Background/Aims: Dysphagia is the fundamental symptom of esophageal disorders. However, because dysphagia can be caused by difficulty in the passage of food anywhere from the mouth into the stomach, with oropharyngeal dysphagia, patients accurately recognize that their swallow dysfunction is in the oro pharynx; they may perceive food accumulating uncontrollably in the mouth or an inability to initiate swallow. Similarly they can generally recognize aspiration before, during, or after a swallow. Such is not the case, however, with esophageal dysphagia wherein patient identification of the location of obstruction is of limited accuracy. (Patients are cored in identifying the location of dysphagia only 60% of the time) to evaluate the accuracy of identification of location of dysphagia by patient and the ability of recognizing oropharyngeal dysphagia by general practitioners and internists, we carried out a prospective study. Methods: 437 patients who were referred by general practitioners and internists for evaluation of dysphagia from 2004 to 2006 were evaluated prospectively by using tree steps method. A: identification of symptoms of oropharyngeal dysphagia such as inability to initiating swallowing, nasopharyngeal regurgitation, aspiration, or post swallow coughing or choking, and location of diffculty in swallowing by re evaluation of the history were taken already by referring physicians, B: patients with unclear history, a simple test (drinking water swallowing test) was done. C: finally upper GI endoscopy was carried out by expert endoscopist very cautiously by full attention to the hypo pharynx, shape and movement of larynx and careful inspection of cricopharyngeus sphincter by using direct method of scope insertion under full observation of the patient by GI assistant and video endoscopy. Results: 26 patients were found to have oropharyngeal dysphagia among 437 patients who were labeled to have esophageal dysphagia by GP and internists (42%) patient were correct in identifying the location of dysphagia. Conclusions: 1-ability to recognize orophgaryngeal dysphagia is poor among Afghan general parishioners and even internists, 2-our patients are less accurate to localize esophageal dysphagia than developed courtiers (60% vs 42%), both findings urges CME and upgrading medical knowledge of the general population 3- a thoughtfully reevaluation of the history is critical before endoscopy.
Therapeutic Effect of Majoon Mundi and Qairooti Karnab in Dā al-Ṣadaf (Psoriasis): A Case Series

Introduction: Psoriasis is a major health concern around the world. Physicians of the Unani system of medicine have been treating psoriasis for centuries. Aim: The purpose of our study was to assess the effect of Majoon Mundi (a semisolid Unani medication intended for oral intake used as blood purifier) and Qairooti Karnab (a Unani medication in paste form intended for topical application used as emollient) in the treatment $na{\ddot{i}}ve$ psoriasis cases and to collect data to warrant further clinical trials. Material and Methods: Psoriasis cases were diagnosed clinically. Data were collected during treatment of five patients of psoriasis treated with the Majoon Mundi (oral intake of 5 gm twice daily with 200 ml of water for 12 weeks) and Qairooti Karnab (topical application on affected sites twice a day for 12 weeks). Patients were treated for 12 weeks. Treatment response was seen with clinical improvement in skin lesions and measurement of Psoriasis Area and Severity Index (PASI Scoring) before and after treatment. Results: Reduced PASI Score was observed in all five patients after 12 weeks of treatment [PASI before and after treatment was (mean${\pm}$SD) $20.7{\pm}4.6$ vs. $3.2{\pm}1.8$; p-value <0.05.]. Clinical improvement was noticed within an average of 4 weeks of treatment. Conclusion: Preliminary findings indicate the potential therapeutic role of Majoon Mundi and Qairooti Karnab in the treatment of psoriasis. Clinical trials based on this Unani pharmacopeial formulation should be conducted to explore the therapeutic potential of this formulation in psoriasis

In the present paper we introduce some sequence spaces over $n$-normed spaces defined by fractional difference operator and Musielak-Orlicz function $\mathcal{M} = (\mathfrak{F}_i)$. We also study some topological properties and prove some inclusion relations between these spaces.
