      • Poster Session : PS 0831 ; Upper GI Tract : Clinical Factors to Predict Angiographically Detectable Non-Variceal Upper Gastrointestinal Bleeding in Patients Refractory to Endoscopic Treatment

        Background: Non-variceal upper GI bleeding (NVUGIB) is a common medical problem that has signifi cant association with morbidity and mortality. Angiographic detection and subsequent transarterial embolization (TAE) is a primary treatment option when medical and endoscopic treatments fail. We investigated clinical factors that could affect the success of the angiographic detection and prognosis after TAE in patients with NVUGIB refractory to endoscopic therapy. Methods: A retrospective analysis of the clinical data was done in patients with failed endoscopic treatment who underwent angiography for the treatment of acute NVUGIB between May 2002 and May 2013. Patients were divided into detection or non-detection groups according to the presence of bleeding stigmata in angiographic fi nding. Rebleeding defi ned as subsequent bleeding event within 7 days and mortality within 30days were analyzed as outcome parameters after TAE following detection in angiography. Results: A total 45 patients 37 (male, mean age, 65.9±14.9 years) were analyzed and classifi ed as a detection group (n=25, 55.5%) and non-detection group (n=20, 44.6%). Peptic ulcers were the most common cause of refractory NVUGIB. Larger transfusion amount (5.7±3.9 unit vs. 3.5±2.8 unit; P=0.03), prolonged aPTT level (34.2±17.3 sec vs. 21.8±13.8 sec; P=0.01) and short time interval between last endoscopy and angiography (17.5±25.9 hours vs. 34.3±59.5 hours; P=0.04) were found to be signifi cant factors for predicting angiographic detection. TAE was performed in all patients detected in angiography. Rebleeding (44%) was significantly associated with higher Rockall score (8.3±1.5 vs. 6.6±2.4; P=0.046) and mortality (12%) was signifi cantly associated with higher Rockall score (9.3±0.6 vs. 7.1±2.2; P=0.002) and higher level of BUN (55.3±47.4 vs. 27.6±17.4; P=0.01). Conclusions: Clinical characteristics associated with angiographic detection in patients with NVUGIB refractory to endoscopic therapy were severe bleeding, bleeding tendency and early angiographic intervention. The Rockall score is useful parameter for predicting rebleeding and mortality after TAE.

      • Identification of Redox Sensitive Proteins in Prdx II Deficient Red Blood Cells

        Red blood cells (RBCs) have been studied as models for infectious diseases, various symptoms of anemia, hemolysis, and erythrocyte aging. Although do not directly affect RBCs, other diseases may cause RBC physiological alterations that could be advanced for diagnostic aim or to convince better understanding of a certain pathological pattern. In this study, comparative RBC proteomics between healthy and abnormal conditions involve to promote aging related‐biomarker discovery. Peroxiredoxin II (Prdx II; a typical 2‐Cys Prdx) has been originally isolated from erythrocytes, and its structure and peroxidase activity have been adequately studied. Mice absent to Prdx II proteins had heinz bodies in their peripheral blood, and morphologically aged cells were detected in the dense RBC fractions, which contained markedly higher levels of reactive oxygen species (ROS). In addition, a labeling experiment with the thiol‐modifying reagent biotinylated iodoacetamide (BIAM) in Prdx II‒/‒ mice revealed that a variety of RBC proteins were highly oxidized. To identify oxidation‐sensitive proteins in Prdx II‒/‒ mice, we performed RBC comparative proteome analysis by nano‐UPLC‐MSE shotgun proteomics with relative protein quantitative analysis. We found oxidation‐sensitive 18 membrane and 41 cytosol proteins from 32 and 85 peptides containing cysteine oxidation, and analyzed comparative expression pattern in healthy RBCs of Prdx II+/+ mice (W1), healthy RBCs of Prdx II‒/‒ mice (K1), and abnormal RBCs of Prdx II‒/‒ mice (K2). These proteins belonged to cellular functions related with RBC lifespan maintain, such as cytoskeleton, stress‐induced proteins, amino acid/nucleic acid metabolic enzymes, signal transduction, and molecular transporters. Furthermore, protein networks among identified oxidation sensitive proteins were analyzed to associate with aging consequence. Consequently, we expected that RBC proteome may provide clues to understand redox‐imbalanced diseases.

