We report herein a case of acral lentiginous melanoma in a 65-year-old female. An about 2x3cm sized erythematous to brownish mass with central ulceration was located on the left heel accompanied by enlargement of the left inguinal lymph nodes. Histopathologic findings showed lentiginous proliferation of atypical melanocytes with pseudoepithliomatous hyperplasia and invasion of tumor cells from the epidermis into the dermis. Focal pigmented melanocytes were also noted. Complete surgical excision and chemotherapy with DTIC were done. Marked edematous swelling on the left lower leg was developed after 8 months and 1 month later she was expired.

Glutaraldehyde로 처리된 자가 및 동종피부이식에 대한 실험적 연구

Autologous tissue graft, homologous tissue graft or foreign materials are implanted of transplanted to correct facial contour in reconstructive and aesthetic plastic surgery. The autogenous cartilage is most popularly used as an ideal material for ear reconstruction. However it has several disadvantages-limitation of donor site and difficulty in shaping. Both homologous graft and foreign implant permit delicate shaping, but they also show the tendency of foreign body reaction and extrusion. Glutaraldehyde-tanned porcine heterografts have been used extensively as cardiac-valve replacements. They have been well tolerated with minimal toxicity, antigenicity, or material breakdown. Based on these experiences as a support structure in the body. In an experimental study in rabbits, two pieces of skin from each rabbit were obtained, molded, and tanned in glutaraldehyde for getting some degree of stiffness and decreasing antigenicity. And then, these materials were implanted under the dorsal skin of the rabbit, one for autogenous skin and another for homogenous skin. They removed at 2 weeks, 4 weeks, and 8 weeks, and these were examined grossly and microscopically. None of the implants were extruded and infected. Grossly, glutaradehyde-treated implants had some degree of stiffness at initial implantation. Microscopically, all three groups showed a thin densely adherent fibrous capsule around the implant at 8 weeks, and early inflammatory change that subsided later. There were no significant differences between each groups. So these glutaraldehyde-treated skin might be used as substitute for cartilage framework or implant.

The breasts of the women have major cosmetic aspect on the body. In the cases of the abnormal sized and shaped breasts, breast reconstruction or reduction mammoplasty are needed. In these operation, new nipple site and areolar diameter should be determined prior to surgery. The authors measured the physical constitution (height, weight, chest circumference) and the nipple site and areolar diameter in 215 nulliparous women. Statistical analysis of these results shows close correlation between the physical constitutions and the breast measurements. So the authors could induce the equations, which calculate the estimated normal values of nipple site and areolar diameter in relation to body constitutions. The results are as follows; 1. Mean values of nipple site and areolar diameter a. Midclavicular point to nipple 18.37±1.80 cm in Rt. 18.35±1.77 cm in Lt. b. Sternal notch to nipple 18.34±1.86 cm in Rt. 18.44±1.96 cm in Lt. c. Distance between nipples 17.65±1.49 cm d. Areolar diameter 2.80±0.66 cm in Rt. 2.80±0.65 cm in Lt. 2. Regression equations of breat measurements in relation to body constitutions(height and weight) a. Midclavicular point to nipple (cm) = -0.0829 ×height (cm) + 0.2448 ×weight (Kg) + 19.46 b. Sternal notch to nipple (cm) = -0.1086 ×height (cm) + 0.2394 ×weight (Kg) + 23.86 c. Distance between nipples (cm) = -0.0212 ×height (cm) + 0.0499 ×weight (Kg) + 19.53 d. Although recent advance in plastic surgery such as microsurgery, myocutaneous free flap, tissue expander, even pharmacologic control of graft rejection has been achieved, it is projected that skin grafting will stand as this century's most important reconstructive surgical innovation. Full thichkness skin graft was superior to split thickness skin graft in many aspects. But an available amount of full thickness skin was limited. The prepucial skin in circumcision was usually discarded. We tried to use the prepucial skin in skin defect for the prupose of diminishing donor site scar. In this clinical experience, we conclude that the prepucial skin could be used as good donor source for coverage of skin defect especially joint region with minimal scar contracture for it's good elasticity, in spite of color disharmony. The advantages of prepucial skin graft are; 1. no donor site scar 2. good elasticity 3. relatively large amount of available skin (81.2㎠) 4. spontaneous circumcision The disadvantages of prepucial skin graft are; 1. color disharmony 2. limited availability
