Impaired Diastolic Recovery after Acute Myocardial Infarction as a Predictor of Adverse Events

<P><B>Background</B></P><P>To investigate the impact of left ventricular (LV) diastolic functional recovery on major adverse cardiac events (MACE) 6 months after acute myocardial infarction (AMI) in patients with preserved LV systolic function.</P><P><B>Methods</B></P><P>A total 463 patients with preserved LV systolic function at 6 months after an AMI were divided into two groups based on the extent of diastolic recovery assessed by echocardiography: group I (n = 241) showed improving diastolic function and group II (n = 222) did not. MACE included death, recurrent myocardial infarction, and rehospitalization due to heart failure, and these events were compared with the patients' characteristics at baseline.</P><P><B>Results</B></P><P>Compared with group I, the patients in group II were older and had a higher prevalence of hypertension and diabetes. Blood levels of hemoglobin and triglyceride were lower in group II, whereas the levels of N-terminal pro-B-type natriuretic peptide (NT-proBNP) and of high-sensitivity C-reactive protein were higher in this group than in group I. MACE were significantly more frequent in group II than in group I. Age, elevated NT-proBNP, and impaired diastolic recovery were significant independent predictors of MACE.</P><P><B>Conclusion</B></P><P>Despite improvement in LV systolic function, LV diastolic function had not improved in 222 patients (47.9%) by the 6-month follow-up after the index AMI, and impaired diastolic functional recovery was found to be an independent predictor of MACE. Evaluation of diastolic function would be a useful way to stratify risk in patients discharged after an index AMI.</P>

This study aimed to investigate correlations between concentrations of extractable Arsenic (As) with varying chemical solutions (0.1 M $Ca(NO_3)_2$, 0.1 M $(NH_4)2HPO_4$, 0.5 M EDTA, Mehlich 3, and 0.5 M $NaHCO_3$) and those of As in crops, and then to seek the most suitable soil extraction method for predicting the potential of As uptake in crops cultivated in soils contaminated with As. For a mesocosm experiment, pepper (Capsicum annuum L.), soybean (Glycine max L.), and rice (Oryza sativa L.) were cultivated for three months in pots containing soils taken from the arable areas near abandoned mines in Korea. Following the cultivation, soil pH and DOC significantly increased by treatments of lime and lime plus compost, respectively, while insignificant influences in changing total and all extractable As concentrations were found in all soils. Arsenic concentration in edible part of all crops considerably depended on the extractable As concentration in the soils, particularly with Mehlich 3. All extractable As concentrations in the soils of C. annuum and G. max were significantly correlated with As concentration in their edible parts. For O. sativa, the extractable concentrations of Mehlich 3 ($R^2$: 0.18 at p: 0.006) and EDTA ($R^2$: 0.11 at p: 0.036) showed only marked relationships with As concentration in the edible part. These results may indicate that the Mehlich 3 and EDTA are soil extractants to determine phytoavailable As in soil that provide better prediction for As transfer from soil to crop.

The MuSI is known as a multiple stress resistant gene with several lines. A previous study using RT-PCR showed that the expression of MuSI gene in tobacco plant induced its tolerance to Cd stress. This study was conducted to examine the enhanced Cd tolerance of the MuSI transgenic tobacco plant through germination test and to understand the role of the involved antioxidant enzymes for the exhibited tolerance. Germination rate of MuSI transgenic tobacco was more than 10% higher than that of wild-type tobacco, and seedlings of MuSI transgenic tobacco grew up to 1.6 times larger and greener than seedlings of wild-type tobacco at 200 and 300 μM Cd. From the third to the fifth day, CAT activities at 100 and 200 μM Cd and APX activities at 100, 200 and 300 μM Cd of MuSI transgenic tobacco were up to two times higher than those of wild-type tobacco. MuSI gene is shown to enhance the activities of antioxidant enzymes resulting in higher tolerance to oxidative stress compared with the control plant.

Study Design : This study assessed the final functional results after treatment of thoracolumbar burst fractures and compared the relationship between the results and the parameters of reduction and state of the fractures. Objectives : To define prognostic factors affecting the final results and to present some precautions to minimize the treatment failure. Summary of Literature Review : In the treatment of the thoracolumbar burst fractures in which flexion loads are predominant, the sagittal contour is crucial to achieve permanent pain-free stability, but definitive therapeutic guidelines have remained a controversal topic. Materials and Methods : We reviewed 37 thoracolumbar burst fractures with an average follow up period of 1.8 years: group I consisting of 20 cases treated conservatively and group 2 consisting of 17 cases treated surgically with posterior instrumentation. Finally functional results were analyzed with the Denis' pain and work scores, and were compared between groups on anterior body height and local kyphosis. Results : A satisfactory pain score less than or equal to P3 was in 15(75%) in group 1 and 15(88.2%) in group 2(p>0.05), but satisfactory work score less than or equal to W3 was in 12(60%) in group I and in 14(82.4%) in group 2(p<0.05). But, some loss of body height and local kyphosis in group 1 was not reversely related with functional outcomes. Eight cases in group 1 showing unsatisfactory result in work scores were analyzed as 4 osteoporosis(Jikei grade I, Ⅱ /Ⅲ), 3 associated compression fracture of the contiguous vertebra and one combined osteoporosis and compression fracture, showing significant loss of vertebral height and increase of kyphosis(p<0.01). Conclusions : Functional results of group 1 showing loss of vertebral height less than 50% and increased kyphosis less than 200 were comparable to those of group 2. The osteoporosis and associated compression fracture of adjacent vertebra were the risk group to develop posttraumatic kyphosis and might be added to the surgical indication of the thoracolumbar burst fractures.
