      • 한국 청소년을 대상으로 한 측두하악장애에 관한 역학적 연구

        An epidemiological investigation was carried out at Yongin, Kyungkido, South Korea to determine the prevalence of symptoms of temporomandibular disorders in adolescent aged 16, 17 and 18 years. 2,098 students(male 507, female 1,591) were randomly selected and investigated with the questionnaire. The results were obtained as follows: 1. The prevalence of symptoms of temporomandibular disorders was 80.9%, 77.3% for male and 82.0% for female, with significance between male and female(p<0.05). 2. The prevalence of symptoms of temporomandibular disorders was gradually increased according to increasing age with significant difference among ages(p<0.001) 3. The most frequent symptom in the temporomandibular disorders in pain on chewing(65.9%), and then clicking(50.7%), pain on mouth opening(41.8%), and pain on TMJ(36,8%) 4. There were little relationships between clenching habits and symptoms of temporomandibular disorders(r=0.166), and also between bruxing habits and those(r=0.057).

      • 백서에서 실험적 치아이동시 치조골 흡수에 미치는 Bisphosphonate의 영향 : Biochemical & Histological Observations

        This study was performed to examine the effect of bisphosphonate, an inhibitor of bone resorption, on the formation of osteoclast and bone resorption during experimental tooth movement. Whether bisphosphonate has a cytotoxicity in high dose was also examined. Eighty-seven male Sprague-Dawley rats, weighing 260-350g, were classified into normal (no appliance + 0.9% NaCl), control (appliance + 0.9% NaCl) and four bisphosphonate-treated (appliance + 0.8, 4, 20 or 100 ㎎/㎏) groups. The maxillary left first molar was moved mesially with the tipping movement using 50-70g of force. Bisphosphonate(etidronate disodium) was injected intraperitoneally with a dose of 0.8, 4, 20, or 100 ㎎/㎏ simultaneously with the application of the orthodontic force. They were killed at day 1, 3, or 7 after the application of the orthodontic force. The activities of serum acid phosphatase and lactate dehydrogenase (LDH) were assayed, and osteoclasts and the degree of bone resorption were examined histologically. The results obtained were as follows: 1. Acid phosphatase activities were significantly higher in the appliance groups, both control and bisphosphonate-treated (4, 20, and 100 ㎎/㎏) groups, at day 1 and 3 than these in normal. At day 1, bisphosphonate-treated(4, 20㎎/㎏) groups showed even higher acid phosphatase than control. However, at day 7, no significant difference was noted between the control and bisphosphonate-treated groups. 2. LDH activities in the 4, 20㎎/㎏ bisphosphonate-treated groups were increased during the experimental periods examined, but there were no significant differences in the 0.8, 100㎎/㎏ bisphosphonate-treated groups. 3. There was no bone resorption at day 1, but severe bone resorption was observed at days 3 and 7 in the control. Bone resorption was reduced by bisphosphonate-treatment at day 3. Bone resorption observed at day 7 was similat between the control and bisphosphonate-treated groups. 4. Few osteoclasts were observed at the alveolar bone in the control and bisphosphonate-treated groups at day1. At day 3, numerous osteoclasts were shown in the control, the degree of which was reduced in bisphophonate-treated groups. These results suggest that the inhibition of the osteoclast formation was not the mechanism of bone resorption by the bisphosphonate-treated during experimental tooth movement. There was no distinct cytotoxicity with a high dose of bisphosphonate. And the drug should be administrated repeatedly to maintain the inhibitory effect of bone resorption.

