<B>BACKGROUND</B><P>Integrated <SUP>18</SUP>fluorine fluorodeoxyglucose (<SUP>18</SUP>F-FDG) positron emission tomography/computed tomography (PET/CT) has shown somewhat variable sensitivity and specificity for mediastinal nodal staging in granulomatous disease endemic areas. The purpose of the study was to prospectively evaluate the efficacy of PET/CT for mediastinal nodal staging in nonsmall cell lung cancer (NSCLC) patients in a tuberculosis-endemic country.</P><B>METHODS</B><P>Prospective assessment of the diagnostic efficacy of integrated PET/CT for detecting mediastinal nodal metastasis was performed in 674 patients (M:F ratio = 502:172; mean age, 61 years) with NSCLC. Patients underwent an integrated PET/CT examination and subsequent surgical nodal staging (by mediastinoscopy only in 121 patients and by thoracotomy in 553). Nodes showing greater <SUP>18</SUP>F-FDG uptake than mediastinum at PET without benign calcification or high attenuation >70 household unit (HU) at unenhanced CT were regarded as being positive for malignancy. The histologic nodal assessment results were used as reference standards.</P><B>RESULTS</B><P>Of 2477 mediastinal nodal stations evaluated in 674 patients, 275 (11%) stations in 180 (27%) patients proved to be malignant. On a per-person basis, the overall sensitivity, specificity, and accuracy of PET/CT for mediastinal nodal staging were 61% (110 of 180), 96% (473 of 494), and 86% (583 of 674), respectively. On a per-nodal station basis, they were 46% (126 of 275), 98% (2154 of 2202), and 92% (2280 of 2477).</P><B>CONCLUSIONS</B><P>Integrated PET/CT provides high specificity and reasonably high accuracy, but somewhat low sensitivity for mediastinal nodal staging of NSCLCs. The high specificity is achieved at the expense of sensitivity by interpreting calcified nodes or nodes with high attenuation at CT, even with high FDG uptake at PET, as benign in a tuberculosis-endemic region. OBJECTIVE:: To investigate the employment status of lung cancer survivors and the work-related problems they face. BACKGROUND:: Although the number of lung cancer survivors is increasing, little is known about their employment and work-related issues. METHODS:: We enrolled 830 lung cancer survivors 12 months after lung cancer curative surgery (median time after diagnosis, 4.11 years) and 1000 volunteers from the general population. All participants completed the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire, Core 30-item and a questionnaire that included items relating to their jobs. We used logistic regression analysis to identify independent predictors of unemployment. RESULTS:: The employment rate of lung cancer survivors decreased from 68.6% at the time of diagnosis to 38.8% after treatment, which was significantly lower than the employment rate of the general population (63.5%; adjusted odds ratio = 2.31, 95% confidence interval: 1.66–3.22). The posttreatment unemployment rate was higher for women than for men. Among survivors, employment was inversely associated with older age, household income, number of comorbidities, and poor social functioning. Fatigue (78.6%) was the most common work-related problem reported by survivors. CONCLUSIONS:: Lung cancer survivors experienced more difficulties in employment than did the general population. Age, monthly household income, number of comorbidities, and social functioning appear to be important factors influencing employment status. These findings suggest that lung cancer survivors need support to cope with the financial impact of cancer.
피부 병원균에 대한 톱니모자반 추출물의 항균 시너지 효과

S. aureus, S. epidermidis, P. aeruginosa, P. acnes와 C. albicans는 사람의 피부에서 발생하는 질병과 밀접한 관련을 가지는 대표적인 병원성 미생물로 알려져 있다. 본 연구에서는 해조류 중에서도 항균 활성에 대한 연구가 미미한 모자반을 대상으로 피부 병원균에 대한 항균 효과를 조사하였다. 국내의 연해에 자생하는 7종의 모자반 추출물 중에서 disc diffusion assay와 MIC assay를 통해 가장 뛰어난 항균 효과를 나타낸 톱니모자반을 후속 연구를 위한 후보 물질로 선정하고 연구를 진행하였다. 톱니모자반 추출물의 유기용매 분획층 중에서, 노말-헥세인 분획층이 S. aureus, S. epidermidis, P. aeruginosa, P. acnes 및 C. albicans에 대한 MIC값이 32-256 μg/mL로 가장 뛰어난 항균 활성을 나타내었다. 이에 피부 병원균들에 대한 치료제로 사용되고 있지만 내성균의 출현으로 효능이 거의 없는 항생제들인 테트라사이클린, 에리트로마이신, 린코마이신과 플루코나졸과 항균 활성이 뛰어난 것으로 나타난 톱니모자반의 노말-헥세인 분획층과의 병용 사용에 의한 항균 시너지 효과를 조사하였다. 그 결과, 톱니 모자반 노말-헥세인 분획층과 이들 항생제와의 병용 사용에 의해 피부 병원균에 대한 항생제와 톱니모자반 노말-헥세인 분획층의 MIC값이 4-32배 감소되었고, 톱니모자반 노말-헥세인 분획층과 이들 항생제와의 병용 시 median FIC값이 0.26-0.55로 항균 시너지 효과를 나타내었다. 즉, 항생제와 톱니모자반 노말-헥세인 분획층과의 병용 사용은 피부 병원균에 대한 이들 항생제의 감수성을 회복시키는데 크게 기여하는 것으로 나타났다. The object of this study was to develop an alternative way to treat human skin pathogens using marine algae. During this study, we observed that the ethanolic extract of the edible brown algae [Sargassum serratifolium (C. Agardh) C. Agardh] exhibited potential antimicrobial activity against pathogenic commensal bacteria related with acne vulgaris (Propionibacterium acnes, Staphylococcus epidermidis, Staphylococcus aureus and Pseudomonas aeruginosa), and Candida albicans which causes cutaneous candidiasis. Among the solvent-soluble fractions from the ethanolic extract, a hexane-soluble fraction showed the strongest antimicrobial activity against all tested human skin pathogens with MIC values ranging from 32 to 512 μg/mL. In addition, the hexane fraction exhibited a synergistic antimicrobial activity with commercial antibiotics used in the treatment of acne vulgaris or cutaneous candidiasis. Thus, this study suggests that S. serratifolium extract could be a potential source of natural antimicrobial agents or a pharmaceutical component against human skin pathogens.

