<P><B>Abstract</B></P> <P>A 5.6 kDa antimicrobial peptide (AMP) was purified from acidified gill extract of the pen shell, <I>Atrina pectinata</I>, by cation exchange and C<SUB>18</SUB> reversed-phase high performance liquid chromatography. Comparison of the amino acid sequences and molecular weight of this peptide with those of other known AMPs revealed that it had high sequence homology with that of cgMolluscidin or hdMolluscidin; it was designated apMolluscidin. apMolluscidin comprises 59 amino acid residues containing several dibasic residue repeats and sequence repeats such as Lys-Lys and Lys-Gly. apMolluscidin exhibited potent antimicrobial activity against both Gram-positive bacteria including <I>Bacillus subtilis</I> (minimal effective concentration [MEC], 2.1 μg/mL), and Gram-negative bacteria including <I>E</I>. <I>coli</I> D31 (MEC, 0.5 μg/mL), without hemolytic activity. However, it did not show any activity against fungi such as <I>Candida albicans</I>. Secondary structure prediction suggested that it might form two helical regions and have an amphipathic structure. Full-length apMolluscidin cDNA contained 812 base pairs (bp), including a 5′-untranslated region (UTR) of 82 bp, a 3′-UTR of 547 bp, and a coding sequence of 183 bp encoding 60 amino acids (containing Met). Furthermore, qPCR analyses revealed that the mature peptide translated from <I>apMolluscidin</I> mRNA is expressed in a tissue-specific manner in locations such as the gill and siphon. These results indicate that apMolluscidin might be related to the innate immune defense system of abalone and may not act directly on the bacterial membrane. This is the first report of an AMP from the pen shell with a fully identified amino acid sequence.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A novel antimicrobial peptide was purified from the pen shell, <I>Atrina pectinata</I>. </LI> <LI> This was named apMolluscidin because it showed a high sequence homology with cgMolluscidin or hdMolluscidin. </LI> <LI> We found that apMolluscidin had broad-spectrum antibacterial activity and low hemolytic activity. </LI> <LI> Full-length apMolluscidin cDNA was cloned and its mRNA expression in tissues was evaluated with qPCR. </LI> </UL> </P>

<P><B>Abstract</B></P><P>A number of studies have demonstrated an association of neuropathic pain and chronic low back pain (CLBP), but the outcome difference in each medical management is poorly understood. This study is aimed to investigate treatment patterns of neuropathic pain in CLBP patients and to explore patient-reported outcomes (PROs) including quality of life (QoL) and functional disability by treatment patterns.</P><P>Data were extracted from the neuropathic low back pain (NLBP) outcomes research. It was a multicenter and cross-sectional study in which 1200 patients were enrolled at 27 general hospitals, from 2014 to 2015. Of total, 478 patients classified as neuropathic pain were used for this subgroup analysis. The patients were divided into 2 groups according to treatment patterns (with vs. without the targeted therapy [TT] of neuropathic pain). Demographic and clinical features were collected by chart reviews and PROs were measured by patient's survey. QoL was assessed by EuroQoL 5-dimension (EQ-5D) questionnaire. Functional disability was measured by the Quebec Back Pain Disability Scale (QBPDS). Multiple linear regression analyses were conducted to compare the PROs between TT group and non-targeted therapy (nTT) group.</P><P>Among the NLBP patients (mean age 63years, female 62%), EQ-5D index, EuroQoL-Visual Analog Scale (EQ-VAS), and QBPDS Scores (mean ± standard deviation) were 0.40 ± 0.28, 54.98 ± 19.98, and 46.03 ± 21.24, respectively. Only 142 (29.7%) patients had pharmacological TT of neuropathic pain. Univariate analyses revealed no significant mean differences between TT group and nTT group in the EQ-5D index (0.41 ± 0.27 and 0.39 ± 0.28), EQ-VAS (56.43 ± 18.17 and 54.37 ± 20.69), and QBPDS (45.31 ± 21.32 and 46.31 ± 21.24). After adjustment with covariates, TT group had higher scores of EQ-5D index (β = 0.07; <I>P</I> < 0.01) and EQ-VAS (β = 4.59; <I>P</I> < 0.05) than the nTT group. The TT group's QBPDS score was lower than the nTT group, although its statistical significance still has not been reached (β = −4.13; <I>P</I> = 0.07).</P><P>We found that considerable proportion of the NLBP patients remains untreated or undertreated. Although TT group had significantly better QoL than nTT group, only 29.7% of NLBP patients had pharmacological TT. Therefore, clinicians should consider using TT for better QoL of neuropathic pain patients.</P>

<P><B>Purpose</B></P><P>In this prospective, randomized, double-blinded study we sought to evaluate the efficacy and safety of combined use of intravenous ketorolac and acetaminophen in small children undergoing outpatient inguinal hernia repair.</P><P><B>Materials and Methods</B></P><P>We studied 55 children 1 to 5 years old who were undergoing elective repair of unilateral inguinal hernia. After induction of general anesthesia children in the experimental group (28 patients) received 1 mg/kg ketorolac and 20 mg/kg acetaminophen intravenously. In the control group (27 patients) the same volume of saline was administered. All patients received 1 μg/kg fentanyl intravenously before incision. We also evaluated the number of patients requiring postoperative rescue fentanyl, total fentanyl consumption, pain scores and side effects.</P><P><B>Results</B></P><P>Significantly fewer patients receiving ketorolac-acetaminophen received postoperative rescue fentanyl compared to controls (28.6% vs 81.5%). A significantly lower total dose of fentanyl was administered to patients receiving ketorolac-acetaminophen compared to controls (0.54 vs 1.37 μg/kg). Pain scores were significantly higher in the control group immediately postoperatively but eventually decreased. The incidences of sedation use (55.6% vs 25.0%) and vomiting (33.3% vs 10.7%) were significantly higher in controls.</P><P><B>Conclusions</B></P><P>Preoperative intravenous coadministration of ketorolac and acetaminophen is a simple, safe and effective method for relieving postoperative pain, and demonstrates highly significant fentanyl sparing effects in small children after outpatient inguinal hernia repair.</P>
Thick SmBCO/IBAD-MgO Coated Conductor for High Current Carrying Power Applications

<P>Long-length (Sm<SUB>1+x</SUB>B<SUB>2-x</SUB>C<SUB>3</SUB>O<SUB>7-y</SUB>) coated conductor was fabricated using ion beam assisted deposition (IBAD)-MgO template (LaMnO<SUB>3</SUB>/IBAD - MgO/Y<SUB>2</SUB>O<SUB>3</SUB>/Al<SUB>2</SUB>O<SUB>3</SUB>/Hastelloy) by the reactive co-evaporation method for electric power applications. SmBCO superconducting layers with thicknesses from 1 to 3 ¿m were deposited to investigate the effect of SmBCO thickness on the in-plane texture, surface morphology and superconducting properties. In this study, the optimal deposition conditions were examined to make thick SmBCO coated conductor without degradation of critical current density. The oxygen partial pressure and temperature were controlled in the range of 1~20 mTorr and 600 ~ 800°C, respectively. SmBCO/IBAD-MgO coated conductors showed high critical current densities over 2.0 MA/cm<SUP>2</SUP> with small amount of degradation by increasing the thickness of superconducting layer up to 3 ¿m. The critical current of 637.5 A/cm-w (J<SUB>C</SUB> = 2.12 MA/cm<SUP>2</SUP>) at 77 K in self- field was obtained in 3 ¿m-thick and 1 m long coated conductor. This is the highest critical current reported so far of the coated conductor with SmBCO superconducting layer deposited by co-evaporation.</P>
