      • Cytosolic Hsp60 orchestrates the survival and inflammatory responses of vascular smooth muscle cells in injured aortic vessels

        <P><B>Aims</B></P><P>Pro-inflammatory response of vascular smooth muscle cells (VSMCs) is triggered by endothelial damage and a causative step for thrombosis and neointimal thickening in the injured arterial vessels. Therefore, we investigate a role of cytosolic Hsp60 as a novel pro-inflammatory mediator in VSMCs.</P><P><B>Methods and results</B></P><P>Hsp60 was detected in the cytosol of VSMCs. The selective depletion of cytosolic Hsp60 in VSMCs reduced the IκB kinase activation, repressed the induction of nuclear factor (NF)-κB-dependent survival genes (MnSOD and Bfl-1/A1), and enhanced apoptotic death in response to TNF-α. Moreover, a quantitative RNA sequencing revealed that the expression of 75 genes among the 774 TNF-α-inducible genes was significantly reduced by the depletion of cytosolic Hsp60. In particular, the expression of pro-inflammatory cytokines/chemokines, such as CCL2, CCL20, and IL-6, was regulated by the cytosolic Hsp60 in VSMCs. Finally, the depletion of cytosolic Hsp60 markedly inhibited the neointimal thickening in the balloon-injured arterial vessels by inducing apoptotic cell death and inhibiting chemokine production.</P><P><B>Conclusions</B></P><P>This study provides the first evidence that cytosolic Hsp60 could be a therapeutic target for preventing VSMC hyperplasia and inflammatory response in the injured vessels.</P>

        Association Between Hearing Level and Mental Health and Quality of Life in Adults Aged >40 Years

        Background and Objectives: In this study, we investigated the effects of hearing loss on mental health and quality of life (QoL) using survey data in adults aged >40 years.Subjects and Methods: We obtained data from 10,921 individuals who responded to the fifth Korean National Health and Nutrition Examination Survey. Primary outcomes were measured using questionnaires that recorded stress perception, depressive mood, and suicidal ideation to evaluate mental health and motor ability, self-management, activities of daily living, pain or discomfort, and anxiety or depression to evaluate QoL.Results: On multivariate analysis, suicidal ideation was more prevalent among older adults with hearing loss than in older adults without hearing loss. Motor ability was lower in individuals with hearing loss of >20 dB than in older adults with normal hearing. Categorization of the EuroQol-5 Dimension (EQ-5D) health state into upper and lower groups showed that the EQ-5D values were lower in the moderate-tosevere hearing loss group than in the mild hearing loss group.Conclusions: Among adults aged >40 years, those with hearing loss experienced more suicidal ideation, had lower motor ability, and lower overall QoL compared with these variables in older adults with normal hearing. Additionally, QoL scores were low in individuals with poor hearing.

      • Cardiopulmonary Resuscitation in cancer patients -Is it really necessary and unavoida ble?

        Introduction: In-hospital cardiopulmonary-resuscitation (CPR) is one of the disastrous situations. There are concerns whether patients in a palliative phase of cancer are the candidate for CPR or not. The avoidance of unnecessary CPR is important issues in palliative care of cancer. Methods: We reviewed medical records of division of hematology and medical Oncology, department of Internal Medicine in Seoul National University Bundang Hospital and analyzed characteristics of CRP situations from March 2003 toJune 2015. Total a 137 patients with cancer happened cardio-pulmonary- arrest and were given CPR including cardiac massage, intubation, DC cardioversion and intensive care unit treatment. We used modified pre-arrest morbidity (mPAM) score to evaluate medical conditions which result in arrest. Results: The mean age was 67 years (range, 39 99). Eighty eight patients (64.2%) were men and 50 patients (36.4%) were women. The primary malignancies were acute leukemia in 13 patients (9.4%), lung cancer in 40 patients (29.1%), lymphoma in 20 patients (14.6%), gastric cancer in 23 patients (16.7%), and other tumor type in 42 patients (30.2%). The causes of arrest were asphyxia from pulmonary aspiration in 15 patients (10.9%), cancer-related bleeding in 21 patients (15.3%), infection related chemotherapy in 23 patients (16.7%), infection not-related chemotherapy in 13 patients (9.4%), cancer progression in 33 patients (24.0%) and other causes in 34 patients (24.8%). The mean of mPAM score was 1.35. The 52 patients (37.9%) were given unnecessary CPR. Of unnecessary CPR, their relatives wanted resuscitation in 4 (2.9%). 85 patients (62.0%) received necessary CPR because they were on chemotherapy cycle or had chemotherapy plans. However, first return of spontaneous circulation (ROSC) rate was 43.7%, but re-arrest happened to most of them. So in-hospital survival rate was 8.7%. Discussions: The prognosis in patients who received CPR in palliative care of cancer was poor. Therefore, we should make an effort to avoid arrest events.

