        Proton Pump Inhibitor-unresponsive Laryngeal Symptoms Are Associated With Psychological Comorbidities and Sleep Disturbance: A Manometry and Impedance-pH Monitoring Study

        Background/AimsLaryngeal symptoms are largely treated with empiric proton pump inhibitor (PPI) therapy if no apparent pathology shown on ear, nose, and throat evaluation and reflux-related etiologies are suspected. However, treatment response remains unsatisfactory. This study aimed to investigate the clinical and physiological characteristics of patients with PPI-refractory laryngeal symptoms. MethodsPatients with persistent laryngeal symptoms despite PPI treatment for ≥ 8 weeks were recruited. A multidisciplinary evaluation comprising validated questionnaires for laryngeal symptoms (reflux symptom index [RSI]), gastroesophageal reflux disease symptoms, psychological comorbidity (5-item brief symptom rating scale [BSRS-5]) and sleep disturbance (Pittsburgh sleep quality index [PSQI]), esophagogastroduodenoscopy, ambulatory impedance-pH monitoring, and high-resolution impedance manometry were performed. Healthy asymptomatic individuals were also recruited for comparison of psychological morbidity and sleep disturbances. ResultsNinety-seven adult patients and 48 healthy volunteers were analyzed. The patients had markedly higher prevalence of psychological distress (52.6% vs 2.1%, P < 0.001) and sleep disturbance (82.5% vs 37.5%, P < 0.001) than the healthy volunteers. There were significant correlations between RSI and BSRS-5 scores, and between RSI and PSQI scores (r = 0.26, P = 0.010, and r = 0.29, P = 0.004, respectively). Fifty-eight patients had concurrent gastroesophageal reflux disease symptoms. They had more prominent sleep disturbances (89.7% vs 71.8%, P < 0.001) than those with laryngeal symptoms alone but similar reflux profiles and esophageal motility. ConclusionsPPI-refractory laryngeal symptoms are mostly associated with psychological comorbidities and sleep disturbances. Recognition of these psychosocial comorbidities may help optimize management in these patients.

      • Subsampling-Based Image Tamper Detection and Recovery Using Quick Response Code

        Quick response (QR) code is the prevalent trademark for a type of matrix barcode symbol. That code is always scanned to efficiently acquire data, especially for mobile device users. It involves the capabilities of data storage, reliable readability and strong error correction. This paper utilizes these properties of a QR code to propose an image tamper detection and recovery scheme for grayscale images. The image to be protected is first subsampled and decomposed the principal energy compaction for image blocks. These are regarded as the image authentication and recovery data and then are encoded into the pattern of a QR code. The QR code is subsequently embedded into the original image. Experimental results showed that the authentication and recovery data in a QR code is able to sustain certain perceptible distortions such that the proposed scheme can detect tampered regions clearly and recover them roughly. Moreover, the proposed scheme also provides a better embedded image quality in comparison with the previous method.

        Prediction of Mortality after Emergent Transjugular Intrahepatic Portosystemic Shunt Placement: Use of APACHE II, Child-Pugh and MELD Scores in Asian Patients with Refractory Variceal Hemorrhage

        Objective: This study was designed to determine if existing methods of grading liver function that have been developed in non-Asian patients with cirrhosis can be used to predict mortality in Asian patients treated for refractory variceal hemorrhage by the use of the transjugular intrahepatic portosystemic shunt (TIPS) procedure. Materials and Methods: Data for 107 consecutive patients who underwent an emergency TIPS procedure were retrospectively analyzed. Acute physiology and chronic health evaluation (APACHE II), Child-Pugh and model for end-stage liver disease (MELD) scores were calculated. Survival analyses were performed to evaluate the ability of the various models to predict 30-day, 60-day and 360-day mortality. The ability of stratified APACHE II, Child-Pugh, and MELD scores to predict survival was assessed by the use of Kaplan-Meier analysis with the log-rank test. Results: No patient died during the TIPS procedure, but 82 patients died during the follow-up period. Thirty patients died within 30 days after the TIPS procedure; 37 patients died within 60 days and 53 patients died within 360 days. Univariate analysis indicated that hepatorenal syndrome, use of inotropic agents and mechanical ventilation were associated with elevated 30-day mortality (p < 0.05). Multivariate analysis showed that a Child-Pugh score > 11 or an MELD score > 20 predicted increased risk of death at 30, 60 and 360 days (p < 0.05). APACHE II scores could only predict mortality at 360 days (p < 0.05). Conclusion: A Child-Pugh score > 11 or an MELD score > 20 are predictive of mortality in Asian patients with refractory variceal hemorrhage treated with the TIPS procedure. An APACHE II score is not predictive of early mortality in this patient population.       • Hydroxyapatite prepared from eggshell and mulberry leaf extract by precipitation method

