        Prevalence of diffuse idiopathic skeletal hyperostosis among elderly subjects referred for radiological investigation in tertiary hospital at Oman

        Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by calcification of different entheseal sites including the anterior longitudinal ligament of the spine. There is no documented information about DISH in Oman. This study determined the prevalence of DISH and associated factors among elderly subjects in a national tertiary care referral hospital in Oman. This retrospective study reviewed chest X-rays of all patients aged more than 50 years, referred to the radiology department of Sultan Qaboos University Hospital in the year 2016, based on the Resnick s criteria. The prevalence was expressed as proportions across age groups and sex. Chi-square test and logistic regression analysis was done to determine the association of the age and sex with DISH. A total of 1305 chest X-rays of patients were reviewed. The overall prevalence of DISH was 10%, with male to female ratio of 1.56:1. The odds ratio for males and increasing age were 1.63 (95% confidence interval [CI], 1.12‒2.3; P<0.05) and 1.34 (95% CI, 1.14‒1.58; P<0.001) respectively. The prevalence increased with age to maximum of 13% in the age group of ≤80. The pre-stage DISH prevalence was 9.3% and more frequently observed among males. Prevalence of DISH in the national tertiary care referral center in Oman is lower than in Jewish population, almost similar to Japanese, but higher than in Koreans. DISH prevalence is positively associated with age and sex. It is necessary to take appropriate precautionary measures to target the ageing population in Oman, especially elderly males.

        Background: Highly active antiretroviral therapy (HAART) is available free of cost in India as a part of our national ART programme since 2004.Early diagnosis and initiation of HAART are key components for the success of HIV control programmes. Late presentation with low baseline CD4 cell count is a strong prognostic marker for early mortality among HIV patients. The main aim of our study was to identify late presenters based on initial CD4 cell count testing. We also wanted to identify factors associated with late presentation. Methods: This retrospective hospital based study was carried out at Kasturba Medical College. The study group included 474 newly diagnosed HIV patients registered in the ART clinic of KMC between February 2012 and February2013. Subjects with CD4 T cell count below 200 cells per cubic millimetre were classifi ed as late presenters. Subjects with CD4 T cell counts less than 50 cells per cubic millimetre were termed as very late presenters. Statistical analysis was performed with SPSS software. Results: The mean age of our study population was 41.60±9.28years. Majority of our study population were males 327(69%). Median CD4 count at diagnosis was 188 cells per cubic millimetre ([IQR] 93-366 ). 208(44%) patients had WHO stage 4 disease at presentation. 251(53%) were late presenters and 223(47%) were early presenters. 47(9.9%) were very late presenters.Male gender(p=.03) and residing in rural area (p=.001) had a signifi cant positive association with late presentation. Conclusions: More than fi fty percent of patients were late presenters despite ART being available free of cost in India. Late presentation poses a signifi cant threat to the success of our national ART programme.HIV/AIDS care intervention programmes must target men and the rural population as they are vulnerable for late presentation as shown in our study.

        We witness noteworthy developments in multifunctional materials progress through additive manufacturing techniques,enhanced by the revolution of Industry 4.0 and Internet of Things. Still, in specific circumstances, the performance of usedmaterials is sometimes limited. Among the various existing techniques, the additive manufacturing (AM) process has gainedmuch popularity over the last two decades and is one of the most revolutionary manufacturing techniques. In this comprehensivereview, we have addressed the fundamentals of various Additive Manufacturing processes, including binder jetting,fused deposition modelling, Stereolithography, selective laser sintering/melting, direct energy deposition. Furthermore, recentadvancements and emerging new technology in AM domain named electrochemical additive manufacturing is highlightedin this review as a major part. These processes’ capabilities, advantages, limitations, and applications are also discussed. Inthe concluding sections of this work, future trends are offered and discussed.

        Dynamic stiffness based computation of response for framed machine foundations

        The paper deals with the applications of spectral finite element method to the dynamic analysis of framed foundations supporting high speed machines. Comparative performance of approximate dynamic stiffness methods formulated using static stiffness and lumped or consistent or average mass matrices with the exact spectral finite element for a three dimensional Euler-Bernoulli beam element is presented. The convergence of response computed using mode superposition method with the appropriate dynamic stiffness method as the number of modes increase is illustrated. Frequency proportional discretisation level required for mode superposition and approximate dynamic stiffness methods is outlined. It is reiterated that the results of exact dynamic stiffness method are invariant with reference to the discretisation level. The Eigen-frequencies of the system are evaluated using William-Wittrick algorithm and Sturm number generation in the $LDL^T$ decomposition of the real part of the dynamic stiffness matrix, as they cannot be explicitly evaluated. Major's method for dynamic analysis of machine supporting structures is modified and the plane frames are replaced with springs of exact dynamic stiffness and dynamically flexible longitudinal frames. Results of the analysis are compared with exact values. The possible simplifications that could be introduced for a typical machine induced excitation on a framed structure are illustrated and the developed program is modified to account for dynamic constraint equations with a master slave degree of freedom (DOF) option.
