Squamous cell carcinoma of the external auditory canal is a rare tumor and they can easily invade adjacent organs at an early stage. And the etiology is uncertain. Radiation treatment of nasopharyngeal carcinoma may induce malignancy in the external ear. Because squamous cell carcinoma of the temporal bone and external auditory canal is so rare, developing an adequate tumor staging and treatment has been difficult. We present a case of squamous cell carcinoma of the external auditory canal in 58-year-old female who presented with a 7-year history of bilateral otorrhea after radiation treatment to nasopharyngeal carcinoma 12 years ago.
간호대학생의 좋은 죽음 인식, 생명의료 윤리의식이 사전의료의향서 태도에 미치는 영향

본 연구는 간호대학생의 좋은 죽음 인식, 생명의료 윤리의식, 사전의료의향서 태도의 차이를 파악하고, 사전의료의향서 태도에 영향을 미치는 요인을 규명하기 위해 실시되었다. 연구 대상자는 전북지역에 소재한 3개 대학 간호학과에 재학 중인 대학생을 대상으로 연구목적과 방법을 설명한 후 연구에 동의한 128명에게 2022년 6월 1부터 2022년 7월 31일까지 자료수집을 하였다. 수집된 자료는 SPSS/WIN 20.0 프로그램을 이용하여 기술통계, independent t-test, one-way ANOVA, pearsons correlation, multiple regression으로 분석하였다. 연구 결과 대상자의 사전의료의향서 태도에 영향을 미치는 요인은 생명의료 윤리의식(β=.36, p <.001), 좋은 죽음 인식(β=.18, p =.032)순서이며, 사전의료의향서 태도를 설명하는 설명력은 18.0%으로 나타났다. 좋은 죽음 인식은 생명의료 윤리의식, 사전의료의향서 태도와 정적 상관관계이고 생명의료 윤리의식은 사전의료의향서 태도와 정적 상관관계로 나타났다. 연구 결과 간호대학생의 생명의료 윤리의식과 좋은 죽음 인식은 사전의료의향서 태도의 영향요인으로 확인되었다. 따라서 예비 간호사인 간호대학생에게 좋은 죽음 인식과 생명의료 윤리의식을 향상시키기 위해서는 교육과정에서부터 교과 및 비교과 과정을 통한 교육과 호스피스 관련 교육 프로그램 개발이 필요하다. This study was conducted to identify the differences between good death perception, biomedical ethics awareness, and attitudes toward advance directives among nursing students, and to identify factors affecting attitudes toward advance directives. The target population was composed of college students attending the nursing departments of three universities in Jeollabuk-do Province. Data were collected from June 1, 2022, to July 31, 2022, from 128 students who agreed to participate in the study after receiving an explanation of the studys purposes and methods. Collected data were analyzed using descriptive statistics, the independent t-test, one-way ANOVA, Pearsons correlation coefficients, and multiple regression in SPSS/WIN 20.0. Factors found to influence attitudes toward advance directives in order were biomedical ethics awareness (β=.36, p<.001) and good death perception (β=.18, p=.032), and the explanatory power of attitude towards advance directives was 18.0%. Good death perception was positively correlated with biomedical ethics awareness and attitude toward advance directives, and biomedical ethics awareness was positively correlated with attitude towards advance directives. The study shows that good death perception and awareness of biomedical ethics influence the attitudes of nursing students to advance directives. Thus, a curriculum is required based on curricular and extra-curricular courses and hospice-related educational programs to improve good death perception and biomedical ethics awareness among nursing students.
기하학적 비선형을 고려한 얕은 스페이스 트러스 구조물의 동적 좌굴 거동 분석

Shell-like space truss structure system has advantage that large space structure. Destruction mechanism occurs due to unstable behaviors with snapping by snap-through and bifurcation. Therefore Structural instability has important problem. In this study, direct and indirect snapping of shallow space truss are numerically investigated by the finite element analysis considering geometrical nonlinearity under the up-and-down excitation, and compared with the static critical load. The dynamic critical loads are determined by the numerical integration of the geometrical nonlinear equation of motion, and the mechanisms of the direct and the indirect snapping are examined by using the backbone curves and displacement response curves, time history curves, phases curves, response spectrums. Finally, the influence of damping to the dynamic buckling is discussed by comparing with the results under the step, the sinusoidal excitations.
상용치료원 보유가 의료기관 종별 선택에 미치는 영향: 대형병원 환자집중현상 완화방안을 중심으로

Background: Concentration of patients to large hospitals is serious problem in Korea. The purpose of this paper is to propose appropriate policy direction to relieve concentration of patients to large hospitals. It is focused on evaluation of the possibility of family doctor system as a policy alternative to relieve concentration of patients to large hospital by empirically analyzing the effect of usual source of care (USC) on large hospitals medical care use. Methods: Korea Health Panel conducted 2009, 2012, 2013 by KIHASA (Korea Institute for Health and Social Affairs) and NHIS (National Health Insurance Service) was used for analysis. For dependent variables, first, the ratio of the amount of using large hospital to total amount of using medical care, and second, the amount of using large hospital are estimated. Independent variables are having an USC and type of USC. Panel analysis was done with above variables. Results: Main results are as follows. First, having an USC increases using large hospital. Second, having a domestic clinic type USC decreases using large hospital and ratio of using large hospital. Third, the effect of domestic clinic type USC is greater in older group, less income group, worse health status group, not having private insurance group, and having chronic disease group. Conclusion: These results show that family doctor program can be a policy alternative to relieve concentration of patients to large hospital. Nonetheless, primary care system in Korea is unsatisfied. It is recommended to reinforce primary care system and family doctor system to relieve concentration of patients to large hospitals.
정현파 하중을 받는 EP(Elliptic Paraboliodal)쉘 구조물의 동적 불안정 특성 분석

