      • Simplified Screening Method for Rice Cultivars with Adaptability to Flooded Paddy

        Rice direct seeding on flooded paddy has a shortcoming of instability of seedling establishment due to hypoxia. This study was conducted to clarify the interrelationship between seeding rate and seedling growth in flooded condition and find out a optimum screening condition for anaerobic tolerant cultivars. The germinated seeds were sown in 250 ml flasks containing 150 ml (in water) and 5 ml distilled water(in air) at 25℃ respectively. The number of seeds sown (seeding rate) were 5, 15, 25, and 35 seeds per 250 ml flask. The characteristics measured, length of shoot and root, fresh weight of shoot and root and chlorophyll content, were not affected by seeding rate and cultivars in air while those in water were decreased as seeding rate increased. The dissolved oxygen in the soak water were affected by seeding rate and cultivars. The difference between the two cultivars in the characteristics were greatest when seeding rate was35 seeds in 250 ml flask containing 150 ml water. The characteristics at seeding rate of 35 seeds in 250 ml flask containing 150 ml water would be used in the screening of anaerobic tolerant cultivars for direct seeding.

      • Effects of Transforming Growth Factor Beta on Cytoskeleton Structure and Extracellular Matrix in Mv1Lu Mink Epithelial Cells

        Previous studies have shown that transforming growth factor beta ($TGF-{\beta}$) is a potent regulator of cell growth and differentiation. To study the effects of $TGF-{\beta}$ on cell morphology and cytoskeleton reorganization, we conducted a survey using Mv1Lu mink lung epithelial cells with antibodies to cytoskeletal proteins and an extracellular matrix protein. While the untreated cells showed a cuboidal shape of typical epithelia, the Mv1Lu cells displayed a drastic shape change in the presence of $TGF-{\beta}$. This alteration was most prominent when near-confluent cells were treated with $TGF-{\beta}$. Since the morphology alteration is known to be accompanied by the reorganization of cytoskeletal proteins in other cell types, we investigated the intracellular distribution of the three major cytoskeletal structures: microfilaments, microtubules, and intermediate filaments. In the microfilament system, $TGF-{\beta}$ induced new stress fiber formation, which was caused primarily by the polymerization of cytoplasmic G-actin. However, $TGF-{\beta}$ appeared not to induce any significant changes in microtubular structures and vimentin filaments as determined by indirect fluorescence microscopy. Finally we confirmed the rapid accumulation of fibronectin by immunoblot analysis and chased the protein locations by immunofluorescence microscopy.

        Long Term Follow-up Results of External Beam Radiotherapy as Primary Treatment for Retinoblastoma

        <P>The authors reviewed their experiences of external beam radiotherapy (EBR) as an initial treatment in retinoblastoma patients to determine its long-term effect on subsequent tumor control and complications. A total of 32 eyes in 25 patients that underwent EBR for retinoblastoma were reviewed retrospectively. The patients consisted of 21 boys and 4 girls of median age at treatment of 7.1 months. Radiation doses ranged from 35 to 59.4 Gy. The 10-yr ocular and patient survivals were 75.4% and 92.3%, respectively. Nine of the 32 eyes progressed; 7 of these were enucleated and 2 were salvaged by focal treatment. According to the Reese-Ellsworth classification, 4 of 5 eyes of Group II, 13 of 16 Group III eyes, 2 of 4 Group IV eyes, and 5 of 7 Group V eyes were retained, and of the 32 eyes, 13 had visual acuity better than 20/200. Eleven patients experienced a radiation-induced complication. No patient developed a second malignancy during follow-up. Despite the limited number of patients enrolled, EBR may provide a mean of preserving eyeball and vision for some advanced lesions.</P>

      • Effect of the ultraviolet light treatment and storage methods on the biological activity of a titanium implant surface

