Background and Objectives: Degradation of basement membranes and extracelluar matrix is a essential step in tumor cell migration, invasion and metastasis formation. Two types of Matrix metalloproteinases (MMPs), MMP-2 and MMP-9 are active in the digestion of type Ⅳ collagen, the main constituent of the basement membrane. Based on this background, we investigated the expression patterns of Type Ⅳ collagenase (MMP-2 and MMP-9) in squamous cell carcinoma, adenoid (acantholytic) squamous cell carcimoma, and its precancerous conditions, actinic keratosis and Bowen's disease by immunohistochemical technique. Materials and Methods: Formalin-fixed and paraffin-embedded tissues from 5 actinic keratosis, 5 Bowen's diseases, 5 adenoid squamous cell carcinoma, and 5 squamous cell carcinoma were immunolabelled with monoclonal antibodies directed against MMP-2 and MMP-9. Result: For MMP-2, actinic keratosis, Bowen's disease, adenoid squamous cell carcinoma, and squamous cell carcinoma was expressed to relative low level in tumor cells. Bowen's disease showed a negative staining for MMP-9, but in microinvasive area of actinic keratosis and adenoid lining cells of adenoid squamous cell carcinoma was a positive staining or MMP-9. In Bowen's disease, squamous cell carcinoma, and adenoid squamous cell carcinoma, dyskeratotic cells and acantholytic cells were stained for MMP-2 but for MMP-9, only in adenoid squamous cell carcinoma. Conclusion: This finding suggested that MMP-9 may be related to tumor invasion to surrounding stroma and that MMP-2 and MMP-9 may be involves in the apoptotic process of the tumor cells.

Pleomorphic lipoma is a asymptomatic, slowly growing, solitary, well-circumscribed dermal or subcutaneous benign tumor. The neck, shoulder region and back are the sites of predilection. On gross examination, the lesion resembles an ordinary lipoma. However microscopically, a misdiagnosis of liposarcoma is often made, on occasion leading to excessive and unnecessary surgical procedures. Smookler and Enzinger described 48 cases of lipomatous tumors having bizarre histologic features, but a benign clinical course. The term pleomorphic lipoma was selected to refer to these tumors. To date, there have been only three case reports of pleomorphic lipoma in the dermatologic literature. We describe a case of pleomorphic lipoma that developed as a subcutaneous nodule on the scalp of 72-year-old man, and showed a typical histopathologic findings of pleomorphic lipoma. Background and Objectives: Telemedicine uses computer and telecommunication technologies to provide medical information and services. Telemedicine can be useful in any situation in which there is a physical barrier between the doctor and the patient. We analyzed medical consultations from Chosun University Hospital is home page (http://hosp.chosun.ac.kr) to find problems on the home page and to offer high quality medical information services. Methods : We collected 544 queries of medical consultation on our home page from May 1998 to May 1999. From medical consultation data on the internet web site of Chosun University Hospital, consulted documents were analyzed with regard to medical department, patients’ age, consultation by attorney, and dermatologic consultations. Results : The telemedicine system is a relatively satisfactory method to both patients and physicians due to easier access and almost rea1-time medical consultation through the world wide web. But several problems should be solved to provide a more effective and lawful telemedicine environment. Conclusion : As a consequence to analyzed medical consultations on our home page, there were several problems in our store-and-forwarding system. To establish an effective telemedicine system, we have to prepare protocols to receive sufficient information and patients have to send digital images to make help an accurate diagnosis. Also problems of liability, reimbursement and coordinated national policy need to be solved.
