Purposes: This study aims to investigate the changes in the length of stay and medical expenses by the impact of the admission decided by patients or their family. Methodology: The analysis was conducted using data from elder patients aged 65 or older who have used admission services more than once from year 6(2012) to year 12(2017) of the Korean Medical Panel Data(KOWEPS). The statistical analysis was performed through the Chi-square test, mean difference analysis, and generalized estimation equation analysis. Findings: Compared to the patients who used admission services decided by doctors, those who used admission services decided by patients or their family had a longer length of stay and a lower daily medical expense. Practical Implications: Inappropriate admission decided by patients or their family can increase the risk of hospital-acquired infections, impairing the health of elderly patients, and has a negative impact on the efficiency of health resources. Purposes: The purpose of this study is to investigate the factors which affect the promotion of the employees working at university hospitals in Korea. Methodology: The data of this study were obtained from personnel management database of 1,634 employees working at 4 university hospitals owned by an education foundation located in the City of Seoul and Kyoung-kee Province. The collected data were analyzed using frequency analysis, correlation analysis and probit regression analysis using SAS 9.4. Findings: The main findings of the study were as follows: First, appraisal score of the employees was the most important factors affect the possibility of the promotion and the following variables of tenure, higher degree, number of transfers, managers were also found to have significant effect on the possibility of the promotion. Second, Impact of those are varied among promotion ladders and the occupations like nurses, administrative staff, and technicians, which revealed that the important competency needed for the promotion was different. Practical Implications: The study results imply that, in order to motivate the employees through the promotion system at university hospitals, it is strongly recommended to use different criteria for evaluating the employees’ performance and qualification for the promotion in terms of the promotion ladder and the characteristics of each occupation.
보훈병원 통합의료복지서비스가 의료이용과 의료비 절감에 미치는 효과 분석

This study aims to examine the effect of integrated health and welfare(IHW) services on medical use and medical expenses in Korean Veteran Hospitals. Data on the volume of medical use and medical expenses were collected from two patient groups of 5 Veterans' Hospitals: the beneficiary group who has received IHW services and the non-beneficiary group who has not. Each group was composed of 265 patients respectively. The results of the study revealed that the volume of home nursing services, home rehabilitation services, ambulatory services have increased significantly, whereas the average length of stay, number of using emergency services, and average medical expenses have decreased after receiving integrated health services in the beneficiary group. In contrast, the non-beneficiary group showed much more increase both in the volume of inpatient and outpatient services, as well as the average medical expenses, compared to those of the beneficiary group. This results imply that the provision of IHW services have positive impacts on the enhancing the effectiveness of the medical resources utilization for the veteran patients.
15세 이하 입원 진료비 본인부담 경감 정책이 평균재원일수 및 입원 진료비 변화에 미친 영향 분석

Purpose: This study aims to examine the effect of the copayment reduction policy on the health care utilization of patients under age 15 after the policy started in 2017. (이하는 아래 methodoloty로 이동) Methodology: Data on the ALOS, the average admission cost, and the out-of-pocket expenditure for patients under 15 years of age from 2015 to 2019 were obtained from the National Health Insurance database. Policy effects were measured by analyzing three dependent variables before and after policy: the average length of stay (ALOS), the average admission cost, and the out-of-pocket expenditure for patients under 15 years of age. The collected data were analyzed using the SAS package, and the analysis methods used in this study were the mean difference test and linear regression analysis. Findings: The study results reveal that, after the copayment reduction policy in the year 2017, the ALOS and the out-of-pocket expenditure were significantly decreased, but the average admission cost was significantly increased. Practical Implications: These results imply that the policy of copayment reduction for the patients under the age of 15 has contributed to mitigating the patients' financial burden with little concern about growing medical utilization.
병원간호사의 조직냉소주의 영향요인 및 조직효과성에 대한 매개효과 검증

본 연구는 병원에서 간호사의 조직 냉소주의에 대한 영향요인을 조사하고 효과적인 조직 관리 및 조직의 효율성에 기여하는 요인을 알아보기 위해 수행되었다. 연구에 사용된 데이터는 2005년 일개 병원에 근무하는 간호사 전체 320명을 대상으로 자료를 수입하여 분석에 부적합한 설문지를 제외하고 최종 295개 설문지를 SPSS WIN 21.0 버전을 활용하여 분석 하였다. 연구결과, 리더십, 과업 상호 의존성, 직무 스트레스, 긍정적 및 부정적인 정서는 병원 간호사의 조직 냉소주의에 통계적으로 유의한 영향을 미치는 것으로 나타났다. 또한, 조직 냉소주의는 직무 만족과 조직몰입에 유의한 매개 효과가 있었고, 조직의 분위기는 직무만족과 조직 몰입에 유의한 간접 효과를 보여 주었다. 결론적으로 병원경영진과 간호관리자는 조직 냉소주의를 감소시키기 위해서 즐거운 조직 분위기 조성, 적절한 지원, 공정한 보상 및 우호적인 조직 문화 조성, 원활한 의사소통과 피드백, 직원들간의 신뢰, 정보공유, 부서 내 및 부서간의 협력과 같은 전략을 수용할 필요가 있다. The purpose of this study was to investigate the influential factors of organizational cynicism of nurses in hospitals, and to examine data contributing the effective organizational management and the organizational effectiveness. Data were collected from 320 nurses working in a hospital in 2005. A total of 295 questionnaires were analyzed by the SPSS WIN 21.0. The leadership, job interdependence, job stress, positive and negative affectivity had significant effect on the organizational cynicism of hospital nurses. Also, the organizational cynicism had significant mediating effects on job satisfaction and organization commitment, and organizational climate showed significant indirect effects on the job satisfaction and organizational commitment. In conclusion, hospital managers and nursing directors need to adopt following strategies; open communication, friendly feedback, trust among staff, co-operation between departments and intra-departments, sharing information, creating joyful organizational climate, adequate organizational supports, fair compensation, and favorable organizational culture.

IgG4-related disease (IgG4-RD) is a new disease entity characterized by elevated serum IgG4 and infiltration of IgG4+ plasma cells in tissue. IgG4-RD can involve various organs, and skin could also be affected. The manifestations of IgG4-related skin disease (IgG4-RSD) are not characteristic, however it usually presents with multiple erythematous nodules or plaques with itching sensation. We report two cases of IgG4-RSD. Histological studies of these cases revealed infiltration of numerous plasma cells and mononuclear cells in the entire dermis. Some plasma cells were strongly positive for IgG4 stain. IgG4-RSD is a treatable disease with systemic corticosteroids. Thus, clinicians should obtain a biopsy and identify IgG4-positive cells for an accurate diagnosis. (Korean J Dermatol 2018;56(3):197∼201)