        고정식 교정장치 장착환자에서 슬림모 칫솔과 V형 교정칫솔 간의 구강위생관리 효과 비교

        본 연구는 고정식 교정장치 장착환자에서 V형 교정칫솔과 비교하여 슬림모 칫솔의 구강위생관리 효과를 평가해보기 위해 시행되었다. 고정식 교정장치를 장착하여 치료를 받고 있는 환자 34명(평균연령 21.9 ± 7.5세)을 대상으로 무작위로 슬림모 칫솔(Nano silver slim care.34, Aiio, Seoul, Korea)을 사용한 군과 V형 교정칫솔(Oral-B Laboratories Ltd., P&G Korea, Seoul, Korea)을 사용한 군으로 구분한 후 연구시작 시(Baseline), 2주 후, 4주 후 및 6주 후에 걸쳐서 하나의 칫솔을 교환하지 않고 변형 바스법으로 하루 2회 2분씩 닦도록 하여 치태지수, 치은염지수 및 치은출 혈지수를 측정하고 비교 분석하였다. 연구 결과 6주간의 실험기간에 걸쳐 슬림모 칫솔군과 V형 교정칫솔군 간에 치태지수, 치은염지수 및 치은출혈지수 모두 통계적으로 유의한 차이를 보이지 않았다. 실험시기에 따른 변화에 대해서 두 칫솔군 모두 치태지수와 치은염지수는 연구시작 시를 기점으로 점차적으로 감소하여 4주 후에 저점을 보이고 6주 후에는 다시 상승하였고, 치은출혈지수의 경우는 V형 교정칫솔군의 경우 2주 후에 최저점을 보이다가 4주 후와 6주 후에는 점차적으로 상승하는 양상을 보인 반면 슬림모 치솔군의 경우 연구시작 시부터 6주 후까지 점차적으로 감소하는 양상을 보였다. 연령에 따른 두 칫솔군 간의 구강위생 관리 효과 비교에서 슬림모 칫솔군은 치태지수와 치은염지수에서 20대가 10대에 비해 통계적으로 유의하게 작은 수치를 보였다 (p < 0.013). 이상의 연구 결과로 고정식교정장치로 치료받는 환자에서 두 칫솔 모두 구강위생관리에 동일하게 효과적이었으나 20대 이상의 성인 교정환자에서 슬림모 칫솔이 V형 교정칫솔보다 치태제거 효과가 더 우수함을 보여 도움이 될 수 있음을 시사하였다. Objective: The purpose of this study was to evaluate the efficacy of a slim bristled toothbrush compared with a V-shaped orthodontic toothbrush in patients with fixed orthodontic appliances. Methods: Thirty four orthodontic patients receiving edgewise treatment were randomly assigned to two groups, a slim bristled toothbrush (Nanosilver slim care) and a V-shaped toothbrush (Oral-B). Plaque index, gingival index and bleeding index were recorded at the beginning of the study (baseline), 2 weeks, 4 weeks and 6 weeks after new toothbrushes were supplied and the results were analyzed statistically. Results: No statistically significant differences were found for plaque index, gingival index and bleeding index between toothbrush groups during the 6 weeks. Plaque and gingival indices were decreased at 2 weeks and 4 weeks but increased at 6 weeks in both toothbrush groups. Bleeding index in the V-shaped toothbrush group showed the lowest value at 2 weeks then increased at 4 weeks and 6 weeks but in the slim bristled toothbrush group decreased from 2 weeks through 6 weeks to under baseline levels. Patients in their twenties had significantly lower values than teenagers in the slim bristled toothbrush group (p < 0.013). Conclusions: The results would suggest that both of the toothbrushes are equally effective but the use of a slim bristled toothbrush may be of benefit in promoting gingival health for fixed orthodontic appliance patients in their twenties and over.

