        지능박약 환자에서 Carbamazepine에 의해 발생한 골연화증 1예

        Anticonvusant therapy with any of several agents, especially phenytoin, phenobarbital, and primidone causes disturbances in bone mineral metabolism. Anticonvulsants stimulate the hepatic microsomal mixed-oxidase enzymes and hence increase the rate of clearance of vitamin D and its metabolism. The severity of clinical manifestations in any given individual appears to be a function of the combined effects of variety of factors including drug type and total drug dose, dietary vitamin D intake, sunlight exposure, and physical activity level. We report a case of osteomalacia associated with long term carbamazepine therapy in a 21-year-old male with less exposure to sunlight(J Kor Soc Endocrinol 15:286?290, 2000).

        A study on an efficient prediction of welding deformation for T-joint laser welding of sandwich panel PART I : Proposal of a heat source model

        The use of I-Core sandwich panel has increased in cruise ship deck structure since it can provide similar bending strength with conventional stiffened plate while keeping lighter weight and lower web height. However, due to its thin plate thickness, i.e. about 4~6 mm at most, it is assembled by high power $CO_2$ laser welding to minimize the welding deformation. This research proposes a volumetric heat source model for T-joint of the I-Core sandwich panel and a method to use shell element model for a thermal elasto-plastic analysis to predict welding deformation. This paper, Part I, focuses on the heat source model. A circular cone type heat source model is newly suggested in heat transfer analysis to realize similar melting zone with that observed in experiment. An additional suggestion is made to consider negative defocus, which is commonly applied in T-joint laser welding since it can provide deeper penetration than zero defocus. The proposed heat source is also verified through 3D thermal elasto-plastic analysis to compare welding deformation with experimental results. A parametric study for different welding speeds, defocus values, and welding powers is performed to investigate the effect on the melting zone and welding deformation. In Part II, focuses on the proposed method to employ shell element model to predict welding deformation in thermal elasto-plastic analysis instead of solid element model.

        Bulk-micromachined, SOI-based half-bridge silicon strain gauges for high pressure applications

        <P>This paper presents a new half-bridge silicon strain gauge fabricated on a silicon-on-insulator (SOI) substrate by MEMS bulk-micromachining technology. These gauges have holes etched through the wafer by deep reactive ion etching (DRIE) and a closed shape with four sides, unlike the current competitive devices with open structures. This unique design minimizes the shifting or rotation of gauge position and enhances the bonding strength during glass-frit bonding, leading to an improved sensor performance and yield, and hence, a reduction in sensor cost. The prototype half-bridge gauges were tested under strains ranging from  −170 to 170 <img ALIGN='MIDDLE' ALT='' SRC='http://ej.iop.org/images/0960-1317/28/12/125004/jmmaae592ieqn001.gif'/> and were shown to have a linear output with a typical gauge factor of about 112 and an average hysteresis of 0.0192% FS. In addition, the full bridge output for 0–50 bar pressure shows a typical sensitivity of about 0.345 mV/V/bar, a maximum thermal zero shift of  −7.33% FS, and a thermal sensitivity shift of  −0.17% FS/°C.</P>

      • Systematic Lymphadenectomy for Survival in Epithelial Ovarian Cancer : A Meta-Analysis

        <P>BACKGROUND: The role of systematic lymphadenectomy (SL) remains unclear for improving overall survival (OS) in epithelial ovarian cancer (EOC). To evaluate the role of SL in EOC, we performed a meta-analysis for comparing the efficacy for OS between SL and unsystematic lymphadenectomy (USL). METHODS: After the extensive literature search between January 1995 and December 2008, we analyzed 9 studies (2 randomized controlled trials [RCTs] and 7 observational studies) with 21,919 patients with EOC who underwent staging laparotomy including SL or USL. RESULTS: Although SL was a favorable factor for OS compared with USL (hazard ratio [HR], 0.72; 95% confidence interval [CI], 0.68-0.76), the efficacy of SL on increased OS could not be determined in all-stage and early-stage EOC owing to the lack of RCTs and the deviation of weight by large-scale observational studies, whereas SL improved OS in advanced-stage EOC (HR, 0.70; 95% CI, 0.67-0.75). Moreover, SL increased OS in patients with all-stage disease who underwent optimal debulking surgery (HR, 0.84; 95% CI, 0.69-0.99), whereas it could not improve OS statistically in early-stage and advanced-stage diseases. However, SL showed increased OS with marginal significance in advanced-stage EOC patients treated with optimal debulking surgery (HR, 0.82; 95% CI, 0.66-1.02). CONCLUSIONS: These findings suggest the possibility that SL can improve OS in advanced-stage EOC. However, the efficacy of SL on OS is still unknown because of the lack of RCTs, which requires more relevant studies for investigating the role of SL in EOC.</P>

