      • 수술칼, CO_2 및 Nd : YAG 레이저에 의한 절개 창상의 치유에 관한 비교 연구

        The purpose of this study was to compare the wound healing process after skin incision using scalpel, CO_2 laser and pulsed Nd:YAG laser in rats. After skin on the back was incised 3 cm long, rats were sacrificed at 1, 3, 7, 14, 21 and 28 days. Macroscopic, histologic and immunohistochemiocal examinations using the collagen type Ⅳ and the CD34 antibodies which are necessary to the forming process of new capillary were performed. Results obtained were as follows ; Macroscopically the initial wound healing of the laser group was about 1-2 weeks slower than that of the scalpel group. There weren't however any remarkable differences in all groups in 4 weeks after incision. By histologic finding, acute inflammatory cells were more prominent during the initial wound healing in the scalpel group than in the other groups. Epithelialization started in the order of scalpel, CO_2 and Nd:YAG laser group after skin incision. By the Masson's trichrome stain, collagen synthesis in the Nd:YAG laser group was more slowly initiated than in the other groups. But it was completed at the 3~4 weeks in all groups. Immunohistochemically, collagen type Ⅳ and CD34 expression were markedly increased at 2 weeks in the scalpel and CO_2 laser group. Meanwhile, in the Nd:YAG laser group, these reactions were observed later than the other groups. Collagen type Ⅳ and CD34 expression were decreased in all groups after 4 weeks. These results suggest that CO_2 and Nd:YAG laser showed similar healing process compared with scalpel and a potential substitute for scalpel in skin incision.

      • 폐염간균에 대한 Quinolone계 약제의 항균작용에 미치는 Rifampin의 영향

        Antimicrobial activities of nalidixic acid(NA),norfloxacin(NX), oxolinic acid(OA), pefloxacin(PX), pipemidic acid(PA), oxacillin, cefotaxime(CFT), cephaloridine, cefazoline, moxalactam(MX), gentamicin, amikacin(AK) and rifampin(RP) were compared by the agar dilution method against 31 strains of K. pneumoniae isolated from various clinical spectimens. And then combined activities of RP with each of the quinolones (NA, MX, OAA, PX, and PA) against the test strains were measured by checkerboard dilution and time-killing methods. The growth inhibitory activities of PX, NX, OA, MX and AK were exellent, and those of PA, NA, RP and CFT were moderate, while the other's being almost negligible. The bactericidal activity of PX was more active than that of the other quinolones. Increasing concentrations of the quinolones resulted in a parallel increment of killing rate. However, when levels of quinolones were excessive beyond the most effective bactericidal ones, the bactericidal activity was decreased. In the combined growth inhigitory tests of RP with quinolones by checkerboard agar dilution method, synergistic and additive effects were rarely observed, and antagonism was noticed among the combinations in the following order: RP-NA, 22 strains (71%); RP-PA, 21 strains (67.7%); RP-OA, 17 strains (54.8%); RP-PX, 15 strains (48.4%) and RP-NA, 14 strains (45.2%). In the combined bactericidal tests measured by time-killing method, RP was found to reduce the activities of all five quinolones tested. And the gradual increasing of RP dose was revealed to reduce progressively the bactericidal action of the quinolones. But when RP was added to quinolones after 1 hour of incubations, the bactericidal activities were not affected.

        편측성 완전구순열에서 Cronin 삼각피판법과 회전신전법에 의한 수복의 비교

        The present study was carried out to compare the postoperative results of Cronin triangular flap and Millard rotation-advancement repair techniques in repairing the unilateral complete cleft lip. Thirteen patients with unilateral complete cleft lip underwent 6 Cronin triangular flap and 7 Millard rotation-advancement repairs at the Department of Oral and Maxillofacial Surgery of Chonnam University Hospital over a period of 6 years (January 1994 to December 1999). The surgical results following the operation were assessed on the basis of scoring, vertical lip length and scar hypertrophy. The mean score was better in Cronin triangular flap group(74.77±0.86) than in Millard rotation-advancement group(66.50±1.14) for both lip and nose segments. Scar hypertrophy developed in the Millard rotation-advancement repair group with 28.6% incidence but not in the Cronin triangular flap group. No significant difference was found in the ratio of lip length between Millard rotation-advancement repair and Cronin triangular flap groups(0.73±0.10 and 0.80±0.96) . These results suggest that Cronin triangular flap shows better results than Millard rotation-advancement repair in repairing the unilateral complete cleft lip.

      • 임프란트에 부착하는 세균의 동정 및 효과적인 항생제 선택

        This study was done to examine adherence of oral bacteria to titanium dental implant and to know the effective prophylactic antibiotics using an in vivo model. Three samples each of the implant material were set in an acrylic resin flange and placed in the maxillary buccal sulcus of twenty volunteers. At 6-and 54-hour intervals, each sample was placed on blood agar plate(BAP) and chocolate agar, and then they were incubated and identified. Also antibiotic susceptibility test was performed. The results obtained were as follows : 1. The microorganisms were chain-like Gram positive cocci and staphyline Gram positive cocci, Gram positive bacilli in order of frequency were found at 6-hour and 54-hour samples by Gram staining. 2. Streptococci was found predominantly at both 6-hour and 54-hour samples, but number of streptococci was decreased as compared to 6-hour samples. 3. There was no difference in the bacterial species adherent to implant between 6-hour and 54-hour samples. 4. All the microbes were sensitive to AMC(amoxacillin clavulanic acid), chloramphenicol, quinolone and vancomycin in the antibiotic susceptibility test. Above results suggest that streptococcus are mainly adhered to titanium implant after implant was placed in the oral cavity and AMC is the most recommendable antibioties to prevent the peri-implant inflammation.

        L-형 장골이식을 이용한 구순열 비변형의 수정

        Patients with a cleft lip nasal deformities hope for a pleasing nasal profile, a well-defined angular nasal tip, and symmetry of the nose. The main objective of the correction of cleft lip nasal deformity is to achieve nasal symmetry and an improved nasolabial relationship. The present study was carried out to assess the effect and prognosis of correction of cleft lip nasal deformity with L-shaped iliac bone graft. We performed correction surgery of the severe cleft lip nasal deformity with the autogenous L-shaped iliac bone graft in five patients and described the technique of correction. The columella portion of the L-shaped graft provided stabilization, eliminating the see-saw effect of the bridge on the bony fulcrum of the bony bridge, The disadvantage of this technique was stiffening of the nasal tip, which included the distal end of the grafted bone. However, the nasal tip gradually becomes softer, probably due to resorption of the bony edge several months after the operation. We obtained stable and symmetric reconstruction of the supporting elements of the nose. These results suggest that L-shaped iliac bone graft is stable and esthetic reconstruction method for correction of the severe cleft lip nasal deformity
