        Intravenous Tenecteplase for Acute Ischemic Stroke Within 4.5–24 Hours of Onset (ROSE-TNK): A Phase 2, Randomized, Multicenter Study

        Background and Purpose Intravenous tenecteplase (TNK) efficacy has not been well demonstrated in acute ischemic stroke (AIS) beyond 4.5 hours after onset. This study aimed to determine the effect of intravenous TNK for AIS within 4.5 to 24 hours of onset. Methods In this pilot trial, eligible AIS patients with diffusion-weighted imaging (DWI)-fluid attenuated inversion recovery (FLAIR) mismatch were randomly allocated to intravenous TNK (0.25 mg/kg) or standard care within 4.5–24 hours of onset. The primary endpoint was excellent functional outcome at 90 days (modified Rankin Scale [mRS] score of 0–1). The primary safety endpoint was symptomatic intracranial hemorrhage (sICH). Results Of the randomly assigned 80 patients, the primary endpoint occurred in 52.5% (21/40) of TNK group and 50.0% (20/40) of control group, with no significant difference (unadjusted odds ratio, 1.11; 95% confidence interval 0.46–2.66; <i>P</i>=0.82). More early neurological improvement occurred in TNK group than in control group (11 vs. 3, <i>P</i>=0.03), but no significant differences were found in other secondary endpoints, such as mRS 0–2 at 90 days, shift analysis of mRS at 90 days, and change in National Institutes of Health Stroke Scale score at 24 hours and 7 days. There were no cases of sICH in this trial; however, asymptomatic intracranial hemorrhage occurred in 3 of the 40 patients (7.5%) in the TNK group. Conclusion This phase 2, randomized, multicenter study suggests that intravenous TNK within 4.5–24 hours of onset may be safe and feasible in AIS patients with a DWI-FLAIR mismatch.

        In vitro stability evaluation of coated lipase

        Objective: The study was conducted to evaluate the stability of commercial coated lipase (CT-LIP) in vitro. Methods: The capsules were tested under different conditions with a range of temperature, pH, dry heat treatment and steaming treatment, simulated gastric fluid (SGF) and simulated intestinal fluid (SIF) in this work, respectively. Free lipase (uncoated lipase, UC-LIP) was the control group. Lipase relative activities measured in various treatments were used as a reference frame to characterize the stability. Results: The lipase activities were decreased with increasing temperatures (p<0.05), and there was a markedly decline (p<0.01) in lipase comparative activities of UC-LIP at $80^{\circ}C$ compared with CT-LIP group. Higher relative activities of lipase were observed in CT-LIP group compared with the free one under acidic ambient (pH 3 to 7) and an alkaline medium (pH 8 to 12). Residual lipase activities of CT-LIP group were increased (p<0.05) by 5.67% and 35.60% in dry heat and hydrothermal treatments, respectively. The lipase relative activity profile of CT-LIP was raised at first and dropped subsequently (p<0.05) compared with constantly reduced tendency of UC-LIP exposed to both SGF and SIF. Conclusion: The results suggest that the CT-LIP possesses relatively higher stability in comparison with the UC-LIP in vitro. The CT-LIP could retain the potential property to provide sustained release of lipase and thus improved its bioavailability in the gastrointestinal tract.

      • Association Between the Ku70-1310C/G Promoter Polymorphism and Cancer Risk: a Meta-analysis