        Posterior Spinal Reconstruction with Pedicle Screws, Multiple Iliac Screws and Wisconsin Spinal Wires in a Patient with Neurofibromatosis Scoliosis: A Case Report

        A 54-year-old female with neurofibromatosis type 1 presented with progressing truncal shift owing to spinal deformity. On plain radiograph, the Cobb angle was 54 degree in coronal plane. Radiological examinations showed severe dystrophic change with dysplastic pedicles, bony scalloping, neural foraminal widening from dural ectasia. The patient underwent deformity correction and reconstruction surgery from the T9 to the pelvis using multiple iliac screws and Wisconsin interspinous segmental instrumentation by wiring due to maximize fixation points. The postoperative course was uneventful. One-year follow-up radiographs showed a successful curve correction with solid fusion. We report a case of pedicle dysplasia and dystrophic change treated by posterior segmental spinal instrumentation and fusion with help of multiple iliac screws and modified Wisconsin interspinous segmental wiring.

        Isolation and Identification of a Marine Bacterium, Pseudomonas sp. BK1 Producing Extracellular Enzymes Capable of Decomposing Multiple Complex Polysaccharides

        한천, 키틴, 셀룰로오스, 자일란, 만난과 같은 복합다당류들에 대한 분해능을 나타내는 효소들을 생산하는 해양미생물을 홍조류인 Porphyra dentata로부터 분리하였다. 분리균 BK1은 그람음성, 호기성 간균으로 DNA의 G+C함량은 51.6 mol%를 나타내었다. 주요 isoprenoid quinine 구성 성분은 ubiquinone-8로 확인되었고, 주요 세포 지방산은 C16:0, C16:1 w6c, C18:1 w7c로 밝혀졌다. 16S rRNA서열의 비교분석 결과는 분리균 BK1이 Pseudomonas 속의 일원인 것으로 확인되었다. 이러한 결과를 바탕으로 분리균 BKl은 Pseudomonas sp. BKl으로 명명하였다. A marine bacterium (strain BKl) that produces extracellular enzymes capable of decomposing complex polysac-charides, such as agar, chitin, carboxymethylcellulose, xylan and mannan, was isolated from the marine red alga Porphyra dentata. Strain BKl was gram-negative, aerobic, catalase- and oxidase-positive, polarly flagellated bacilli that produce gelatinase and urease, but not decarboxylases. The G+C content of the DNA was 51.6 mol%. The major isoprenoid quinone component was identified as an ubiquinone-8, and the major cellular fatty acids were C16:0, C16:1 w6c and C18:1 w7c. Comparative 16S rRNA sequence analysis placed strain BK1 with members of the genus Pseudomonas. On the basis of phenotypic and genotypic data, the strain BK1 was shown to be a member of the subgroup of Pseudomonas, and named as Pseudomonas sp. BK1.

        A study on an efficient prediction of welding deformation for T-joint laser welding of sandwich panel Part II : Proposal of a method to use shell element model

        I-core sandwich panel that has been used more widely is assembled using high power $CO_2$ laser welding. Kim et al. (2013) proposed a circular cone type heat source model for the T-joint laser welding between face plate and core. It can cover the negative defocus which is commonly adopted in T-joint laser welding to provide deeper penetration. In part I, a volumetric heat source model is proposed and it is verified thorough a comparison of melting zone on the cross section with experiment results. The proposed model can be used for heat transfer analysis and thermal elasto-plastic analysis to predict welding deformation that occurs during laser welding. In terms of computational time, since the thermal elasto-plastic analysis using 3D solid elements is quite time consuming, shell element model with multi-layers have been employed instead. However, the conventional layered approach is not appropriate for the application of heat load at T-Joint. This paper, Part II, suggests a new method to arrange different number of layers for face plate and core in order to impose heat load only to the face plate.

