<P>Glutamatergic dendritic EPSPs evoked in cortical pyramidal neurons are depressed by activation of hyperpolarization-activated cyclic nucleotide-gated (HCN) channels expressed in dendritic spines. This depression has been attributed to shunting effects of HCN current (<I>I</I><SUB>h</SUB>) on input resistance or <I>I</I><SUB>h</SUB> deactivation. Primary sensory neurons in the rat mesencephalic trigeminal nucleus (MTN) have the somata covered by spine-like microvilli that express HCN channels. In rat MTN neurons, we demonstrated that <I>I</I><SUB>h</SUB> enhancement apparently diminished the glutamate receptor (GluR) current (<I>I</I><SUB>GluR</SUB>) evoked by puff application of glutamate/AMPA and enhanced a transient outward current following <I>I</I><SUB>GluR</SUB> (OT-<I>I</I><SUB>GluR</SUB>). This suggests that some outward current opposes inward <I>I</I><SUB>GluR</SUB>. The <I>I</I><SUB>GluR</SUB> inhibition displayed a U-shaped voltage-dependence with a minimal inhibition around the resting membrane potential, suggesting that simple shunting effects or deactivation of <I>I</I><SUB>h</SUB> cannot explain the U-shaped voltage-dependence. Confocal imaging of Na<SUP>+</SUP> revealed that GluR activation caused an accumulation of Na<SUP>+</SUP> in the microvilli, which can cause a negative shift of the reversal potential for <I>I</I><SUB>h</SUB> (<I>E</I><SUB>h</SUB>). Taken together, it was suggested that <I>I</I><SUB>GluR</SUB> evoked in MTN neurons is opposed by a transient decrease or increase in standing inward or outward <I>I</I><SUB>h</SUB>, respectively, both of which can be caused by negative shifts of <I>E</I><SUB>h</SUB>, as consistent with the U-shaped voltage-dependence of the <I>I</I><SUB>GluR</SUB> inhibition and the OT-<I>I</I><SUB>GluR</SUB> generation. An electron-microscopic immunohistochemical study revealed the colocalization of HCN channels and glutamatergic synapses in microvilli of MTN neurons, which would provide a morphological basis for the functional interaction between HCN and GluR channels. Mathematical modeling eliminated the possibilities of the involvements of <I>I</I><SUB>h</SUB> deactivation and/or shunting effect and supported the negative shift of <I>E</I><SUB>h</SUB> which causes the U-shaped voltage-dependent inhibition of <I>I</I><SUB>GluR</SUB>.</P>

Purpose: To evaluate the effects of phosphodiesterase type 5 inhibitor (PDE5i) on urinary continence recovery after bilateral nerve-sparing radical prostatectomy (BNSRP). Materials and Methods: Between 2002 and 2012, 137 of 154 consecutive patients who underwent BNSRP in our institution retrospectively divided into 3 groups that included patients taking PDE5i immediately after surgery (immediate PDE5i group, n=41), patients starting PDE5i at an outpatient clinic after discharge (PDE5i group, n=56), and patients taking no medication (non-PDE5i group, n=40). Using self-administered questionnaires, the proportion of patients who did not require incontinence pads (pad-free patients) was calculated preoperatively and at 1, 3, 6, 12, 18, and 24 months after BNSRP. Severity of incontinence was determined based on the pad numbers and then compared among the 3 groups. Results: Proportions of pad-free patients and severity of incontinence initially deteriorated in all of the groups to the lowest values soon after undergoing BNSRP, with gradual improvement noted thereafter. The deterioration was most prominent in the immediate PDE5i group. As compared to the non-PDE5i group, both the PDE5i and immediate PDE5i groups exhibited a better final continence status. Conclusions: PDE5i improves final continence status. However, administration of PDE5i immediately after surgery causes a distinct temporary deterioration in urinary incontinence.

In recent years, the debasement of the power quality of a distribution system such as deterioration of voltage imbalance and current imbalance or the increase of harmonics has been apprehended with diversification of the load by progress of power electronics technology and with increase of the amount of connection of a distributed generators for home use with the reduction of the effect on the environment. In this paper, the authors propose the restraint method of harmonics and improvement of voltage imbalance in distribution system utilizing the power conditioner systems of photovoltaic systems and verify the validity of the proposal method by the numerical calculation using an analytical model of distribution system.

Research into first episode psychosis (FEP) is a growing field in psychiatry. In this paper, the authors provide an overview of neurocognitive findings and therapeutic approaches indicated for patients with FEP. Focus will be placed on evidence from neuropsychological assessment and brain imaging of electrophysiological activities, as well as the ability of some psychotropic drugs, such as second generation antipsychotics, to improve functional outcome. Efforts to identify biological signals characteristic of FEP are expected to facilitate early intervention into schizophrenia and related disorders.
