덤핑 증후군은 소아의 위공장 경관식이의 합병증으로 발생할 수 있는 질환으로, 19개월된 여아에서 식후 나타나는 특징적인 임상증상과 당부하 검사상 조기 고혈당 후 나타나는 후기 저혈당으로 진단할 수 있었으며, 생옥수수 전분을 포함한 식이요법만으로도 증상의 호전과 영양상태의 호전을 보일 수 있었다. Dumping syndrome is a known complication of gastric surgery in adults, but a very rare disease in the pediatric population. We report on a case of dumping syndrome in a 19-month-old child, who underwent gastrojejunal feeding tube insertion for the treatment and prevention of gastroesophageal reflux and frequent aspiration pneumonia. At 17 months of age, 2 months after the beginning of gastrojejunal tube feeding, postprandial diaphoresis, palpitation, lethargy, bloating, and diarrhea occurred, and a single episode of convulsion with hypoglycemia were noted. Early and late dumping syndrome was confirmed by an abnormal oral glucose tolerance test with early onset hyperglycemia followed by delayed onset hypoglycemia. Diet therapy including uncooked corn starch then improved the postprandial diaphoresis, abnormal glucose levels, and her nutritional status. We conclude that dumping syndrome may be considered as a complication of gastrojejunal tube feeding in a child.

An effective thermal conductivity measurement for suspensions of microparticles in oil mixture is conducted in order to evaluate the shear rate-dependence of the thermal conductivity of suspensions. Measurements are made for rotating Couette flows between two concentric cylinders. The rotating outer cylinder is immersed into a constant temperature water bath while the stationary inner cylinder is subject to a uniform heat fluff. Test fluids are made to be homogeneous suspensions, in which neutrally buoyant microparticles ($d=25{\sim}300{\mu}m$) are uniformly dispersed. The present measurements show strong shear-rate dependent thermal conductivities for the suspensions, which are higher than those at zero shear rate. The shear rate dependent thermal conductivity increases with the particle size and volume concentration.4 new model for shear rate-dependent thermal conductivity of microparticle suspensions is proposed; the correlation covers from zero shear rate value to asymptotic plateau value at moderately high shear rates.

Bending deformation and energy absorption of aluminum square tube beams reinforced by composite layer have been evaluated using experimental tests combined with theoretical and finite element analyses. Six kinds of reinforced aluminum square tube beams composed of glass fabric/epoxy layer wrapped around on aluminum tube were employed for the bending test. A theoretical modification of hinge model was developed to predict the resistance to the collapse of reinforced tube beams subjected to a bending load. Theoretical ultimate bending moment and energy absorption capability were in good agreement with experimental ones. Aluminum square tube beams strengthened by composite layer on the whole web and flange showed an excellent bending strength and energy absorption capability.

The membrane deflection in piezoelectric actuators for micropump applications has been analyzed using theoretical and finite element analyses. A modified theoretical model was developed to predict the deflection of membrane. The theoretical results were in good agreement with the results from numerical simulation. Based on this theoretical model, the effects of several important parameters and variables on actuator performance have been investigated. These parameters and variables include the dimensions and mechanical properties of the piezoelectric disk, the membrane and the bonding layer material. As a result, a viable design of a piezoelectric actuator for micropump system can be achieved and some experimental works are needed for further study.

Background/Aims: Partial tracheal narrowing can occur during expiration in the normal population. It is not certain whether the trachea collapses more readily in chronic airway disease. We evaluated the tracheal narrowing at end-expiration using computed tomography (CT). Methods: We investigated 45 patients and 22 normal subjects who underwent high-resolution CT and pulmonary function tests. In each subject, two CT images at the same level of the aortic arch were compared: one at end-inspiration and the other at end-expiration. The cross-sectional area and sagittal diameter of the trachea were measured using a hand-tracing method, using the in-program measuring tools of Medical Image Viewer, and the percentage changes of each value were calculated. Results: Of the 45 patients with chronic airway disease, 21 had chronic obstructive pulmonary disease, 16 had bronchial asthma, and 8 had bronchiectasis. The mean change in the cross-sectional area was 13.3% in the patients and 9.0% in the normal subjects (p<0.05), and the mean change in sagittal diameter was 11.8 and 9.6%, respectively (p>0.05). The decrease in tracheal cross-sectional area was greatest in bronchiectasis, while the greatest decrease in sagittal diameter was in bronchial asthma. There was no significant difference in tracheal collapsibility among the disease groups. The percent change in the tracheal cross-sectional area was correlated with the % predicted FVC (r = -0.033, p<0.05) and FEV1 (r = 0.277, p<0.05) in both the patients and normal controls. Conclusions: The decrease in tracheal cross-sectional area at end-expiration measured using chest CT was greater in chronic airway disease than in normal subjects, and was associated with ventilatory function.
