        A case of omental mass during Pregnancy Omental cyst is a rare lesion and occur primarily during childhood and young adulthood. When encountered, most of them are benign and intraabdominal masses. It shows a large mass but not palpable and often asymptomatic. The precise diagnosis is made by most commonly at surgery or during pathologic examination. So, we report this case with brief review of literatures.

        Pelvic inflammatory disease (PID) is a disease that affects young, sexually active, reproductive age women. Most pathogens are Chlamydia trachomatis and Neisseria Gonorrheae transmitted sexually. One of serious acute complication of PID is tubo-ovarian abscess (TOA) that involves the fallopian tube, ovary and adjacent structures (eg, bowel, pelvic peritoneum). The microbiology of TOAs is polymicrobial with anaerobic organisms and causal organisms of PID. Common anaerobic organisms are Peptostretococcus spp, Bacteriodes spp, Prevotella spp, Escherichia coli and aerobic streptococci. Treatment is necessary to admission for intravenous broad-spectrum antibiotics and surgery is often reserved for patients with suspected rupture or patients who fail to respond to antibiotics. So, we reported this case found pelvic abscess due to ruptured tube-ovarian abscess.

        전자의무기록의 충실성 검토를 통한 미비기록 개선 활동

        문제: 전자의무기록(EMR) 시행 후 의무기록 정리율의 저하와 질적인 측면에서의 충실성과 정확성에 대한 문제점이제기되었다. 목적: 전자의무기록의 정리율과 충실성 검토를 통하여 문제점을 파악하고개선점 찾아 의무기록 정리율을 향상시키고 충실성을 높이고자 하였다. 의료기관: 서울시에 소재한대학병원 의무기록과 질 향상 활동: 전자의무기록의 문제점을 개선히키 위하여 사용자 편의를 위한 EMR프로그램 수정 및 보완, 진단·수술 관련 작업, 업무개선, 교육, 홍보 등의 활동을실시하였다. 개선효과: 의무기록 정리율, 전자인증미비, 경과기록 기재일수, 퇴원요약주진단적합률, 기록지별 필수항목기 재율, 충실성에서 향상이 이루어졌다.

        Two cases of primary small cell carcinoma of the ovary occurred. One was a case of 43-year-old woman associated with hypercalcemia and the other was a case of 40-year-old woman with pulmonary type. Ovarian small cell carcinoma of the hypercalcemic type have clinicopathological, immunohistochemical, ultrastructural, and flow cytometric features that from differ from those of small cell carcinomas of the pulmonary type. It is important for therapeutic reasons to distinguish these two types of ovarian tumors from each other and from a variety of malignant small cell tumors that ovary primarily or secondarily. So, we reported these cases with a brief review of the concerned literatures.

      • 임상간호사의 분권화, 전문성 및 의사결정의 관계

        This study were conducted to analyze the correlations of decentralization, expertise and decision-making perceived by clinical nurses and to another relational factors. The subjects of this study were 190 clinical nurses who worked at two hospitals in Taejon city. The data were collected from August 4th to August 8th, 1997. The instruments for this study were Van de Van and Ferry's Job Authority Scale(4 items), Hage and Aiken's Hierarchy of Athoriy Scale(5 item), Vroom's Measure of Psychological Participation Scale(4 items), Hage and Aiken's Participation in Decision Making Scale(4 items), and 3 single items for expertise. The data were analyzed by use of percentage, ANOVA, and Pearson-Correlation Coefficient. The results of this study were as follows; 1. The degrees of participation in decision-making classified by education levels showed significant differences(P<0.05). 2. There was significant correlation between decentralization and participation in decision-making(P<0.01). Also there was significant correlation between participation in decision-making and education levels(P<0.01), and self-rating for expertise(P<0.05). 3. There was significant correlation between decentralization and total career of RN (P<0.01), and subject's age(P<0.05). Also significant correlation between participation in decision-making and total career of RN (P<0.001), and pattern of duty(P<0.01), and subject's age(P<0.001).