        전정편두통 환자에서 비디오 두부충동검사로 확인되는 반고리관의 이상 소견과 증상 호전과의 관계

        Objectives: Video head impulse test (vHIT) can evaluate function of the vestibulo- ocular reflex for high frequency range of head rotation. We aimed to characterize the abnormal patterns of canal dysfunction by vHIT in vestibular migraine (VM) and evaluate the relationship between the presence of canal dysfunction and symptomatic improvement. Methods: Eighty-seven patients with VM were included. Abnormality of vHIT at the initial examination was determined by the vHIT gain and the degrees of the corrective saccades at each canal and each side. The relationship between the abnormal patterns and the symptomatic improvement (no need for preventive medication) after modification of life styles and preventive medications for 1, 3, and 6 months was evaluated. Results: Abnormal vHIT of the lateral canal was 13.8% when determined by the gain criteria and 31.0% when based on both gain and corrective saccade, regard-less of the side. Abnormal vHIT of the superior and posterior canals were 18.4% and 27.6%, regardless of the side. Abnormal vHIT at any canal and side was ob-served in 47%. Patients showed symptomatic improvement in 29.9%, 71.3%, and 88.5% after modification of life styles and preventive medications for 1, 3, and 6 months. Abnormal vHIT results of canals were significantly related to the poor response to preventive mediations. Conclusions: Prolonged preventive medication was required for symptomatic im-provement in VM patients when vHIT results of any canals were abnormal, sug-gesting that peripheral vestibular abnormality is closely related to the pathophy-siology of vestibular migraine.

        Characteristics and Treatment Trends for Emphysematous Pyelonephritis in Korea: A 10-Year Multicenter Retrospective Study

        Purpose: This study examined the characteristics, current treatment trends, and outcomes of patients with emphysematous pyelonephritis (EPN) in Korea. Materials and Methods: Two hundred and seventeen patients diagnosed with EPN were evaluated using abdominal computed tomography in 2011-2021 at 15 institutes in Korea. The patients’ demographic and clinical characteristics, treatment modalities, and treatment outcomes were analyzed. The total study period was divided arbitrarily into groups A (2011-2014), B (2015-2017), and C (2018-2021) to analyze the trends in the EPN treatment. Results: The mean age of the patients was 65.1 years; there were more female patients (74.2%) than male patients. The overall mortality rate was 10.6%. Ninety-five (43.8%), 98 (45.2%), and 24 (11.0%) patients were treated with medical, minimally invasive, and surgical management, respectively; the corresponding mortality rates were 13.7%, 6.1%, and 16.7%. There was no significant change in the proportion of patients treated with medical management over time (group A=46.5%, group B=47.0%, and group C=38.8%). The proportion of patients treated with minimally invasive management gradually increased over time (group A=35.2%; group B=43.9%; group C=55.0%), while those who underwent surgical management decreased gradually over time (group A=18.3%, group B=9.1%, and group C=6.3%). No differences in mortality rates were observed between the groups. Conclusions: EPN with medical and minimally invasive management had a relatively high treatment success rate, which increased gradually, while surgical management decreased gradually over time in Korea. The mortality rate was relatively lower than that reported in studies published before the 2010s.

