Purpose: To evaluate the effect of intermittent bleeding method in the distal phalanx replantation. Materials and Methods: From January 2007 through June 2009, authors have replanted 117 cases of distal phalangeal amputation in adults at Soo Hospital and Chonbuk National University Hospital. Cases of zone II were 60 cases and zone III 57 according to Allen classification. Male to female ratio was 8.7:1.3. The most common cause was machinery injury in the factory, 98 cases(83.8%), next one was belt injury of the machine, 11 cases(9.4%) and others, 8(6.8%). At least one digital artery and digital nerve were anastomosed under the operating microscope, but vein was impossible to anastomosis as unable to find out in the zone II and III. After anastomosis of one or more digital arteries and nerves, heparine(6,000-10,000 units) was kept to intravenous injection for 24 hours and at the same time fish mouth incision in 2-3 millimeter diameter was made in the distal radial and ulnar margin of the replanted distal phanlanx. From the first 30 minutes to an hour after replantation, incision site was swabbed with heparinized cotton ball for 5 minutes in every 30 to 40 minutes to make sure perfusion for 24 hours, every an hour at the second day, every two hours at the postoperative third to fifth day. Results: 92 cases(78.6%) was completely survived at average postoperative third week follow-up and satisfied with preservation of the finger nail, digit length, good range of motion of the distal interphalangeal joint and acceptable sensibility at average 1.2 years follow-up. Conclusions: Intermittant bleeding method in replantation of crushed distal phalanx impossible to anastomosis of vein at zone II and III of Allen classification was regarded as one of the notable salvage procedure.

작금에 皇龍寺 49萬소斤 巨鐘의 행방이 문제가 되는 것은, 최근 경주시가 그 종을 찾아서, 우리 민족 문화의 국보를 되살리고, 이를 경주 황룡사 종루에 복원하면 그 종소리와 더불어 영원할 것이라는 데 있다. 그리하여 해군에서도 최신 장비를 동원하여 그 종이 빠졌다는 동해구를 뒤지고 있다. 그러나, 그 종은 고려 숙종조에 녹여 져서 신종으로 만들어 졌다는 것이 三國遺事 卷三, 皇龍寺 鐘條에 나와 있다는 것을 발견했다. 그리고, 그것을 기준으로 계산해 보니 皇龍寺 巨鐘: 무게 41.0 ton, 높이 3.14 m, 구경 2.44 m 皇龍寺 新鐘: 무게 7.4 ton, 높이 2.07 m, 구경 1.61 m임이 드러났고 종의 收率(종의 무게/입증)이 聖德大王 神鐘은 34.7 %, 皇龍寺 巨鐘은 13.6 % 임을 알았다. 그리고, 1103년에 皇龍寺 新鐘(7.4 ton)이 원 황룡사 거종이 달려있던 자리에 매달려 있다가, 1238년 11월에, 몽고 제3차 침입군에 의하여 황룡사 전체는 타 버리고, 그 新鐘은 지금의 ④번 國道의 course를 따라 토함산맥 제일 낮은 고개를 넘어, 보문양북면 大鐘川동해구로 끌려가서 동해구에서 몽고행 배에 싣다가 그 곳에 빠뜨렸다고 보여진다. 이번에 잘하면 해군이 7.4 ton의 황룡사 신종을 건질 수 있을런지 모른다. Recently, the search for the whereabout of the huge Bell Imperial-Dragon-Temple becomes a great issue. If it happens to be found out and ringing at the original location of the Bell in Kyungjoo City, the Bell might be a great national treasure and lasting to the eternity with her beautiful sound. The Bell was so huge that the total weight of the raw material put into crucibles was 497,581 Kun (289 tons), the shoulder weight 10.3 Chuk (3.14 m) and the maximum thickness 9 Chon (27.4 cm). The Bell was erected in 754 in Shilla Dynasty and was assumed to be lost during the war time by the 3rd invasion of Mongolians (1235~8). However, the author found out that the huge Bell was recast into a new small Bell (8.1 ton) in 1103 by the people of Koryu Dynasty and then the new small Bell was hung in the same position as in the original huge Bell. 135 years later, the new small Bell was carried out by Mongolian forces as a spoil of war from Kyungjoo to the Bay Tonghaegoo, through the saddle point of Mountain Toham, Yangbuk and Riber Great Bell. At the bay, Mongolian forces wished to bring back the Bell to Mongolia by a ship, but they dropped the Bell into the sea by accident. So, if this was the case, the bell at the seabed may be the new small bell (7.4 ton) but not the original huge Bell (41.0 ton) For the evaluation of missing data of the two bells, the author sets up two equations relating all the dimensions and their weights, which seems to be a useful guide to the design of bells. The results of the evaluation of the Bells are as follows. 유리 측두 근막판을 이용한 수배부 및 족배부 연부조직 결손의 재건

Vascularized tissue coverage is necessary for treatment of soft tissue defect with bone and tendon exposure on hand and foot dorsum, which cannot be successfully covered with simple skin graft or local flap. The temporal fascia is one of the most ideal donor for coverage of soft tissue defect of dorsum of hand or foot in term of ultra-thin, pliable and highly vascular tissue. Also, this flap offers the advantage of a well-concealed donor site in the hair-bearing scalp and smooth tendon gliding. We have experienced 11 cases of reconstruction for soft tissue defect in the hand or foot using temporal fascial flap with skin graft. All cases survived completely and we could gain satisfactory functional results. There were no specific complications except one donor site alopecia We think that the free temporal fascial flap coverage is a highly reliable method for soft tissue defect in hand and foot dorsum. However, the potential pitfalls is secondary alopecia and requirement of skin graft after its transfer.

a simpel coordinate transformaton method is suggested that converts Schr$\"{o}$dinger's equation involving a position-dependent effective mass in a heterostructure to an equation involving a positon-independent effective mass. This method enables the conceptual study of the effect of the positon-dependent effective mass inserted between the divergence operator and the gradient operator in Schr$\"{o}$dinger's equation. It is also shown that the characteristics such as a transmission coefficient in various heterostructures involving a position-dependent effective masses can be obtained iwth ease by the suggested method.