      • 임상실습을 경험한 간호학생들이 인식한 간호사의 역할개념 양상

        The purpose of this study was to analyze nurses' role conception perceived by nursing student experienced the clinical practice. The subject of this study were consisted of 109 nursing students, 62 university students and 47 college students. The data was collected form January 16th to February 11th, 1997. Subscale of Nursing Role Conceptions(Pieta, 1976) were used to measure bureaucratic, professional, and service role conceptions. Data were analyzed by SPSS for Windows program. The result of this study were summarized as follows ; 1. There was statistically significant differences between ideal and actual role conceptions(p<.001). 2. In the ideal role conception and role discrepancy, three role conception score is high in sequence of professional, service, bureaucratic role conceptions(p<.001). In the actual role conception , three role conception score is high in sequence of bureaucratic, professional, service role conceptions(p<.001). 3. There was no relationship between ideal role conceptions and educational level. In actual role conceptions, college students had significantly higher bureaucratic, professional, and service role conception scores than that of university students(p<.05). In the role discrepancy, university students had significantly higher bureaucratic, professional, and service role conception scores than that of college student(p<.05).

      • 임상실습 경험 유무에 따른 간호학생의 죽음에 대한 태도 조사연구

        The purpose of the study was to investigate the nursing students' attitudes toward death, care for the hopeless patients, suicide, brain death, euthanasia and organ transplant according to their practice experience. This study will help nursing practitioners in reviewing and evaluating the importance of hospice care and nursing teaching staff in formulating the related subjects in a nursing curriculum. The questionnaires were mailed to nursing students (at one Junior College and one University) in Daejeon area from March 15, 1993 to March 22, 1993. The survey instrument was a questionnaire of 26 questions based on Edwin Shneidman's 21 questions and 5 questions in regard to euthanasia, brain death and organ transplant. The SPSS was used to analyze the data collected according to Chi-square test. The conclusions can be drawn from the data gathered in this study : 1. The questions relating to nursing students' attitudes toward death according to practice experience : 1) 88.3% of the practice experienced students and 70.0% of the practice unexperienced students responsed that they 'have talked about death with others' (P<0.001). 2) 70.8% of the practice experienced students and 52.0% of the practice unexperienced students indicated that they 'have their philosophy toward life and death' (P<0.001). 3) 89.6% of the practice experienced students and 77.3% of the practice unexperienced students responsed that they `want to hear of that have a terminal disease and a limited time to live' (P<0.01). 2. The questions relating to nursing students's for hopeless patients according to practice experience : Suppose patients are aware of their hopelessness, 87.7% of the practice experienced students and 67.3% of the practice unexperienced students responded that they would 'help the patients to accept death and to arrange the rest of life' (p<0.001). 3. The questions relating to nursing students' attitudes toward suicide according to practice experience : 71.3% of the practice unexperienced students and 59.7% of the practice experienced students thought that 'suicide has no hereditary factor' (P<0.01). 4. The questions relating to nursing students' attitudes toward euthanasia, brain death and organ transplant according to practice experience : 1) 63.6% of the practice experienced students and 51.3% of the practice unexperienced students 'supported for the euthanasia' if they were aware of their incurable diseases (p<0.05). 2) 67.5% of the practice experienced students and 54.0% of the practice unexperienced students indicated that they 'agreed to accept brain death' (P<0.05). 3) 84.4% of the practice experienced students and 71.3% of the practice unexperienced students 'agreed with organ transplant' (P<0.05).

        This study was aimed at examing the ideal, actual role perception and role discrepancy for clinical nurses. And that will being help to hospital nursing management. Here, role conceptions were devided into the professional role conception, bureaucratic role conception and service role conception. This study was based on 205 registered nurses who worked in one hospital. And the data was collected through questionnaire from July 24th to July 30th, 1995. Data analysis method included the SPSS program for Paired T-test, Duncan's multiple range test, X_2 test, Pearson's Correlation. The result and finding were as follows. 1. There was statistically significant differences between ideal and actual role perception for clinical nurses(P<0.001). 2. There was statistically significant differences. 1) Ideal, actual role discrepancy for professional role conception according to educational level(P<0.01). 2) Ideal, actual role discrepancy for bureaucratic role conception according to educational level(P<0.001). 3) Ideal, actual role discrepancy for professional role conception according to present servise (P<0.05).