      • WHAT TRIGGERS CUSTOMER EMOTION IN THE LUXURY RETAIL SETTING: CUSTOMER EMOTION BEFORE VS. AFTER ENTERING A LUXURY SHOP AND ITS CUSTOMER EFFECT

        Customer emotion evoked during shopping is closely linked to customers’ central attitude toward the brand. Few studies, however, have examined what type of customer emotion is triggered at what stage of shopping by what factors, particularly in the luxury fashion retail context. This study intends to fill this gap in the research. To differentiate between customer emotions displayed before and after entering a luxury shop, we first identified what emotions are displayed at each stage and what factors triggered such emotions. We then examined the impact of customer emotions at each stage on customer responses, such as evaluations of service quality and brand attitude. Further, we attempted to identify patterns in the change in the type of customer emotions between before and after entering a luxury shop (i.e., from positive to negative or from negative to positive) and examined their effect on brand attitude. We collected 298 valid responses through a self-administered survey among luxury shop visitors in a major duty free complex in Seoul, Korea. The results show that customers display diverse emotions prior to entering a luxury shop. When customers were divided into four emotion groups (positive emotions only, mixed emotions, negative emotions only, no emotion), the positive emotion only group was the largest. As for specific emotions, happiness, contentment, and feeling comfortable were the three most experienced emotions. Negative emotions such as fear or shame were rarely experienced. Brand familiarity affected the type of emotion customers felt. While customers with high brand familiarity tended to feel privileged and comfortable, those with low brand familiarity tended to feel fear and shame. The emotions customers felt also differed by the shopping motive. When the motive was browsing only, customers tended to feel shame. When purchasing was a part of the motive, they tended to feel comfortable. Customer emotions displayed after entering a luxury shop were also diverse. The positive emotion only group was the largest, but it was not as large in the case of emotions felt prior to entering the shop. The effect of the retail service quality (RSQ) dimensions on in-store emotions varied depending on the specific emotion. The RSQ dimensions of "reliability" and "physical aspect" influenced the greatest number of in-store customer emotions. The moderating effect of brand familiarity was significant. The number of emotions affected significantly by RSQ dimensions was higher with the group with low familiarity. The moderating effect of the shopping motive was also significant. The RSQ dimensions that influenced emotion differed by the shopping motive. In terms of the effect of in-store emotions on brand attitude, it differed by the specific emotion. A significant effect was only present in three positive emotions (happiness, pride, privileged) and one negative emotion (anger). This implies that not all in-store emotions are equally influential on customer attitude. As for the effect of emotion change pattern on brand attitude, due to a large difference in the sample size of the emotion change pattern group (positive to positive (PP), positive to negative (NP), negative to positive (NP), negative to negative (NN)), we could not conduct a meaningful statistical analysis but could only observe some directions and tendencies. The brand attitude of the NP group tended to be higher than that of the PP group. This study contributes academically by extending the research on customer emotion, particularly in the luxury fashion retail context, in three ways. Firstly, we differentiated customer emotion before and after entering a shop. Secondly, we identified the triggers for specific emotions. Thirdly, we attempted to identify and examine the patterns of changes in customer emotion between before and after entering a shop and the impact of this pattern on customer responses. Managerially, this study contributes in two ways. Firstly, we demonstrated the importance of managing customer emotion at various stages of shopping at a specific emotional level. Secondly, we showed what factors should be managed for a particular customer emotion. Our research could be further extended in several ways. Customer experience research has shown that other customers present in the service setting can influence customer emotion; hence, in a luxury retail store, the degree of customers’ sense of compatibility with other customers may affect customer emotion and therefore should be explored. Another factor for future consideration is gender. Extant research in a luxury setting shows that the characteristics of luxury consumption tend to vary by gender. Considering that males’ luxury consumption is increasing, the difference in customer emotions based on gender might be worth investigating.

