      • 肝癌患者 193例에 對한 臨床的 考察

        Auther observed 193 cases of hepatoma patient who were admitted to surgical Department of Pusan Gosple Hospital during the past 5 years. Among 193 patients, exploratomy laparotomy was carried out for the 82 cases, and hepatic resection was done for the 27 cases. And resectability was only 32. 9 percent. During the operation of hepatic resection or within 24 hours following the hepatic resection there were no death cases. 3 cases out of the 27 cases who were received hepatic resection, have been in good health until 2 yeerrs following hepatic resection.

        화상환자의 영양치료

        Severe burns evoke a substantial metabolic response causing physiological derangement leading to a hyper-metabolicstate. The hyper-metabolic response is accompanied by severe catabolism and a loss of lean body mass and also by aprogressive decline of host defenses that impairs the immunological response. The protective functions of an intact skin arelost, leading to increased risk of infection and protein loss. Therefore, nutritional therapy is a cornerstone of burn care fromthe early resuscitation phase to the end of rehabilitation. Careful assessment of the nutritional state of the burn patientis also important to reduce infection and recovery time, and improve long-term outcomes. Evidence-based research hasshown that nutritional therapy contributes to improved clinical outcomes.

      • 2-3 : Effects of Early Enteral Feeding After Severe Burn

        Background: Early enteral feeding is recommended in cases of critical illness. However, It is unclear whether this recommendation is of most benefit to extremely ill patients. From our experiences, our authors believed that early enteral nutrition can lead to better prognosis of BICU patients. The aim of this study is to evaluate the efficacy of early enteral feeding. Methods: one hundred fifty six critically ill patients were enrolled and grouped as “Early enteral feeding (EF)” and “Delayed enteral feeding (DF)” for this cross-sectional, retroospective randomized observational study from January 2012 to December 2013. To reduce the selection bias, we compare our groups to 20-39% (group A) TBSA (total body surface area, and 40-59% (group B) TBSA burned. length of ICU stay, length of hospital stay, hospital mortality, serum prealbumin, serum transfferin and lymphocyte count data were collected over 28 days. Results: There were no statistical differences in measured outcomes between early and late feeding groups. In serologic test; prealbumin, transferrin and lymphocyte, there is also no statistical difference except 2nd, 4th week of lymphocyte. EF group has higher lymphocyte than DF group. In the group A, however, prealbumin and transferrin was high in EF group during the whole 4 weeks of study. Comparing the ICU stay and hospital mortality, there was also no statistical significance. Conclusions: In this study, there is no significant association between hospital outcomes and timing of enteral feeding initiation. More active trials and many-sided studies will be needed to maximize the effect of early enteral nutritional support as a method to improve treatment for major burned patients.

        수도사 카르피니(Carpini)의 『몽골인들의 역사』 (Ystoria Mongolorum ) 연구 - 몽골 방문의 목적 및 저술의도 연구 -

        교황 이노센트 4세가 공식적으로 파견했던 프란체스코회 수도사 플라 노 드 카르피니는 몽골제국을 여행했던 첫 번째 유럽인으로서, “우리가 타 타르인이라고 부르는 몽골인들의 역사”를 저술했다. 이는 연대기적이 아 니라, 주제별로 기술되어 있으며, 유럽인 최초로 아시아를 직접적-사실적 으로 묘사한 작품이라고 평가받고 있다. 『몽골인들의 역사』는 두 번의 편 집작업을 통하여 작성되었다: 그의 리옹 귀환 직후에 수행된 첫 번째 편집 작업을 통하여 몽골군의 유럽재침공을 대비하기 위한 정탐보고서로서 서문과 제1장부터 제8장이 작성되었다. 두 번째 편집작업은 약간의 휴식 후 에 진행된 두 번째 편집작업을 통하여 본문의 진정성에 대한 의심을 방지 하기 위한 시도로서, 제9장이 추가되었다. 카르피니에게 있어서 교황의 친서전달이 공식적인 몽골방문의 중요한 목적이었으며, 부수적으로 정탐의 임무를 부여받았다. 그에게 있어서 교 황친서의 전달은 그가 가지고 있었던 프란체스코회 수도사로서의 선교적 사명감과 부합했다. 그러나 교황대사로서 구육칸을 가톨릭 신앙으로 회심 시키지 못하고, 유럽재침공의 의도를 암시했던 구육칸의 매우 위협적인 친서를 받은 이후, 몽골인들의 차후 계획과 의도가 유럽 재침공이라는 것 을 확신하게 되었다. 이와 같은 과정을 통하여 작성된 그의 몽골방문보고 서로서의 『몽골인들의 역사』는 유럽인들이 두려워했던 몽골에 대한 첫 번째 인상을 확립할 수 있도록 도와 주었으며, 전쟁준비를 위한 유익한 정보를 전달했다. John of Plano Carpini as a Franciscan friar is the first noteworthy European traveller in the Mongol Empire, to which he was sent on a formal mission by Pope Innocent IV. He wrote an excellent firsthand account of the Mongols called Ystoria Mongolorum (The History of the Mongols Whom We Call the Tartars). He successfully recorded a detailed account as the first direct authentic description of Asia based on his personal observation throughout the journey, which was formed not chronologically but thematically. Ystoria Mongolorum was written in two separate moments: the first redaction was realized shortly after his return to Lyon. The preface and Chapters 1-8 were written as a spy report to prepare for the reinvasion of Europe by the Mongols.; the second after some rest. Chapter 9 was entirely added in the second redaction and it looks like the attempt to reject the accusation of the text. The delivery of the papal letter was the official purpose of his visit to Mongolia. This purpose coincided with his sense of mission as a Franciscan friar. Incidentally. he was assigned a spy mission. But as the papal legate, John of Plano Carpini might not have accomplished the goal of converting Güyük Khan to Catholic faith or manifest the power of Almighty God and his supreme vicar, Pope Innocent IV. Furthermore, he received a very threatening letter from Güyük Khan hinting at the intention of reinvading Europe. It became clear that the Mongols’ intention was a reinvasion of Europe. Ystoria Mongolorum as the report of his visit to Mongolia written through this process helped Latin Christendom establish an initial impression on the Mongol Empire the Europeans feared, which was also beneficial for the preparation of a war.