        수학능력의 발달과 산술장애 : 발달 정신병리적 관점을 중심으로

        본 논문에서는 발달 정신병리적 접근을 토대로 수학 능력의 발달과 산술장애에 대해 검토하였다. 최근의 발달 정신병리는 정상 발달과 이상 발달을 분리하지 않고 통합적으로 다룸으로써 정상 집단과 비정상 집단을 보다 폭넓게 이해할 수 있는 패라다임을 제공하고 있다. 본 논문에서는 정상적인 수학 능력의 발달과 산술장애를 Miller의 수학 발달의 일반적 틀에 따라 상징적 측면, 연산적 측면, 개념적 측면으로 기술하면서 정상 아동과 산술장애 아동의 진단 및 처치에 시사점을 줄 수 있다고 보며 후속적 연구를 통해 보다 구체적 자료가 모아지리라고 여겨진다. At this study, mathematical development and mathmatics disorder were examined with a perspective of developmental psychopathology. Recent developmental psychopathology provides the paradigm that combines both normal and abnormal development as an integrated unit. If we assumed that mathematical ability developed by symbolic, algorithmic and conceptual competence as Miller suggested, mathematical development and mathmatics disorder can be understood with a same framework. The attempt to combine the normal and mathmatics disorder children will provide the good suggestion to both groups, and more detail data will be collected by following studies.

        청소년 흡연예방을 위한 멀티미디어 CAI 개발

        Background : The purpose of this study was to develop a structured and individualized smoking prevention program for adolescents by utilizing a multimedia computer-assisted instruction model and to empirically assess its effect. Method : For the purpose of this study, a guide book of smoking prevention program for middle and high school students was developed as the first step. The contents of this book were summarized and developed into an actual multimedia CAI smoking prevention program according to the Gané &riggs instructional design and Keller’s ARCS motivation design models as the second step. At the final step, the short-term effects of this program were examined by an experiment. This experiment were made for middle school and high school students and the quasi experimental design was the pretest - intervention - posttest. The measured data was attitude, belief and knowledge about smoking, interest in the program, and learning motivation. Result : The results of this study were as follows: First, the guide book of a smoking prevention program was developed and the existing literature on adolescent smoking was analyzed to develop the content of the guide book. Then the curriculum was divided into three main domains on tobacco and smoking history, smoking and health, adolescent smoking and each main domain was divided into sub-domains. Second, the contents of the guide book were translated into a multimedia CAI program of smoking prevention througn Powerpoint software according to the instructional design theory. The characteristics of this program were interactive, learner controllable, and structured. The program contents consisted of entrance(5.6%), history of tobacco(30%), smoking and health(38.9%), adolescent smoking(22.2%), video(4.7%), and exit(1.6%). Multimedia materials consisted of text(121), sound and music, image(still 84, dynamic 32), and videogram(6). The program took about 40 minutes to complete. Third, the results on analysis of the program effects were as follows: 1) There was significant knowledge increase between the pre-test and post-test with total mean difference 3.44, and the highest increase was in the 1st grade students of high school(p<001). 2) There was significant decrease in general belief on smoking between the pre-test and post-test with total mean difference 0.28. In subgroup analysis, the difference was significantly higher in the 1st grade of high school (p<001), low income class (p<001), and daily smokers (p<01). 3) There was no significant difference in attitudes on his personal smoking between the pre-test and post-test. 4) The interest in the program seemed to lower as students got older. The score of motivation toward this prevention program was the highest in the middle school 3rd grade. Among sub-domains of motivation, the confidence score was the highest. Conclusion : To be most effective, the smoking prevention program for adolescents should utilize the most up-to-date and accurate information on smoking, and then instructional material should be developed so that the learners can approach the program with enjoyment. Through this study, a guide book with the most up-to-date information was developed and the multimedia CAI smoking prevention program was also developed based on the guide book. The program showed positive effect on the students' knowledge and belief in smoking.
