There have been many reports about infections in patients with rheumatoid arthritis (RA) being more frequent than general population. Increased susceptability to infections in RA could be related to disease itself, comorbid conditions, and use of immunosuppressive drugs and steroid. Herpes zoster viral infections also have been reported to be more frequent in RA. We describe a case of 60-year-old woman with RA presented with ophthalmoplegia associated with herpes zoster ophthalmicus. Initially she complained of right orbital and periorbital pain. Six days later, she noted a cutaneous herpes zoster eruption involving the right 1st division of trigeminal nerve. Three days after developing skin eruption, she developed a right ptosis with external ophthalmoplegia and pupil dilatation resulting from the 3rd and 6th cranial nerve involvement. She was placed on intravenous acyclovir for 7days. Eight weeks after the onset of ophthalmoplegla, she recovered nearly completely.

연구논문 : 플라톤의 미학에 관한 고찰

본 글은 인류 문화사에 지대한 영향을 미친 플라톤의 철학적 특징과 그의 예술론을 통해 미학을 재조명해 보았다. 본 글에 따르면 플라톤의 미학은 첫째, 철학적 관점으로 미의 문제를 정립하고 그 본질을 규명하기 위한 토대를 마련했다. 아울러, 현대에이르러 철학자들이 가지고 있는 미에 대한 것들을 철학적 관점으로 정립시키는데 영향을 미쳤다. 둘째, 플라톤의 철학적 이념과 미학에 관한 것들은 합리주의, 낭만주의, 관념주의 등과같은 현대 철학 사조에 지대한 영향을 미쳤다. 셋째, 플라톤의 철학적 관점에 의한 미의 탐구는 근대 미학의 변천사와 함께 미학에 대한 활발한 논의를 가능하게 했으며, 철학적 사유에 의한 미의 이해는 예술의 위상을 크게높였을 뿐만 아니라 인간의 근원적인 창조성에 대한 일반인들의 시각을 변화시켜 주었다. 넷째, 플라톤은 미학의 여러 가지 물음들에 대한 해답을 제시해 주었다. 즉, 어떤 국면들이 대상을 아름답게 만드는가, 미적 기준이 존재하는가, 자연에 대한 예술작품의 관계는 어떤 것인가 등을 비롯하여 예술철학에서 제기되는 다양한 물음들에 대해 예술론과 철학을 통해 해답을 제시하고 있다. 다섯째, 플라톤은 예술이 국민들에게 미치는 영향에 대해서 경고하였다. 이 점에서 오늘날 예술을 통한 청소년 교육에 시사하는 바가 컸다. 예술은 누구에게나 향유 가능성을 열어주고 있는 만큼 경우에 따라 유해 또는 무해할 수도 있다는 것이다. 그것이 아름다운한 예술은 순수하고 무해한 기쁨을 가져다 준다. 그러나 바람직하지 못한 예술은 유해하다. 플라톤은 이러한 이유에서 신이나 영웅들의 비도덕적 행동에 대한 이야기가 청소년들의 교육에서 배제되어야 한다고 했다. 이는 현대 청소년 교육에서 수준 높은 인문학적 소양과 예술교육이 필요한 이유를 설명해 주고 있다. This text is reviewing on the Esthetics upon the philosophical features of Plato and his art theory, which gave a large affect on not only the ancient Greek cultural history but also the Human cultural history. According to the text, Plato’s aesthetics has, first, established the matter of beauty by philosophical perspective and laid out the groundwork to investigate the essentials. Also, it has affected establishing the philosophers’ having of beauty in the present, into philosophical perspectives. Second, Plato’s philosophical idea and esthetics had a profound effect on modern philosophy trend such as rationalism, romanticism, idealism etc. Third, Plato’s study of beauty in regard to philosophical perspectives enabled active discussions about aesthetics and the modern aesthetics transitions, and the understanding of beauty in regard to philosophical reasons did not only improve the status of art but also had changed common people’s viewpoints toward the original creation of human. Forth, Plato has given answers to many questions on aesthetics. Namely, via art theory and philosophy, it has given answers to diverse questions brought up from philosophy of art including which conditions make the objects beautiful?, does standard of beauty exist?, what is the relation of artwork that carries nature? etc. Fifth, Plato warned about the art’s affect on people. In respect to this, today, education for youths through the arts is full of suggestions. As art gives the chance of its enjoyment to whomever, in some cases it could be harmful or harmless. A beautiful art brings pure and innocuous pleasure. But undesirable art is noxious. For these reasons Plato claimed that the stories of immoral acts upon gods and heroes should be ought to be taken out from the education of the youths. This explains why the modern adolescent education requires high level of liberal arts knowledge and art education.