        Eggshell is a waste material after the usage of egg. In this work, biowaste chicken eggshells were used for preparing carbonated hydroxyapatite (HA) nanoparticles of high purity through aqueous precipitation method at room temperature. The eggshell-derived HA will be a cost-effective bioceramics for biomedical applications and an effective material-recycling technology. Additionally, mulberry leaf extract was used as a template to regulate the morphology, size and crystallinity of HA, and the effects of pH value were also examined. Characterization of the samples was performed by X-ray diffraction (XRD) and Fourier transform infrared (FT-IR) spectroscopy. Scanning electron microscopy (SEM) was used to determine the size, shape and morphology of HA. The results indicate that only one phase of HA were synthesized in the both absence and presence of mulberry leaf extract at pH of 7 and above, while DCPD or DCPA/DCPD phase was observed at pH 4 condition. The crystallite sizes of the HA samples obviously decreased when adding mulberry leaf extract as a template, while they decreased gradually as the solution pH levels increased. With increasing pH level from 7 to 14, the rod-like HA nanoparticles gradually changed to spherical shape at pH 14. Note that, the obtained product is Mg and Sr containing A- and B-type carbonate HA at alkaline pH and it can be a potential material for biomedical applications.

        Mest Attenuates CCl4-Induced Liver Fibrosis in Rats by Inhibiting the Wnt/β-Catenin Signaling Pathway

        Background/Aims: The Wnt/β-catenin signaling pathway has been reported to play an important role in liver fibrosis. This study was designed to investigate whether mesoderm-specific transcript homologue (Mest), a strong negative regulator of Wnt/β-catenin signaling, could inhibit liver fibrosis. Methods: pcDNA-Mest was transfected into hepatic stellate cells (HSCs) and rats. Rats were randomly divided into four groups: normal group (normal saline), treatment group (pcDNA- Mest+CCl4), control group (pcDNA-neo+CCl4), and model group (normal saline+CCl4). Changes in liver pathology were evaluated by hematoxylin and eosin and Masson’s trichrome staining. The levels of alanine transaminase, aspartate transaminase, lactic dehygrogenase, hyaluronic acid, and laminin in the serum and hydroxyproline in the liver were detected by biochemical examination and radioimmunoassay, respectively. The expression and distribution of β-catenin, α-smooth muscle actin (α-SMA), Smad3, and tissue inhibitor of metalloproteinase type I were determined, and the viability of the HSCs was tested. Results: Our data demonstrate that Mest alleviated CCl4-induced collagen deposition in liver tissue and improved the condition of the liver in rats. Mest also significantly reduced the expression and distribution of β-catenin, α-SMA and Smad3 both in vivo and in vitro, in addition to the viability of HSCs in vitro. Conclusions: We found that Mest attenuates liver fibrosis by repressing β-catenin expression, which provides a new therapeutic approach for treating liver fibrosis.

        Characterization of nanosized hydroxyapatite prepared by an aqueous precipitation method using eggshells and mulberry leaf extract

        Hydroxyapatite (HA) has been vigorously studied for orthopedic and dental applications due to its excellent bioactivity, osteoconductivity, and osteoinductivity. This study aims to present a simple room-temperature aqueous precipitation method for obtaining carbonated HA nanoparticles of high purity from biowaste chicken eggshells. Biowaste chicken eggshells were used for preparing HA through aqueous precipitation method. Moreover, mulberry leaf extract was used as a template to regulate the morphology, size, and crystallinity of HA. All produced nanocrystalline HA powders exhibited rod-like particle agglomerates of a size below 100 nm. The HA particles synthesized with the mulberry leaf extract showed higher crystallinity and slightly decreased crystallite sizes compared to the samples prepared without adding the extract. Carbonate peaks observed for the specimens closely matched those of A- and B-type carbonates, which can contribute to the low crystallinity. Notably, the HA synthesized from the eggshell powders contains several important trace elements such as Mg and Sr, which are benefi cial to the overall biological performance as biomaterials. The morphology, size, structure, and composition of the obtained HA products are similar to those of natural bones, and consequently, the products show potential as a material for biomedical applications.

