        Management of stage II endometrial cancer and subsequent oncologic outcomes: a National Cancer Database study

        Objective: The management of stage II endometrial cancer (EC) is challenging due to thewide variation in surgical practice and adjuvant treatment recommendations. We sought todescribe the treatment patterns for patients with stage II EC and to evaluate the associationbetween surgical management and adjuvant therapy on survival outcomes in a large cohort ofpatients with stage II EC. Methods: Using data from the National Cancer Database, we identified 9,690 women withstage II EC. We used logistic regression to identify association of sociodemographic and tumorcharacteristics with surgery type and receipt of adjuvant therapy. We used Cox proportionalhazards regression models to estimate hazard ratios (HRs) and 95% confidence intervals (CIs)for associations between adjuvant therapy, hysterectomy type, and overall survival. Results: Almost 11% of the cohort underwent radical hysterectomy; however, there wasno difference in survival between surgical types even when adjusted for adjuvant therapy(HR=0.94; 95% CI=0.82–1.07). Compared to no adjuvant treatment, radiation only(HR=0.66; 95% CI=0.61–0.73) and combination radiation and chemotherapy (HR=0.53;95% CI=0.45–0.62) were associated with lower risk of death. There was no survival benefit ofchemotherapy alone even when separated by histologic subtype (HR range, 0.55–1.46). Conclusions: Women with stage II EC do not appear to benefit from routine radical hysterectomythough all patients appear to benefit from receipt of radiation therapy (RT), regardless ofmodality. Additionally, there may be an added survival benefit with the combination of computedtomography and RT in patients with non-endometrioid, high-risk histologies.

        Case Report of a Laparoscopic Primary Diaphragmatic Repair in Acute Penetrating Trauma: Is It Within the Realm of an Acute Care Surgeon?

        The laparoscopic repair of traumatic diaphragmatic injuries in the acute phase has not been well established. The current consensus view is an elective repair by a surgeon with an advanced laparoscopic skill set. This case report illustrates the position that with judicious patient selection an acute repair is not only possible but well within the realm of an acute care surgeon. Anterior diaphragmatic injuries with penetrating trauma can even be repaired primarily without the need for prosthetic reinforcement. It is hoped that this case report will add to the surgical armamentarium of the acute care surgeon.

        Low-Sampling Rate UWB Channel Characterization and Synchronization

        We consider the problem of low-sampling rate high-resolution channel estimation and timing for digital ultrawideband (UWB) receivers. We extend some of our recent results in sampling of certain classes of parametric non-bandlimited signals and develop a frequency domain method for channel estimation and synchronization in ultra-wideband systems, which uses sub-Nyquist uniform sampling and well-studied computational procedures. In particular, the proposed method can be used for identification of more realistic channel models, where different propagation paths undergo different frequency-selective fading. Moreover, we show that it is possible to obtain high-resolution estimates of all relevant channel parameters by sampling a received signal below the traditional Nyquist rate. Our approach leads to faster acquisition compared to current digital solutions, allows for slower A/D converters, and potentially reduces power consumption of digital UWB receivers significantly.

        Mineral Composition of Different Strains of Edible Medicinal Mushroom Agaricus subrufescens Peck

        Agaricus subrufescens Peck is a well-known Basidiomycota fungus (Royal Sun Agaricus), with rising demand in consumption and production worldwide. This particular mushroom with high medical value has been used successfully in cancer therapy and in the treatment of some bacterial and viral diseases. Four strains of A. subrufescens (Si2.2, 853, 1105, and 2603) were cultivated, and 22 mineral elements of basidiomes (fruit bodies) were analyzed (caps and stipes separately). The data obtained about the mineral compositions were compared to the “reference” Agaricus bisporus (white button mushroom) and to the average of wild growing Agaricus species. The mineral composition of A. subrufescens strains can be characterized by the following: (1) high levels of valuable macroelements, i.e., potassium (28–30,000mg/kg of dry matter), phosphorus (7–11,000mg/kg of dry matter), and calcium and magnesium (for both elements, 1,000–1,500mg/kg of dry matter); (2) significantly higher level of copper (compared to A. bisporus, 70–150mg/kg of dry matter) and zinc (140–250mg/kg of dry matter); (3) low quantity of sodium (140–180mg/kg of dry matter); (4) attention should paid to the detectable amount of cadmium (2–17mg/kg of dry matter) in strain Si2.2; (5) low or undetectable concentrations of some other poisonous microelements like As, Cr, and V; and (6) the distribution of elements in caps and stipes is characteristic—the majority of beneficial elements have higher contents in caps than in stipes, but some other elements, such as Ca, Fe, and Na, appear in an inverse proportion. In conclusion, it can be said that the mineral composition of A. subrufescens is definitely positive, with the exception of the above-mentioned Cd level.

