        Psychological Distress and Perceived Burden in Parents of Korean Children With IgE-Mediated Food Allergy

        Background: Food allergy (FA) can have a profound effect on quality of life (QoL), stress, and anxiety in the family. We aimed to validate the Korean version of the Food Allergy Quality of Life-Parental Burden (FAQL-PB) and identify factors related to the parental psychosocial burden of caring for children with FAs. Methods: Parents of children aged between 6 months and 17 years with immunoglobulin E (IgE)-mediated FAs from the Pediatric Allergy Department of five university hospitals in Korea were enrolled in the study. Parents were asked to complete the FAQL-PB, Food Allergy Independent Measure-Parent Form (FAIM-PF), Child Health Questionnaire-Parents Form 28 (CHQ-PF28), Beck’s Anxiety Inventory, Connor-Davidson Resilience Scale, and Patient Health Questionnaire-9 for depression. Statistical analyses included internal consistency, test-retest reliability, concurrent validity, discriminative validity, and logistic regression analyses. Results: A total of 190 parents were enrolled. Social activity limitation was the item with the highest FAQL-PB scores. The Cronbach’s α for each item was higher than 0.8. The test-retest reliability was good (intra-class correlation coefficient, 0.716; 95% confidence interval [CI], 0.100–0.935). An increase in the FAQL-PB was significantly associated with an increase in the FAIM-PF (β = 0.765, P < 0.001) (concurrent validity). There was a positive correlation between parental burden, anxiety, and depression, while resilience was inversely correlated with parental burden (all P < 0.001). The total FAQL-PB score in parents of children who had experienced anaphylaxis was significantly higher than that in parents of children who did not experience it (P = 0.008). When adjusting for age, sex, and underlying diseases, anaphylaxis (β = 9.32; 95% CI, 2.97 to 15.68), cow’s milk (CM) allergy (β = 8.24; 95% CI, 2.04 to 14.44), soybean allergy (β = 13.91; 95% CI, 1.62 to 26.20), higher anxiety (β = 1.05; 95% CI, 0.07 to 1.41), higher depression (β = 2.15; 95% CI, 1.61 to 2.69), and lower resilience (β = −0.42; 95% CI, −0.61 to −0.2) were significantly associated with greater parental burden in children with IgE-mediated FAs. Conclusion: FAQL-PB is a reliable and valid tool for use in Korea. Anaphylaxis, CM or soybean allergies, more anxiety and depression symptoms, and lower resilience are associated with poorer QoL in parents of children with FAs.

        오메가-3 지방산 및 푸코잔틴 고함량 토착 규조류 아크난티디움 균주의 분리 및 특성

        Omega-3 fatty acids and carotenoids, which are known as representative high-value substances derived from microalgae, are being studied from various diatoms. Most of the diatoms contain fucoxanthin and omega-3 fatty acid. Fucoxanthin produced by diatom has been reported as bioactive compounds exhibiting strong antioxidant, anticancer and anti-inflammatory activities. However, the low growth rate and fucoxanthin content of diatoms are one of the big obstacles to the industrial application. In this study, indigenous marine diatom Achnanthidium sp. BS-001 was isolated for a candidate of fucoxanthin producer. Light intensity and temperature for the culture of Achnanthidium sp. BS-001 were optimized on PhotoBiobox. Optimization of silicate concentration for increasing BS-001 biomass productivity was confirmed in F/2 medium with various concentration of sodium silicate. As a result, condition of light intensity, temperature, and silicate concentration for optimal cultivation were 150 μmol·m<SUP>-2</SUP>·s<SUP>-1</SUP>, 18℃ and 0.106 mM, respectively. Maximum biomass productivity reaches to 154.3 mg·L<SUP>-1</SUP>·day<SUP>-1</SUP>, and then the content of omega-3 fatty acids and fucoxanthin were 19.4 mg·g<SUP>-1</SUP>, 9.05 mg·g<SUP>-1</SUP>, respectively. These results indicate that Achnanthidium sp. BS-001 has the potential to be used as a source of omega-3 fatty acids and fucoxanthin.

        After the global food crisis that took place in 2007 and 2008, food importing countries around the world actively took part in discussion on food security, and the competition to secure food resources is becoming more intense around the world. Since 2008, China, Japan and GCC(Gulf Cooperation Council) countries have been competing against one another to secure overseas farmlands. Those countries are, in particular, looking to farmlands in Africa and Southeast Asia and are in the process of securing numerous overseas farmlands, with the combined number of overseas farmlands that were secured by those countries standing at more than 100 in 2008 alone. Korea, for its part, is coming up with ways to increase food self-sufficiency rate for food security, and introducing and implementing overseas agricultural development polices. As part of overseas agricultural development policy, Korea established 10-year-plan for overseas agricultural development in June 2008 and has been carrying out overseas agricultural development projects since 2009. Although Korea came to establish the system in place to support overseas agricultural development at a policy level, but we need to establish an effective operating system for overseas agricultural development projects given that Korea’s entry into this field is relatively new and competition over overseas agricultural development is getting intense around the world. Among others, it is necessary to review and evaluate the implementation of overseas agricultural development carried out after 10-year-plan for overseas agricultural development, establish the necessary complementary measures, and improve the relevant institutions. Amid increasing discussion over food security among food importing countries after 2007 and 2008 global food shortage crisis and rapid in crease in international agricultural investment through securing of overseas farmlands, there exist two opposite views on securing of overseas farmlands: negative view that securing overseas farmlands equals Neo-Imperialism or Land Grab, and positive view that it is a new opportunity for agricultural development or the win-win strategy for countries. In order to facilitate a responsible overseas agricultural development investment, an appropriate international principle should be established that can allow for win-win situation where both investing country and recipient country benefit.

