      • 임상 : 사대 척삭종의 수술 치료에 대한 비교 분석; 내시경을 이용한 경접형동 접근법의 유용성

        Objective:Chordomas, whether involving the clivus or simply situated anterior to the brainstem, present a technical challenge for adequate exposure and safe resection. Transsphenoidal approach is safer and easier than any other approaches, but it has some limitation of resectability in laterally extended tumor. The objective of this paper is to demonstrate the advantages of endoscope assisted transsphenoidal approach to remove paraclival lesion on the midline. Methods:Nine patients with clival chordomas underwent surgery in our institution from 2002 to 2009. There were two men and seven women who ranged in age between 4 and 59 years. Four patients underwent stage operation by both transsphenoidal surgery and transpetrosal or transcavernous surgery. Three patients underwent only transsphenoidal surgery and two underwent only transpetrosal surgery. For image guidance, both endoscope and neuronavigation system were used in all surgical procedures. Results:Gross total removal was achieved in 33.3% and subtotal removal was achieved in 55.5%. The mean follow-up period was 44.2 months. The mortality rate during the follow-up period was 11.1%. Conclusion:For accessing clival chordoma via an anterior route, visualization of the tumor with the endoscope and navigation system is readily achievable and have the advantages of confirmation the anatomy that cannot be seen in the conventional transsphenoidal approach. This study shows transsphenoidal approach become safer and easier if the tumor is located on upper two thirds of the clivus and located on the midline.

        Follicle-stimulating hormone-secreting pituitary adenomas are rarley diagnosed and likely considered as a non-functioning pituitary tumor. Because they usually do not present until of sufficient size to cause neurologic deficits by mass effect or hypopituitarism, whereas other functioning tumors - acromegaly, prolactinoma, Cushing s disease - frequently present earlier with symptoms caused by physiologic effects of excess hormones that they secrete. 62-year-old man suffered from visual disturbance. Endocrinologic evaluation showed high serum FSH level. Suprasellar extended pituitary mass with compression to optic chiasm & cavernous sinus was imaged on MRI and removed by endoscope assisted endonasal transsphenoidal surgery. It was composed of gonadotroph showed strong immunohistochemical reactivity against FSH and LH. Uncommon pituitary tumors such as FSH-secreting adenomas can be defined early and accurately through correlation between their clinical, endocrionological, radiographic and immunohistochemical characteristics.

        문둥이박쥐(Eptesicus serotinus)의 배설물을 이용한 먹이원 분석

        문둥이박쥐(Eptesicus serotinus)의 먹이원 구성을 파악하기 위하여 총 25개체의 암컷 성체로부터 배설물을 수집하여 분석하였다. 박쥐의 포획은 경북 안동시의 문둥이박쥐 집단 서식지가 확인된 교량을 대상으로 하였으며, 2014년 5월부터 9월까지 매월 5개체를 대상으로 하였다. 총 100개의 배설물이 수집되었으며, 고배율 현미경(50 × binocular microscope)을 이용하여 곤충의 목(Order)별로 구기(mouthpart), 다리(leg), 날개(wing), 더듬이(antenna), 딱지날개 (elytra) 등 구분할 수 있는 특징을 이용하여 동정하였다. 배설물 분석결과 문둥이박쥐의 먹이원으로 이용된 분류군은 딱정벌레목(Coleoptera), 벌목(Hymenoptera), 파리목(Diptera), 노린재목(Hemiptera) 등 4개였다. 먹이원 가운데 딱정 벌레목은 6월을 제외하고 5, 7, 8, 9월 모두에서 확인되었으며, 특히 5월과 7월에는 오직 딱정벌레목만 확인되었다.그 외 노린재목, 파리목, 벌목은 각각 9월, 6월, 8월에 먹이원으로 이용되는 것으로 나타났다. Faecal analysis was carried out to identify the diet composition of Eptesicus serotinus after collecting the faeces from 25 female bats. Five bats were captured at a bridge where a colony of E. serotinous was found in Andong city, Gyeongbuk, every month from May to September 2014. A total of 100 faeces were collected and the parts of insects obtained from the faeces were examined using a high-resolution microscope (50× binocular microscope) and then the orders of the insects were identified by the characteristics of the parts of the insect (mouthpart, leg, wing, antenna, elytra, etc.). According to the results of the faecal analysis, 4 orders: Coleoptera, Hymenoptera, Diptera and Hemiptera were used as food resource of E. serotinus. Coleoptera was found in May, July, August, and September. Interestingly, only Coleoptera were found in May and July. Hemiptera, Diptera, and Hymenoptera were found to be used as food resources in September, June, and August.

