Kyung-p-1 main line is characterized by its curves radii of which are considerably small. It is essential for running time reduction of train to improve capabilities of curve negotiation. This improvement can be achieved by designing a bogie with flexible suspension system. The effect of the improvement is mainly concerned in the primary yaw stiffness of bogie suspension. This paper gives a linear analysis for the motion of railway vehicle on curved track and gives also computer simulation results for Semaul Train. The results introduce a conclusion that the primary yaw stiffness of Semaul train is too rigid to be self-steering on Kyung-pu main line curves.

Dynamics of railway vehicles are strongly influenced by the wheel/rail contact forces. Wheel/rail contact geometric characteristics are important parameters to determining wheel/rail contact forces. In general, geometric relations between wheel and rail are represented by nonlinear functions of the wheelset lateral excursion and the relative yaw angle. There are some analytical and experimental studies to show the influences of the wheelset lateral displacement on wheel/rail geometric relations. Recently radial steering bogie which is designed to have flexible yaw motions of wheelsets was developed to improve curve negotiation performance. The radial steering bogie makes it important problem to study the effects of wheelset yaw motion on wheel/rail geometric relations. This paper describes the method to analyze 3-dimensional wheel/rail contact geometry considering wheelset yaw motion and describes also some computer simulation results.

A trauma surgeon is always concerned about child abuse when he or she meets injured children. Abused children will be neglected if trauma surgeons only concentrate on the injured site or physical dynamics. Lately, violence on children has increased in Korea. Therefore, in this study, we considered child abuse through a review of the literatures. An eleven-year-old boy visited the emergency room vomiting with abdominal distension. He had been kicked in the abdomen by his step-mother 10 days earlier. The computed tomography revealed a transected pancreas tail and neck with a large pesudocyst (Fig. 1) and laboratory findings showed an elevated amylase level of more than 6,500 IU/L. Because he complained of severe pain with rebound tenderness on the whole abdomen, he underwent an emergent laparotomy, a distal pancreatectomy of the tail portion with an anti-leakage procedure on the cut surface of the pancreas. However, he underwent a distal pancreatectomy again on the neck portion of the pancreas because of a continuing pseudocyst with severe pain that could not be controlled with conservative managements. After that, his symptoms were improved and he returned to his daily life. (J Trauma Inj 2012;25:283-286)

Purpose : Postoperative cerebrospinal fluid (CSF) leakage after neurosurgery could lead to potentially lethal complications. Reoperations, multiple operations, cranial radiotherapy and poor vascularization of the dura graft may lead to delayed healing of the suture site and resultant persistent CSF leaks. In this report, we present a technical experience of duroplasty with omental free flap. Methods : Between 2007 and 2011, the authors performed duraplasty in nine patients with recurrent subdural empyema and intracranial infection. The omental free flap transfer was intended for patients who could not be cured with conventional surgical debridement and lyophilized dura appliance. Patient ages ranged from 14 to 47 years (mean, 31.5 yr). There were five men and four women. Treatment was required for malignant tumors (n=4), cerebral hematomas (n=3) and the result of traumas (n=2). After harvesting the omentum laparoscopically, the flap was sutured to the dural defect in a watertight fashion. The most commonly used recipient vessels were the superficial temporal artery and vein. Results : The postoperative course was uneventful and flap survival was excellent in all patients. Cerebrospinal fluid leaks were controlled successfully. Patients were discharged with no evidence of wound discharge and there were no reports of infection or CSF leakage recurrence. Conclusion : Duraplasty using omental flap is an effective technique to control CSF leaks, especially when dura is poorly vascularized and less viable. The delicate nature and great pliability of the omental flap make it unique and competent for reconstructions of dural defects.
성인 입원 환자에서의 영양치료 관련 합병증: 국내 다기관 연구

Purpose: Nutritional therapy (NT), such as enteral nutrition (EN) or parenteral nutrition (PN), is essential for the malnourished patients. Although the complications related to NT has been well described, multicenter data on symptoms in the patients with receiving NT during hospitalization are still lacking. Methods: Nutrition support team (NST) consultations, on which NT-related complications were described, were collected retrospectively for one year. The inclusion criteria were patients who were (1) older than 18 years, (2) hospitalized, and (3) receiving EN or PN at the time of NST consultation. The patients’ demographics (age, sex, body mass index [BMI]), type of NT and type of complication were collected. To compare the severity of each complication, the intensive care unit (ICU) admission, hospital stay, and type of discharge were also collected. Results: A total of 14,600 NT-related complications were collected from 13,418 cases from 27 hospitals in Korea. The mean age and BMI were 65.4 years and 21.8 kg/m<sup>2</sup>. The complications according to the type of NT, calorie deficiency (32.4%, n=1,229) and diarrhea (21.6%, n=820) were most common in EN. Similarly, calorie deficiency (56.8%, n=4,030) and GI problem except for diarrhea (8.6%, n=611) were most common in PN. Regarding the clinical outcomes, 18.7% (n=2,158) finally expired, 58.1% (n=7,027) were admitted to ICU, and the mean hospital days after NT-related complication were 31.3 days. Volume overload (odds ratio [OR]=3.48) and renal abnormality (OR=2.50) were closely associated with hospital death; hyperammonemia (OR=3.09) and renal abnormality (OR=2.77) were associated with ICU admission; “micronutrient and vitamin deficiency” (geometric mean [GM]=2.23) and volume overload (GM=1.61) were associated with a longer hospital stay. Conclusion: NT may induce or be associated with several complications, and some of them may seriously affect the patient’s outcome. NST personnel in each hospital should be aware of each problem during nutritional support.

The characteristics of unsteady pipe flow to an external impact such as valve modulation can be detected in a frequency domain. Fast Fourier Transformation is employed to convert the head variation in the time domain to the amplitude of the frequency domain. Application of the method of characteristics to a pipeline provides a range of frequency representing a particular unsteady condition. Inverse Fast Fourier Transformation and a Finite Impulse Response Filter can be used to remove noise present in the frequency range. Characteristics of the frequency range for various conditions such as valve modulation and a minor modification of a pipeline are explored to demonstrate the effectiveness of the noise control method in the frequency domain. A noise contaminated signal was successfully filtered using a low pass filter, which was designed from a frequency analysis of head variation.