      • Postoperative Changes in Moderate to Severe Nonspecific Low Back Pain After Cervical Myelopathy Surgery

        <P><B>Objective</B></P> <P>Cervical myelopathy patients sometimes experience concurrent nonspecific moderate to severe low back pain (msLBP). However, postoperative changes in msLBP after cervical myelopathy surgery have rarely been reported. Awareness of postoperative changes in msLBP may be helpful in consultation. Therefore, the objective of this study was to examine postoperative changes in msLBP.</P> <P><B>Methods</B></P> <P>Patients with cervical myelopathy and msLBP (a visual analog pain score ≥5/10) were reviewed prospectively, and 53 patients (male:female ratio, 28:25; mean age, 63.1 years) were enrolled. Cervical myelopathy was assessed with the Japanese Orthopedic Association score. Cervical laminoplasty was performed in 49 patients, and anterior cervical discectomy and fusion were performed in 4 patients. The patients were followed up postoperatively at 1, 3, 6, and 12 months and yearly thereafter. The primary endpoint was improvement of the visual analog scale score for back pain (VAS-B) by greater than 2.6/10. Prognostic factors were analyzed postoperatively at 12 months. The mean follow-up period was 16 ± 9 months.</P> <P><B>Results</B></P> <P>MsLBP improved in 58%, 49%, 53%, 52%, and 59% of the patients at 1, 3, 6, 12, and 24 months postoperatively, respectively. The VAS-B worsened after improvement or vice versa in approximately 30% of the patients during the follow-up period. Lumbar decompression operations were performed in 5 patients at 4, 6, 7, 15, and 16 months postoperatively. The recovery rate of the JOA score was a positive prognostic factor.</P> <P><B>Conclusions</B></P> <P>Although the exact pathophysiology was not demonstrated, cervical myelopathy surgery may directly and indirectly improve msLBP.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Cervical myelopathy patients sometimes have nonspecific low back pain (msLBP). </LI> <LI> Awareness of postoperative changes in msLBP may be helpful in consultation. </LI> <LI> MsLBP was improved in approximately 50% of patients after cervical myelopathy surgery. </LI> <LI> MsLBP symptoms fluctuated in approximately 30% of patients. </LI> </UL> </P>

      • Nitrogen modulation under chemostat cultivation mode induces biomass and lipid production by <i>Chlorella vulgaris</i> and reduces antenna pigment accumulation

        <P><B>Abstract</B></P> <P>Algal growth limitation in large-scale cultivation mostly results from high level synthesis of photosynthetic pigments, owing to self-shading effects and attenuation of light distribution. To overcome this problem, here we investigated the influence of nitrogen modulation on changes in antenna pigments as well as biomass and lipid production by <I>Chlorella vulgaris</I> under a chemostat continuous cultivation mode. The production of algal antenna pigments, including chlorophylls and carotenoids, was promoted in a total nitrogen (TN) concentration-dependent manner. Maximum algal biomass and lipid production were obtained from 70 mg/L of TN concentration along with a significant increase in light transmittance and reduction in antenna pigments. Furthermore, the composition of polyunsaturated fatty acids remarkably augmented at low TN concentrations. These results suggest that the reduction in algal antenna pigment synthesis via modulation of nitrogen concentration may serve as an effective strategy to enhance algal biomass and lipid production.</P> <P><B>Highlights</B></P> <P> <UL> <LI> TN-modulated algal cultivation was performed under chemostat mode. </LI> <LI> Antenna pigment reduction was observed at low TN concentration. </LI> <LI> Algal biomass and lipid production increased from OLTZ. </LI> <LI> PUFA production significantly increased at low TN concentration. </LI> </UL> </P>

        잠재적 푸코잔틴 및 오메가-3 지방산 생산자로서 토착 규조류 오돈텔라의 분리 및 배양 특성

        Fucoxanthin has been reported as bioactive compounds exhibiting strong antioxidant, anticancer and anti-inflammatory activities. Owing to its a wide range of applications and potentials, commercial production of fucoxanthin from algae has been attracted many attentions. Although, most of seaweeds and diatoms contain fucoxanthin as major carotenoid contents, low productivity of fucoxanthin still hinder the industrial application. Here, we newly isolated and identified indigenous marine diatom Odontella sp. BS-003 as a resource of fucoxanthin production. The characterization, optimization and production of the fucoxanthin, along with other bioactive compound omega-3 fatty acid from odontella sp. BS-003 were analyzed in this study, and the results represented optimal culture condition (two-fold silicate containing F/2 medium) significantly enhanced the algal biomass productivity. The maximum biomass (1.83 g/L), fucoxanthin (3.88 ㎎/g), along with omega-3 fatty acid (10 %, w/w) were obtained from the 10 L of photobioreactor. Based on the results, it is speculated that the microalga Odontella sp. BS-003 can be a promising natural resource for the production of bioactive compounds.
