A 59-year Korean woman had suffered from vaginal spotting for three months and underwent TLH (total laparoscopic hysterectomy) with BSO (bilateral salpingo-oophorectomy) on August 11, 2011 at other University affiliated hospital. The final histopathological results showed totally unexpected malignancy of the left salpinx. And then, she visited my office to discuss the further evaluation and better management. There were no abnormal findings of her serum tumor markers, pelvic MRI scan, and PET-CT scan (in the preoperative evaluations). She underwent laparoscopic restaging surgery including laparoscopic pelvic lymphadenectomy and laparoscopic para-arotic lymphadectomy on August 30, 2011 Firstly, right laparoscopic pelvic lymphadenectomy was performed. And then, laparoscopic para-aortic lymphadenectomy up to the left renal vein level, presacral lyhadenectomy and left pelvic lymphadenectomy were performed. Next, laparoscopic omentectomy using a harmonic shears and laparoscopic appendectomy using an endoscopic linear cutter with white cartilage were performed. The final histopathological results showed periappendiceal metastatic nodule, omental metastasis and three out of 22 para-aortic lymph nodes metatsasis. She received adjuvant chemotherapy and healthy since then.

The primary objective of this study was to compare cosmetic outcomes and patients` satisfaction between women underwent single port laparoscopic surgery (SP-LS) or conventional laparoscopic surgery (conventional-LS). The secondary objective was to evaluate the cosmetic effect of SP-LS in women with abdominal scars from prior surgeries Patients who were followed up after SP-LS or conventional-LS for gynecologic disease were included. Cosmetic outcome was evaluated at least six months after surgery. Objective and subjective scar analysis were performed using the Vancouver scar scale, the patient and observer scar assessment scale (POSAS), and a visual analog scale (VAS). Overall satisfaction with the surgery was evaluated with VAS. 130 women were included in this study, with 65 women in each group. No difference in objective scar assessment for only umbilical scar was showed. In the subjective scar assessment and satisfaction with the surgery, SP-LS group was more satisfied. SP-LS group with abdominal scar had higher satisfaction in overall satisfaction with the scar SP-LS offers better cosmesis and overall patients` satisfaction with surgery in women with gynecologic disease. SP-LS also seems to have considerable cosmetic benefit in women with abdominal scars from prior surgeries.

This study aimed to evaluate the safety and surgical outcomes of laparoscopically assisted vaginal hysterectomy(LAVH) for women with anterior wall adherence after Cesarean section. We analyzed a retrospective chart review of 328 women who had prior Cesarean section history who underwent LAVH from March 2003 to July 2013, selected from a total of 2,034 women who underwent LAVH during that period. We divided the women into Group A, with anterior wall adherence, and Group B, without anterior wall adherence. Mann-Whitney test and chi-square test were used to compare differences between Group A and B. p<0.05 was considered statistically significant. Of the total 328 patient, forty-nine(14.9 %) patients had anterior wall adherence. Analysis of the clinical characteristics of the two groups showed that they did not differ significantly in terms of age, parity,BMI, the number of Cesarean section, indications for hysterectomy, and previous abdominal operations other than Cesarean section. Analysis of the surgical results showed that they did not differ significantly in the postoperativechange in the hemoglobinconcentration, hospital stay, or surgical complications. There was significant differences of the operating time between two groups. There were two cases of bladder laceration during a vaginal procedure,one in group A and the other one in group B.The both lacerations were repaired vaginally. There was no conversion to abdominal hysterectomyin both groups. LAVH is effective and safe for women with anterior wall adherence after Cesarean section.