        Peroxiredoxin V, an atypical thioredoxin peroxidase, is widely expressed in mammalian tissues. In addition, Prdx V is localized in mitochondria, peroxisome, cytosol, and nucleus. Prdx V has been reported to protect a wide range of cellular environments as antioxidant enzyme, and its dysfunctions may be implicated in several diseases, such as cancer, inflammation, and neurodegenerative disease. Identification and relative quantification of proteins affected by Prdx V may help identify novel signaling mechanisms that are important for oxidative stress response. However, the role of Prdx V in the modulation of hypoxia‐related cellular response is not studied yet. In order to examine the function of endogenous Prdx V in hypoxic condition in vivo, we generated a transgenic mouse model with Prdx V siRNA expression controlled by U6 promoter. Of many tissues, the knockdown of Prdx V expression was displayed in kidney, lung, and liver, but not spleen and skin. We conducted on the basis of nano‐UPLC‐MSE proteomic study to identify the Prdx V‐affected protein networks in hypoxic kidneys. In this study, we identified protein networks associated with oxidative stress, fatty acid metabolism, and mitochondrial dysfunction. Our results indicated that Prdx V affected to regulation of kidney homeostasis under hypoxia stress.

We found oxidation‐sensitive 54 proteins from 61 peptides containing cysteine oxidation, and analyzed comparative expression pattern in healthy RBCs of Prdx II+/+ mice, healthy RBCs of Prdx II‒/‒ mice, and abnormal RBCs of Prdx II‒/‒ mice. These proteins belonged to cellular functions related with RBC lifespan maintain, such as cytoskeleton, stress‐induced proteins, metabolic enzymes, signal transduction, and transporters. Furthermore, protein networks among identified oxidation sensitive proteins were analyzed to associate with various diseases. Consequently, we expected that RBC proteome may provide clues to understand redox‐imbalanced diseases.

      • Targeting Multiple Myeloma Cell Death with Polyphenol EGCG

        (‐)‐Epigallocatechin 3‐gallate (EGCG) is a potent antioxidant polyphenol in green tea that acts as an anticancer agent via both direct and indirect pathways. Although the relationship between EGCG’s anticancer effects and its antioxidant activity is not fully understood, it is known that EGCG stimulates production of reactive oxygen species (ROS), which induce oxidative stress leading to cell death. In IM9 multiple myeloma cells, EGCG acted in a dose‐ and time‐dependent manner to induce apoptotic cell death. Among the antioxidant enzymes expressed in IM9 cells, levels of peroxiredoxin (Prdx) V were selectively and significantly reduced by EGCG. Moreover, the ROS scavenger NAC completely inhibited EGCG‐induced apoptosis and PrdxV reduction, while overexpression of PrdxV, but not a PrdxVC48S mutant, protected IM9 cells from EGCG‐induced apoptosis. EGCG‐induced reductions in cell viability and PrdxV levels were also observed in primary CD138+ multiple myeloma cells from patients. These results suggest that PrdxV is a key target via which EGCG mediates its anticancer effects.

        Effect of Developmental Lead Exposure on the Expression of Hippocampal NMDA Receptor Subunit mRNA

        목적: in vivo 및 vitro에서 해마 신경세포의 발생단계별 NMDA 수용체 아단위 mRNA 발현에 대한 연 폭로 영향을 알아보고자 하였다. 방법: 흰쥐 해마 신경세포의 발생단계별 NMDA 수용체 NR2A, NR2B 아단위 mRNA 발현에 대한 연의 영향은 정상군과 연 폭로군의 출생 후 7일, 14일, 22일 흰쥐의 해마에서 in situ hybridization으로 mRNA 발현 정도를 densitometer로 측정하여 비교하였고, 연과 NMDA 의 세포독성은 해마 신경세포 일차배양 후 도립현미경을 이용한 형태학적인 관찰과 LDH 활성도를 이용하여 측정하였다. 결과: 연 과 NMDA 에 의한 세포독성에 대한 in vitro 실험에서 형태학적 소견과 LDH 활성도에서 해마 미성숙 신경세포와 성숙 신경세포사이의 차이가 있었으므로, 연과 NMDA 독성효과는 해마 신경세포의 발달 단계에 따라 차이가 있다. 정상군의 해마에서의 NR2A mRNA 발현은 출생 후 연령이 증가함에 따라 점진적으로 증가하였으나, NR2B mRNA 발현은 출생 후 연령이 증가함에 따라 점진적으로 증가하였으나 NR2B mRNA 발현은 연령의 증가에 따른 변화가 없었다. 연 폭로에 희한 NR2A mRNA 발현은 유의하게 감소하였으나(p<0.05), NR2B mRNA 발현은 변화가 나타나지 않았다. 만성적 연 폭로는 NR2A를 포함하는 NMDA 수용체를 감소시킬 수 있음을 알 수 있다. 결론: 연은 해마신경세포의 발생단계에서 NMDA 수용체 아단위 특히 NR2A mRNA 발현의 변화를 야기하여 시냅스 신호 전달에 영향을 나타냄을 알 수 있었다.         괴산군 지역 농업 생태계의 곤충 다양성 비교 분석