        <P><B>Abstract</B></P> <P><B>Objective</B></P> <P>We evaluated whether the biological activity of the surface of titanium, when stored in an aqueous solution, in low vacuum, and under ambient conditions after ultraviolet light (UV) treatment is comparable to that of the surface immediately after UV treatment for 15min and that after dielectric barrier discharge (DBD) plasma treatment for 15min.</P> <P><B>Methods</B></P> <P>Grade IV titanium discs with machined surfaces were irradiated with UV and their surface properties were evaluated immediately and after storage for 28days in distilled H<SUB>2</SUB>O (dH<SUB>2</SUB>O), a vacuum desiccator (31.325kPa), and a sealed container under air. Their surface characteristics were evaluated by atomic force microscopy, X-ray diffraction, contact angle analysis, and X-ray photoelectron spectroscopy. Biological activities were determined by analyzing the albumin adsorption, MC3T3-E1 cell adhesion, and cytoskeleton development.</P> <P><B>Results</B></P> <P>Hydrophilicity of titanium surfaces stored in dH<SUB>2</SUB>O was comparable to that immediately after UV treatment and higher than that immediately after DBD plasma treatment (<I>P<</I> 0.001). Storage in dH<SUB>2</SUB>O and in low vacuum immediately after UV treatment prevented hydrocarbon contamination and maintained elevated amounts of titanium and oxygen. After 28 days, protein adsorption, cellular adhesion, and cytoskeletal development of MC3T3-E1 cells on the titanium surfaces stored in dH<SUB>2</SUB>O were significantly enhanced compared to those stored in low vacuum and under ambient conditions while being comparable to those immediately after UV and DBD plasma treatments.</P> <P><B>Significance</B></P> <P>UV treatment of the titanium implants followed by wet storage is useful for maintaining enhanced biological activity and overcoming biological aging during shelf storage.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Surface properties of UV-irradiated titanium discs stored differently are compared. </LI> <LI> The storage period is 28days after 15min UV treatment. </LI> <LI> The discs were stored in distilled water, low vacuum, and ambient air. </LI> <LI> Hydrophilicity of titanium surface was maintained after storage in distilled water. </LI> <LI> Samples stored in distilled H<SUB>2</SUB>O showed enhanced biological activity. </LI> </UL> </P>

      • ATLS Hypovolemic Shock Classification by Prediction of Blood Loss in Rats Using Regression Models :

        <P>In our previous study, our input data set consisted of 78 rats, the blood loss in percent as a dependent variable, and 11 independent variables (heart rate, systolic blood pressure, diastolic blood pressure, mean arterial pressure, pulse pressure, respiration rate, temperature, perfusion index, lactate concentration, shock index, and new index (lactate concentration/perfusion)). The machine learning methods for multicategory classification were applied to a rat model in acute hemorrhage to predict the four Advanced Trauma Life Support (ATLS) hypovolemic shock classes for triage in our previous study. However, multicategory classification is much more difficult and complicated than binary classification. We introduce a simple approach for classifying ATLS hypovolaemic shock class by predicting blood loss in percent using support vector regression and multivariate linear regression (MLR). We also compared the performance of the classification models using absolute and relative vital signs. The accuracies of support vector regression and MLR models with relative values by predicting blood loss in percent were 88.5% and 84.6%, respectively. These were better than the best accuracy of 80.8% of the direct multicategory classification using the support vector machine one-versus-one model in our previous study for the same validation data set. Moreover, the simple MLR models with both absolute and relative values could provide possibility of the future clinical decision support system for ATLS classification. The perfusion index and new index were more appropriate with relative changes than absolute values.</P>

        Utility of sentinel lymph node biopsy in papillary thyroid microcarcinoma

        Purpose: There are many studies on sentinel lymph node (SLN) biopsy in thyroid carcinoma but SLN biopsy (SLNB) in papillary thyroid carcinoma (PTC) remains open to debate. Therefore in this retrospective study, the usefulness of SLNB in thyroid carcinoma patients who had micro-PTC without cervical lymphadenopathy was assessed. Methods: SLNB was performed in 114 patients who were diagnosed with micro-PTC in a single lobe without palpable or ultrasound-detected lymph node at the tertiary center between January 2012 and December 2013. After SLNB, all patients underwent total thyroidectomy and central neck dissection or thyroid lobectomy and central neck dissection of the single side. Results: SLNs were identified in 112 of 114 patients with 41 positive SLNs and 71 negative SLNs on intraoperative frozen sections. However, eight negative patients were found to be positive in the final pathology. Sentinel node identification rate and false negative value of SLNB were 98.2% and 11.3%, respectively. In the univariate analysis, higher lymph node metastasis was detected in men than in women. Higher detection number of SLN showed higher probability of lymph node metastasis. Conclusion: SLNB may be helpful in papillary thyroid cancer, especially in male patients. Also, it is useful for the staging of nodal status and clearance of persistent disease

