A non-invasive respiratory gated radiotherapy system like those based on external anatomic motion gives better comfortableness to patients than invasive system on treatment. However, higher correlation between the external and internal anatomic motion is required to increase the effectiveness of non-invasive respiratory gated radiotherapy. Both of invasive and non-invasive methods need to track the internal anatomy with the higher precision and rapid response. Especially, the non-invasive method has more difficulty to track the target position successively because of using only image processing. So we developed the system to track the motion for a non-invasive respiratory gated system to accurately find the dynamic position of internal structures such as the diaphragm and tumor. The respiratory organ motion tracking apparatus consists of an image capture board, a fluoroscopy system and a processing computer. After the image board grabs the motion of internal anatomy through the fluoroscopy system, the computer acquires the organ motion tracking data by image processing without any additional physical markers. The patients breathe freely without any forced breath control and coaching, when this experiment was performed. The developed pattern-recognition software could extract the target motion signal in real-time from the acquired fluoroscopic images. The range of mean deviations between the real and acquired target positions was measured for some sample structures in an anatomical model phantom. The mean and max deviation between the real and acquired positions were less than 1mm and 2mm respectively with the standardized movement using a moving stage and an anatomical model phantom. Under the real human body, the mean and maximum distance of the peak to trough was measured 23.5mm and 55.1mm respectively for 13 patients' diaphragm motion. The acquired respiration profile showed that human expiration period was longer than the inspiration period. The above results could be applied to respiratory-gated radiotherapy.
제 1-2 경추간 아탈구에 의한 경추 척수증에 시행한 분절 나사 고정술을 이용한 간접 감압술

제 1-2 경추간 아탈구로 인한 심한 척수 압박과 척수증이 있는 경우, 수술의 목적은 척수를 감압하고 견고한 내고정 및 유합을 얻는 것이다. 척수를 압박하는 골조직을 절제하는 직접 감압술에는 경구 감압술과 제 1 경추 후궁 절제술이 있으나, 이들은 합병증 발생률이 높거나 골 유합을 얻기 힘들다는 단점이 있다. 간접 감압은 골조직 절제 없이 아탈구만을 정복하여 내고정함으로써 척수를 감압하는 것으로, 심한 척수 압박으로 인한 척수증에 대한 간접 감압은 국내에서는 보고된 바 없다. 저자들은 제 1-2 경추간 아탈구로 인한 심한 척수증에 대해 제 1-2 경추간 분절 나사 고정술을 이용한 간접 감압술을 시행하여 만족할 만한 정복 및 감압과 척수증의 호전을 얻었기에 이를 보고하고자 한다. The main aims of surgery for severe cord compression and myelopathy caused by atlantoaxial subluxation are decompression of the spinal cord and achievement of rigid fixation and fusion. Direct decompression by resecting the bony structures that compress the spinal cord includes transoral decompression and resection of the posterior arch of the atlas. The shortcomings of these procedures are a high complication rate and a relatively low rate of union. Indirect decompression can be performed by a reduction of the subluxation and fixation without bone resection. To the best of our knowledge, there are no domestic reports on the use of indirect decompression for severe cord compression and myelopathy for atlantoaxial subluxation. We report a case of a patient that had atlantoaxial subluxation and severe myelopathy; satisfactory reduction of the subluxation and decompression with an improvement in the myelopathy symptoms was achieved by indirect decompression using segmental screw fixation.

In this study, 7 dof (Including warping) beam element was developed to estimate the effects of wagner effects and load height effects on the lateral buckling strength of mono-symmetric Ⅰ beam. Finite element buckling analysis of mono-symmetric Ⅰ-shaped girders subjected to transverse loading applied at different heights on the cross-section were conducted. Linear moment gradient were considered, too. In these cases, girders are subjected to both single-curvature and Reverse-curvature bending. An applicability of current LRFD C<SUB>b</SUB> on the mono-symmetric Ⅰ beam was studied from the finite element results. The problems of current LRFD C<SUB>b</SUB> occurring from load height effects and reverse curvature bending in unbraced length when applied on the mono-symmetric Ⅰ beam were studied. Solutions to these problems are also presented.
Caillois의 사회.문화적 관점에서의 놀이의 교육적 기능