Background: Inflammation has been known to associate with many human diseases. The objective of this study was to evaluate an anti-inflammatory effect of ozonated krill (Euphausia superba) oil, which was prepared by the treatment of krill oil using ozone gas. The anti-inflammatory activity was evaluated in lipopolysaccharide (LPS)-stimulated RAW 264.7 macrophages. Results: Ozonated krill oil significantly inhibited nitric oxide (NO) production and suppressed the mRNA and protein expression of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) in LPS-stimulated RAW 264.7 macrophages. Ozonated krill oil also reduced the mRNA expression of inflammatory cytokines such as interleukin (IL)-$1{\beta}$, IL-6, and tumor necrosis factor (TNF)-${\alpha}$ in LPS-stimulated RAW 264.7 macrophages. To elucidate the mechanism underlying the anti-inflammatory activity of ozonated krill oil, we evaluated the effects of ozonated krill oil on the activation of mitogen-activated protein kinases (MAPKs) pathway. Ozonated krill oil suppressed the LPS-stimulated phosphorylation of p38 MAPK and c-Jun N-terminal kinases (JNK). Conclusion: This study revealed that the ozonated krill oil exhibited an anti-inflammatory effect in LPS-stimulated RAW 264.7 macrophages. To the best of our knowledge, this is the first report that ozonated krill oil suppressed pro-inflammatory mediator and cytokine expression in LPS-stimulated RAW 264.7 macrophages by inhibiting the phosphorylation of p38 MAPK and JNK.

Campsis grandiflora K. Schum. flower was extracted with 80% aqueous MeOH, and concentrated extract was successively partitioned with EtOAc, n-BuOH, and $H_2O$. From n-BuOH fraction, two cyclohexylethanoids were isolated through repeated silica gel and Sephadex LH-20 column chromatographies. Based on physico-chemical data obtained from NMR, MS, and IR, chemical structures of compounds were determined as 1,4-dihydroxy-3,4-(epoxyethano)-5-cyclohexene (1) and cornoside (2). These compounds were isolated for the first time from C. grandiflora K. Schum flower.

<P><B>Abstract</B></P> <P>Piperlongumine (PL), isolated from <I>Piper longum</I> L., is receiving intense interest due to its selectively ability to kill cancer cells but not normal cells. We synthesized a number of analogues by replacing the cyclic amide of PL with aliphatic amides to explore structural diversity. Compound <B>CG-06</B> had the strongest cytotoxic profile of this series, showing potent effects in human prostate cancer DU-145 cells, in which signal transducer and activator of transcription 3 (STAT3) is constitutively active. <B>CG-06</B> inhibited STAT3 phosphorylation at tyrosine 705 in a dose- and time dependent manner in DU-145 cells and suppressed IL-6-induced STAT3 phosphorylation at Tyr-705 in DU-145 and LNCaP cell lines. <B>CG-06</B> decreased the expression levels of STAT3 target genes, such as cyclin A, Bcl-2, and survivin. Notably, we used drug affinity responsive target stability (DARTS) to show that <B>CG-06</B> binds directly to STAT3, and the reactive oxygen species (ROS) scavenger N-acetyl cysteine (NAC) rescued the <B>CG-06</B>-induced suppression p-STAT3. Surgical Resection of Recurrent Lung Cancer in Patients Following Curative Resection

<P>We reviewed our experience with resection of recurrent lung cancer to evaluate the benefit and risk of the procedure. From December 1994 to December 2003, 29 consecutive patients underwent pulmonary resections for recurrent lung cancer. The mean duration from the first resection to second surgery was 25.4±15.1 months for the definite 2nd primary lung cancer (n=20) and 8.9±5.7 months for metastatic lung cancer (n=9). The procedures at the second operations were completion-pneumonectomy in 11 patients, lobectomy in 5 patients, wedge resection in 12 patients and resection and anastomosis of trachea in 1 patient. Morbidity was observed in 6 (21%) of the patients and the in-hospital mortality was two patients (7%) after the repeated lung resection. Tumor recurrence after reoperation was observed in 14 patients (48%). The actuarial 5-yr survival rate was 69% and the 5-yr disease free rate following reoperation was 44%. No significant difference was found in overall survival and disease free survival between the 2nd primary lung cancer group and the metastatic lung cancer group. The recurrence rate following reoperation was significantly different between the wedge resection group and lobectomy/completion pneumonectomy group (<I>p</I>=0.008), but the survival rate was not significantly different (<I>p</I>=0.41). Surgical intervention for recurrent lung cancers can be performed with acceptable mortality and morbidity. If tolerable, completion pneumonectomy or lobectomy is recommended for resection of recurrent lung cancer.</P>