        Rapid growth of management Information system (MIS) and evolution of software system enlarge maintenance necessity and it need a process to implement information system service in business. This paper proposes service-oriented maintenance process of component software based on service design and implementation provided by existing processes for maintenance, MaRMI-RE and ITIL. The Proposed process derives activities and tasks of the process by identifying services according to service guidelines. Service-oriented maintenance process is produced by analyzing correlation between the derived activities and tasks and MaRMI-RE's and adding the tasks to MaRMI-RE. The verify availability of the proposed service-oriented maintenance process, correlation between artifacts is proved.

        간성 뇌중 환자에서 Flumazenil 의 효과 및 안정성에 관한 전향성 연구

        N/A Objectivea: Ther are still many controversies on the pathogenesis of the hepatic encephalopathy. Recently, it has been postulated that endogenous benzodiazepine like substance (s) may be involved in the neuroinhibitory effect of gamma-aminobutyric acid (GABA). For this reason, benzodiazepine antagonist, flumazenil, has been advocated as a new treatment for hepatic encephalopathy. Several studies reported clinical improvement of hepatic encephalopathy in patients treated with flumazenil but these were not controlled studies. Methods: To investigate the effect of flumazenil, we performed a double-blind, prospective study in the 13 patients with hepatic encephalopathy who fulfilled the inclusion criteria of the protocol. The drug was administered intravenously (flumazenil 2.2 mg bolus injection followed by a continuous infusion with a rate of 1 mg/ h or placebo) in coded vial. After the code was open, 6 patients received flumazenil and the other 7 palcebo. The ages were 36 to 61(mean 54) years and 7 were male and 6 female. The causes of liver cirrhosis were HBV related in 7, HCV related in 3, and 3 were alcoholics. Results: In the fumazenil group, 4(67%) demonstrated immediate improvement of mental state with various degrees (from stage III to II in 1 and from stage I & II to 0 in 3). The significant and immediate improvement in the electroencephalographic (EEG) findings were noted in 3(50%), In the placebo group, however, 2(29%) showed an immediate improvement in EEG findings but only one (14%) showed the immediate improvement of mental status. There was no evidence of significant adverse reaction related to the treatment. Conclusion: This study shows that the benzodiazepine antagonist flumazenil may be effective and safe in patients with hepatic encephalopathy in the immediate mangement. Long term beneficial effect may be expected by treating the patients with more convinient oral administration.

        Locally Advanced, Unresectable Pancreatic Cancer Treated by Stereotactic Radiation Therapy

