Background: Efficacy of chemotherapy plus bevacizumab has been shown in patients with metastatic colorectal cancer (mCRC) compared with chemotherapy alone. The aim of the present study was to evaluate the efficacy and safety of FOLFIRI or XELIRI regimens in combination with bevacizumab for mCRC patients in a first-line setting. Materials and Methods: A total of 132 patients with previously untreated and histologically confirmed mCRC were included. They were treated with either FOLFIRI-Bevacizumab (Bev) or XELIRI-Bev according to physician preference. The efficacy and safety of the two regimens were compared. Results: Between 2006 and 2010, 68 patients were treated with the XELIRI-Bev regimen, while the remaining 64 patients received the FOLFIRI-Bev regimen. The median age was 58.5 years (53.6 years in the FOLFIRI-Bev and 59.7 years in the XELIRI-Bev arm, p=0.01). Objective response rate was 51.6% for FOLFIRI-Bev versus 41.2% for XELIRI-Bev (p=0.38). At the median follow-up of 24.5 months, the median progression-free survival (PFS) was not different between two groups (14.2 months in FOLFIRI-Bev vs. not reached in the XELIRI-Bev, p=0.30). However, median overall survival time for the FOLFIRI-Bev arm was better than that for patients treated with XELIRIBev, but these differences was not statistically significant (37.8 months vs. 28.7 months, respectively, p=0.58). Most commonly reported grade 3-4 toxicities (FOLFIRI-Bev vs XELIRI-Bev) were nausea/vomiting (7.8% vs. 14.7%, p=0.27), diarrhea (10.9% vs 22.1%, p=0.10), hand-foot syndrome (0% vs 8.8%, p=0.02) and neutropenia (18.7% vs 27.9%, p=0.22). Conclusion: Our results showed that FOLFIRI-Bev and XELIRI-Bev regimens were similarly effective treatments in a first-line setting for patients with untreated mCRC, with manageable adverse event profiles.

The study is about the bounds of the spectral norms of r-circulant and geometric circulant matrices with the sequences called biperiodic Jacobsthal numbers. Then we give bounds for the spectral norms of Kronecker and Hadamard products of these r-circulant matrices and geometric circulant matrices. The eigenvalues and determinant of r-circulant matrices with the bi-periodic Jacobsthal numbers are obtained.
Characteristics of nanocomposite films deposited by atmospheric pressure uniform RF glow plasma

Characteristics of nanocomposite films synthesized and deposited by atmospheric pressure Radio-Frequency (RF) (13.56 MHz) uniform glow discharge are examined. The nanocomposite thin film deposition is carried out in the presence of titanium dioxide (TiO_2) with polymerization of pyrrole,thiophene and furan monomers in acetonitrile medium containing lithium perchlorate (LiClO_4). The chemical, morphological, thermal and electrical characteristics of polypyrrole/TiO_2 (PPy/TiO_2), polythiophene/TiO_2 (PT/TiO_2) and polyfuran/TiO_2 (PF/TiO_2) composites are examined by FTIR spectroscopy,scanning electron microscopy-energy dispersive X-ray analysis (SEM-EDX), atomic force microscopy (AFM), thermogravimetry (TGA) and four-probe conductivity measurements. The presence of PT bands in the FTIR spectra of PT/TiO_2 indicates that plasma polymerization did not cause thiophene ring opening as was reported in the literature. The thermograms indicate that the PPy/TiO_2 composite has the best thermal stability, whereas PT/TiO_2 has the best electrical conductivity. Morphology studies reveal that the composite film deposition occurs uniformly through polymerization of the monomer onto TiO_2nanoparticles.
Transumbilical scarless surgery with thoracic trocar

Purpose: Single-site laparoscopic surgery has become increasingly common. We herein report an easy and low-cost thoracic trocar technique (TTT) for these types of procedures and recommend the simpler name “transumbilical scarless surgery” (TUSS) to minimize confusion in nomenclature. Methods: We retrospectively reviewed patients who underwent TUSS by TTT using a thoracic trocar and surgical glove in our hospital between November 2011 and November 2012. Operating time, postoperative stay, and complications were detailed. Results: A total of 101 TUSS by TTT were successfully performed, comprising appendectomy (n = 63), ovarian cyst excision (n = 7), splenectomy (n = 5), nephroureterectomy (n = 5), orchidopexy (n = 4), pyeloplasty (n = 3), nephrolithotomy (n = 2), orchiectomy (n = 2), varicocelectomy (n = 2), lymphangioma excision (n = 2), ureterectomy (n = 1), Morgagni diaphragmatic hernia repair (n = 1), ovarian detorsion (n = 1), antegrade continence enema (n = 1), intestinal resection anastomosis (n = 1), and intestinal duplication excision (n = 1). Kirschner wires were used for some organ traction. Nine patients required an additional port, but no major complications occurred. The postoperative stay (mean ± standard deviation) was 3.2 ± 1.4 days, and operating time was 58.9 ± 38.3 minutes. Conclusion: We recommend the simpler name of TUSS to minimize confusion in nomenclature for all transumbilical single-incision laparoendoscopic surgeries. TTT is an easy and low-cost TUSS technique.

Objective: In recent years, many Syrian asylum seekers has died while they were crossing over sea to migrate to Europe. We aimed to identify the determinants of choice of Syrian asylum seekers for emigration to Europe; therefore, we selected the sample from refugee mental health clinic in Turkey. Methods: Our sample consists of 100 Syrian who applied to the refugee mental health branch polyclinic. Participants filled out the (e1) sociodemographic form, WHO-5 well-being index, and WHOQL. Then their future plans assessed by the interviewer. Results: It was seen that people who prefer to emigrate to Europe have higher level education level, lower income status, and had worse physical conditions at home. Also, They live generally in urban area, they do not have close relatives living in Syria, have poor mental well-being and have low quality of life scores. Job duration, absence of close relatives living in Syria, having low WHOOL score and physical conditions of home were found significant in regression analysis. Conclusion: We found that their economic circumstances significantly predicted their preference, unlike clinical variables. We believe in that, improving their economic conditions and the life quality of Syrian asylum seekers can prevent illegal migration by dangerous routes to Europe and policy makers over the world should consider this situation.
Colonic Metastasis from Carcinoma of the Breast that Mimicks a Primary Intestinal Cancer

Although the lung, liver, or bones are the most common location for distant metastases in breast cancer patients, metastases to the intestinal tract are very rarely recognized in the clinic. We will present an unusual case of colonic metastasis from a carcinoma of the breast that mimics a primary intestinal cancer, along with a through review of English language medical literature. Despite the fact that isolated gastrointestinal (GI) metastases are very rare and much less common than benign disease processes or second primaries of the intestinal tract in patients with a history of breast cancer, metastatic disease should be given consideration whenever a patient experiences GI symptoms.

A composite of polythiophene (PT) and nano-titanium dioxide (TiO2), possessing core–shell structure, was synthesized via oxidative polymerization of thiophene using FeCl3 in the presence of three different surfactants: anionic, cationic, and nonionic. The morphology of the obtained composite materials was investigated by SEM, proving the core–shell structure of the prepared nanocomposite. The formation and thermal stability of the PT onto TiO2 nanoparticles were confirmed by FTIR and TGA analyses, respectively. XRD data show all of composite materials were amorphous structures. The electrical properties of the nanocomposites were investigated in the presence of surfactant materials, and the best semiconductor property was observed for PT/TiO2-anionic system. This difference in the conductivity has been attributed to differences in the stability of the composites.
