Background: Rate of cancer-positive lateral margin (LM) after endoscopic submucosal dissection (ESD) for early gastric cancer (EGC) have been reported 2 to 12%. Sometimes cancer cells abut to multiple lateral margins. The aim of this study was to compare clinicopathologic characteristics and treatment outcomes between single and multiple cancer-positive LM group. Methods: From September 2010 to April 2014, 1346 patients underwent ESD for EGC. Among them, 33 patients (2.5%) had cancer-positive LM. Clinicopathological data of these patients were investigated. Results: There were 25 patients with single positive LM (group LM1+) and 8 with multiple positive LM (group LM2+). No patients met the absolute indication of ESD, and all patients met the expanded indication (n=17) or beyond indication (n=16). LM2+ group tended to have more undifferentiated carcinoma (62.5% vs. 36.0%) and larger size of the lesion (>30mm) (62.5% vs. 28%), but there were no statistical signifi cance. Sixteen patients (64%) and 1 patient (12.5%) met expanded indication of ESD in LM1+ and LM2+ group, respectively. After ESD, 5 patients (20.0%) and 1 patient (12.5%) underwent regular endoscopic follow-up, and 11 patients (36.4%) and 1 patient (12.5%) underwent local control with redo-ESD or argon plasma coagulation in LM1+ and LM2+ group, respectively. There were no locoregional or distant metastasis during the follow-up periods (median 25.1 months). Nine (36.0%) in LM1+ group and 6 (75.0%) in LM2+ group underwent surgery after ESD. Residual cancer was detected in 69.2% (9/13) and 66.7% (4/6) after surgery or redo-ESD in LM1+ and LM2+ group, respectively. Conclusions: Even though there was no statistical signifi cance, proportions of large lesion size (>3cm) and undifferentiated carcinoma were higher in LM2+ group than LM1+ group. Careful investigation of margin is necessary for these patients.
Approach to cytomegalovirus infections in patients with ulcerative colitis

Cytomegalovirus (CMV) reactivation is common in patients with severe ulcerative colitis (UC), and may ref lect exacerbation of mucosal inf lammation and/or administration of immunosuppressants. The question of whether CMV is an active pathogen or `an innocent bystander` in the exacerbation of UC remains controversial. Patients with UC exacerbated by reactivated CMV experience worse prognoses than those without CMV reactivation and antiviral therapy significantly reduces the need for colectomy in patients with severe UC and high-grade CMV infection, indicating that CMV plays a role in UC prognosis. Therefore, the CMV status of patients on immunosuppressants, particularly those with steroid-refractory or -dependent UC, should be tested. When CMV is detected, be performed based on should adequate treatment the extent of the viral load and the presence of certain clinical features including a large ulcer. Anti-tumor necrosis factor agents may be useful for treating CMV colitis complicating UC.

Bardet-Biedl syndrome (BBS) is a rare ciliopathy generally inherited with an autosomal recessive pattern. BBS is characterized by 6 primary features namely retinal dystrophy, obesity, postaxial polydactyly, renal dysfunction, learning difficulties, and hypogonadism and a wide range of secondary features. To date, mutations in 16 genes have been identified as causative factors for BBS. Among them, the BBS1 and BBS10 genes are major disease-causing genes, and each of these gene mutations presents in more than 20% of all BBS patients. Genotype-phenotype correlations have not been observed in BBS, and there can be phenotypic overlap between BBS and other ciliopathies. In Korea, no molecular, genetically confirmed case of BBS has been reported to date. Herein, we describe the case of the first Korean siblings with BBS resulting from 2 BBS10 gene mutations who showed typical clinical phenotypes, including retinal dystrophy, obesity, intellectual disability, cystic tubular disease, and postaxial polydactyly.

Background: In 2015, workers dismantling a fluorescent lamp factory in Korea were affected by mercury poisoning from exposure to mercury vapor. Case presentation: Eighteen out of the 21 workers who participated in the demolition project presented with symptoms of poisoning and, of these, 10 had persistent symptoms even at 18 months after the initial exposure to mercury vapor. Early symptoms of 18 workers included a general skin rash, pruritus, myalgia, sleep disturbance, and cough and sputum production. Following alleviation of these initial symptoms, late symptoms, such as easy fatigue, insomnia, bad dreams, and anxiety disorder, began to manifest in 10 out of 18 patients. Seven workers underwent psychiatric care owing to sleep disturbance, anxiety disorder, and depression, and three workers underwent dermatologic treatment for hyperpigmentation, erythematous skin eruption, and chloracne-like skin lesions. Furthermore, three workers developed a coarse jerky movement, two had swan neck deformity of the fingers, and two received care at an anesthesiology clinic for paresthesia, such as burning sensation, cold sensation, and pain. Two workers underwent urologic treatment for dysfunction of the urologic system and impotence. However, symptomatic treatment did not result in satisfactory relief of these symptoms. Conclusion: Awareness of the perils of mercury and prevention of mercury exposure are critical for preventing health hazards caused by mercury vapor. Chelation therapy should be performed promptly following mercury poisoning to minimize damage.

A new facing target sputtering (FTS) method that has two facing targets and substrate locates vertically to the targets was used for ITO anode deposition in OLED. The ITO different from it by a conventional DC or RF magnetron sputtering was optimized in crystallinity, grain morphology, surface roughness and oxygen content by plasma treatment, and the effect on the OLED I–V characteristic was identified. Crystallinity of ITO was improved with a short plasma treatment time less than 15 min. The grains were an equi-axed as deposited state and changed to columnar grains when the plasma treatment time was exceeded above 20 min. The surface roughness was maintained within the targeted 1.8–2.0 nm range with plasma treatment from 5 to 20 min. Oxygen content measured using XPS analysis was increased with 15 min plasma treatment during the ITO grains maintained an equi-axed, but it was decreased again with 25 min plasma treatment. The I–V characteristic of OLED was relatively good with 15 min plasma treatment. It was considered because the ITO anode can contain higher oxygen in equi-axed grains, and can have better crystallinity and relevant surface roughness by 15 min plasma treatment. Background/Aims: DA-9701, a standardized extract of Pharbitis Semen and Corydalis Tuber, is a new prokinetic agent that exhibits an analgesic effect on the abdomen. We investigated whether DA-9701 affects visceral pain induced by colorectal distension (CRD) in rats. Methods: A total of 21 rats were divided into three groups: group A (no CRD+no drug), group B (CRD+no drug), and group C (CRD+DA-9701). Expression of pain-related factors, substance P (SP), c-fos, and phosphorylated extracellular signal-regulated kinase (p-ERK) in the dorsal root ganglion (DRG) and spinal cord was determined by immunohistochemical staining and Western blotting. Results: The proportions of neurons in the DRG and spinal cord expressing SP, c-fos, and p-ERK were higher in group B than in group A. In the group C, the proportion of neurons in the DRG and spinal cord expressing p-ERK was lower than that in group B. Western blot results for p-ERK in the spinal cord indicated a higher level of expression in group B than in group A and a lower level of expression in group C than in group B. Conclusions: DA-9701 may decrease vis-ceral pain via the downregulation of p-ERK in the DRG and spinal cord. (Gut Liver 2014;8:140-147)