        목 적: 국소적으로 진행된, 절제 불가능한 췌장암 치료에 있어 고식적 방사선 치료와 비교하여 CyberKnife (CK)를 이용한 정위 방사선 치료의 생존율 및 급성 독성에 대해 분석하고자 하였다. 대상 및 방법: 2003년 4월부터 2004년 4월까지의, Eastern Cooperative Oncology Group (ECOG) 활동도 3 이하이며 CT 및 PET/CT로 평가하여 원격 전이 없는 국소 진행된 췌장암 환자 19명을 대상으로 하였다. 대상 환자는 점차 방사선량을 증가시키는 방법으로 33 Gy, 36 Gy, 39 Gy를 3분할로 각각 6명, 4명, 9명에서 CK를 이용한 정위 방사선 치료를 시행하였으며, 생존율 및 Radiation Therapeutic Oncology Group (RTOG) acute radiation morbidity criteria에 의한 위장관 독성을 분석하였다. 또한 나이, 성별, ECOG 수행 점수, 항암 치료,우회로 조성술(bypass surgery) 여부, 방사선량, CA19-9, 계획용 표적 체적(planning target volume, PTV), CT상 주위 장기 및 혈관 침범 여부 등을 Log Rank test를 이용하여 예후 인자를 평가하였다.결 과: 본 연구에서 중앙 생존 기간은 11개월, 1년 생존율은 36.8%였다. 추적 조사 기간 중(범위 3∼20개월, 중앙값 10개월) 유의한 위장관 급성 독성은 관찰되지 않았다. 단일 인자 분석에서 계획용 표적 체적만이 유의한 예후 인자로 80 cc 이하인 경우가 80 cc 이상인 경우보다 생존율이 높았으며(p-value<0.05), 나이, 성별, ECOG 수행 점수, 항암 치료, 우회로 조성술, CA19-9 수치, 주위 장기 및 혈관 침범 여부 등에서는 통계적으로 유의한 차이를 보이지 않았다. 다인자 분석에서는 65세 이하인 경우와 PTV 80 cc 이하인 경우에서 생존율이 높았다. 결 론: 고식적 방사선 치료, 고선량 입체조형 방사선 치료(high dose conformal radiation therapy), 수술 중 방사선 치료(intraoperative radiation therapy) 또는 세기 조절 방사선 치료(intensity modulated radiation therapy, IMRT)를 이용한 최근의 결과와 비교하여 CK를 이용한 정위 방사선 치료는 생존율 측면에서 비슷하거나 나은 결과를 보였다. 또한 심각한 부작용은 관찰되지 않았으며 짧은 기간의 치료로 환자에게 편의를 제공할수 있어 결과적으로 삶의 질을 향상시킬 수 있을 것이다. 따라서, 이 새로운 치료 방법은 국소 진행된, 절제불가능한 췌장암 환자에서 심각한 부작용 없는 효과적인 치료가 될 것으로 생각된다. 또한 계획용 표적 체적은 CK 치료의 유용한 예후 인자로 사용될 것이다. Purpose: In order to find out whether stereotactic radiation therapy (RT) using CyberKnife (CK) could improve survival rate and lower acute toxicity compared to conventional RT. Materials and Methods: From April 2003 through April 2004, 19 patients with Eastern Cooperative Oncology Group (ECOG) performance status ≤3 and locally advanced pancreas cancer without distant metastasis, evaluated by CT or PET/CT, were included. We administered stereotactic RT consisting of either 33 Gy, 36 Gy or 39 Gy in 3 fractions to 6, 4 and 9 patients, respectively, in an effort to increase the radiation dose step by step, and analyzed the survival rate and gastrointestinal toxicities by the acute radiation morbidity criteria of Radiation Therapeutic Oncology Group (RTOG). Prognostic factors of age, sex, ECOG performance score, chemotherapy, bypass surgery, radiation dose, CA19-9, planning target volume (PTV), and adjacent organ and vessel invasion on CT scan were evaluated by Log Rank test. Results: The median survival time was 11 months with 1-year survival rate of 36.8%. During follow-up period (range 3∼20 months, median 10 months), no significant gastrointestinal acute toxicity (RTOG grade 3) was observed. In univariate analysis, age, sex, ECOG performance score, chemotherapy, bypass surgery, radiation dose, CA19-9 level, and adjacent organ and vessel invasion did not show any significant changes of survival rate, however, patients with PTV (80 cc showed more favorable survival rate than those with PTV>80 cc (p-value<0.05). In multivariate analysis, age younger than 65 years and PTV>80 cc showed better survival rate. Conclusion: In terms of survival, the efficacy of stereotactic radiation therapy using CK was found to be superior or similar to other recent studies achieved with conventional RT with intensive chemotherapy, high dose conformal RT, intraoperative RT (IORT), or intensity modulated RT (IMRT). Furthermore, severe toxicity was not observed. Short treatment time in relation to the short life expectancy gave patients more convenience and, finally, quality of life would be increased. Consequently, this could be regarded as an effective novel treatment modality for locally advanced, unresectable pancreas cancer. PTV would be a helpful prognostic factor for CK.

        <P>Robotic prostatectomy (RP) has been reported to be technically challenging in patients with a history of prior complex lower abdominal or pelvic surgery, morbid obesity, large prostate, prior pelvic irradiation, neoadjuvant hormonal therapy, or prior prostate surgery. In this paper, we report an experience of RP in a prostate cancer patient with abdominoperineal resection, adjuvant chemotherapy, and pelvic irradiation for rectal cancer.</P>
