        공정의 계속적인 향상을 위해서는 여러 가지 통계적 방법이 필요하다. 본 논문에서는 우선 공정의 문제점 파악에 아주 중요한 역할을 하는 부분군의 형성에 대해 알아보고, 다음으로 공정의 향상에 많이 이용되는 관리도 및 실험계획법에 대해서 알아본다. The various statistical techniques are required for the process improvement. The process capability studies are active investigations whose purposes are to detect and remove special cause sources of variability to produce performance. Ration subgrouping is the basis for the identification and removal of possible problem in the process. Control charting techniques and desighed experiment generate process improvement by locating and removing causes of process variability.

        환자호소에 대한 해결시간 단축을 위한 기초조사연구

        Background : An important issue in health care today is in trying to center service around the hospitalized patient. There is a "kindness Movement" developing now. Where in the focus of Health Care is being changed from simply treating the basic physical needs of the patient with sophisticated technology. to keeping emotional well being healthy with more humane and attentive treatment. In our attempt to reach the goal of a completely satisfied patient, we undertook a study of the common complaints of patients, and the subsequent nature of the interventions. Method : The study was carried out in two stages, first the patients made known their complaints by filling out questionnaires, then we collected data on the attempts to alleviate the complaints. The questionnaire provided 19 different complaints, which were then analysed for such variables as content, source of complaint, persons treating the complaint, and length of time and method used to solve the complaint. Result : 1. The Chief complaints made by patients(99.1%) were of physical discomfort, such as pain, nausea, vomiting, indigestion, diarrhea and constipation. 2. The complaints were vioced primarily by either a member of the patient's family, or by the patients themselves(78.4%). 3. The complaints were intervened by nurses alone(53.5%), physicians alone(25.5%), or by nurses and physicians together(19.25%). 4. The method by which the complaints were resolved included the utilization of prescriptions(55.7%), turther explanation and education (25.5%) and notification after treatment(13.2%). 5. Most complaints were voiced during the dayshift (42.6%, 7:00~15:00), followed by the evening shift(36.0%, 15:00~22:00) ;and then the nightshift(21.3%, 22:00~07:00). 6. The time required for successful resolution of the patient's problems varied from 10~88.9mi, according to the nature of the complaint. Conclusion : Hopefully by knowing beforehand the nature of both complaint and intervention, we can anticiapte problems and shorten reaction time, in order to provide for a more satisfied patient.

        Study Design : Two patients who had painful Schmorl's node at thoracic level from acute trauma were adopted and taken operation for its treatment Objectives : To document the effect of operation for the treatment of painful Schmorl's node at thoracic level summary of background data : Schrmorl's nodes are common spinal lesion generally believed to be asymptomatic, but in some cases there were painful symptom by Schmorl's node. This case report is to explain the effect of operative treatment for the painful Schmorl's node. Methods : Two patients were adopted who had traumatic painful Schmorl's node at thoracic level for our case report. They were treated conservative methods at first but it's effect were poor, so they were taken anterior decompression and fusion, and checked back pain symptom. Result : In all two patients, the symptom of back pain by Schmorl's nodes at thoracic level was almost subsided. conclusion : For the treatment of traumatic painful Schmorl's node, most of surgeon says that conservative treatments were treatment of choice, but operative treatments are also benefit to subsidence of back pain by Schmorl's node.

      • [P425] Giant cell tumor of the tendon sheath developed on the finger

        Giant cell tumor of the tendon sheath(GCTTS) is a family of lesions usually involving the joint synovia, bursae and tendon sheath. Localized forms mainly involve the fingers and hands. GCTTS may occur at any age, but it is most common between the ages of 30 and 50 years. GCTTS is firm in consistency and measures from 1 to 3 cm in diameter. A 51-year-old male presented to the dermatology with a painless, firm skin colored mass on the radial aspect of distal phalanx of the left index finger. The lesion was asymptomatic, about 1.5 x 2 cm, sized skin colored, firm tumor. Microscopically, the tumor located in dermis was encapsulated and showed highly cellular proliferation within a distinctly hyalinized collagenous stroma. In cellular area, most cells were numerous histiocyte-like cells with kidney-shaped, oval neuclei and abundant eosinophilic cytoplasm. Some osteoclast-like multinucleated giant cells were also distributed in the cellular area. Smooth muscle actin stain was negative. Finally, the diagnosis of GCTTS was made on the basis of the histologic findings. He was transferred to orthopedic surgery department and underwent excision of the lesion. The histopathology result of excised specimen was also consistent with GCTTS. Our case was typical for the clinical characteristic of GCTTS. Korean reported cases are currently seven in the dermatologic literature. We report a rare case that suggests GCTTS clinically and histologically.