        Ku70 plays an important role in DNA double-strand break repair. Studies revealing conflicting results on the role of the Ku70-1310C/G promoter polymorphism on cancer risk led us to perform a meta-analysis to investigate this relationship. Ten case-control studies with 2566 cases and 3058 controls were identified. Odds ratios (ORs) and 95% confidence intervals (CIs) were used to assess the strength of associations. The overall results suggested no association between the Ku70-1310C/G promoter polymorphism and total cancer risk. However, on stratified analysis, significantly increased risks were observed among the Asian population (GG vs. CC: OR=1.50, 95%CI=1.10-2.06; GG vs. CC/CG: OR=1.47, 95%CI=1.07-2.01) and population-based case-control studies (GG vs. CC: OR=1.57, 95%CI=1.12-2.22; CG vs. CC: OR=1.35, 95%CI=1.11-1.64; CG/GG vs. CC: OR=1.37, 95%CI=1.14-1.65). Additionally, variant genotypes were associated with a significantly increased breast cancer risk (GG vs. CC: OR=1.80, 95%CI=1.26-2.56; GG vs. CC/CG: OR=1.40, 95%CI=1.01-1.95).

        Dynamic Characteristics of Saturated Soft Clays under Cyclic Loading in Drained Condition

        This study presents an experimental investigation on the effects of cyclic loading waveform and cyclic stress ratio (CSR) on dynamic behaviors through laboratory tests on a source of soft clay under cyclic loading in drained condition. Test results showed that under different cyclic loading waveforms at a given frequency (f = 1 Hz) and CSR = 0.6, the axial strain increases significantly at a relatively lower number of cycles (N) and then became stable, while the axial strain consistently decreases with the increasing N and tend to be stable. At a given N, specimen under sine wave cyclic loads shows a higher axial strain rate than that under triangle wave or square cyclic loads. Test results also indicated that at different CSRs, the axial strain increases rapidly at a relatively lower N and then gradually tends to be stable with the increasing N at lower CSR yet there is no stable trend for specimens at higher CSR, which indicates there is a critical value of CSR. It is recommended to apply the cyclic loading in sine waveform with CSR close to and less than the critical value to get a better treatment effect.

        An Improved CPTu-based Method to Estimate Jacked Pile Bearing Capacity and Its Reliability Assessment

        This paper presents an improved method for predicting the bearing capacity of single jacked pile based on piezocone penetration test (CPTu) data which contain tip resistance (qt), side friction ( fs) and excess dynamic pore water pressure (u2). Firstly, the average value of qt was determined to calculate the pile unit base resistance according to the influence zone of the logarithmic spiral concerning the effect of soil types. Secondly, fs and u2 were used to calculate the pile unit shaft resistance considering the effect of friction fatigue. The pile jacking force was obtained in terms of unit base resistance and unit shaft resistance. The ultimate bearing capacity was finally determined using pile jacking force multiplied by a force ratio denoted as Rf. The ultimate bearing capacities calculated for two case projects show a good agreement with the measured results, which indicates that the proposed method is feasible and effective. Reliability assessment was carried out on the proposed method and other five estimation methods with respect to the reliability index obtained by advanced first-order second-moment (AFOSM) method. Comparison result showed that the method proposed in this study has a higher reliability index over other five existing methods. Therefore, the proposed method can be used as a reliable alternative to predict the bearing capacity in the engineering practice of jacked pile foundation.

