저자들은 32세 여자가 성교 후 질출혈을 주소로 내원하여 자궁경부에 종물이 발견되었고, 수술 후 조직검사상 소세포암으로 판명된 원발성 자궁경부 소세포암 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Small cell carcinoma of the uterine cervix is a rare malignancy representing less than 5% of all cases of cervical carcinoma. It is clinically characterized by frequent early nodal and distant metastasis. Survival is relatively poor with hysterectomy alone. Currently, there is no consensus for the optimal management of small cell carcinoma of the uterine cervix. Most patients are treated with a multimodality approach, using chemotherapy and radiotherapy. We treated a 32-year-old woman with primary small cell carcinoma of the uterine cervix. The pelvic MR imaging showed a 3 cm mass in the uterine cervix and no evidence of other abnormalities in the adnexa. The patient treated with a radical hysterectomy alone and adjuvant therapy was not provided. Five months later, the patient had recurrent disease with multiple bone and liver metastases. We treated the patient with cisplatin and etoposide combination chemotherapy. Here we report the case of a primary small cell carcinoma of the uterine cervix with rapid progression after surgical treatment alone and review the medical literature. 요추 추간판 탈출증 환자에 대하여 기타 한방치료를 병행한 가열식 화침과 일반침의 효능 비교 연구: 무작위배정, 평가자 맹검, 평행군 설계

Objectives : The purpose of this study is to compare the efficacy of fire needling therapy and general acupuncture for the patient with lumbar herniated intervertebral disc. Methods : We observed 20 lumbar herniated intervertebral disc patients from March, 2014 to October, 2015. This study was randomized and assessor blinded. The patients were divided into two groups: one group(group A) was treated by general acupuncture only, and the other group(group B) was treated by fire needling therapy and general acupuncture. We evaluated the treatment effect experienced by each group using the visual analogue scale(VAS) and Oswestry disability index(ODI). Results : 1. In group A, VAS and ODI from before treatment to treatment 4, 8, 12, 16 were significantly improved in statistics. 2. In group B, VAS from before treatment to treatment 4, 8, 12, 16 was significantly improved, but ODI was significantly improved in statistics only from before treatment to treatment 12, not in treatment 4, 8, 16. 3. As a result of comparisons between group A and B, there were no statistical significance in VAS and ODI. Conclusions : This study was not able to determine statistical significance between the group treated by general acupuncture only and the group treated by fire needling therapy and general acupuncture, so further studies are required in the future.

저자들은 특별한 증상 없이 흉부전산화단층촬영에서 발견된 종격동내 흉선 종양으로 내원하여 수술적 제거 후 추적 검사상 발견된 잔존 종양에 대해 국소 방사선 치료를 시행한 후 현재까지 15개월간 무병 생존을 보인 원발성 흉선 MALT 림프종 1예를 경험 하였기에 문헌고찰과 함께 보고하는 바이다. Primary thymic MALT lymphoma is a rare thymic tumor, with only eight previous cases having been described worldwide to date. We report a case of a 60-year-old Korean woman diagnosed as primary thymic MALT lymphoma. She was found to have an anterior mediastinal tumor during a medical check-up in 2006 and was referred to our hospital for further examination and treatment. The thymus was resected through a median sternotomy and pathology revealed primary thymic MALT lymphoma. Two months later, a follow-up chest CT showed a residual mediastinal soft tissue mass and increased FDG uptake was detected on PET CT scan. The patient was irradiated with 4,140 cGy. After radiation therapy, no evidence of residual soft tissue was found in follow-up chest CT scan and the patient is alive and well 15 months after treatment. We report the details of this case of primary thymic MALT lymphoma treated with irradiation and also offer a review of the literature. (Korean J Med 75:343-348, 2008)

투사형 전도등의 특성을 조사하기 위해 광선 추적법으로 광학계를 분석하고 그 결과를 포물경 전도등과 비교하였다. 포물경 전도등의 경우 광학계가 단순한 장점이 있지만 광원의 일부분 만을 반사하며 실제 광원의 크기와 위치를 고려할 때 높은 수차가 불가피 했다. 투사형 전도등은 여러개의 광학부품 사용하는 복잡함과 전체적 길이가 길어지는 단점이 있지만 포물경 전도등 보다 40%이상의 빛을 더 활용하며 전체적인 수차도 상당히 줄일 수 있었다. 그러나 투사형 전도등 광학계의 성능은 비구면 렌즈의 형태에 매우 민감한 것으로 나타났다. To investigate characteristics of the projection type headlights for automobiles, we used ray tracing methods and compared the results with those from parabolic type headlights. The parabolic type headlights are simple but have two limits: first, they require large apertures. To reflect 50% of a source light, the aperture must be 4 times of focal length and to have more reflected light, apertures must grow geometrically. second, a parabolic mirror have large aberration when a source is away from a focus, since output of a headlight cannot be divergent, a source should locate far side from a mirror and moreover a light source has a finite size, large aberration is unavoidable. The projection type headlights have many optical components and longer depth compared with parabolic types, but their apertures can be made small. Advantage of using a ellipsoidal mirror is to reduce beam divergence by half and displace a focal position to another focal point, so that spherical aberration free and short focal length aspherical lens can be placed in proper position. In our study, we adopt a commercially available aspheric lens and the results were quite good compared with those from parabolic types but not optimal since output experienced total reflection at an aspheric lens. We found to improve performance of headlights, proper design of an aspheric lens was crucial.
