In this paper, a novel 1 DOF hand rehabilitation robot is proposed in consideration of ADL training for stroke patients. To perform several ADL trainings, the proposed robot can move the thumb part and the part of 4 fingers simultaneously and realize the full ROM (Range of Motion) in grasp. Based on these characteristics, the proposed robot realizes several types of grasp such as cylindrical grasp, lateral grasp, and pinch grasp by using a passive revolute joint that can change the thumb movement direction. The movement of the thumb is driven by a cable mechanism and the part of 4 fingers is moved by a four-bar linkage mechanism.
발목관절 보조기 착용이 뇌졸중 편마비 환자의 균형에 미치는 영향

The purpose of this study was to investigate the changes of postural control and the effect of ankle foot orthosis (AFO) on the static balance in patients with post-stroke hemiplegia. Fourteen hemiplegic stroke patients with independent ambulation more than 10 m were included. The movement of center of gravity (COG) during static postural control was measured with Biodex Balance System SD<SUP>®</SUP> (Biodex Medical System, New York, USA) at three different conditions (barefoot, shoes and AFO) randomly for three times on same condition. Percent (%) time in quadrant(anterior, posterior, left and right), overall, anterior-posterior and medio-lateral movement index were measured. COG were distributed to the right and posterior quadrant during static standing for maintaining postural stability in patients with post-stroke hemiplegia, and this patterns were not changed with shoes or AFO application. There was tendency of weight shift to the sound side for maintaining postural stability in hemiplegic patients, but not statistically significant. Application of AFO displayed lower overall (p<0.002), anterior-posterior (p<0.008) and medio-lateral instability index scores (p<0.008) than barefoot. The impaired static postural control in patients with post-stroke hemiplegia was improved with AFO application.

뇌졸중이란 뇌혈류 차단으로 산소와 영양공급이 막혀 뇌조직 일부의 손상이 초래되는 질병이다. 뇌졸중에 의한 사망률은 감소하는 추세지만 후유장애, 특히 기능장애로 불편함을 호소하는 환자는 늘어나고 있다. 그러나 뇌졸중 환자를 대상으로 한 연구만 지속될 뿐, 재발 환자를 포함하지 않은 초발 뇌졸중 환자를 대상으로 한 연구는 아직 부족한 실정이다. 이에 본 연구는 한국판 수정 바델 지수(K-MBI)라 불리는 재활 분야에서 흔하게 사용하는 뇌졸중 환자의 일상생활활동 평가 도구를 기능점수의 기준으로 사용해 초발 뇌졸중 기능에 영향을 미치는 요인을 규명하고 궁극적으로 보건의료 정책에 도움을 주고자 했다. 본 연구에서 사용된 자료는 각 개체에 대한 반복적인 측정값으로 이루어진 종단자료로 상관성을 고려하는 분석 방법이 필요해 다수준 분석을 시행하였다. 일반화 추정 방정식 모형(GEE)으로는 모든 변수가 유의하게 나왔지만 다수준 모형은 배우자 유무(미혼보다 기혼), 입원 기간(짧을수록), 나이(적을수록)에 따라 기능점수 회복이 유의미했다(p-value<0.05). 또한, 초기 뇌졸중 중증도에 따른 기능점수의 차이가 있음을 확인했다. 따라서 본 연구는 초발 뇌졸중 환자의 기능향상을 위해서 초기 뇌졸중 중증도에 따라 다른 의료적 접근의 필요성을 제시한다는 점에서 의의가 있다. Stroke is a disease in which oxygen and nutrition are blocked by blocking cerebral blood flow, causing damage to some of the brain tissue. The mortality rate from stroke is decreasing, but the number of patients complaining of discomfort due to aftereffects, especially dysfunction, is increasing. However, only studies on stroke patients continue, and studies on initial stroke patients that do not include recurrence patients are still insufficient. Accordingly, this study tried to identify factors affecting initial stroke function and ultimately help health care policies by using a tool for evaluating daily life activities of stroke patients commonly used in the rehabilitation field called the Korean version of the K-MBI. The data used in this study is longitudinal data consisting of repeated measurements for each object, and a multi-level analysis was conducted because an analysis method that considers correlation was needed. All variables were significant in the generalized estimation equation model (GEE), but the majority model showed significant functional score recovery depending on the presence of a spouse (married than unmarried), hospitalization period (shorter), and age (less); p-value<0.05. In addition, it was confirmed that there was a difference in function scores according to the severity of the initial stroke. Therefore, this study is meaningful in that it presents the need for a different medical approach depending on the severity of the initial stroke to improve the function of patients with initial stroke.

Global effort has resulted in tremendous progress with energy harvesters that extract mechanical energy from ambient sources, convert it to electrical energy, and use it for systems such as wrist watches, mobile electronic devices, wireless sensor nodes, health monitoring, and biosensors. However, harvesting a single energy source only still pauses a great challenge in driving sustainable and maintenance-free monitoring and sensing devices. Over the last few years, research on high-performance mechanical energy harvesters at the micro and nanoscale has been directed toward the development of hybrid devices that either aim to harvest mechanical energy in addition to other types of energies simultaneously or to exploit multiple mechanisms to more effectively harvest mechanical energy. Herein, we appraise the rational designs for multiple energy harvesting, specifically state-of-the-art hybrid mechanical energy harvesters that employ multiple piezoelectric and triboelectric mechanisms to efficiently harvest mechanical energy. We identify the critical material parameters and device design criteria that lead to high-performance hybrid mechanical energy harvesters. Finally, we address the future perspectives and remaining challenges in the field.

Anastomosis between the median and ulnar nerves in the forearm provides variation in the motor innervation of the intrinsic hand muscles and influences the interpretation of electrophysiologic data in normal subjects and patients with traumatic or entrapment lesions of the median or ulnar nerve. Incidence of median-to-ulnar or ulnar-to-median nerve anastomosis was obtained by studying the upper extremities bilaterally in 50 persons with no neuromuscular disease. The median-to-ulnar anastomosis in the forearm was seen in 18 subjects(36.0%) and in 25 limbs(25.0%). The anomalous median nerve axon to the first dorsal interosseous muscle was more common than the hypothenar and thenar muscles, occuring in the first dorsal interosseous 72.0% of the time, hvpothenar 36.0% and thenar 16.0%.
