Antiepileptic drug monitoring is essential to the rational medical therapy of the epilepsies. Present methods for monitoring plasma phenobarbital measure total drug levels, wherease only the non-protein-bound (free) fraction is considered to be biologically active. Since CSF fluid and saliva are both low-protein fluids, salivary phenobarbital levels correlate with plasma free and CSF levels. Measurement of phenobarbital in saliva provides a noninvasive method of obtaining repeated samples. Salivary phenobarbital concentration is dependent on the PH of the salive at the moment of sampling. This study was performed to develop a correlation between plasma total and salivary phenobar bital levels by the High Performance liquid Chromatography in 39 Patients. The following results were obtained; 1) Among 39 Patients males were 24(61.5%), and females were 15(38.5%), giving sex ratio 1.6:1, Majority(41%) of cases were between 6 and 10 years. 2) The mean salivary PH is 7.28±0.25. 3) The mean plasma total phenobarbital concentration was 13.77±5.00ug/ml and salivary concent. was 5.04±2.20 ug/ml. In correlation between plasma total phenobarbital concent. and salivary phenobarbital concent., liner regression equation is y=0.077+0.359×(y:salivary phenobarbital concent., x:plasma total phenobarbital concent) with correlation efficient(r)=0.73. 4) The mean salivary phenobarbital levels as a percent of plasma total levels were 34.7% at salivary PH values less than 7.0, 37.1% at PH values from 6.0 to 7.4, 37.5% PH values greater than 7.4 and 36.2% in overall cases. 5) In correlation between salivary phenobarbital concent./plasma total ptenobarbital concent (%) and salivary PH, linear regression equation is y=-1.52+5.24×(y:salivary phenobarbital concent./plasma total phenobarbital concent. x 100, x:salivary PH) with correlation efficient (r)=0.696.
