        갑상선 결절의 수술 범위 결정시 세침흡인세포검사와 동결절편생검의 유용성

        Purpose: Traditionally the extent of thyroidectomy in patients with nodular thyroid disease has been based on fine needle aspiration cytology and intraoperative frozen section examination. The value of routine frozen section examination for intraoperative diagnosis of thyroid cancer and determination of extent of thyroidectomy is controversial and needs to be evaluated. Methods: We reviewed the fine needle aspiration cytology, frozen section examination, and final pathology of 142 consecutive patients who underwent thyroidectomy for nodular thyroid mass in an 3-year period. The diagnosis were classified as indeterminant, benign, or malignant. The utility and impact of the diagnosis from fine needle aspiration or frozen section on the operative procedure performed was analyzed. Results: Fine needle aspiration cytology (FNA) as a diagnostic test for thyroid nodules demonstrated an indeterminant rate of 23.9% (34 patients), with diagnostic accuracy of 93.9% for malignant disease. In frozen section (FS) results, the indeterminant rate was 19.7%, and the diagnostic accuracy 98.3% for malignant disease. Of the 50 patients with the benign results on FNA, 3 patients was diagnosed as malignancy on FS. Of the 34 patients with indeterminant results on FNA, the intraoperative FS diagnosis showed 16 patients of benign, 3 patients of malignancy, and diagnosis on 15 patients was deferred to permanent section; in 15 patients, benign disease was diagnosed in 12 patients, and 3 patients were diagnosed as malignancy. And one of 10 patients with inadequate result on FNA was diagnosed as malignancy on FS. Therefore the decision about the extent of surgical thyroid resection was changed in 7 patients (4.9%) based on the FS results, and including the 16 patients diagnosed as benign on FS with indeterminant results on FNA, overall, in 23 patients (16.2%) the intraoperative surgical decision was affected by FS. Conclusion: The fine needle aspiration cytology and intraoperative frozen section examination showed the limitation on diagnosis of follicular neoplasm, but intraoperative frozen section examination proved useful in determining the extent of operation and affected intraoperative decision making in thyroid surgery.       • 사람 태반조직 Peroxidase의 부분정제 및 특성에 관한 연구

        The purification and some properties of peroxidase of human term placenta was made in this study. The activity of peroxidase was measured by the method of Pu‥tter(1974). The results were summarized as follows; 1. The activity of peroxidase in cytosolic fraction of human term placenta was found to be 31.09 units per g wet tissue. 2. The peroxidase from human term placenta was purified about 22-fold by ammonium sulface precopitation and DEAE-cellulose column chromatography. 3. The most rapidly oxidized substrate was o-dianisidine. Benzidine, ascordic acid and guaiacol were oxidized with decreasing order of rats, while oxidation of dihydrooxyphenols and trihydrooxyphenol were very slow. 4. The partially purified peroxidase was strongly inhibited by i.omM of sulfhydryl reagents and cyanide. 5. The optimal tempreature for enzyme astivity was 40℃ and the optimal pH was 5.5. 6. The half life of the enzyme astivity was 30 min. at 60℃. 7. the Km values for hydrogen peroxid and guaiacol of peroxidase were 0.50mM and 0.44mM, respectively. 8. The purified enzyme was a typical hemoprotein.

      • Dynamics of connexin expression in the lycopene treated oocytes of Korean native cow

        Ovarian folliculogenesis and the production of fertilizable oocytes depend on gap junctional intercellular communication within both the developing and the mature follicle. Gap junctions connect oocytes with granulosa cells and granulosa cells with each other. Various nutritional bio-molecules are known to be transferred to the growing oocyte from the granulosa cells via gap junction. Signals that regulate meiotic maturation of fully-grown oocytes pass through the oocyte-granulosa cell gap junctions. Gap junctions also play a critical role in regulating uterine blood flow, contributing to the maternal recognition and also implantation during pregnancy. Due to the challenge of various stressors the in vitro embryo developmental potentials are still suboptimal compared to in vivo. To identify the molecular mechanism of these stressors and to improve the existing embryo developmental potentials, the singlet oxygens quencher lycopene was added to the culture media to counterbalance the oxidative damage caused by ROS. In this study, we have patterned connexin like Cx43, Cx37, Cx32 and Cx26 at protein and transcription level during follicular growth, atresia and blastocyst stage by using immunohistochemistry, conventional PCR and RT-qPCR. Lycopene (0.2 μM) significantly (P < 0.05) increased the gap junctional communication protein (connexin) expression of Cx43, Cx37, Cx32, Cx26 as compared to the control group at both transcription and translation level during follicular growth, atresia and blastocyst stage. Lycopene potentiates ovarian folliculogenesis, provides the production of fertilizable oocytes and improved embryo developmental capabilities by increasing gap junctional intercellular communication.
