Clinical Approaches to Vascular Anomalies of the Lip

Background The incidence of vascular anomalies in the head and neck is higher than in the extremities. It is especially common for vascular anomalies to involve the lip. The lips are a functionally and aesthetically important component of the head and neck area. A retrospective analysis of data from our vascular anomaly center was performed in order to understand the characteristics and treatment requirements of vascular anomalies of the lip and to establish which treatments are likely to lead to the best outcomes. Methods A retrospective review was performed of the medical records of patients diagnosed with vascular anomalies of the upper or lower lip from January 2001 to September 2013. Using clinical photographs, radiologic findings, and patient records, the diagnosis of each case and the location of the vascular anomaly were recorded, along with information about treatment and follow-up. Results A total of 1,606 patients were diagnosed with vascular anomalies over this time period, of whom 127 (7.9%) were found to have vascular anomalies in the lip only. Surgical treatment with or without embolization, sclerotherapy, laser therapy, medication only, and observation were the treatment strategies adopted in these cases. Conclusions Vascular anomalies of the lip should be diagnosed accurately. Radiologic diagnosis played a crucial role in treatment planning, and several techniques were used to treat vascular anomalies of the lip. When surgical excision is indicated for the correction of vascular anomalies of the lip, the aesthetic and functional characteristics of the lip should be considered.
Our Experiences in Nipple Reconstruction Using the Hammond flap

Background: Nipple reconstruction following breast mound reconstruction is the final step inbreast reconstruction. Although nipple reconstruction is a simple surgery, the psychologicalaspects of nipple reconstruction are thought to be important. Nipple projection is a key factorin determining patient satisfaction with the surgery. In the present study, the Hammond flaptechnique was introduced for nipple reconstruction. Methods: Twenty-six patients who had undergonebreast reconstruction from February 2008to March 2012 were enrolled in this prospectivestudy. All patients were evaluated based onpreoperative photos, and their nipple diametersand heights were measured. Postoperativeevaluation was conducted 3, 6, 9, and 12 months following nipple reconstruction. Aquestionnaire on patient satisfaction with the nipple reconstruction was administered 12months after nipple reconstruction. Moreover, the same plastic surgeon scored nippleprojection and overall cosmetic result of the new nipple. Results: The mean projection was 4.4 mm (range, 3–6 mm), and it well matched the contralateralnipple. Twelve months following nipple reconstruction, the mean reduction rate inthe nipple projection was 43.6%. Patients were satisfied or very satisfied with the nippleprojection and the overall cosmetic result in 80.7% cases. Conclusions: In the present study, compared with other techniques, the use of the Hammondflap technique in nipple reconstruction showed competitive results with regard to nippleprojection and patient satisfaction.
양흡입 원심펌프의 내부 표면 거칠기 개선에 따른 에너지 절감 실험 연구

본 연구에서는 유량 70.7㎥/min, 양정 87m의 양흡입 원심펌프의 수력 손실 절감을 위해 표면 마찰 거칠기를 변화시킨 실험을 수행하였다. 초기 표면 거칠기를 100~110㎛에서 0 ~ 0.08㎛로 변경한 후 표면 거칠기 변화가 전체 펌프의 효율 증가 및 펌프 운전시 소요되는 전력 원단위 변화 측정을 수행하였다. 표면 마찰 거칠기 개선 부위는 물과 접하는 펌프 내부 임펠러와 케이싱 표면으로 하였고 코팅 방식을 적용하였다. 그 결과 펌프 효율은 약 0.8~1.79% 증가하였으며, 전력원단위는 4.38 ~ 6.04% 절감 효과를 가져 왔다. 이와 같은 결과는 임펠러 및 내부 케이싱의 표면 거칠기가 펌프의 성능에 작지 않은 영향을 미칠 뿐만 아니라 표면 거칠기 개선을 통해 마찰에 의한 수력 손실을 줄여서 펌프 성능 향상과 상당한 에너지의 절감 효과를 얻을 수 있음을 확인하였다. In this study, we test the effects of the surface roughness on the hydro efficiency of double-suction centrifugal pumps (Q = 70.7㎥/min, H = 87 m). The original surface of the impeller and inner casing were coated, resulting in surface-roughness reductions ranging from 100~110 ㎛ to 0~0.08 ㎛. We conducted experimental studies to measure the efficiency of the pumps and operating-pump electro energy variations with different surface roughness values. The experimental results showed that the efficiency of the pumps increased by about 0.8~1.79% and the electric energy of the operating pump was reduced by around 4.38 ~ 6.08%. These results indicate that the performance of the pumps depend largely on the surface roughness of the impeller and inner casing, and by reducing the surface roughness, we can improve the efficiency of the pump and reduce costs by reducing the electric energy consumption.
