        소아형 만성 골수성 백혈병과 신경섬유종증을 동반한 연소성 황색육아종

        Juvenile xanthogranuloma(JXG) is a a benign histiocytic proliferative disorder most frequently seen in infants. The usual course is self-limited, but it may be a manifestation of juvenile chronic myeloid leukemia(JCML). In addition to JXG, some patient with JCML may have multiple cafe-au-lait spots and a family history of neurofibromatosis(NF). The association of JXG and cafe-au-lait spots in patients with JCML is of interest in light of description of otherwise healthy children with JXG who also had cafe-au-lait spots. We report herein a case of JXG in 18-month-old boy who also had multiple cafe-au-alit spots, which predated the JCML.

      • 母乳內 Amylase含量에 關한 硏究

        It is well documented that human milk contains amylase but not in cow’s milk. Besides the level of amylase activity in saliva and in pancreatic juice is very low at birth, but increases during the first month of life. Present study was carried out to determined the amylase activity in breast milk from various stages of lactation and to compared with that of duodenal juice from infant. The effect of low pH on human milk amylase activity and contribution of human milk amylase activity to total amylase activity in duodenal juice were also studied. Total 201 samples of milk were obtained from 109 mothers during the March to July 1983 and results were as follows. 1) amylase activity was high in colostrums and somewhat lower in milk from day 14 to day 30 after delivery. In this period of lactation, breast milk contained higher amounts of amylase than duodenal juice from infants aged 1 to 6 months. 2) Low amylase activity was found in milk from 4 month or more after delivery. 3) A pH of 5.3 does not inactivate the amylase and there was considerable breast milk amylase activity in duodenal juice after a meal of breast milk. Breast milk amylase could thus contribute to the breast-fed infant’s ability to digest starch.

        백서에서 일측성 요관 폐쇄가 PDGF 발현에 미치는 영향

        Unilateral ureteral obstruction(UUO) results in severe renal vascular constriction through the activation of renin-angiotensin system, which causes progressive tubulointerstitial fibrosis. Platelet-derived growth factor(PDGF) plays an important role in stimulating myofibroblasts and regulating synthesis of extracellular matrix in renal interstitial proliferation and fibrosis. This study was designed to investigate the relationship between unilateral ureteral obstruction and PDGF expression in tubulointerstitial fibrosis of the kidney. Eleven adult male Spraugue-Dawley rats were carried out unilateral ureteral ligation and sham operation. After 14 days, control kidney, UUO kidney and intact opposite(IO) kidney were harvested. Tissue fibrosis was quantified morphologically using the point detection method after Masson-Trichrome stain. Expression of PDGF-A and B was determined by immunohistochemical staining, RT-PCR and Western blot assay. Results were as follows: 1) UUO and IO group resulted in reduced kidney weight compared with control group(p$lt;0.05). 2) Collagen deposition was increased in the renal cortex of UUO group(p$lt;0.05). 3) PDGF-A and B mRNA expression was increased significantly compared with control and IO group(p$lt; 0.05). 4) PDGF-A and B protein expression were increased in UUO and IO group(p$lt;0.05). 5) On the immunohistochemical staining for PDGF-A and B, staining intensity was increased significantly at the renal cortex, interstitium and tubular epithelial cells in the UUO group. This results indicated that PDGF-A and B plays important role in tubulointerstitial fibrosis developed after unilateral ureteral obstruction and compensatory fibroproliferative growth in contralateral kidney.