      • Molar Uprighting Spring에 의해 발생되는 치조골내의 응력분포에 관한 광탄성학적 연구

        This study was performed to analyze the effects of forces to the alveolar bone by various molar uprighting spring such as helical uprighting spring, T-loop spring, Modified T-loop spring and open coil spring. The simplified two-dimensional photoelastic model was constructed with a lower left posterior quadrant containing the second molar, the first and second premolars and the canine, with the first molar missing. Several molar uprighting springs were fabricated from 0.017 by 0.022 inch blue Elgiloy and applied to the photoelastic model. Two-dimensional photoelastic stress analysis was performed, and the stress distribution was recorded by photography. The results obtained were as follows ; 1. In all the kinds of the springs, the center of rotation of the mandibular second molar was observed at the apical 1/5-1/6 between the alveolar crest and the root apex. 2. In all the kinds of the spring, the stress induced in the mesial root surface of the manibular second molar was relatively homogeneous but there was some difference in the magnitude of the stress. 3. In the kinds of the springs, the distal crown tipping moment of the second molar was increased in turn as open coil spring, helical uprighting spring, T-loop spring, and modified T-loop spring. 4. The largest extrusive force was occured in the T-loop spring, intrusive force was occured in Modified T-loop spring only, and the largest distal tipping force was occured in open coil spring. 5. In the T-loop spring with activation, the stress induced in the mesial root surface of the second molar was increased gradually from the root apex to the alveolar crest and highly concentrated in the alveolar crest.

        상악 가철식 보정장치인 circumferential comfortable retainer (CCR)에 대한 불편감 평가

        발음장애, 구토감 및 착용 불편감과 같은 보정장치에 대해 환자가 느끼는 불편감을 평가해보기 위해 고정식 교정장치로 교정치료를 받고 교정장치가 제거된 66명(남자 23명, 여자 43명; 평균연령 23.42 ± 10.19)의 교정환자를 대상으로 무작위로 두 군으로 배정한 후 고정식 교정장치를 제거한 다음 날 CWR 장착군에게는 구개를 완전히 덮는 구개 완전 피개형 보정장치인 conventional wraparound retainer (CWR)를 장착시키고 CCR 장착군에게는 구개를 말 발굽 모양으로 부분 피개하는 보정장치인 circumferential comfortable retainer (CCR)를 4주 동안 장착시킨 후 발음장애, 구토감 및 착용 불편감의 정도에 대해 100-mm visual analog scale (VAS)로 표시할 수 있도록 제작된 설문지를 통해 얻은 점수에 대해 통계적으로 비교 분석하였다. 연구결과 발음장애와 착용 불편감의 비교에서 CCR 장착군이 CWR 장착군에 비해 통계적으로 유의하게 낮았다 (p < 0.05). 구토감의 비교에서는 CCR 장착군이 CWR 장착군에 비해 낮은 점수를 보였지만 통계적으로는 유의한 차이를 보이지 않았다 (p = 0.146). 이상의 연구 결과로 circumferential comfortable retainer (CCR)는 발음장애를 감소시키고, 착용 불편감을 완화시킴으로써 환자의 협조도를 증진시켜 줄 수 있는바 고정식 교정장치를 이용한 교정치료 후 치료결과 유지에 도움이 될 수 있음을 시사하였다. (대치교정지 2010;40(5):325-333) Objective: The aim of this study was to illustrate the circumferential comfortable retainer (CCR) as a removable maxillary retainer with good potential patient compliance and to evaluate the discomfort of the retainers including distorted speech, gagging sensation and appliance discomfort. Methods: Sixty-six orthodontic patients (male, 23; female, 43; mean age, 23.42 ± 10.19 years) who received orthodontic treatment with fixed orthodontic appliances were randomly assigned to two groups after debonding, a conventional wraparound retainer (CWR) group that fully covers the palate with an acrylic plate and a highly polished surface, and a circumferential comfortable retainer (CCR) group which has a horseshoe shaped base plate with three folds on the anterior region. A questionnaire that had a visual analog scale (VAS) which consists of a 100-mm horizontal line with 2 end-points labeled “no discomfort” on the left and “worst discomfort” on the right, with regard to distorted speech, gagging sensation and discomfort, was administered to patients after 4 weeks of retainer wear. The Mann-Whitney test was used to test the hypothesis that there was no difference between the two retainers. Results: Comparing distorted speech and discomfort, the CCR group significantly had lower values than the CWR group (p < 0.05). Comparing gagging sensation, the CCR group had lower values than the CWR group but there were no statistically significant differences between groups (p = 0.146). Conclusions: In conclusion, the results suggest that the circumferential comfortable retainer (CCR) might facilitate patient compliance and thereby improve the maintenance of the fixed orthodontic treatment outcome.         백서에서 실험적 치아이동시 치조골 흡수에 미치는 Bisphosphonate의 영향 : 생화학적 및 조직학적 관찰 Biochemical & Histological Observations