        전방 경유 디스크 절제술 및 유합술과 후방 추간공 절제술에서의 경추 추간공의 형태학적 변화 비교

        목적: 경추 추간공 협착증에서 전방경유 경추 추간판 절제술 및 유합술과 추간공 절제술 후 추간공의 크기 변화 및 임상 결과를 비교하여 그 효용성에 대하여 알아보고자 하였다. 대상 및 방법: 2018년 1월부터 2019년 3월까지 전방경유 경추 추간판 절제술 및 유합술 및 추간공 절제술을 시행한 186명 중에서 나이, 성별, 체질량지수 등을 고려한 환자-대조군 연구를 통해 선별된 102예를 대상으로 하여 후향적으로 분석하였다. 술 전 및 술후 5일째 자기공명영상을 촬영하여 전방경유 경추 추간판 절제술 및 유합술을 시행 받은 군(A군-51명)과 추간공 절제술을 시행 받은 군(B군-51명) 사이의 추간공의 크기 변화 및 임상 결과를 비교하였다. 결과: 술 후 A군에서 추간공 수직 직경의 변화는 평균 1.7 mm, B군에서는 평균 1.2 mm로 증가하였으며 A군에서 0.5 mm 높게 나타났다(p=0.042). 추간공 횡단 직경의 변화는 A군에서 평균 1.2 mm, B군에서는 1.8 mm로 B군에서 0.6 mm 넓게 나타났다(p=0.021). Neck disability index (NDI), Japanese orthopaedic association (JOA) score 모두 양 군에서 호전되는 결과를 보였으며 A군에서 더 높은 호전을 보였지만 통계학적으로 유의하지는 않았다(각각 p=0.356, p=0.607). 결론: 미세현미경과 내시경을 이용한 추간공 절제술은 전방경유 경추 추간판 절제술 및 유합술에 필적하는 임상적 결과를 보였고, 수술 후 경부의 통증 및 불편감을 줄이면서 인접분절의 퇴행변화를 줄일 수 있기 때문에 경추의 추간공 협착증 환자에서 유용한 치료 방법 중 하나로 판단된다. Purpose: This study compared the change in foraminal space on magnetic resonance imaging (MRI) and the clinical outcome after anterior cervical discectomy and fusion (ACDF) versus foraminotomy in cervical foraminal stenosis. Materials and Methods: A retrospective case-control study was conducted from January 2018 to March 2019 on 186 patients who underwent ACDF and foraminotomy. One hundred and two cases were selected considering age, sex, and body mass index. MRI was performed before and on the 5<SUP>th</SUP> day after surgery to compare the changes in the foraminal diameter between the ACDF group (group A-51) and foraminotomy group (group B-51). Results: Between groups A and B, the average change in foraminal vertical diameter was 1.7 mm and 1.2 mm, respectively; group A was 0.5 mm larger difference (p=0.042). The average change in foraminal transverse diameter was 1.2 mm and 1.8mm, respectively; group B showed a 0.6 mm larger change (p=0.21). Both the neck disability index (NDI) and Japanese orthopaedic association (JOA) scores improved in both groups. Group A showed more improvement, but there was no significant difference (p=0.356, p=0.607, respectively). Conclusion: Foraminotomy is a useful option for patients with foraminal stenosis of the cervical spine because it showed comparable clinical and morphological results to ACDF and could minimize motion segment loss and muscle and ligament damage.

      • 삼천지의 저서성 대형무척추동물의 군집구조 및 생물학적 수질평가에 관한 연구

        The present study was conducted for the study on the community stucture of the benthic macroinvertebrates and the water quality in Smchun-impoundement based on the biological parameters. The survey area induced eight sites in Samchun-impoundment and the study was carried out from September 1997 to July 1998. The result are as follows; (1) Benthic macroinvertebrates are composed of 74 species including 64 genera, 41 families, 13 orders, 5 classes in three phyla. (2) Hemiptera and Ephemeroptera were dominant taxa in this area. (3) The diversity indices ranged from 2.022 to 2.519 depending on the site. (4) The water quality criteria was investigated as α- mesosaprobic with Group Pollution Index(GPI) 1.91.
