        Mechanical Thrombectomy for Large Vessel Occlusion via the Transbrachial Approach: Case Series

        Mechanical thrombectomy has become a standard treatment for acute ischemic stroke with large vessel occlusion. In aged patients, it is difficult to guide the catheter via the transfemoral approach due to vessel tortuosity and aortic elongation. We report our preliminary clinical experience using the transbrachial approach. Among the 119 patients who underwent thrombectomy from April 2018 to December 2019, a total of 5 patients were treated via the transbrachial approach. Clinical outcomes were retrospectively analyzed. Successful reperfusion was achieved in 4 out of 5 cases. There was 1 death due to symptomatic intracranial hemorrhage. One patient had a good outcome at discharge. There were no access-site complications associated with any of these cases. Transbrachial access for mechanical thrombectomy is feasible and can provide an alternative to the transfemoral approach.

        A Safety Review of Medical Device Robots in JAPAN

        In Japan, the robot evolution in the medical field has been remarkable, robots now often perform actions that are difficult for humans including assisting with elderly care and conducting medical operations that require specific techniques, and Japanese medical robot technology is among the best in the world. As the workforce population decreases in Japan's ageing society, the demand for robots will be ever-increasing, the use of robots will reduce the increasing medical costs. Unfortunately, there has been insufficient research by Japanese law researchers on medical robots. This paper examines whether the law in Japan is a barrier to the development of robot technology. In Japan, medical treatments that utilise robots are promoted by the government and by private organisations, including the Center of Assistive Robotics and Rehabilitation for Longevity and Good Health and the Japan Agency for Medical Research and Development, which are similar to the National Science Foundation and the National Institutes of Health in the U.S. In Europe, the SPARC robot project promotes the private development of robots. One side effect of robot technology is the challenge they present within legal systems because laws commonly lag behind the rapid technological developments. To address this, we may need to modify existing interpretations by the executive branch and revise statues in parliament; otherwise, judges must interpret the meaning of statutes that competing parties dispute. This paper focuses on the legal issues regarding medical robots, including how the government approves the use of medical devices for older people, and proposes a legal framework that encourages the safe implementation of robots in the medical field.

        Mechanical Thrombectomy for Acute Ischemic Stroke due to Thrombus in the Pulmonary Vein Stump after Left Pulmonary Lobectomy: A Case Series

        Cerebral embolic stroke caused by a thrombus in the pulmonary vein stump after left pulmonary lobectomy is a serious complication. We retrospectively analyzed four patients who underwent mechanical thrombectomy for large-vessel occlusion after left pulmonary lobectomy between January 2014 and March 2022. Two cases occurred after left upper lobectomy and the others occurred after left lower lobectomy. All patients presented with cerebral embolic stroke from the day after surgery to the 9th postoperative day, and successful reperfusion was achieved in all cases. Two patients had good outcomes at 90 days. Thrombus in the pulmonary vein stump is the probable cause of cerebral embolism, and mechanical thrombectomy is effective. Further studies are required to establish preventive measures and perioperative management strategies.

        <i>De Novo</i> Vertebral Artery Dissecting Aneurysm after Parent Artery Occlusion of the Contralateral Vertebral Artery

        After treatment of unilateral vertebral artery dissecting aneurysm (VADA), de novo VADA rarely occurs on the contralateral side. In this article, we report a case of subarachnoid hemorrhage (SAH) due to de novo VADA in the contralateral vertebral artery (VA) 3 years after parent artery occlusion of unilateral VADA, with a review of the literature. A 47-year-old woman was admitted to our hospital complaining of headache and impaired consciousness. Head computed tomography showed SAH, and three-dimensional computed tomography angiography showed a fusiform aneurysm in the left VA. We performed an emergency parent artery occlusion. Three years and 3 months after the initial treatment, the patient presented to our hospital with complaints of headache and neck pain. Magnetic resonance imaging revealed SAH, and magnetic resonance angiography revealed de novo VADA in the right VA. We performed a stent-assisted coil embolization. The patient had a good postoperative course and was discharged with a modified Rankin scale score of 0. Long-term follow-up is necessary in patients with VADA because contralateral de novo VADA can develop even several years after the initial treatment.

        The associations of the national health and productivity management program with corporate profits in Japan

        OBJECTIVES: Using a dataset from a survey on national health and productivity management, we identified health and productivity factors associated with organizational profitability. METHODS: The Ministry of Economy, Trade and Industry conducted an annual survey on Health and Productivity Management between 2014 and 2021. We assessed the associations of organizational health and productivity management using survey questions collected in 2017 and 2018, and the rate of change in profits from 2017 and 2018 to 2020. We identified factors associated with organizational profitability using eXtreme Gradient Boosting, and calculated SHapley Additive exPlanation (SHAP) values for each factor. RESULTS: Among 1,593 companies (n= 4,359,834 employees), the mean age for employees at baseline was 40.3 years and the proportion of women was 25.8%. The confusion matrix for evaluating model performance had accuracy of 0.997, precision of 0.993, recall of 0.997; and area under the precision-recall curve of 0.999. The most important factors related to an increase in corporate profits were the percentage of current smokers (SHAP value 0.121), per-employee cost for health services (0.084) and medical services (0.050); the percentage of full-time employees working in sales departments (0.074), distribution or customer service departments (0.054); the percentage of employees who sleep well (0.055); and the percentage of employees who have a habit of regular exercise within a company (0.043). CONCLUSIONS: The lifestyle health risk factors of employees and organizations’ management systems were associated with organizational profitability. Lifestyle medicine professionals may demonstrate a significant return on investment by creating a healthier and more productive workforce.