동적 불안정 좌굴현상에 관한 연구는 다소 발표되고 있으나 주기성을 가진 히중하에서의 동적 좌굴을 다룬 연구는 그리 많지 않은 편이다. 주기성을 가진 하중하에서의 거동은 스텝 하중하에서 거동과는 다르리라 예상된다. 본 논문에서는 동적 불안정의 기본 메커니즘을 파악하기 위하여 2자유도의 얕은 EP(Elliptic Paraboliodal)쉘이 정현파 하중을 받았을 때의 직접좌굴과 간접좌굴 현상을 조사한다. Newma가-$\beta$법에 의한 수치적분을 이용하여 비선형 운동 방정식의 변위응답을 구하고 얻어진 비선형 변위응답으로 위상곡면선의 끌개를 비교하고, FFT(Fast Fourier Transform)에 의한 연속 응답 스펙트럼을 구해 동적 불안정 특성에 관해서 분석한다. 그 결과 동적 좌굴하중은 구조물의 고유주기와 외력 탁월진동수와의 관계에 크게 영향을 받는다. The dynamic instability for snapping phenomena has been studied by many researchers. Few paper deal with the dynamic bucking under the load with periodic characteristics, and the behavior under periodic excitation is expected the different behavior against STEP excitation. We investigate the fundamental mechanisms of dynamic instability when shallow EP(Elliptic Paraboliodal) shell of two degree of freedom are subjected to sinusoidal excitation with direct snapping and indirect snapping. By using Newmark-$\beta$ method, we can get the nonlinear response, and characteristics of the dynamic instability through the running response spectrum by FFT(fast Fourier Transform) and attractors are compared in the phase plane. Dynamic buckling loads are strongly influenced by the relationships between the natural frequency of structures and the dominant frequency of incident excitations.
스페이스 트러스의 강성 방정식을 이용한 비선형 해의 비교

This paper studies approaches to improving the convergence properties of nonlinear stiffness equations. A space truss is large structure that must be built to be quite thin, and for this reason a structure stability review is a critical part of the structural design process. The stability of the shelled structure is sensitive to diverse conditions. This leads to a nonlinear problem with concomitant large deformation. To examine structural stability, the accuracy of nonlinear stiffness equations must be improved. In this study, the space truss is an analysis model. Tangent stiffness equation and nonlinear stiffness equation is using nonlinearity analysis program. The study compares analysis results to investigate the accuracy and the improvement in the convergence properties of nonlinear stiffness equations.
한방병원에 요통, 하지방사통으로 내원한 환자 236명에서 MRI상 추간판 탈출증과 적외선 체열 검사(DITI)와의 연관성 분석

Objectives This study is planned to classify correlation between HIVD of L-spine MRI and Digital Infrared Thermal Image (DITI). Methods We measured the temperature of both leg whose 120 men and 116 women patients with lumbar pain in Bucheon Jaseng Korean Medicine Hospital. And We use Magnetic Resonance Imaging (MRI) for classifying the patients who has lumbar intervertebral disc or not. Results 1) There was no statistical relation between difference of both leg`s temperature and gender (p>0.05). 2) There was meaningful statistical relation between difference of both leg`s temperature and age (p<0.05). 3) There was meaningful statistical relation between direction of HIVD of L-spine and direction of temperature reduction. 4) There was meaningful statistical relation between the severity of HIVD of L5/S1 and degree of temperature reduction. But there was no statistical relation between the severity of HIVD of L3/4, L4/5 and degree of temperature reduction. Conclusions We can use Digital Infrared thermal image (DITI) on low back pain patients for diagnosis. But we should not use DITI alone. DITI has limit in diagnosis. 족관절 염좌에 따른 비골신경 신전 손상 환자에 대한 근에너지기법과 한의종합치료 병행 증례보고

Objectives : The purpose of this study was to explore the effectiveness of Korean medicine treatment and the Muscle Energy Technique on foot drop through a case study with a patient with common peroneal neuropathy. Methods : A patient with severe foot drop after an ankle sprain was hospitalized and treated with acupuncture and traditional medicine and the Muscle Energy Technique. The effect of the treatment was evaluated with the Numeric Rate Scale(NRS), dorsiflexion angle evaluation, and the Manual Muscle Test (MMT). Results : The results of the NRS, dorsiflexion angle evaluation and MMT showed greatly improvementwith results changing from 8 to 2, 0 to 15, and Grade 2 to Grade 4, respectively. Conclusions : This study suggests that Korean medicine treatment and the Muscle Energy Technique could be effective treatments for foot drop with common peroneal neuropathy. However further clinical studies are needed to establish a definite conclusion.

Multi-access Edge Computing(MEC)는 이동통신 기지국과 근거리에 데이터를 처리 및 저장을 위한 서버등 컴퓨팅 시스템을 구축하여 물리적인 데이터 전송거리를 단축 시키고 초고속 및 조저지연 서비스를 가능하게 하는 5G 통신의 핵심기술이다. 현재 상용화 되고 있는 5G 환경에서의 MEC 기술을 사용하는 분야에 대해 기술하고, 이를 자율주행에 어떠한 방법으로 활용 해볼지에 대해 알아본다.
