        Proposal of novel staging system CNM (Crohn’s primary site, nodes, mesentery) to predict postoperative recurrence of Crohn’s disease

        After oncologic resection, histological grading and staging of the tumor give important prognostic information about the future risk of recurrence and hence influence the subsequent management plan. Several studies and their meta-analysis have shown that various histological features (e.g., microscopic positive resection margins, plexitis, granuloma, mesenteric inflammatory activity) can predict postoperative clinical/endoscopic/surgical recurrence after resection in Crohn’s disease (CD). Inclusion of mesentery in surgical resection specimens has been shown to reduce surgical recurrence after ileocolonic resection in CD. However, there is no uniform histopathological staging system for risk stratification in postoperative CD to systematically predict postoperative recurrence. This is because the prediction to date is based on clinical characteristics (smoking status, disease phenotype, surgical history). Histopathological predictors are still not adopted in routine clinical practice due to the lack of a uniform staging system, heterogeneity of published studies and lack of standardized definition of histological features. In this article, we attempted to incorporate all such histological features in a single histological staging system CNM (Crohn’s primary site [resection margin positivity, plexitis, granuloma, depth of infiltration], nodes [presence of granuloma], mesentery [involved or not]) in surgical resection specimen in CD. The proposed CNM classification would help to enable systematic reporting, design future clinical trials, stratify postoperative recurrence risk and choose appropriate postoperative prophylaxis.

      • Fostering e-Governance to fight Corruption: The Indian Experience

        Much has been said and written about corruption the world over. Various strategies and mechanisms to cope with this basic issue have been adopted, but with less success. Corruption became systemic in India, especially, when the licence-permit quota raj was a preferred option. Globalization, in fact, spawned a new wave of corruption, albeit going by the name of corporate lobbying. This phenomenon is not entirely new, either for the country or its people. It`s not that we are confronting the problem of corruption alone. We have to think of its other variants ? crony capitalism, corporate favors, doling out land, water and energy and topping it up with tax holidays. These variants are subtle and their dynamics are hard to be understood by the common man and society. Also, these have taken a form of public policy that drives the national economy. In this context, this paper tries to put into perspective the possibility of tackling corruption, at least partially, by adopting the process of e-governance and the structures of e-government. Egovernance offers a partial solution to the multifaceted problem of corruption. It reduces discretion, thereby curbing some opportunities for arbitrary action. It increases chances of exposure by maintaining detailed data on transactions, making it possible to track and link the corrupt with their wrongful acts. By making rules simpler and transparent, e-government emboldens citizens to question illogical procedures and their arbitrary application.

        Post-Neutron Mass Yield Distribution and Photo-Neutron Cross-Section Measurements in 209Bi with 65-MeV Bremsstrahlung

        The post-neutron-emission mass yield distribution and the photo-neutron cross-section σ (γ,3n)) of 209Bi with 65-MeV bremsstrahlung have been determined using an off-line gamma-ray spectrometric technique in the test electron linac at Pohang Accelerator Laboratory. The post-neutron mass yield distribution in 65-MeV bremsstrahlung induced fission of 209Bi is symmetric with average mass around 103 and a FWHM of 22 mass units, which is higher than the literature value of 19 mass units in the energy range of 28 -- 40 MeV. The increasing trend of the FWHM of the post-neutron mass yield distribution with increasing breamsstrahlung energy has been explained from the point of view of the increase in the multi-chance fission probabilities with increasing excitation energy. The (γ,3n) reaction cross-section is determined for the first time. It was also calculated using the TALYS code and was found to be in good agreement with the experimental value.         <P>Chemical investigation of a Korean marine sponge, <I>Monanchora</I> sp., yielded nine new sesterterpenoids (<B>1</B>–<B>9</B>) along with phorbaketals A–C (<B>10</B>–<B>12</B>). The planar structures were established on the basis of NMR and MS analysis, and the absolute configurations of <B>1</B>–<B>9</B> were defined using the modified Mosher’s method and CD spectroscopic data analysis. Compounds <B>1</B>–<B>8</B>, designated as phorbaketals D–K, possess a spiroketal-modified benzopyran moiety such as phorbaketal A, and their structural variations are due to oxidation and/or reduction of the tricyclic core or the side chain. Compound <B>9</B>, designated as phorbin A, has a monocyclic structure and is proposed to be a possible biogenetic precursor of the phorbaketals.         Factors Affecting Trial Without Catheter for First Spontaneous Acute Urinary Retention

        Purpose: To find the association of trial without catheter (TWOC) outcome for first spontaneous acute urinary retention (AUR) in benign prostatic obstruction with age, prior lower urinary tract symptoms (LUTS), retention volume at catheterization (RV), and size of prostate. Methods: Our prospective observational analytical (interventional) study enrolled 77 cases of spontaneous AUR over 24 months. After clinical evaluation, digital rectal examination, and transabdominal ultrasonography, all patients were catheterized per urethra and their RV was recorded. TWOC was administered after 2 or 3 doses of 0.4 mg tamsulosin-oral absorption control system and after 48-72 hours had passed. A successful endpoint was defined as a maximum flow-rate, >5 mL/sec; voided volume, >100 mL; postvoid residue, <200 mL; and voiding within 6 hours of catheter removal. Data obtained from 58 patients were analyzed after excluding the cases lost to follow-up and secondary exclusion. Age, RV, duration of LUTS, and prostate volume on examination and ultrasonography (PUSG) were recorded and statistically analyzed. Prostate-specific antigen levels were obtained on follow-up and cases of cancer, as seen on transrectal ultrasound-guided biopsy, were secondarily excluded. Results: The patients had a mean age of 65.89±8.67 years. Prior LUTS was seen in 35 patients (2.07±2.91 months). The mean PUSG and RV were 46.81±20.58 mL and 854.8±36.26 mL, respectively. Thirty patients underwent a successful TWOC; a mean age of 63.13±8.58 years (mean±standard deviation; unpaired t-test; P=0.0053) and a PUSG of ≤45 mL (Pearson chi-square test; P=0.0427) were significantly associated with a successful outcome. Conclusion: There is a significant association between TWOC outcome, age (P=0.0053), and PUSG (P=0.0427).