      • A superconducting joint for GdBa2Cu3O7 (-delta)-coated conductors

        Second-generation (2G) GdBCO-coated conductors (CCs) are promising for superconducting magnet applications because of their high critical current (I-c) density, low dependency of the Ic on the external magnetic field, good mechanical properties and reasonable cost, which offer opportunities to develop ultra-high-field magnets. However, they have not been used in hightemperature superconducting (HTS) applications with persistent current mode (PCM) operation such as nuclear magnetic resonance/magnetic resonance imaging magnets owing to unavailability of fabrication techniques for proper joining and contacts. Here we report a resistance-free joint, termed a 'superconducting joint', for 2G GdBCO CCs that forms a direct connection to establish a superconducting closed loop for PCM operation. The Ic value of the joined CCs is identical to that of the parent conductors in a liquid nitrogen bath (77 K). Moreover, the initially induced magnetic field of a model GdBCO coil containing a superconducting joint is maintained without decreasing, indicating the complete absence of electrical resistance. Thus, this fabrication method is a unique practical solution for lengthening the 2G HTS CCs and, more importantly, achieving PCM operation in 2G HTS magnet applications, including ultra-high-field nuclear magnetic resonance/magnetic resonance imaging magnets generating more than 1 GHz.

        ENACT 프로그램 참여를 통한 중·고등학생의 과학기술자의 사회적 책임에 대한 인식 및 과학기술 관련 사회쟁점에 대한 태도 탐색

        본 연구에서는 중·고등학생을 대상으로 ENACT 프로그램을 적용하여, 이들의 과학기술자의 사회적 책임에 대한 인식과 과학기술 관련 사회쟁점에 대한 이해를 탐색해보았다. 중·고등학생 40명을 대상으로 한 학기간 ENACT 프로그램을 도입하였으며, 과학기술자의 사회적 책임에 대한 인식, 과학기술 관련 사회쟁점에 대한 태도, 역량, 참여의향을 묻는 검사도구 응답인 양적 자료와 반성적 글쓰기인 질적 자료를 함께 이용하여 학생들의 인식 향상을 분석하였다. 연구결과, 본 연구에 참여한 학생들은 ENACT 프로그램을 통해 과학기술자가 갖추어야 할 사회적 책임에 대한 인식과 SSI에 대한 이해가 높아졌으며, 쟁점에 대한 이해를 바탕으로 문제를 해결하는 역량과 참여의향 또한 높아진 것으로 드러났다. 특히 학생들은 과학기술 관련 사회쟁점을 다각도로 탐색해보고 본인만의 해결방안을 도출하여 실천에 옮기는 과정을 통해 과학기술자들이 보다 적극적으로 사회문제를 해결할 필요가 있음을 깨닫게 되었으며, 스스로도 과학기술 관련 사회문제 해결에 관심을 갖고 참여할 의지를 표명하는 모습을 보였다. 또한 학생들은 이해관계자 지도와 퓨처스휠을 통해 쟁점을 깊숙이 탐색하고 미래를 다각도로 예상해봄으로써, 과학기술이 양면성을 지니고 있으며 그에 따라 미래에 바람직한 방향으로 나아갈 수 있도록 관심과 노력이 필요하다는 것을 인식하게 되었다. In this study, we implemented the ENACT program on secondary school students and examined their understanding of socioscientific issues and their views of social responsibility of STEM professionals. A total of 40 secondary school students participated in the ENACT program over the semester. Quantitative data sources such as Views of Social Responsibility of Scientists and Engineers and Perceptions on SSIs, and qualitative data including reflective essay, were collected for an analysis. The results showed that participating students presented statistically significant improvement in their views of the social responsibility as well as their perceptions of SSIs after the program. Reflective essays supported that students perceived the importance of social responsibility and problem-solving of STEM professionals and they also showed an improvement in their willingness to act for resolving the issues by exploring the views of various stakeholders surrounding the issues and anticipating the future consequences. Additionally, students also realized the ambivalence of science and technology and understood the necessity of putting efforts on desirable future by visualizing stakeholder map and futures wheel.