        Agricultural ecosystem biodiversity monitoring and community variation analysis of insects were conducted from 2016 to 2018 in selected conventional and organic farming fields in Goesan district, Chungcheongbuk-do, South Korea. The total number of 1,125 species in 16 orders and 207 families were identified. The numbers of species collected in the locations practicing organic farming were greater than the conventional farming both in the paddy fields (564 vs. 383 species) and the upland fields (471 vs. 365 species). Among them, Hemiptera had the most abundant of species, followed by Diptera, Hymenoptera, Coleoptera and Araneae. We calculated various index values of biodiversity (diversity index H’, richness index R, evenness index J’, dominance index D, and similarity index QS) based on quantitative measurements of species and individuals collected over three years of field monitoring. Variations in biodiversity index values in different agricultural systems show that the positive effect of organic farming is to produce more biodiversity than conventional farming systems. When compared to other index results reported in Korea, Japan and China, the richness index was higher and other index values were at similar levels.

        조선후기 월송만호와 울릉도 수토제

        월송만호는 삼척영장과 함께 울릉도 수토제가 확립된 이후 울릉도 수토가 폐지되는 시기까지 울릉도 수토를 책임진 관직이다. 월송만호로써 울릉도에 간 수토관은 총 25명인데 이들 중 수토시기를 알 수 있는 인원은 전회일, 김창윤, 한창국, 이태근, 이규상, 지희상이 있다. 이들은 경우 울릉도에 대략 4박 5일 정도를 머물며 수토를 거행하였고, 도형(圖形)과 토산물을 비변사에 올렸다. 특히 김창윤과 한창국의 경우는 울릉도 지도에 여정의 이동 경로를 표기하여 나타내었다. 수토관의 임무 중 하나는 울릉도에 잠입한 범금인을 적발하는 것이다. 조선 시대 울릉도는 1403년(태종 3) 이후 공식적으로는 무인도가 되었지만 계속해서 사람들이 들어가 살았다. 이러한 범금인(犯禁人)은 조선 후기가 되면 조직적으로 변모한다. 물주들이 선주와 격군을 모집하고, 이들에게 임금을 지급하였다. 조정에서는 적발하고자 했는데 이들을 적발하는 방법은 황죽에 대해서 조사하는 것이었다. 울릉도에서 생산되는 황죽은 쉽게 알 수 있을 정도로 눈에 띄기 때문에 이것을 통해 적발할 수 있었다. 이렇게 적발된 범금인들에 대해서 채취한 물건을 몰 수 하고 율문(律文) 대로 처벌하였다. 이와 더불어 울릉도에 가는 것을 방조한 것에 대한 책임으로 수토사들을 처벌하였다. 본고에서는 기존의 수토현황에 오흥량, 안재소, 안광로, 한두석을 추가로 기재하였다. 이들은 기존기록에 수토기록이 남아 있으나 월송만호의 인명이 기재되어 있지 않았던 경우로 승정원일기의 월송만호 임명기록을 통해 추정되는 인물을 기재하였다. 향후 과제로 수토에 관해서 수토물자의 조달 문제와 삼척과 월송포의 주민들이 수토비용을 어떻게 마련했는지에 대한 연구가 필요하다고 생각된다. Wolsong Manho is a government position responsible for search and destroy of Ulleungdo Island until the period discontinue the search and destroy of Ulleungdo Island after established search and destroy system of Ulleungdo Island together with Samchuk Youngjang. There are Heo-il Jeon, Chang-yoon Kim, Chang-gook Han, Tae-geun Lee, Gyu-sang Lee and Hee-sang Ji possible to understand their search and destroy time among all 25 people of the person in charge of search and destroy as Wolsong Manho. They executed search and destroy staying at Ulleungdo Island about five days and four night and submitted the figure and local products to the Office of Border Defense. One of the aim for the person in charge of search and destroy was to uncover the wicked people infiltrated to Ulleungdo Island. Ulleungdo Island in Joseon Dynasty became an uninhabited island officially after 1403, however, the people came in and lived continuously. These wicked people changed as systematically when it became in late Joseon Dynasty. Financiers collected shipowner and fierce force and paid wages to them. The government wanted to uncover them and the method to uncover them was to investigate about yellow bamboo. The yellow bamboo The articles collected were forfeited and punished as the provisions of a criminal code for uncovered wicked people. In addition, the person in charge of search and destroy were punished for the responsibility of abet to come into Ulleungdo Island. Heung-ryang Oh, Jae-so An, Gwang-ro An and Doo-seok Han were filled in to exiting search and destroy status additionally in this study. The people estimated by appointment record as Wolsong Manho in Seungjeongwon Ilgi, the Diaries of the Royal Secretariat were filled in who were in case not written their name of Wolsong Manho though search and destroy record is remained in existing literature. The study for how to procure search and destroy articles and how the people in Samchuk and Wolsong port prepared the cost for search and destroy will be necessary in the future task.