        Optically Controlled Silicon MESFET Fabrication and Characterizations for Optical Modulator/Demodulator

        An optically controlled silicon MESFET (OPFET) was fabricated by diffusion process to enhance the quantum efficiency, which is the most important optoelectronic device performance usually affected by ion implantation process due to large number of process induced defects. The desired impurity distribution profile and the junction depth were obtained solely with diffusion, and etching processes monitored by atomic force microscope, spreading resistance profiling and C-V measurements. With this approach fabrication induced defects are reduced, leading to significantly improved performance. The fabricated OPFET devices showed proper I-V characteristics with desired pinch-off voltage and threshold voltage for normally-on devices. The peak photoresponsivity was obtained at 620 ㎚ wavelength and the extracted external quantum efficiency from the photoresponse plot was found to be approximately 87.9%. This result is evidence of enhancement of device quantum efficiency fabricated by the diffusion process. It also supports the fact that the diffusion process is an extremely suitable process for fabrication of high performance optoelectronic devices. The maximum gain of OPFET at optical modulated signal was obtained at the frequency of 1 MHz with rise time and fall time approximately of 480 nS. The extracted transconductance shows the possible potential of device speed performance improvements for shorter gate length. The results support the use of a diffusion process for fabrication of high performance optoelectronic devices.

      • Comparison of mass operator methods considering test uncertainties

        In the space industry, structures undergo several vibration and acoustic tests in order to verify their design and give confidence that they will survive the launch and other critical in-orbit dynamic scenarios. At component level, vibration tests are conducted with the aim to reach local or global interface loads without exceeding the design loads. So, it is often necessary to control and limit the input based on a load criterion. This means the test engineer should be able to assess the interface loads, even when load cannot be measured. This paper presents various approaches to evaluate interface loads using measured accelerations and by referring to mass operators. Various methods, from curve fitting techniques to finite element-based methods are presented. The methods are compared using signals with known imperfection to identify strengths and weaknesses of each mass operator definition.

      • Identification of p54<sup>nrb</sup> and the 14-3-3 Protein HS1 as TNF-α-Inducible Genes Related to Cell Cycle Control and Apoptosis in Human Arterial Endothelial Cells

        TNF-$\alpha$ plays a pivotal role in inflammation processes which are mainly regulated by endothelial cells. While TNF-$\alpha$ induces apoptosis of several cell types like tumor cells, endothelial cells are resistant to TNFa mediated cell death. The cytotoxic effects of TNF-$\alpha$ on most cells are only evident if RNA or protein synthesis is inhibited, suggesting that de novo RNA or protein synthesis protect cells from TNF-$\alpha$ cytotoxicity, presumably by NF-${\kappa}B$ mediated induction of protective genes. However, the cytoprotective genes involved in NF-${\kappa}B$ dependent endothelial cell survival have not been sufficiently identified. In the present study, the suppression subtractive hybridization (SSH) method was employed to identify rarely transcribed TNF-$\alpha$ inducible genes in human arterial endothelial cells related to cell survival and cell cycle. The TNF-$\alpha$-induced expression of the RNA binding protein $p54^{nrb}$ and the 14-3-3 protein HS1 as shown here for the first time may contribute to the TNF-$\alpha$ mediated cell protection of endothelial cells. These genes have been shown to play pivotal roles in cell survival and cell cycle control in different experimental settings. The concerted expression of these genes together with other genes related to cell protection and cell cycle like DnaJ, $p21^{cip1}$ and the ubiquitin activating enzyme E1 demonstrates the identification of new genes in the context of TNF-$\alpha$ induced gene expression patterns mediating the prosurvival effect of TNF-$\alpha$ in endothelial cells.

        Patient and nonradiographic tumor characteristics predicting lipid-poor angiomyolipoma in small renal masses: Introducing the BEARS index

        Purpose: To create a simple model using clinical variables for predicting lipid-poor angiomyolipoma (AML) in patients with small renal masses presumed to be renal cell carcinoma (RCC) from preoperative imaging. Materials and Methods: A series of patients undergoing partial nephrectomy (PN) for renal masses ≤4 cm was identified using a prospectively maintained database. Patients were excluded if standard preoperative imaging was not consistent with RCC. Chi square and Mann-Whitney U analyses were used to evaluate differences in characteristics between patients with AML and other types of pathology. A logistic regression model was constructed for multivariable analysis of predictors of lipid-poor AML. Results: A total of 730 patients were identified that underwent PN for renal masses ≤4 cm between 2007–2015, including 35 with lipid-poor AML and 620 with RCC. In multivariable analysis, the following features predicted AML: female sex (odds ratio, 6.89; 95% confidence interval, 2.35–20.92; p<0.001), age <56 years (2.84; 1.21–6.66; p=0.02), and tumor size <2 cm (5.87; 2.70–12.77; p<0.001). Sex, age, and tumor size were used to construct the BEnign Angiomyolipoma Renal Susceptibility (BEARS) index with the following point values for each particular risk factor: female sex (2 points), age <56 years (1 point), and tumor size <2 cm (2 points). Within the study population, the BEARS index distinguished AML from malignant lesions with an area under the curve of 0.84. Conclusions: Young female patients with small tumors are at risk for having lipid-poor AML despite preoperative imaging consistent with RCC. Identification of these patients may reduce the incidence of unnecessary PN for benign renal lesions.
