      • Effect of nonpharmacological interventions for the prevention of delirium in the intensive care unit: A systematic review and meta-analysis

        <P><B>Abstract</B></P> <P><B>Purpose</B></P> <P>We aimed to classify nonpharmacological interventions used for preventing delirium in the intensive care unit (ICU), and estimate their effect size.</P> <P><B>Materials and methods</B></P> <P>In this systematic review and meta-analysis, the literature was searched and studies were selected based on the PRISMA flow chart. Data sources included MEDLINE, Cochrane, CINHAHL, PsyInfo, and EMBASE. We used Cochrane's Effective Practice and Organisation of Care (EPOC) criteria in study design and quality assessment of the meta-analysis.</P> <P><B>Results</B></P> <P>This systematic review and meta-analysis included 35 and 15 studies, respectively. Studies were grouped into nine intervention types: multicomponent (16 studies), physical environment (9), daily interruption of sedation (2), exercise (2), patient education (2), automatic warning system (1), cerebral hemodynamics improving (1), family participation (1), and sedation reducing protocol (1). The effect size of preventive nonpharmacological interventions had an odds ratio (OR) of 0.66 (95% confidence interval [CI], 0.50–0.86) for delirium occurrence, and an OR of 0.31 (95% CI, 0.10–0.94) for delirium duration. Although relevant studies by interventions were lacking, a partial subgroup analysis by intervention was performed.</P> <P><B>Conclusions</B></P> <P>Nonpharmacological interventions were effective in reducing the duration and occurrence of delirium. Consistent application and development of nonpharmacological interventions for use in the ICU are important.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Systematic review of preventive nonpharmacological interventions for ICU delirium </LI> <LI> Nonpharmacological interventions reduced delirium duration and occurrence. </LI> <LI> However, they did not reduce the length of ICU stay and ICU mortality rates. </LI> <LI> Findings provide basic data for the development of interventive measures. </LI> </UL> </P>

        Effect of Wall Thickness on Thermal Behaviors of RC Walls Under Fire Conditions

        The objective of this paper is to investigate the effect of thickness and moisture on temperature distributions of reinforced concrete walls under fire conditions. Toward this goal, the first three wall specimens having different thicknesses are heated for 2 h according to ISO standard heating curve and the temperature distribution through the wall thickness is measured. Since the thermal behavior of the tested walls is influenced by thickness, as well as moisture content, three additional walls are prepared and preheated to reduce moisture content and then tested under fire exposure. The experimental results clearly show the temperatures measured close to the fire exposed surface of the thickest wall with 250 mm thickness is the highest in the temperatures measured at the same location of the thinner wall with 150 mm thickness because of the moisture clog that is formed inside the wall with 250 mm of thickness. This prevents heat being transferred to the opposite side of the heated surface. This is also confirmed by the thermal behavior of the preheated walls, showing that the temperature is well distributed in the preheated walls as compared to that in non-preheated walls. Finite element models including moisture clog zone are generated to simulate fire tests with consideration of moisture clog effect. The temperature distributions of the models predicted from the transient heat analyses are compared with experimental results and show good agreements. In addition, parametric studies are performed with various moisture contents in order to investigate effect of moisture contents on the thermal behaviors of the concrete walls.

      • Embracing the new vulnerable self: A grounded theory approach on critical care survivors’ post-intensive care syndrome

        <P><B>Abstract</B></P> <P><B>Objectives</B></P> <P>This study explored critical care survivors’ experience of post-intensive care syndrome.</P> <P><B>Research design and setting</B></P> <P>This qualitative study employed a classic grounded theory approach with 13 survivors after intensive care unit treatment.</P> <P><B>Main outcome measures</B></P> <P>Data were collected through one-to-one in-depth interviews. Open, axial and selective coding were performed based on interview transcripts, field notes, and memos.</P> <P><B>Findings</B></P> <P>The analysis of survivors’ experience with post-intensive care syndrome led to the ‘<I>embracing the new vulnerable self</I>’ theory. After discharge from the intensive care, survivors became physically, psychologically, cognitively and socially vulnerable. The survivors were struggling for recovery; they faced new crises and some were devastated. However, their internal and external resources helped them recover. Through coping processes, survivors gained a new perspective on normality.</P> <P><B>Conclusion</B></P> <P>These results contribute to understanding the post-intensive care syndrome phenomenon and coping process of critical care survivors. This syndromes is a multidimensional phenomenon and the recovery process is a continuum. An integrated rehabilitation program that extends beyond utilising family support requires development.</P>