<P>The histamine H4 receptor (H4R), a member of the G-protein coupled receptor family, has been considered as a potential therapeutic target for treating atopic dermatitis (AD). A large number of H4R antagonists have been disclosed, but no efficient agents controlling both pruritus and inflammation in AD have been developed yet. Here, we have discovered a novel class of orally available H4R antagonists showing strong anti-itching and anti-inflammation activity as well as excellent selectivity against off-targets. A pharmacophore-based virtual screening system constructed in-house successfully identified initial hit compound <B>9</B>, and the subsequent homology model-guided optimization efficiently led us to discover pyrido[2,3-<I>e</I>]tetrazolo[1,5-<I>a</I>]pyrazine analogue <B>48</B> as a novel chemotype of a potent and highly selective H4R antagonist. Importantly, orally administered compound <B>48</B> exhibits remarkable efficacy on antipruritus and anti-inflammation with a favorable pharmacokinetic (PK) profile in several mouse models of AD. Thus, these data strongly suggest that our compound <B>48</B> is a promising clinical candidate for treatment of AD.</P> [FIG OMISSION]</BR>
사람 난포에서 angiotensin 2에 의한 steroidogenic pathway의 조절

Objective: The exact mechanism of angiotensin II to steroidogenesis is still speculative in spite of many researches especially in human and these were performed indirectly with serum or follicular fluid. Under the hypothesis that ovarian RAS increases androgen, decreases progesterone synthesis in normal human ovary, we investigated the exact action of angiotnesin II on human ovary. Methods: After appliance of angiotensin II and saralasin to the normal human ovarian follicles, we measured sex steroids like progesterone, testosterone, DHEA and enzymes like HSD3β2, CYP 17 to see the action of angiotensin II and its antagonist, saralasin. The results were analyzed by ANOVA test. Results: Angiotensin II increased androgen synthesis but did not affect progesterone synthesis. There were no difference of HSD 3β2 mRNA expression in angiotensin II and saralasin group compared with control group. The expression of CYP17 mRNA was increased by angiotensin II but did not reach statistically significant level. Conclusion: Angiotensin II could increase androgen production probably via overexpression of CYP17, but had no efffect on progesterone production.
일광 노출과 관련된 표피 종양에서 MAPK 발현에 관한 면역조직화학적 연구

N/A Background : Mitogen-activated protein kinase(MAPK) is an important molecule in transducing extracellular signal from cell surface to the nucleus. MAPK family includes ERK(extracellular signal-regulated protein kinase), JNK(stress-activated c-Jun N-terminal kinase), p38 kinases. Not only various growth factors and cytokines, but also other signals such as UV light are able to activate MAPK, resulting in various cellular responses including proliferation, differentiation and apoptosis. Objective : The purpose of our study was to determine patterns of MAPK expression in epidermal tumors including seborrheic keratosis(SK) on sun-exposed skin and unexposed area, actinic keratosis(AK), Bowen`s disease(BD), acantholytic squamous cell carcinoma(ASCC), and other squamous cell carcinoma(SCC). Methods : Using pan ERK, JNK-2 and p-JNK, we have examined MAPK expression immunohistochemically in epidermal tumors(total 30 cases) including SK on sun-exposed skin(5 cases) and unexposed areas(5 cases), AK(5 cases), BD(5 cases), ASCC(5 cases), and SCC(5 cases). Results : ERK was expressed in AK and ASCC positively but not in BD, and the staining pattern with ERK also showed positivity in microinvasive area and dysplastic cells of SCC. JNK was not expressed in all tumor cells of SK, AK, BD, and ASCC with the exception of dyskeratotic cells in BD and acantholytic cells in ASCC. Conclusions : We suggest that ERK expression may be related to sun exposure and responsible for proliferation of tumor and invasion into surrounding tissue. JNK may not be related with malignant progression of tumor and carcinogenesis cauesd by sun exposure, but with apoptosis of tumor cells. (Korean J Dermatol 2003;41(5) : 578~585)

This paper presents a simple but valid loss calculation method of grid-connected photovoltaic system based on normalized yield model. The proposed method can be represented as a quantitative value for five losses and performance of grid-connected photovoltaic system with three years monitored data. These results will indicate that it is useful to investigate various loss factors causing the performance obstruction, enhance the lifetime yield for changing meteorological conditions, and determine the optimal design and performance improvement of grid-connected photovoltaic system.

This paper presents a simple but valid design factor calculation method of grid-connected photovoltaic system using normalized yield model. The proposed calculation method can be represented as a quantitative value about five design factors from irradiance to system output power. The validity of this method is identified by analyzing design factor with three years monitored data. These results will indicate that it is useful to determine the optimal design and selection of grid-connected photovoltaic system to meet different user purposes and enhance long-term reliability and stability of grid-connected photovoltaic system.
