        전신성 홍반성 루프스에서 고용량 경정맥 당질코르티이드로 치료된 단백질 소실성 장질환 1예

        Generalizes edema and hypoalbuminemia are relatively common presenting manifestations in many clinical situations. The differential diagnosis of hypoalbuminemia include; Kwashiorkor, synthetic dysfunction of the liver, and excessive protein loss as in nephrotic syndrome. In systemic lupus erythematosus (SLE), hypoalbuminemia and generalized edema are most commonly due to protein loss associated with lupus nephritis; gastrointestinal involvement is uncommon, and therefore protein loss through the gastrointestinal tract is quite rare. We report a case of a protein losing enteropathy (PLE) associated with SLE. The patient was referred to out hospital for generalized edema, arthralgia and facial rash. After clinical evaluation, the patient met the criteria for the SLE diagnosis; hypoalbuminemia with general edema was consistent with a protein losing enteropathy. After two weeks of therapy with parenteral high dose glucocorticoid, the patients was improved in laboratory findings as well as clinical symptoms.

        특수건강진단 자료를 이용한 소음성난청 판정기준의 비교

        Although noise-induced hearing loss (NIHL) is the most prevalent occupational disease in Korea, only 10% of the diagnosed cases are compensated. Old (1989-94) and current diagnostic criteria, criteria for workers' compensation of NIHL in Korea, compensation formulas of American Medical Association/American association of Ophthalmology and Otolaryngology (AMA/AAOO), the Committee on Hearing, Bio-Acoustics, Biomechanics (CHABA), American Academy of Otolaryngology (AAO) recommendation were compared. Each criterion was applied on the audiomety data of 4044 workers (8023 ears), who had received the second line screening test of Special Periodic Health Examination Program for noise-exposed workers during 1991-2. First, the resulting proportions of NIHL cases by employing each criterion were compared and strength of agreement was measured using kappa value. Temporary Threshold Shift (TTS) was corrected by noise free interval, and the reduction ratio of NIHL cases was calculated. Theoretical progression model of NIHL was reconstructed from previous studies on the natural course of NIHL to evaluate the change of diagnosis result of each criterion in the model. The kappa value between old and current criteria was 0.19, 0.55 for current criteria and workers' com. criteria, ranging from 0.08 to 0.78, highest coincidence was observed between current criteria and CHABA formula. The current criteria produce most similar results with CHABA formula. If TTS is corrected for NFI, there is about 14% reduction of NIHL cases. The results of applying on NIHL progression model divided the formulas roughly into 3 groups, of which compensation criteria was the most, old criteria and AAO the least conservative. In conclusion, the result of 4 ㎑ audiometry should be excluded in evaluation of hearing level and a new hearing conservation program should be set out. Current diagnostic criteria has an ambiguity in that managerial concept for prevention and purpose of compensation is mixed up. The current diagnostic criteria and compensation criteria could be incorporated into a new formula which is based on the state of the art test for estimating everyday hearing disability.

        VDT작업자에서 업무로 인한 정신사회적 스트레스에 대한 인지가 근골격계장애에 미치는 영향

        업무에 의한 정신사회적 스트레스가 근골격계 질환의 유병에 미치는 영향을 파악하기 위하여 전화안내원을 대상으로 단면적 조사연구를 수행하였다. 연구 결과 나이, 교육정도, 근무연한, 가사부담 등의 인구학적 변수는 근골격계질환의 유병에 영향을 미치지 않는 것으로 분석되었다. 정신사회적 스트레스 중 업무요구도가 높다고 느끼는 집단에서 그렇지 않은 집단에 비해 근골격계 질환의 유병률이 통계적으로 유의하게 높게 나타났으나 업무재량도나 사회적지지도는 근골격계질환과 유의한 상관관계를 발견할 수 없었다. 그러나 업무요구도는 높고 업무재량도는 낮다고 느끼는 '근무긴장도가 가장 높은 군'은 업무요구도는 낮고 업무재량도는 높다고 느끼는 '근무긴장도가 낮은 군'에 비해 근골격계 질환의 발생위험이 통계적으로 유의하게 높은 것으로 나타났다. 본 연구의 결과는 업무로 인한 정신사회적 스트레스에 대한 인지 정도에 따라 근골격계질환의 유병률이 달라짐을 시사하고 있다. 따라서 작업관련성 근골격계질환의 예방을 위해서는 기존의 인간공학적 접근 이외에 스트레스를 감소시키려는 노력이 동시에 진행되어야 할 것으로 생각한다. A cross-sectional study was conducted to assess the association between perceived occupational psychosocial stress at work and the work-related musculoskeletal disorder(WRMSD) among employees using video display terminals. The study included 111 female telecommunication employees from three companies at ChungBuk province. Cases of WRMSD were defined using symptom questionnaire and physical examination conducted by rehabilitation specialist. Information on demographics, individual factors, and perceived psychosocial stress, were obtained by self-administered questionnaire. For assessing perceived psychosocial stress, we used variables from 'job strain model' proposed by Karasek. Associations between perceived psychosocial stress and WRMSD were assessed by multiple logistic regression models. Forty nine(44% to the total) subjects met our operational definition for WRMSD. Age, seniority, housing load were not associated with WRMSD. Perceived psychosocial stress was associated with WRMSD[odds ratio=3.28, 95% confidence interval: 1.05-10.19]. This study suggests that perceived occupational psychosocial stress is related to the prevalence of WRMSD.