        Xunzi"s Theory of Ethics and Universality Thesis from a Stand point of Virtue Ethics

          순자철학은 서구의 덕 윤리학(virtue ethics)와 유사성을 지니고 있다. 또한 순자 철학은 현재 지적되는 덕 윤리학의 한계의 일부분을 극복할 논점도 제시할 수 있다.<BR>  덕 윤리학에서 인간존재는 구체적이고 특정한 문화전통에 뿌리를 둔 인간 존재일 뿐, 보편적 의무 개념에 지배 되는 추상적 인간 존재를 가정하지 않는다. 그리고 덕의 개념 역시 그 시대 사회 구조와 전통 가치에 의해서 정의된다. 그러므로 덕 윤리학은 특정 시대, 문화에 국한되어진다는 한계성을 가진다. 덕 윤리학이 특정한 문화의 한계성을 벗어나 보편성을 지니기 위해 어떻게 할 것인가? 이 부분에 대해 덕 윤리학자들은 순자윤리학의 구조를 주시할 필요가 있다.<BR>  먼저 필자는 순자윤리학과 서구 덕 윤리학의 유사한 부분을 설명한다. 다른 유가 윤리학에서 보이듯 순자윤리학에서도 역시 한 인간이 공동체 속에서 자신의 사회적 역할에 맞는 사회적 의무를 습득하고 준수하는 과정에서 덕을 습득한다고 본다. 또한 도덕함양의 과정에서 전통을 전수하는 교사의 역할을 중시하는데 순자 윤리학에서 전통과 공동체를 연결시키는 점은 덕 윤리학과 유사하다.<BR>  그리고 순자윤리학에서 덕윤리학의 한계를 뛰어넘는 보편성에 관한 문제를 탐구하기 위해 인간본성론과 수양, 성인, 예에 대한 논의를 살펴본다. 성인은 도덕성의 기초를 세우기 가능한 존재로 성인을 공동체 내의 전통전수의 기반으로 세우려한 점이 순자 윤리학의 장점이다.<BR>  필자는 순자 윤리학이 인간 본성 안에 덕성이 실현될 가능성을 존재론적으로 남겨둔 점, 예라는 사회적 장치와 사회 도덕의 기초로서 성인의 존재에 대한 설명에서, 특수한 사회, 문화의 한계성을 뛰어 넘는 보편성을 담지하고자 했고, 이 점이 서구 덕윤리학의 한계를 극복할 이론적 틀을 제공한다고 본다.   Xunzi"s philosophy has common threads with that of Western virtue ethics. Xunzi"s philosophy, however, presents its own that go beyond the limits of virtue ethics.<BR>  In virtue ethics, human beings are viewed as rooted in particular cultural traditions and thus not as abstract individuals governed by universally applicable concept of obligation. Furthermore, the concept of “virtue” itself is defined specifically within the boundary of social structures and traditional values of a particular era. In view of this, virtue ethics has its own congenital limitation that it is just one ethics in a local culture and community, thus losing the ground for claiming universally applicable governing ethics of human beings beyond the limits of time and space. To begin with, author explains similarities between Xunzi"s ethics and virtue ethics. According to Xunzi, students of texts and rituals learn directly a teacher who has already achieved understanding and knowledge of Confucian tradition. Rituals are important guides in organizing society and in the process of moral cultivation.<BR>  The author that especially in his ethics that makes for realization of virtues within human nature, in his emphasis on rituals as social contrivances and in exploration of the existence of sage, may provide theoretical frames that may limits of virtue ethics.
