Background: Antiemetic triplet therapy including dexamethasone (DEX) is widely used for patients receiving highly emetogenic chemotherapy (HEC). In Japan, the appropriate dose of DEX has not been established for this combination. Materials and Methods: To assess the efficacy and safety of increased-dose DEX, we retrospectively examined patients receiving HEC with antiemetic triplet therapy. Results: Twenty-four patients (fosaprepitant group) were given an increased-dose of DEX (average total dose: 45.8mg), fosaprepitant, and 5-HT3 antagonist. A lower-dose of DEX (33.6mg), oral aprepitant, and 5-HT3 antagonist were administered to the other 48 patients (aprepitant group). The vomiting control rates in the fosaprepitant and aprepitant groups were 100% and 85.4% in the acute phase, and were 75.0% and 64.6% in the delayed phase. The incidences of toxicity were similar comparing the two groups. Conclusions: Triplet therapy using an increased-dose of DEX is suggested to be safe and effective for patients receiving HEC.

More viscous fuels such as residual oil tend to be burned in boilers. However, the use of high viscous fuels in conventional plants will degrade combustion characteristics, such as combustion efficiency and emission characteristics, which greatly depend on the fuel spray characteristics. The present study investigated the effects of the fluid viscosity and the atomizing air flow rate on the spray characteristics of a Y-jet atomizer. The test liquid and atomizing gas were aqueous glycerol solutions and air, respectively. Spray shape was visualized at different axial positions by using short-exposure backlight method. Droplet size and velocity were measured by a Phase Doppler Particle Analyzer (PDPA).The results show that the spray shape was similar to hollow-cone regardless of viscosity and air flow rate. Spatial distribution of total droplet volume measured by PDPA per minute had the maximum value at the opposite side of test liquid supply line. The largest Sauter mean droplet diameter (SMD) existed at the side of test liquid supply line. SMD increased with the test liquid viscosity. On the other hand, as the atomizing air flow rate was increased, SMD decreased.

<P>We present a spectroscopic analysis of the metallicities, ages, and alpha-elements of the globular clusters (GCs) in the giant elliptical galaxy (gE) NGC 4636 in the Virgo Cluster. Line indices of the GCs are measured from the integrated spectra obtained with Faint Object Camera and Spectrograph on the Subaru 8.2 m Telescope. We derive [Fe/H] values of 59 GCs based on the Brodie & Huchra method, and [Z/H], age, and [alpha/Fe] values of 33 GCs from the comparison of the Lick line indices with single stellar population models. The metallicity distribution of NGC 4636 GCs shows a hint of a bimodality with two peaks at [Fe/H] = -1.23(sigma = 0.32) and -0.35(sigma = 0.19). The age spread is large from 2 Gyr to 15 Gyr and the fraction of young GCs with age < 5 Gyr is about 27%. The [alpha/Fe] of the GCs shows a broad distribution with a mean value [alpha/Fe] approximate to 0.14 dex. The dependence of these chemical properties on the galactocentric radius is weak. We also derive the metallicities, ages, and [alpha/Fe] values for the GCs in other nearby gEs (M87, M49, M60, NGC 5128, NGC 1399, and NGC 1407) from the line index data in the literature using the same methods as used for NGC 4636 GCs. The metallicity distribution of GCs in the combined sample of seven gEs including NGC 4636 is found to be bimodal, supported by the KMM test with a significance level of > 99.9%. All these gEs harbor some young GCs with ages less than 5 Gyr. The mean age of the metal-rich GCs ([Fe/H] > -0.9) is about 3 Gyr younger than that of the metal-poor GCs. The mean value of [alpha/Fe] of the gE GCs is smaller than that of the Milky Way GCs. We discuss these results in the context of GC formation in gEs.</P>