        극한 기후 지수를 이용한 여름철 극한 고온 현상의 시공간적 변화특성에 관한 연구

        전 세계적 극한 고온 현상의 잦은 출현으로 고온 기상 변화에 대한 관련 연구가 활발히 진행되고 있지만, 한국에서 장기간 축적된 기상자료를 이용한 시공간 분석에 대한 연구는 부족한 실정이다. 본 연구에서는 전국 59개 기상관측소의 40년간 일일 기상관측 자료를 바탕으로 6개의 극한고온 지수를 이용하여 여름철 극한 고온 현상의 시공간적 변화에 대한 연구를 수행하였다. Mann-Kendall 테스트는 시계열 자기 상관 관계에 의해 영향을 받을 수 있기 때문에 이 연구에서 변이점을 확인하기 위해 Sliding t 테스트를 추가로 사용하였으며, 그 결과 2005~2009년에는 상승 경향성이 급격히 변화하는 현상이 발생한 것을 알 수 있었다. 그리고 공간자기 상관에 대해 가장 일반적으로 사용되는 통계량 Morans I 지수를 이용하여 주로 고-고 밀집과 저-저밀집의 두 가지 공간밀집 형태이 나타났다. 추세변화 결과 공간분포 절대지수 SU와 TR의 변화율이 가장 빠르게 진행되었고, 전반적인 추세는 남서쪽에서 북동쪽으로 점차 감소하는 것으로 나타났다. With the frequent occurrence of extreme high temperature in the world, the research on high temperature weather changes is being actively carried out, but the research on temporal and spatial analysis using meteorological data accumulated in Korea for a long time is insufficient. In this study, based on 40 years of daily weather observation data from 59 weather stations across the Korea, a study was conducted on spatiotemporal changes in extreme high temperature phenomena in summer using six extreme high temperature indices. Since the Mann-Kendall test can be affected by time-series autocorrelation, the Sliding-t test was additionally used to confirm the variation point in this study, and as a result, it was found that the upward tendency changed rapidly from 2005 to 2009. And using the most commonly used statistic Morans I index for spatial autocorrelation, two types of spatial density were mainly found: high-high density and low-low density. As a result of the trend change, the rate of change in the spatial distribution absolute index SU and TR progressed the fastest, and the overall trend gradually decreased from the southwest to the northeast.

        According to the China tumor registry 2013 annual report, breast cancer, lung cancer, and ovarian cancer are three common cancers in China nowadays, with high mortality due to the absence of early diagnosis technology. However, proteomics has been widespreadly implanted into every field of life science and medicine as an important part of post-genomics era research. The development of theory and technology in proteomics has provided new ideas and research fields for cancer research. Proteomics can be used not only for elucidating the mechanisms of carcinogenesis focussing on whole proteins of the tissue or cell, but also seeking the biomarkers for diagnosis and therapy of cancer. In this review, we introduce proteomics principles, covering current technology used in exploring early diagnosis biomarkers of breast cancer, lung cancer and ovarian cancer.

      • MicroRNAs May Serve as Emerging Molecular Biomarkers for Diagnosis and Prognostic Assessment or as Targets for Therapy in Gastric Cancer

        Gastric cancer (GC) is one of the most common cancers, with high incidences in East Asia countries. Most GC patients have been reported with low early diagnosis rate and show extremely poor prognosis. Therefore, it is necessary to develop novel and more sensitive biomarkers to improve early diagnosis and therapy in order to provide longer survival and better quality of life for gastric cancer patients. MicroRNAs (miRNAs) play crucial roles in GC development and progression. miRNAs have emerged as a novel molecular biomarker for cancer diagnosis, prognosis and therapy with surprising stability in tissues, serum or other body fluids. This review summarizes major advances in our current knowledge about potential miRNA biomarkers for GC that have been reported in the past two years.

      • An Experimental Model for Induction of Lung Cancer in Rats by Chlamydia Pneumoniae

        Objective: To assess induction effects of Chlamydia pneumoniae (Cpn) on lung cancer in rats. Methods: A lung cancer animal model was developed through repeated intratracheal injection of Cpn (TW-183) into the lungs of rats, with or without exposure to benzo(a)pyrene (Bp). Cpn antibodies (Cpn-IgA, -IgG, and -IgM) in serum were measured by microimmunofluorescence. Cpn-DNA or Cpn-Ag of rat lung cancer was detected through polymerase chain reaction or enzyme-linked immunosorbent assay. Results: The prevalence of Cpn infection was 72.9% (35/48) in the Cpn group and 76.7% (33/43) in the Cpn plus benzo(a)pyrene (Bp) group, with incidences of lung carcinomas in the two groups of 14.6% (7/48) and 44.2% (19/43), respectively (P-values 0.001 and <0.000 compared with normal controls). Conclusions: A rat model of lung carcinoma induced by Cpn infection was successfully established in the laboratory for future studies on the treatment, prevention, and mechanisms of the disease.
