Maternal and Neonatal Outcomes during Dengue Infection Outbreak at a Tertiary National Hospital in Endemic Area of Indonesia

Ryan Saktika Mulyana,Evert Solomon Pangkahila,Tjokorda Gede Astawa Pemayun 대한가정의학회 2020 Korean Journal of Family Medicine Vol.41 No.3

Background: Dengue infection is the most common mosquito-borne disease in Indonesia. Dengue incidence during pregnancy increases with its prevalence among women of reproductive ages. It potentially causes serious maternal and neonatal morbidity and mortality; however, its impact during a period of disease outbreak has not been described. We aimed to assess the outcomes of pregnancies affected by dengue infection during an outbreak in an endemic area of Indonesia. Methods: We conducted a prospective observational study at Sanglah General Hospital, Bali, Indonesia from April 2016 to April 2017. All pregnant women with clinical symptoms of dengue infection who tested positive for dengue nonstructural protein 1-antigen or anti-dengue immunoglobulin M were included in the study. Clinical features and hematological and biochemical parameters were documented to assess their relationship with maternal and neonatal outcomes. Results: Our study population consisted of 41 pregnant women. Most were multigravida (58.5%). Dengue infection without warning signs was observed in 31.7% of the cases, while 53.7% had dengue infection with warning signs, and 14.6% had dengue shock syndrome/severe dengue infection. Most pregnancies were in the third trimester (78%); five cases (12.2%) suffered clinical complications. Preterm delivery was observed in seven cases (17.1%). Most patients (75.6%) decided to seek medical treatment at a critical phase of dengue infection (days 3–6). Conclusion: The high incidence of associated morbidity and mortality should warrant clinicians in endemic regions to consider diagnoses of dengue infection when treating pregnant women with clinical signs and symptoms. Patients should be treated at centers with adequate resources for monitoring and emergency procedures. A more intensive study is needed in the future aiming to make a more comprehensive guideline.

Background: Indonesian Basic Health Research Study (RISKESDAS) indicate that prevalence of stroke is 12.1%; and is considered as one of the major cause of death in Indonesia. It has been reported that ischemic stroke shown about 80-85% of the total cases, that continues to be the leading cause of long term disability worldwide. Primarily due to a loss of motor abilities and subsequent impairment in activities of daily living. Transcranial magnetic stimulation (TMS) is a non invasive tool that has been used in addition to other existing diagnostic tools to investigate motor pathophysiology in stroke patients. It has been shown to quantify the corticomotor excitability properties of clinically affected and unaffected muscles. Therefore, the objective of the study is to determine the usefulness of the device to rehabilitate the motoric function of the post-ischemic stroke patients Methods: March 2013 - March 2014, there has been 4380 cases of ischemic stroke, and only 2146 (48.9%) undergone intra-arterial heparin fl ushing guided with digital subtracted angiography (3D-DSA). Prior and post heparin-fl ushing procedure, all patients are undergone functional MRI test and assessment of the motoric function using TMS to examine the resting motor threshold (RMT), the amplitude (A), the latency (L), as well as Central Motoric conduction Time (CMCT) at the lesion and non lesion areas. Results: 2146 ischemic stroke patients, the RMT are 55.60±11.75 to 54.40±8.93 (p=0.52), and 52.78±10.59 to 52.22±8.59 (p=0.794); Amplitude are 1.57±1.76 to 1.7±1.72 (p=0.44), and 1.21±1.36 to 1.8±1.67 (p=0.005); Latency 23.76±4.51 to 23.51±4.36 (p=0.555) and 22.85±3.37 to 22.45±3.95 (p=0.104); and RMT 9.69±3.36 to 9.20±3.78 (p=0.238) and 8.87±2.92 to 8.02±2.58 (p=0.002) at the non lesion and lesion areas respectively. Conclusions: TMS provide a convincing evidence as new modality for post stroke rehabilitation.

Background: Indonesian Basic Health Research Study (RISKESDAS) indicate that prevalence of stroke is 12.1%; and is considered as one of the major cause of death in Indonesia. It has been reported that ischemic stroke shown about 80-85% of the total cases. Conventional ischemic stroke management has indicated some major obstacles in the stroke management program; therefore an approach using intra-arterial heparin fi ushing method has been used to overcome the above limitations. Controversy has been around for sometimes, therefore 2 groups of scientifi c forum have been established InaCVF (Indonesia CerebroVascular Forum) and InaDOCAR Prevent (Indonesia DOCAR Prevent) to fi nd out the scientifi c rational in the management of ischemic stroke using intra-arterial heparin fi ushing. The objective of this approach is for recovering the cerebral blood perfusion at the optimal time. Methods: March 2013 - March 2014, there has been 4380 cases of ischemic stroke, and only 2146 (48.9%) undergone intra-arterial heparin fi ushing guided with digital subtracted angiography (3D-DSA). Prior to the heparin-fi ushing procedure, all patients are undergone functional MRI test and the patients are carefully selected for the optimal result. Post procedure, the patients undergone functional MRI. The pre and post functional MRI results are compared to determine the change of cerebral blood perfusion. Results: Of 2146 ischemic stroke patients, undergone intra-arterial heparin fi ushing, none of the patients had severe complication. Sixty-six percent, 32%, and 2% have signifi cant moderate and little improvement respectively. Further biochemical parameters is still under investigation to elucidate the pathomechanism. Conclusions: Intra-arterial heparin fi ushing in patient with ischemic stroke is a promising and effective modality.