        스마트기기 구매과정에서 의사결정모델링에 관한 연구 : 감정요소의 상호작용을 중심으로

        본 연구는 고관여 혹은 고기여도 제품군에 속하는 모바일/스마트 기기 구매시 우선 고려되는 인간의 감정적 요소들에 대한 관점을 우선시 하여 주 소비자층의 의사 결정과정에 대한 검증을 하고자 하였다. 연구결과 정보로서의 감정은 긍정적인 감정반응에 영향을 주며, 그 긍정성을 바탕으로 제품/서비스 평가에 정보로서의 감정이 유의미하게 작용함과 아울러, 제품/서비스에 대한 흥미를 유발하는데 기여한다는 기존의 연구결과들을 본 연구에서도 확인하였다. 그러나 기존의 연구들과는 달리 감정은 흥미를 유발하지만 그 흥미가 가치판단을 주도하지 않으며, 더구나 직접적인 욕구의 형성을 돕지도 않는다는 차별적인 결과도 확인할 수 있었다. 결과적으로 모바일/스마트 기기에 한정한 연구이지만 정보로서의 감정이 특정한 루트를 통해서만 작용하고 있음을 확인할 수 있었다. 이와 같이 충동구매를 이끄는 감정정보의 기능에 대한 제한적인 태도의 확인은 브랜드 이미지와 충성도를 높이기 위한 기존의 광고표현전략의 개념과는 다른 새로운 광고표현전략의 필요성을 시사한다. 아울러 감성적 요소와 제품 정보 간에 어떤 것들이 보다 더 직접적인 반응을 일으키는 지에 대한 구체적인 비교 및 재확인을 제언하였다. This study is to discover the functions of customer emotion in highly involved and information intense buying decision process. This study used smart networking devices (ex. smart phones), which falls into expensive commodity category in its acquisition and use. Known through many other previous studies, information tends to trigger emotional responses. However, this study, assumed emotion also as information, tried to find emotion-triggered consumer responses in buying the products. The results lighted the finding that emotion as information helped to form positive interests on products as well as positive emotion on products. Unlike the previous studies on positive connection between interests and product/service value evaluation, interests triggered by emotion in this study did not show any connection between them. The phenomenon stretched to the relationship between interests and consumer needs. Although the study was only about smart device buyers’ emotional responses, it became obvious that emotion as information majorly works through a limited path when consumers form their buying intention. Based on the findings, ideas of promotion and communication activities that are designed to work around emotion-triggered-buying (a.k.a. impulsive buying) may need to have a better and closer examination to further clarify the cause and effect relationship. The part may need further studies.

        Gesellschaftskonzeption bei Zwingli und Barth

        Zwinglis Betonung der ethischen und sozialen Verantwortung verstärkt den engen Zusammenhang von Kirche und Gesellschaft, der seinem theologischen Ansatz entspricht. Alfred Farner charakterisiert Zwinglis Teilnahme an der Züricher Politik als prophetische Theokratie. Meines Erachtens ist die prophetische Identifikation von Kirche und Obrigkeit bei Zwingli als ein durchaus noch nicht verwirklichtes Ideal verstehbar, bzw. bliebe wegen der menschlichen Sündhaftigkeit grundsätzlich utopisch. Im Vergleich zu Zwingli, dessen politisches Ideal als prophetische Theokratie entfaltet wurde, ist für Barth die Demokratie das angemessene politische Konzept für die Bürgergemeinde. Dabei ist zu beachten, dass für Zwingli die Obrigkeit christlich sein sollte,während Barth um der Neutralität des Staates willen nicht auf eine Verchristlichung der Bürgergemeinde abzielt. Für ihn stellt vielmehr die Rechtsstaatlichkeit ein Gleichnis des Reiches Gottes dar, weshalb die Christengemeinde gerade nicht den Umbau der Bürgergemeinde zu einem kirchlichen und theokratischen Gemeinwesen erstreben,sondern ihre Mitverantwortung innerhalb der Bürgergemeinde übernehmen sollte. Allerdings ist es unbestreitbar, dass der historische Kontext von Zwingli in Zürich, Calvin in Genf und Barth in Deutschland höchst unterschiedlich war. Zusammenfassend möchte ich auf die in Zwinglis, Calvins und Barths theologischer Argumentation implizierte Bedeutung aufmerksam machen: Barth hat unter dem Einfluss Calvins sowohl die Trennung von Christengemeinde und Bürgergemeinde als auch die Herrschaft Christi herausgearbeitet. Aber Barth geht im Vergleich zu Calvin unter dem Einfluss Zwinglis einen Schritt weiter und über dessen Verständnis des Widerstandsrechts hinaus, da er das soziale Engagement der Christenfür die Bürgergemeinde stärker hervorhebt. In diesem Punkt etehen Calvin und Barth mit Zwingli insofern auf der gleichen Linie, als dass der Staat unter dem Evangelium steht, bzw. die Gesellschaft nach dem Wort Gottes reformiert werden sollte.