보육교사의 전문성 인식과 직무만족도가 영유아 권리존중 보육실행에 미치는 영향

The aim of this study was to examine the effects of child care teachers’ perception of expertise and job satisfaction on their respect for young children’s rights. It provides useful data for researchers seeking to improve the quality of child care practice and respect for young children’s rights. The subjects of this study were 280 child care teachers in Incheon Metropolitan City. Using the SPSS 21.0 Program for data analysis, we conducted frequency analysis, reliability analysis, ANOVA, post-test, Pearson’s product-moment correlation, and multiple regression analysis. We found significant differences in the respect for young children’s rights among child care teachers according to their perception of expertise and job satisfaction. In addition, we found significant correlations among the three variables and that the teachers’ age and some sub-factors of their perception of expertise and job satisfaction had significant effects on their respect for young children’s rights. This study suggests that high levels of perception of expertise and job satisfaction among child care teachers can improve their respect for young children’s rights.

A 22-year-old woman presented with fever and pancytopenia. One year ago, she was diagnosed as Salmonella group D bacteremia and myelofibrosis associated with SLE at another hospital. She was placed on high dose steroid, however, there was no improvement. Two months ago, she was diagnosed as recurrent Salmonella group D bacteremia. After admission to our hospital, she was placed on intravenous antibiotics and high dose intravenous immunoglobulin. A significant improvement in laboratory and clinical condition occurred and bone marrow biopsy showed complete resolution of fibrosis. We report a case of SLE with myelofibrosis and recurrent Salmonella group D bacteremia.

미분화 척추관절증 환자의 임상상 ; 강직성 척추염 환자와의 비교

Objectives: To investigate the clinical features of undifferentiated spondyloarthropathy among the HLA-B27 positive arthralgic patients and to compare with those of ankylosing spondylitis. Methods: Two hundred and three HLA-B27 positive subjects among patients with arthralgia in various joints were classified according to the standard diagnostic criteria. The onset age, disease duration, numbers of involved joint, X-ray and bone scan findings were compared between patients with undifferentiated spondyloarthropathy(uSpA) and ankylosing spondylitis(AS). Results: 1) The patients diseases were classified as ankylosing spondylitis(46%), undifferentiated spondyloarthropathy(29%), juvenile spondyloarthropathy(4%), reactive arthritis(3%), psoriatic spondyloarthropathy(1%) and unclassified(17%). 2) In undifferentiated spondyloarthropathy, the durations of the symptoms were shorter than ankylosing spondylitis and the male predominance is less prominent(5; 1 as compared to 17:1 in ankylosing spondylitis). 3) The numbers of involved peripheral joints and the frequency of peripheral joint inflammation were more frequent in undifferentiated spondyloarthropathy while the spinal joint symptoms including sacroiliac joint and the radiological severity grade of sacroiliitis were higher in ankylosing spondylitis. 4) There were no significant differences in bone scan uptake and ESR between undifferentiated spondyloarthropathy and ankylosing spondylitis. Conclusion: The prevalence of undifferentiated spondyloarthropathy was notable high in HLA-B27- posicive arthralgic patients when a new classification criteria was applied, Attention should be paid on the diagnosis and progression of this elusive and challenging clinical entity.

Rheumatoid arthritis (RA) is a chronic autoimmune disease that mainly involves the joints. Although the full etiology remains unclear, both genetic and environmental factors contribute to disease. Recently, genetic studies of RA, which include whole-genome linkage study, candidate gene association study, and especially genome-wide association study, made much progress through the advances in technology and large collection of case-controls. These various approaches provided new susceptible genes such as PTPN22, PADI4, STAT4, TNFAIP3, TRAF1-C5, CD40 gene in addition to HLA-DRB1. This review will cover an update in recent progress in RA genetics, focusing mainly on the several novel loci. (Korean J Med 75:515-524, 2008)