        Changes in growth and active components of Angelica gigas Nakai in different cultivation environments

        Angelica gigas Nakai belongs to the Unbelliferae family and was a representative medicinal plant with antiinflammatory, antioxidant and immunoenhancing activities. Recently, changes in the atmospheric environment, such as temperature, CO2 concentration and solar radiation, have continuously resulted in the reduction of cultivation area and production amount of A. gigas. They could affect the synthesis of secondary metabolites including nodakenin, decursin and decursinol angelate. This study aimed to investigate the atmospheric factors and the growth and content of active compounds of A. gigas. Air temperature and soil temperature were lowest in Jeongseon (northern region). Solar radiation was highest in Bonghwa (central region). Relative humidity was the lowest in Jeongseon from January to June, and it was the lowest in Bonghwa after July. At the Jeongseon experimental site, most of the growth characteristics of A. gigas were poor. The growth characteristics of A. gigas in Chungju and Bonghwa did not show any significant difference. The contents of active compounds were significantly highest in A. gigas collected from Chungju, but were significantly low in Jeongseon. Relative humidity showed a significant positive correlation with the content of the active compounds except for nodakenin. Decursin and decursinol angelate were significantly negatively correlated with solar radiation and soil moisture content, respectively. Most of growth characteristics of A. gigas, except for root length, showed a significant positive correlation with nodakenin content. The results of this study can be used as data for predicting the location of A. gigas cultivation according to the growth and active compounds of A. gigas in different cultivation regions.

        토착 야초류(어저귀, 자귀풀)의 수경재배를 이용한 2,4,6-trinitrotoluene(TNT) 분해 연구

        In this study, uptake and translocation of 2,4,6-trinitrotoluene(ThT) by plant in a hydroponic culture was quantified with two indigenous plant species, Aeschynomene indica and Abutilon avicennae on various initial concentrations of TNT ranging from 20 to 80 ㎎TNT/L. Experiments were sterilized to exclude the activity of microorganisms and conducted in duplicate. Weight loss of two plant species in added TNT culture media was higher than in control. At over 20㎎TNT/L, there appeared to be phytotoxicity from TNT as indicated by severe yellow-chlorosis and increase of falling leaves. TNT removal rate normalized(K) to the plant fresh weight of Abutilon avicennae and Aeschynomene indica was that the higher TNT concentrations resulted in lower TNT removal rate normalized(K) to the plant fresh weight. Approximately 96% of the TNT in viable microflora-hydroponic culture was removed after 96h of the experiments.

        운전자의 체압 분포 및 시트변형에 대한 정량화 측정시스템

        Proper seat design is critical to the safety, comfort, and ergonomics of automotive driver’s seats. To ensure effective seat design, quantitative methods should be used to evaluate the characteristics of automotive seats. This paper presents a system that is capable of simultaneously monitoring body pressure distribution and surface deformation in a textile material. In this study, a textile-based capacitive sensor was used to detect the body pressure distribution in an automotive seat. In addition, a strain gauge sensor was used to detect the degree of curvature deformation due to high-pressure points. The textile-based capacitive sensor was fabricated from the conductive fabric and a polyurethane insulator with a high signal-to-noise ratio. The strain gauge sensor was attached on the guiding film to maximize the effect of its deformation due to bending. Ten pressure sensors were placed symmetrically in the hip area and six strain gauge sensors were distributed on both sides of the seat cushion. A readout circuit monitored the absolute and relative values from the sensors in real-time, and the results were displayed as a color map. Moreover, we verified the proposed system for quantifying the body pressure and fabric deformation by studying 18 participants who performed three predefined postures. The proposed system showed desirable results and is expected to improve seat safety and comfort when applied to the design of various seat types. Moreover, the proposed system will provide analytical criteria in the design and durability testing of automotive seats.