        류마티스 관절염에서 갑상선 자가항체 및 갑상선 기능에 관한 연구

        Background : Autoimmune thyroid diseases have been shown to occur in association with connective tissue disorders. This study was undertaken to elucidate the prevalence of thyroid autoantibodies and thyroid dysfunction in Korean patients with rheumatoid arthritis (RA). Methods : We have compared the prevalence of thyroid autoantibodies in 108 RA patients with 81 age-matched controls. Antimicrosomal antibody (AMA) and antithyroglobulin antibody (ATA) were measured by passive hemagglutinin assay. T3, T4, Free T4, and TSH were measured by chemiluminescence method. Rheumatoid factor was measured by nephelometry. Statistical analysis was performed by chi-square, Fisher's exact test, and Pearson correlation test . Results : 1) The presence of AMA/ATA in patients with RA was significantly higher (28.7%/26.7% in 108 patients) than that of controls (11.1%/9.9% in 81 controls). The positive rates of AMA/ATA in female patients were also higher (28.9%/28.9% in 97 patients) than those of female controls (15.5%/12.1% in 58 controls). 2) Thirty three (17.6%) of 187 patients with RA had the abnormalities of thyroid function; 14 (7.5%) revealed subclinical hypothyroidism, 5 (2.7%) revealed biochemical primary hypothyroidism, 7 (3.7%) revealed sick euthyroid syndrome, and 7 (3.7%) revealed hyperthyroidism. Conclusion : These results suggest that autoimmune thyroiditis is highly associated with RA in Korean patients, showing the increased prevalence of thyroid autoantibodies and thyroid dysfunctions.         What are ‘true normal’ liver stiffness values using FibroScan<sup>®</sup>?: a prospective study in healthy living liver and kidney donors in South Korea

        <P>Abstract</P><P>Aims</P><P>To identify the normal range of liver stiffness (LS) values by recruiting healthy living liver and kidney donors in South Korea.</P><P>Methods</P><P>Liver stiffness measurement (LSM) was performed in 69 healthy living liver and kidney donors who were admitted for transplantation. None of the subjects suffered from diabetes mellitus, hypertension, hepatitis B virus infection, hepatitis C virus infection, heart dysfunction, liver dysfunction or metabolic syndrome.</P><P>Results</P><P>LSM failure rate was 2.7%. Among 12 liver donors (17.4%) with available liver histology, eight showed normal liver histology and four showed liver steatosis of <5% of the hepatocytes. The mean age of our study population was 38.9 ± 11.9 years (35 men and 34 women), with a mean LS value of 4.6 ± 0.5 kPa (range 3.3–5.6 kPa). LS values were not significantly different between men (4.6 ± 0.6 kPa) and women (4.5 ± 0.5 kPa, <I>P</I>=0.636), nor were they significantly different according to age (<I>P</I>=0.851). Using the fifth and 95th percentiles, we determined the normal range of LS values to be 3.9–5.3 kPa.</P><P>Conclusions</P><P>We identified the normal range of LS values and found that LS values were not significantly influenced by age and gender.</P>

      • Preparation of iron-doped titania from flocculated sludge with iron-titanium composite coagulant.

        <P>The main drawback of flocculation process with dye wastewater is the large amount of unrecyclable sludge which needs disposal. A novel process using Ti-salt flocculation to purify wastewater was developed to produced sludge that can be calcined to produce titania. In this study, iron-doped TiO2 nanomaterial was successfully produced from sludge obtained by the flocculation of dye wastewater with a composite floculant including TiCl4 and FeSO4. The titania was characterised using scanning electron microscopy (SEM/EDX), transmission electron microscopy (TEM), X-ray diffraction (XRD), and the photodecomposition of acetaldehyde. The XDR results showed that the anatase and rutile structures were found after sludge calcination at 550 degrees C and 640 degrees C respectively. The elemental analyses were carried out using EDX. The rutile titania sample consisted of Ti (35.7 wt.%), Fe (14.7 wt.%), O (42.3 wt.%), P (2.6 wt.%) and Ca (4.7 wt.%). The photocatalytic activity was monitored for the photodecomposition of gas acetaldehyde. Iron-doped titania seems to play an important role in increasing the photocatalytic activity under UV light irradiation.</P>