A Shift in Information Dissemination Methods in a Cultural Ritual amid the COVID-19 Pandemic

Suhadah, Suhadah,Mulyana, Deddy,Yusup, Pawit M.,Sjafirah, Nuryah Asri Korea Institute of Science and Technology Informat 2021 Journal of Information Science Theory and Practice Vol.9 No.4

This study aims to unravel the shift in the customary method of disseminating information about the ritual of Perang Topat (literally translated as "rice-cake war") as carried out by the custom community of Kemaliq Lingsar in the West Lombok Regency of Indonesia during the COVID-19 pandemic. Grounded in the ethnography of communication research methodology, this study examines the process of the cultural ritual during the 2020 period of the pandemic and compares it to the ones held in 2018 and 2019 (before the current pandemic). Drawing on findings from observations, in-depth interviews, and documentation, it was revealed that the traditional or custom-oriented community of Kemaliq Lingsar abandoned all three prominently used methods of disseminating the information of the Perang Topat ritual that had been used prior to the pandemic, and that they were replaced by a strategy with solely face-to-face (FTF) interpersonal communication carried out by visiting the homes of the target participants of the cultural event. This method was relevant to the current viral crisis because it enabled the committee to minimize the number of spectators which might potentially violate COVID-19 health protocols. This finding also reinforces the hypothetical statement that interpersonal communication via FTF is effective in disseminating information in a limited manner and empowering the emotional bond between the individuals who share relationships and similar interests. The findings of the present study can be a reference for any events where physical distancing must be strictly imposed and require a limit to the number of participants during the pandemic.

      • The Journalism Crisis in the Era of the COVID-19 Pandemic in Indonesia

Dudi Iskandar,Deddy Mulyana,Sitti Murni Kaddi World Association for Triple Helix and Future Stra 2023 Journal of Contemporary Eastern Asia Vol.22 No.2

The focus of this research is the practice of journalism in relation to the Behavioural Change Journalism Fellowship (JFPP) during the COVID-19 pandemic in Indonesia. Journalists and curators who are participants in the Behavioural Change Journalism Fellowship received fundings from the state. The Behavioural Change Journalism Fellowship is a unique model because it is the only one in the world in journalism practice that involves state's financial assistance during the COVID-19 pandemic. These fundings from the state for the media lead to dilemmas and controversies. This research uses a qualitative approach and a single holistic case study method. The Behavioural Change Journalism Fellowship was held from May to December 2021, involving 7,276 journalists from 857 media (print, television, radio, and online media) and 26 curators. This study found that during the Behavioural Change Journalism Fellowship, journalism experienced a crisis and lost its function as a control overpower. Giving cash to journalists and curators during the fellowship makes journalism lose its independence and objectivity. The Behavioural Change Journalism Fellowship has degraded or eliminated the function of the press as a watchdog. Incentives for journalists while participating in the Behavioural Change Journalism Fellowship is a long-term investment by the government to co-opt Indonesian media and journalists in the future. On the other hand, the Behavioural Change Journalism Fellowship raises another aspect; the crisis of journalism. Journalism work produced in the Behavioural Change Journalism Fellowship does not reflect the values and principles of journalism. Journalism values, such as independence and impartiality, are absent from the Behavioural Change Journalism Fellowship product.

This study aims to develop propositions about the factors that influence the purchase intention of private label products. These factors are: in-store promotion, visual merchandising, store image, and customer value. This study elaborates on some of the results of previous studies that have examined the factors that influence purchase intention of private label products that have been published on Google Scholar and indexed by Scopus between 1991- 2020, to develop a proposition. This paper fills a lack of Studies which discuss purchase intention from a consumer behavior perspective. From the perspective of consumer behavior, purchase intention is influenced by three factors, namely: intrinsic factors including: consumer value, extrinsic factors including: in-store promotions, visual merchandising and store image, and consumer factors. This paper defines purchase intention as the effort and strong urge to buy a particular product in the future, the possibility of considering buying the product, the decision to rebuy the product and the desire to recommend the product. The main findings of this research are several propositions, namely: in-store promotion, visual merchandising and store image directly affect customer value and purchase intention. The following propositions are: In-store promotion, visual merchandising and store image influence purchase intention mediated by customer value.

Effective & near real-time track-to-track association for large sensor data in Maritime Tactical Data System

        Nurfalah Adiyasa,Supangkat Suhono Harso,Mulyana Eueung 한국통신학회 2024 ICT Express Vol.10 No.2

The Maritime Tactical Data System is a software system that collects track data from maritime sensors to compile and show on a sea map to provide maritime patrol vessels or maritime surveillance stations with situational awareness. Speed and precision in tracking multiple targets are crucial in achieving situational awareness. A multi-target tracking problem is NP-hard if it involves more than two sensors and a large amount of data since it generates many potential solutions that must be evaluated. Previous research has demonstrated that the Density-based Spatial Clustering of Applications with Noise (DBSCAN) method can perform track-to-track association with pretty good results; nevertheless, the density-reachable concept of DBSCAN poses a problem when two targets are within a distance less than the threshold. Another limitation is the inability of DBSCAN to associate tracks as soon as sensor track data is received. DBSCAN must run after all data has been collected in a database. In this paper, a novel track-to-track association method called Neighborhood Clustering Track Association and Fusion (NCTAF) is proposed to address the limitations of DBSCAN. According to the experiment results, NCTAF overcame the inaccurate cluster form generated by DBSCAN. The most remarkable result is that NCTAF performs track associations in an average of one second after receiving sensor track data involving three sensors, 4000 track data per sensor, and an update rate of 5-12 s per sensor. In contrast, DBSCAN required more than 10 min for the same scenario.
