        Therapeutic Effect of Majoon Mundi and Qairooti Karnab in Dā al-Ṣadaf (Psoriasis): A Case Series

        Introduction: Psoriasis is a major health concern around the world. Physicians of the Unani system of medicine have been treating psoriasis for centuries. Aim: The purpose of our study was to assess the effect of Majoon Mundi (a semisolid Unani medication intended for oral intake used as blood purifier) and Qairooti Karnab (a Unani medication in paste form intended for topical application used as emollient) in the treatment $na{\ddot{i}}ve$ psoriasis cases and to collect data to warrant further clinical trials. Material and Methods: Psoriasis cases were diagnosed clinically. Data were collected during treatment of five patients of psoriasis treated with the Majoon Mundi (oral intake of 5 gm twice daily with 200 ml of water for 12 weeks) and Qairooti Karnab (topical application on affected sites twice a day for 12 weeks). Patients were treated for 12 weeks. Treatment response was seen with clinical improvement in skin lesions and measurement of Psoriasis Area and Severity Index (PASI Scoring) before and after treatment. Results: Reduced PASI Score was observed in all five patients after 12 weeks of treatment [PASI before and after treatment was (mean${\pm}$SD) $20.7{\pm}4.6$ vs. $3.2{\pm}1.8$; p-value <0.05.]. Clinical improvement was noticed within an average of 4 weeks of treatment. Conclusion: Preliminary findings indicate the potential therapeutic role of Majoon Mundi and Qairooti Karnab in the treatment of psoriasis. Clinical trials based on this Unani pharmacopeial formulation should be conducted to explore the therapeutic potential of this formulation in psoriasis

        Decoupling of hyperbolic conservation laws using an improved eigenvector matrix

        The resolution of Harten’s non-oscillatory explicit second-order accurate total variation diminishing scheme was relatively high for computations of nonlinear wave equations. However, this scheme shows false dissipation for sharp discontinuities, such as shocks and contacts, when applied to 1D Euler equation. The selection of the entropy fixing parameter, flux limiter, and appropriate eigenvector matrix is crucial to control the amount of such dissipation. This study focuses on the formulation of eigenvector matrix, which is used for the decoupling of a highly coupled nonlinear 1D Euler equation into a nonlinear wave equation. The selection of the eigenvector matrix is crucial to limit the unwanted false dissipation, which is inherent to the numerical schemes derived for nonlinear wave equations. A generalized form of the scaling factor for the eigenvector matrix is then derived, which is satisfied by the scaling factors used by Harten, Hofmann, and Yee. LAX, SOD, and inverse shock test cases of the shock tube problem are solved to examine and analyze the corresponding physical aspects. The findings show that multiplying the scaling factor by itself or by a constant does not affect the results. However, the variables inside the scaling factor, as well as these variables’ powers play a vital role in controlling false dissipation. These scaling factors can effectively capture physics in one case, but might not exhibit satisfactory performance for other problems. In addition, Yee’s scaling factor can effectively capture shock and contact discontinuities, especially for the inverse shock test case.

        Mochrus (Bombax ceiba Linn.): A Comprehensive Review on Pharmacology Phytochemistry, and Ethnomedicinal Uses

        The medicinal plants are extensively used for curing variegated malady in day to day life. There is an emergent demand for plant based medicines, food supplements, health products, pharmaceuticals etc. Mochrus (Bombax ceiba Linn.) is one of the valuable medicinal plants used in Unani system of medicine since relic belongs to family Bombacaceae. It is a tall tree and widely distributed through India, Africa, Australia and tropical Asia. Many parts of the plant (root, stem bark, gum, leaf, prickles, flower, fruit, seed and heartwood) are used for the treatment of a variety of ailments. It is reported to possess nafe sailanur reham (beneficial in leucorrhea), mujaffif (siccative), muqawwi reham (uterine tonic), qabiz (constipative), muallide mani (production of semen), mumsik wa mughalliz mani (increase consistency of semen), dafe fasaad khoon wa safra (purifies blood and bile) etc. It is used in asthma, diarrhoea, wound, leprosy, boils and many other skin diseases. Also possess some important pharmacological activities such as antioxidant, analgesic, antipyretic, antibacterial, diuretic, hepatoprotective, anticancer, hypoglycaemic and hypotensive etc. It is reported to contain phytoconstituents like polysaccharides, naphthoquinones, anthocyanins, lupeol and naphthol etc. This paper provides a compendium review on pharmacological, phytochemical properties and therapeutic benefits of the plant.

        Borjeson-Forssman-Lehmann Syndrome: Clinical Features and Diagnostic Challenges

        Borjeson-Forssman-Lehmann syndrome (BFLS) is an X-linked recessive disorder resulting from mutations in the PHF6 gene. The syndrome is characterized by short stature, obesity, hypogonadism, hypotonia, intellectual disability, distinctive facial features, fleshy ears, and finger and toe abnormalities. However, the diagnostic challenge in identifying BFLS remains a topic of interest. In this case report, we present the clinical characteristics of a proband with BFLS, highlighting the additional features of hypotonia, intellectual disability, and distinctive facial features. While no definitive treatment exists for BFLS, patients benefit from specialized education and ongoing supervision from early childhood through adulthood. Symptomatic treatment, including close follow-up, may be necessary for complications such as seizures and hearing problems. Mastectomy or testosterone replacement therapy may be considered on a case-by-case basis. Genetic counseling for X-linkage should be offered to affected families.

        Treatment of Pantothenate-Kinase Neurodegeneration With Baclofen, Botulinum Toxin, and Deferiprone: A Case Report

        Pantothenate kinase-associated neurodegeneration (PKAN) is a rare autosomal recessive disorder characterized by progressive motor symptoms, such as dystonia and spasticity. Classical PKAN is the most common subtype of neurodegeneration with brain iron accumulation (NBIA). Currently, there is no established treatment for PKAN. However, baclofen and botulinum toxin have been reported to improve motor symptoms and ease care in these patients. Additionally, Deferiprone is a well-tolerated iron chelator that has been shown to be effective in reducing brain iron accumulation. In this case report, we present the case of a seven-year-old boy who presented to our ward with spastic gait and extrapyramidal signs. Brain magnetic resonance imaging was performed, which showed features of neurodegeneration secondary to brain iron accumulation with a specific appearance of the eye-of-the-tiger sign. Genetic testing was positive for a homozygous mutation in PANK2, and the diagnosis of early-stage classical PKAN was made. This case report highlights the potent efficacy of baclofen, botulinum toxin, and deferiprone in slowing down the disease progression at an early stage and improving the severity of symptoms.