        어린이 놀이 공간에 대한 듀이의 경험론적 특성 연구

        (연구 배경 및 목적) 산업화로 인해 전환점을 맞은 어린이 교육은 일방적인 지식 습득의 주입식 교육보다는 창의성을 강조하는 교육이 기존 패러다임의 전환을 가져오게 되었다. 단순한 기술 습득보다는 창의적 콘텐츠의 중요성이 강조되는 교육의 환경변화가 나타나며 어린이 교육 또한 창의적인 교수법의 개발 및 활용 방안의 적용이 필요하게 되었다. 본 논문은 어린이의 발달적 특성을 파악하고 직접 경험하는 것이 참된 의미의 경험이라고 강조한 존 듀이의 경험론을 이론적 근거로 하여 어린이 놀이 공간에 나타나는 경험적 특성의 상호작용과 어린이의 창의력 개발과의 관계를 밝히고자 한다. 이 연구를 통해 얻어진 결과는 어린이의 창의성 개발을 위한 공간 연구에 새로운 이론으로 제시되기를 기대하며, 더 나아가 공간 디자인에 적용 방법을 모색하는데 도 움이 되고자 한다. (연구방법) 우선 어린이의 발달적 특성 중 인지발달에 대해 문헌을 통해 연구하고 놀이 공간에 대한 개념에 대해 이론적 고찰을 하고자 한다. 이론적 고찰을 바탕으로 어린이 놀이 공간과 존 듀이 경험 이론에 대한 선행연구 분석을 통하여 분석한 키워드를 통해서 경험 이론적 특성이 나타나는 어린이 놀이 공간에 대한 공간 사례 연구를 하고자 한다. 최근 2000년대 이후에 설립된 국외 영, 유아 대상의 교육목적 공간인 유치원 내부 및 외부 놀이 공간과 콘셉트 놀이 공간을 포함한 어린이 전용공간을 위주로 선정하였다. (결과) 듀이의 이론에서 전반적으로 말하고 있는 상호작용적 특성은 어린이가 놀이 공간에서 창의력 개발을 위해 꼭 필요한 것이며, 공간뿐만 아니라, 주변 환경, 타인 등 수많은 경우와 다양한 상호작용을 통한 경험은 어린이의 상상력 및 창의력 개발에 필요한 근거이자 특성이 될 수 있음을 알 수가 있었다. (결론) 경험적 특성이 어린이 놀이 공간에 나타난다는 것은 어린이 놀이 공간이 학습적인 목적을 위한 공간이 아닌 어린이가 자유롭게 공간 안에서 신체의 사용과 놀이 경험을 통한 상호작용으로 어린이의 창의력 개발이 일어날 수 있다는 사실을 증명해준다. 이를 바탕으로 볼 때 어린이 놀이 공간은 경험이론적 특성을 전략으로 놀이 공간에 적용되어야 하며, 자라나는 어린이에게 바람직한 놀이 공간을 제공해줄 수 있는 사회 복지적 측면에서의 고찰이 필요할 것이다. 경험 이론적 특성이 나타나는 어린이 놀이 공간계획을 통해 듀이가 말하고 있는 참된 경험의 발현이 일어날 수 있는 공간이 계획된다면 어린이를 위한 참된 놀이 경험을 할 수 있는 공간이 될 것이라고 생각한다. 이러한 공간의 형성은 아이들의 창의력 발달 및 다양한 인지적 발달을 가져와 창의력 개발에 중요한 역할을 할 것으로 기대한다. (Background and Purpose) Children’s education has reached a turning point because of industrialization, emphasizing creativity instead of cramming curriculum with information for the unilateral acquisition of knowledge. Thus, the shift in the existing paradigm of education is evident. The educational environment has changed from focusing on simple skills to stressing creative content; therefore, it has become necessary to develop creative teaching methods and explore ways to utilize them. This study establishes the relationship between the interaction of empirical characteristics of children’s play spaces and the development of children’s creativity with John Dewey’s empiricism as the theoretical basis. This theory emphasizes that an experience in the true sense of the word grasps children’s developmental characteristics and directly influences their growth. The results obtained through this study are expected to be presented as a new theory aligned with a spatial study on the development of children’s creativity; by extension, it will be helpful in exploring applications to spatial designs. (Method) First, a literature study was conducted regarding cognitive development of children, and the concept related to play space was examined theoretically. Based on the theoretical study, a spatial case study was conducted on children’s play space in which the characteristics of the space theory were presented through analytical key words from past research analyses and John Dewey’s empiricism theory. The internal and external play spaces of kindergartens abroad, which were established since the 2000s, were examined as educational spaces for infants and young children, with the primary focus on play space. (Results) It was found that the interactive characteristics generally mentioned in Dewey’s theory are essential for development of children’s creativity in a play space, and experiences in such spaces (also described in numerous cases) and through diverse interactions can be the basis for the growth of imagination and creativity. (Conclusions) The fact that empirical characteristics are represented in children’s play spaces proves that they are not places for learning but for freedom of interaction; thus, creativity can be developed through physical and playful experiences. Based on findings of this study, children’s play spaces are suitable for strategic applications of the empiricism theory, and social welfare research that verifies the need for growing children to have desirable play space is necessary. When the space is designed in which a true experience described by Dewey can be realized, it will be considered a space where children can have true play experiences. Building such spaces is expected to promote the development of children’s creativity and diverse cognitive abilities.