이 연구는 Caillois의 사회·문화적 관점에서의 놀이의 교육적 기능을 구명(究明)하는데 목적이 있다. 이를 위하여 놀이이론의 특징과 Caillois의 사회·문화적 관점에서의 놀이의 특징을 고찰한 다음, 이에 근거하여 놀이의 교육적 기능을 구명하였다. Caillois는 놀이를 문화 그 자체라고 강조한 Huizinga의 관점을 비판적으로 계승하여 이를 더욱 발전시켰다. Caillois는 놀이를 자유로운 활동, 분리된 활동, 확정되어 있지 않은 활동, 비생산적인 활동, 규칙이 있는 활동, 허구적인 활동으로 정의하고, 경쟁, 우연, 모의, 현기증이라는 네 개의 역할 중 어느 것이 우위를 차지하는가에 따라서 놀이를 아곤, 알레아, 미미크리, 일링크스 등으로 구분하였다. 이러한 Caillois의 놀이구분은 놀이 각각의 특징에 따라 이를 체계적으로 구분한 것으로, 그 활용도가 매우 높다고 할 수 있다. Caillois의 사회·문화적 관점에서 놀이는 문화의 한 현상으로서, 어린이들의 전인적인 성장에 도움을 주며, 특히 정서발달과 사회성 발달, 문화형성 및 신체발달에 도움을 준다. This study aims to examine the educational function of play through analyzing aspects and characteristics of play in Caillois’ socio-cultural perspectives, and to investigate into a new direction of study on Korean play. Caillois critically succeeds to Huizinga who insists that play is culture itself and develops it. He also defines play as a free, separate, uncertain, unproductive, governed by rules, and make-believe activity. Caillois categorizes play as Agôn, Alea, Mimicry, or Illinx as based on whether the role of competition, chance, simulation, or vertiea, is dominant. This devision of play in Caillois systematically divided into each play's distinctive features and that has high applications. Play in Caillois’ socio-cultural perspectives is a culture phenomenon, and can be educationally helpful to growth of for the whole man, especially improvement of emotion and Sociality, growth of cultural and physical condition.
굴곡구축을 동반한 슬관절에서 후방 십자 인대 보존형 슬관절 전치환술의 결과 - 후방 안정형과의 비교 -

Purpose : To evaluate the clinical results of cruciate retaining type total knee arthroplasty in patients with severe flexion deformity by comparison with that of posterior stabilized type. Materials and Methods : From April 1994 to June 2000, 73 cases(63 patients) with preoperative flexion deformity over 15 degrees who underwent cruciate retaining type were categorized as group Ⅰ. 16 cases(16 patients) who underwent posterior stabilized type were categorized as group Ⅱ. We analyzed clinical results according to residual flexion deformity and knee functional score. Results : There was no statistically significant difference in residual flexion deformity(Ⅰ: 4.3±6.5 degree, Ⅱ: 4.5±2.8 degree), HSS knee score(Ⅰ: 94.9±5.1, Ⅱ: 93.0±5.4), TKSCRS score(Ⅰ: 162.0±13.0, Ⅱ: 163.7±12.2) between two groups(p>0.05). Conclusion : Usually in severe flexion deformity patients, posterior stabilized design has been used, but our results show no significant statistical difference between cruciate retaining type and posterior stabilized type.

슬관절 전 치환술 후 발생하는 자발성 혈관절증은 매우 드문 합병증으로 알려져 있고 국내에는 아직 보고된 바 없다. 저자들은 퇴행성 관절염으로 슬관절 전 치환술을 시행 받은 60세 여자 환자에서 수술 후 10개월에 발생한 자발성 혈관절증에 대해서 관혈적 활액막 제거술로 치유된 1예를 경험하였기에 보고하고자 한다. Spontaneous hemarthrosis of the knee after a total knee arthroplasty is a rare complication, and there is no report is available in Korea. We report a case that was cured with an open synovectomy in the spontaneous hemarthrosis, which developed 10 months after the total knee arthroplasty for degenerative osteoarthritis in a 60-year-old woman.