        백서에서 실험적 치아 이동시 bisphosphonate가 파골세포의 형성에 미치는 영향과 골 흡수 억제기전을 규명하고 독성유무를 알아보고자 하였다. 체중 260-350g의 웅성 백서 87마리를 정상군(장치비장착 + 0.9% NaCl), 대조군(장치장착 _ 0.9% NaCl) 및 장치장착후 bisphosphonate 투여군(0.8mg, 4 mg, 20 mg, 및 100mg/kg) 으로 분류하였다. 상악 좌측 제1대구치를 근심으로 치아이동이 일어나도록 50-70g의 교정력을 가하고, 교정장치 장착후 1일, 3일 및 7일째에 혈청 acid phosphatase와 lactate dehydrogenase (LDH)의 활성도를 측정하고, 또한 제1대구치를 포함한 상악골일부에서 파골세포수 및 골흡수 정도를 조직학적으로 관찰하여 다음과 같은 성적을 얻었다. 1. Acid phosphatase 활성도는 장치후 1일째와 3일째에 대조군과 bisphosphonate 투여군에 서 모두 정상군에 비해 2-3배 높았으나, 7일째에는 정상군과 유의한 차이를 보이지 않았다. 2. LDH활성도는 bisphosphonate 4 mg과 20mg/kg 투여군에서 전 실험기간에 걸쳐 증가된 양상을 보였으나 0.8mg과 100mg/kg 투여군에서는 유의한 차이를 나타내지 않았다. 3. 골흡수는 장치후 1일째에 대조군과 bisphosphonate 투여군에서 모두 관찰되지 않았으나, 3일 이후에 나타난 7일째가지 지속되었다. Bisphosphonate 4, 20 및 100mg/kg 군에서의 골흡수정도는 3일째에는 대조군에 비해 미약하였으나 7일째에는 대조군과 유사하게 나타났다. 4. 파골세포는 1일째에 대조군이나 bisphosphonate 투여군 모두에서 거의 관찰할 수 없었다. 3일째에 대조군에서는 파골세포가 다량 출현하였으나 bisphosphonate 투여군에서는 약물의 용량이 증가함에 따라 감소하여 나타났다. 이상의 결과로 실험적 치아이동시 파골세포의 형성억제가 bisphosphonate에 의한 골흡수억제기전이 아님을 알 수 있었고, bisphosphonate는 투여량이 증가에 따른 뚜렷한 세포독성은 관찰되지 않았으며, 골흡수 억제효과를 지속시키기 위해서는 약물이 반복적으로 투여되어야 할 필요가 있음이 시사되었다.       • 잠제의 안정성에 관한 실험적 연구

        In this paper Using a TTP to the experimental study on the stability of the Submerged breakwaters would like to perform. Experiment conditions consisted of structure conditions and incident wave conditions. Submerged breakwater which consisted of two layer installed in wave flume. height and crest width of submerged breakwaters are 8.6cm and 80cm, respectively. The three types of TTP were used: normal type, safety type, safety type(connect). In this study, rocking rate of submerged breakwater applied the general definition in Shore Protection Manual. Damage in terms of displaced blocks is generally given as the relative displacement, D, defined as the proportion of displaced blocks relative to the total number of blocks. Rocking rate was plotted in Figure 5 as function of Lb/L0. h=2.0hs, T=2sec case rocking rate of normal tetrapod are 57.1 %, safety tetrapod are nonrocking. Experimental result shown that stability of safety tetrapod higher than normal tetrapod.