        ENACT 프로그램 참여를 통한 예비 기술교사의 과학기술의 사회적 책임감과 교육 필요성에 대한 인식 변화 탐색

        이 연구는 ENACT 프로그램에 참여한 예비 기술교사의 팀 프로젝트 학습과정과 경험을 탐색하는 것을 목적으로 한다. ENACT 프로그램은 과학기술관련 사회쟁점과 문제해결에 기반을 두고 만들어진 교육프로그램으로, 쟁점발견, 쟁점탐색, 미래 상황 예측, 과학·기술·공학적 쟁점해결, 사회적 실천의 5단계로 구성된다. 본 연구에서는 예비 기술교사 48명을 대상으로 5주 간의 온라인 ENACT 프로그램을 진행하였으며, 해당 프로그램 참여를 통한 예비 교사의 과학기술에 대한 사회적 책무성 및 교수 활동 태도의 변화와 학습요인별 성찰 경험 등을 살펴보았다. 면담분석 결과, 예비 교사들은 과학기술관련 사회쟁점을 과학·공학적으로 해결하고 사회적 실천을 위한 방안을 탐색하는 과정에서 사회적 책무성에 대한 인식이 변화하였으며, 특히 본인을 포함한 모든 사회 구성원이 이에 관심을 가질 것을 촉구하였다. 또한 ENACT 프로그램을 수행한 것이 예비 교사 스스로에도 효과적이었을 뿐만 아니라, 교사로서 학생들에게도 의미있는 교육이 될 것이라 보았다. 특히 예비 교사들은 추상적인 아이디어를 구체적 방안으로 발전시키거나 기존과 다른 학습 방법으로 수행했다는 점, 평소 관심 있던 주제나 실생활과 연관된 주제를 선정하여 문제를 스스로 해결해보는 경험을 했다는 데 흥미를 느끼고 있었다. 이와 더불어, 프로그램의 진행 과정에서 단계별 관련 내용과 사례를 워크북에 담아두어 프로젝트의 완성도를 높일 수 있었으며, 과학기술과 관련된 지식의 증가를 비롯하여 다양한 영역에서 과학기술을 바라볼 기회를 얻었다고 보고하였다. 초기에는 갈등을 겪었지만 점차 팀이 문제를 잘 해결할 수 있을 것이라는 믿음 하에 과제를 성공적으로 수행하였으며, 프로젝트 결과물에 대한 창조적 자신감 또한 보여주었다. The purpose of the study was to investigate pre-service technology teachers’ learning experiences of participating in the ENACT program. The ENACT program, designed by integrating socio-scientific issues education with scientific/engineering problem-solving processes, consisted of five steps: 1) Engage in SSIs, 2) Navigate SSIs, 3) Anticipate future consequences, 4) Conduct scientific and engineering practice, and 5) Take action. We implemented the program for 48 pre-service technology teachers over 5 weeks in online workspaces, and examined their views on social responsibility of science and engineering, attitudes toward the ENACT approaches, and their learning experiences. Results indicated that the pre-service teachers began to recognize the importance of cultivating social responsibility while exploring the nature of SSIs and generating ideas for taking action, and they even encouraged others to join the discussions on the issues. They highly valued their experiences in the ENACT program and believed that such programs will be also beneficial to students. Additionally, they enjoyed the process of elaborating abstract ideas into real solutions for the issues they encountered in their lives. They also mentioned that the online workspaces for the ENACT program supported their learning by guiding them in each step of the program. They believed that such process allowed them to broaden their perspectives on socioscientific issues despite the challenges and conflicts they experienced working with team members. They expressed a sense of accomplishment and satisfaction after the project.
