Background and Objectives : When p53 cancer suppressor gene, occurs gene deletion or point mutation, malignancy develops by loss of p53 function with abnormal p53 protein. The Epstein-Barr virus (EBV) is the causative agent of certain type of lymphoma and undifferentiated nasopharyngeal carcinoma. However, the role of EBV as a causative factor in other head &eck tumors is not fully elucidated except nasopharyngeal carcinoma. This study, the author examined that p53 expression and detection rate of EBV correlate to development of laryngeal and hypopharyngeal squamous cell carcinoma(SCC) and play the possible role of prognostic indicators. Materials and Methods : Formalin-fixed, paraffin-embedded tissue specimen from 32 cases of larynx and hypopharyngeal squamous cell carcinomas were studied by immunohistochemical staining for p53 and EBV. The results of analysis were compared with clinicopathological parameters. Results : p53 expression was 56.3% (18 cases) and the detection rate of EBV was 43.6% (14 cases) of 32 cases in laryngeal and hypopharyngeal squamous cell carcinomas. p53 expression was correlate with histologic grade (p<05) only. Detection of EBV was not correlate with clinicopathological parmeters. Conclusion : These results suggest p53 expression and the detection of EBV may be related with development of laryngeal and hypopharyngeal squamous cell carcinoma. And, the expression of p53 protein can be used as a prognosticator in laryngeal and hypopharyngeal squamous cell carcinoma under certain limitation.
위험평가(Risk Assessment)에 의한 업종별 위험(Risk)의 구분

A total of 514 workplaces in 17 industries were inbestigated through walk-through survey. These industries were all small companies with less than five workers and didn't have work environmental measurement data. These industries were ranked by risk that was evaluated by combination of toxicity and possibility of exposure to chemical hazardous agents produced at operations of industry. Risk Index(RI) was qualitatively combined by the combination between hazard and potential of exposure to chemical hazardous agents. Industries that were regarded as having the highest risk were wood and products of wood, chemicals and chemical products, basic metals, other vehicles, trailers and semi-trailers, and fumiture manufacturing. These industries were found to have operations with higher risk than other industries. This study found that more attentions should be paid to these industries with that more attentions should be paid to these industries with high risk.

Background: Herpes zoster (HZ) infection can significantly impair the quality of life of the affected individuals, and its treatment imposes a considerable cost burden on the health-care system and on society at large. However, there is little information on the perception of this disease and the acceptability of vaccines in Korea. Objective: The aim of this study is to determine the awareness of HZ and its vaccine, and to identify factors associated with the acceptability of the HZ vaccine among outpatients of dermatology clinics. Methods: A questionnaire-based survey was conducted on 607 outpatients who visited the dermatologic clinics. Results: The responses of the patients revealed a high awareness of HZ (85.4%) but a relatively low knowledge about HZ and its vaccine (42.3%). The HZ vaccination rate among patients aged ≥50 years was 9%. A history of HZ infection, being older, and greater knowledge about HZ and its vaccine were associated with a higher HZ vaccine acceptability. Of those who had not been vaccinated, 58.3% were interested in receiving the vaccine. The most frequent reason for this interest was “severe sequelae,” followed by “knowing someone who has HZ” and “recommendation from a doctor.” High cost was the most common reason for unwillingness to receive the vaccination. Conclusion: Despite a high awareness of HZ, vaccine acceptability was extremely low among this study cohort. Vaccination acceptability would be improved by encouraging doctors to educate elderly patients about the disease and the availability of vaccination, and by the provision of insurance coverage for HZ vaccination. Background: Physicians can play a crucial role in the knowledge that patients have about a disease and its prognosis. Recently, patients with atopic dermatitis (AD) are increasingly turning from western medicine to oriental herbal medicine. However, their awareness of AD and attitude toward Western medicine and oriental herbal medicine clinics are scarcely reported. Objective: The aim of this study was to determine the understanding of AD among patients and their parents and to identify their awareness of and attitude toward Western medicine and oriental herbal medicine as treatments for AD. Methods: An online questionnaire was administered to 500 consenting respondents with AD (age, 16∼49 years) and parents of children with AD (age, 0∼15 years). Results: The mean percentage of correct answers to questions about AD was 52.54%. A parental history of AD was independently associated with higher respondent`s knowledge about the disease and its treatment. The satisfaction with treatment outcomes was highest among patients treated at private clinic of dermatology specialists (49.4%), while lowest among those treated at oriental herbal medicine clinics (38.4%). Many participants were aware that oriental herbal medicine requires a longer treatment period for a cure and does not burden the skin, while steroid phobia was seen in most of participants. Conclusion: Physicians need to educate AD patients and their parents about the disease and its treatment. Misconceptions for Western medicine and oriental herbal medicine among AD patients and parents should be corrected to improve their prognosis. (Ann Dermatol 28(6) 725∼732, 2016)
The Mass Infection of COVID-19 in Daegu City of Korea: Vascular Surgeons’ Perspective

Since February 18, 2020, the number of patients with COVID-19 has increased rapidlyafter the infection was initially confirmed in Daegu. Daegu city subsequentlybecame the largest epicenter of the coronavirus outside China. In the early daysof the outbreak of mass infection, Daegu citizens were in various difficult circumstances. Vascular surgery in Daegu was also affected as much as other medicalfield. Most of elective schedule were canceled, and the vascular surgeons cooperatedto utilize the limited medical resources in Daegu region. Fortunately, the citizensof Daegu city could overcome the first crisis because of matured citizenship,sufficient medical resources and the timely-supplied surplus bed for patient isolation. Now all humanity today is interdependent and we are all one community inthe pandemic. To overcome pandemic, it is necessary to respect and share experiencesin various places at the beginning of the crisis.

A hybrid operation in a patient with complex right subclavian artery aneurysm

We report a hybrid surgery including endovascular aneurysm repair and debranching procedures to treat a patient with a complex right subclavian artery aneurysm. The patient was a 70-year-old woman who presented with dry cough and hoarseness. The aneurysm was characterized by the absence of a proximal neck, and involvement of the origin of the right vertebral artery. She underwent carotid-vertebral artery bypass, stent graft from the innomiate artery to the common carotid artery and carotid-axillary artery bypass. Great saphenous vein was used for the carotid-vertebral artery bypass and 7 ㎜ reinforced polytetrafluoroethylene graft was used for the carotid-axillary artery bypass. The postoperative course was uneventful.
