In this research, items and contents of SONAR were derived from investigation of OMS/MP of previously-operated submarines and domestic and international references related to weapon system. Such items and contents were established in detail and were categorized into four separate groups after consulting system design experts and specialists in the field of SONAR's operation. Ultimately, based on wartime and peacetime scenario and operation concept of SONAR, RAM values were obtained through various methods using submarine-operating records and maintenance records for the past eleven years.
모즈 미세도식 수술에 의한 기저세포암의 준임상적 침윤에 관한 연구

Background: Mohs micrographic surgery is an ideal method for removing the BCC completely. Several studies defining proper lateral margin for excision of BCC have been reported. However, the study of invasion depth of BCC has not been specially addressed. Objective : In our study, we evaluated the safety lateral margin as well as depth of excision of BCC and the correlation with the histopathological aspects by using Mohs micrographic surgery technique. Methods : Fourty nine patients with BCCs(29 primary, 20 recurrent) diagnosed in the Department of Dermatology of Dong-A University Hospital from March, 1991 through February, 1997 were studied prospectively. Results: 1. Fifty five percent of tumors in our series had 2mm maxumal margins and 70% of recurrent BCC were eradicated with 4-mm more lateral margins. 2. Sixty two percent of BCC located in the cheek was eradicated with the excision within the mid-subcutaneous fat and more than 76% of the tumor in the nose required the removal of the periosteum or perichondrium. 3. In Ninety five percent of BCC, less than 14mm required a 4mm lateral margin and only 5% required a 6mm margin. However, seventy two percent of BCC was eradicated with a 4mm lateral margin and more than 36% was eradicated with a 6-mm or more lateral margin. 4. Eight percent of the micronodular type required 4mm or more lateral margins and had a tendency to invade more deeply. All nodular micronodular-infiltrative BCC had 4mm or more maximal subclinical extension and periosteum/perichondrial or deeper vertical invasion. Conclusion : Mohs micrographic surgery is an appropriate tool to use in proposing guidelines for treatment for BCC and clinicopathological characteristics of BCC such as anatomic location, tumor size, histopathological subtype may be helpful to plan the proper surgical margins. 광역동요법을 통한 피부암 전구증의 치료효과 및 조직학적 고찰

N/A Background : Photodynamic therapy(PDT) is a treatment modality using a photosensitizer, light and oxygen to cause photochemically induced selective cell death. Topical PDT is an effective therapy for nonmalignant skin disease as well as various skin conditions. Objective : The purpose of this study is to evaluate the effectiveness of PDT in the treatment of precancerous disease, and to evaluate the serial histopathologic findings after PDT. Method : PDT using 5-aminolaevulinic acid(ALA) was performed in 6 patients who had been diagnosed with extramammary Paget`s disease, actinic cheilitis, actinic keratosis, Bowen`s disease and arsenic keratosis. Histopathologic changes after PDT were evaluated by serial biopsy at pre-PDT and 2, 24, 48 hours, 7days post-PDT. TUNEL staining for detecting apoptotic cells was also performed. Light was administered at a wavelength of 600~800㎚. The light intensity was 120㎽/㎠ and the light dose ranged from 144J/㎠ to 216J/㎠. Result : Five patients showed a complete response at 1 month after the PDT and continued to show a complete response over six months. One patient showed a partial response and surgical intervention including laser ablation or conventional surgical excision has been needed. Early histopathologia changes after PDT were epidermal necrosis, band like dermal infiltration of inflammatory cells and thrombi formation in venules. At seven days after PDT, there was significant disappearance of abnormal cells in epidermis. Apoptotic cells were detected at 2, 24 hours post-PDT specimen and decreased from 48 hours post-PDT. Conclusion : PDT using 5-ALA is an effective and useful treatment modality for various precancerous skin temors. (Korean J Dermatol 2003;41(5) : 609~616)

Lipoblastoma is a rare, benign tumor arising from embryonic fat tissues that continue to proliferate in the postnatal period. So, lipoblastoma occurs almost exclusively in infants and children younger than 3 years of age. Published reports showed that 70~90% of cases occur before the age of 3. Histopathologically, lipoblastoma, mimicking liposarcoma, was composed of well-defined lobulated fat tissue with multilobulated lipoblasts, undifferentiated mesenchymal cells, such as stellate or spindle cells and minimal myxoid stromas. The diagnosis of lipoblastoma is important because its management differs from that of lipoma and liposarcoma. To our knowledge, there are only a few reports of lipoblastoma in the Korean dermatologic literature. Herein, we present an interesting and atypical case of lipoblastoma in 68-year-old woman. Pyoderma gangrenosum (PG) is a rare, painful, chronic, ulcerating skin disease of an unknown etiology. It can be associated with systemic diseases such as Crohn`s disease, ulcerative colitis, myeloproliferative disease, monoclonal gammopathy, rheumatoid arthritis and Behcet`s disease, or after minor trauma and surgery. To the best our knowledge, no cases of PG associated with pregnancy have ever been published in the Korean medical literature, and there have been only ten such reported cases in other countries. Herein, we report on a case of PG that developed during pregnancy in a patient with rheumatoid arthritis. (Korean J Dermatol 2011;49(4):365∼369)
TP63 유전자 돌연변이와 연관된 Ankyloblepharon, Ectodermal Defects, and Cleft Lip and Palate 증후군

Ankyloblepharon, Ectodermal defects, and Cleft lip and palate (AEC) syndrome, also known as Hay-Wells syndrome, is a rare form of ectodermal dysplasia initially described by Hay and Wells in 1976. It is an autosomal dominant disorder caused by mutations in the gene encoding the TP63 transcription factor, specifically in the Sterile Alpha Motif (SAM) domain. It is characterized by congenital ectodermal dysplasia, including alopecia, scalp infections, dystrophic nails, hypodontia, ankyloblepharon and cleft lip and/or cleft palate. To our knowledge, no case of AEC syndrome associated with a TP63 gene mutation has ever been published in Korea. Herein, we present a Korean female infant with AEC syndrome due to a novel TP63 gene mutation. (Korean J Dermatol 2011;49(3):274∼278)