      • Effect of ethyl alcohol aging on the apatite formation of a low-modulus Ti-7.5Mo alloy treated with aqueous NaOH

        The purpose of this experiment was to evaluate the apatite-formation abilities of low-modulus Ti-7.5Mo substrates treated with NaOH aqueous solutions and subsequent ethyl alcohol aging before soaking them in simulated body fluid. Specimens of Ti-7.5Mo were initially treated with 5 M NaOH at $60^{\circ}C$ for 24 h, resulting in the formation of a porous network structure composed of sodium hydrogen titanate. Afterwards, the specimens were aged in ethyl alcohol at $60^{\circ}C$ for 5 or 10 min, and subsequently immersed in simulated body fluid at $37^{\circ}C$ for 3, 7 and 14 days. Ethyl alcohol aging significantly increased the apatite-forming abilities of Ti-7.5Mo. The amount of apatite deposited on the Ti-7.5Mo after NaOH treatment and subsequent ethyl alcohol aging was much greater, especially after the Ti-7.5Mo specimens were aged for 5 min. Due to its excellent combination of bioactivity, low elastic modulus and low processing costs, the Ti-7.5Mo treated with NaOH aqueous solutions and subsequently aged in ethyl alcohol has promising heavy load-bearing applications.

      • Differential Protein Expression Profile Between CD20 Positive and Negative Cells of the NCI-H929 Cell Line

        At present, multiple myeloma (MM) remains an incurable disease and cologenic cells may be responsible for disease relapse. It has been proposed that CD20+/CD138- NCI-H929 cells could be hallmarks of MM clonogenic cells. Here, the immunology phenotype of NCI-H929 cells is described. Only a small population of CD20+/CD138- cells (<1%) was found in the NCI-H929 cell line, but CD20+/CD138- cells were not detected. We found that CD20+/CD138+ cells were able to exhibit cologenic capacity by colony formation assay and continuous passage culture. Proteins were analyzed by 1D-SDS-PAGE and TMT based quantitative differential liquid chromatography tandem mass spectrometry (LC-MS/MS). 1,082 non-redundant proteins were identified, 658 of which were differentially expressed with at least a 1.5-fold difference. 205 proteins in CD20+ cells were expressed at higher levels and 453 proteins were at lower levels compared with CD20- cells. Most proteins had catalytic and binding activity and mainly participated in metabolic processes, cell communication and molecular transport. These results proved that there are different biological features and protein expression profile between CD20+ and CD20- cells in the NCI-H929 cell line.

        American ginseng attenuates azoxymethane/dextran sodium sulfate-induced colon carcinogenesis in mice

        Background: Colorectal cancer is a leading cause of cancer-related death, and inflammatory bowel disease is a risk factor for this malignancy. We previously reported colon cancer chemoprevention potential using American ginseng (AG) in a xenograft mice model. However, the nude mouse model is not a gut-specific colon carcinogenesis animal model. Methods: In this study, an experimental colitis and colitis-associated colorectal carcinogenesis mouse model, chemically induced by azoxymethane/dextran sodium sulfate (DSS) was established and the effects of oral AG were evaluated. The contents of representative ginseng saponins in the extract were determined. Results: AG significantly reduced experimental colitis measured by the disease activity index scores. This suppression of the experimental colitis was not only evident during DSS treatment, but also very obvious after the cessation of DSS, suggesting that the ginseng significantly promoted recovery from the colitis. Consistent with the anti-inflammation data, we showed that ginseng very significantly attenuated azoxymethane/DSS-induced colon carcinogenesis by reducing the colon tumor number and tumor load. The ginseng also effectively suppressed DSS-induced proinflammatory cytokines activation using an enzyme-linked immunosorbent assay array, in which 12 proinflammatory cytokine levels were assessed, and this effect was supported subsequently by real-time polymerase chain reaction data. Conclusion: AG, as a candidate of botanical-based colon cancer chemoprevention, should be further investigated for its potential clinical utility.