      • Dermoscopic findings in acute and diffuse total alopecia

        Background: Acute diffuse and total alopecia (ADTA) is a type of alopecia totalis. Although a number of studies have been reported on dermoscopic finding of alopecia areata, studies on dermoscopic finding of ADTA are extremely limited. If a non-invasive method, dermoscopy, can identify ADTA, clinicians will be able to diagnose ADTA faster and reassure patients. Objectives: The purpose of this study was to evaluate the characteristics of ADTA using dermoscopy Methods: We studied whether dermoscopy can enhance the diagnosis of ADTA using Dermlite DL3N. Patients with ADTA diagnosed by clinical and histological findings were examined by dermoscopy. We also photographed 10 hairless areas with a digital camera connected to dermoscopy. Results: In 18 patients, 180 scalp lesions were examined by dermoscopy. In 180 lesions, yellow dots and black dots were observed in 132 lesions (73.3%) and 137 lesions (76.1%), respectively. Broken hairs and short vellus hairs were seen in 114 lesions (63.3%) and 101 lesions (56.1%), respectively. Tapering hair appeared in 43 lesions (23.8%). Conclusion: The simultaneous increase in the number of broken hair and black dot, suggesting sudden hair loss, and short vellus hair, suggesting regrowth of hair, is consistent with the clinical symptoms of ADTA. In other words,, the increase of short vellus hair on the dermoscopy that accompanies clinically severe hair loss is the greatest feature of ADTA.

      • Juvenile localized scleroderma occurring concomitantly with port wine stain

        Juvenile localized scleroderma(JLS) is a fibrosing disease that affects the skin and subcutaneous tissue, and occurs before 16-year-old. Port wine stain(PWS) is a type of capillary vascular malformation composed of dilated blood vessels within the papillary and reticular dermis. A 22-year-old male patient visited our department for slightly erythematous depressed patch on his right thigh. Both the erythematous patch and the depression were found at childhood. He had no history of trauma, injection and other past medical disease. Physical examination revealed a solitary about 10cm x 10cm sized slightly erythematous depressed patch on his lateral side of right thigh. Histopathological examination showed relatively normal appearance of epidermis with hyperkeratosis, proliferation and dilatation of vessels with perivascular inflammatory infiltration in upper dermis and decreased number of adnexal structures. Mild atrophic subcutanous layer seemed to lie higher in the dermis and the collagen bundles in the dermis appeared thickened. Elastic fiber stain confirmed straightened elastic fibers. Masson trichrome stain revealed focally thickened collagen bundles. Based on these findings, a diagnosis was made as juvenile localized scleroderma occurring concomitantly with port wine stain. Considering its underestimated tendency, clinician needs to be concerned about cases of coexistence of JLS and PWS.

      • Syringoid eccrine carcinoma occurred on the thigh

        Syringoid eccrine carcinoma (SEC) is a rare cutaneous malignant tumor thought to be derived from eccrine sweat apparatus. It is usually present in the head, neck and trunk region. SEC shows an infiltrative growth pattern, with deep invasion and often shows recurrences. However, metastases in the regional lymph nodes are extremely rare as are disseminated metastases. A 94-year-old male patient visited our department with an 80-year history of a lesion on his left thigh. Physical examination revealed a 2x2cm sized well-demarcated round shaped erythematous to pinkish colored nodule with ulcer on his left thigh. Histological findings from the punch biopsy of the lesion revealed that tumor consisted mainly of numerous small cords and nests from the reticular dermis extending close to subcutaneous tissue. Most of the tumor nests and cords formed luminal or tubular structures and the tumor cells showed variable atypia. Some ductal structure showed tadpole appearance and were consisted of anaplastic cell with deep basophilic nucleus. Immunohistochemical staining of cytokeratin-7, carcinoembryonic antigen and Ki-67 were reactive. Based on these findings, the final diagnosis was made as SEC. CT scan indicated a solitary lesion without metastasis. The patient was treated with local wide excision and didn’t show any recurrence during the follow-up period of 7 months. Herein, we report a very rare case of SEC which occurred on the left thigh.

      • The efficacy and safety of tofacitinib in patients with alopecia totalis

        Background: In a mouse model of alopecia areata(AA), CD8+NKG2D+ T cells were shown to be central in AA, causing up-regulation of IL-15 in hair follicles and ultimately production of INF-γ. As downstream regulators of INF-γ and IL-15, JAK inhibitors have been shown to eliminate the interferon signature and reverse disease. Objectives: We aimed to evaluate the safety and efficacy of the JAK 1/3 inhibitor, tofacitinib, in a series of patients with recalcitrant alopecia totalis (AT). Methods: This is a prospective open label study performed. Nine recalcitrant AA patients with a total baldness, unresponsive to other therapeutic modalities were included and had taken 5mg tofacitinib twice daily for 6months. The endpoint at 6 months was assessed by the percent change in Severity of Alopecia Tool(SALT) score during treatment. Results: Three patients who experienced hair growth were judged as responders showing 25% to 75% regrowth at 6 months. The patients with a shorter duration of current AT and more preserved hair follicles showed better efficacy in tofacitinib. The relationship between of age of onset and hair growth improvement also showed positive relationship pattern. Conclusion: In this study, the response rate was unsatisfactory. However, as the patients in this study did not respond to any other treatments of AA, tofacitinib might be considered as an alternative treatment of recalcitrant AA.
