        Probabilistic computation of the structural performance of moment resisting steel frames

        This study investigates the reliability of the performance levels of moment resisting steel frames subjected to lateral loads such as wind and earthquake. The reliability assessment has been performed with respect to three performance levels: serviceability, damageability, and ultimate limit states. A four-story moment resisting frame is used as a typical example. In the reliability assessment the uncertainties in the loadings and in the capacity of the frame have been considered. The wind and earthquake loads are assumed to have lognormal distribution, and the frame resistance is assumed to have a normal distribution. In order to obtain an appropriate limit state function a linear relation between the loading and the deflection is formed. For the reliability analysis an algorithm has been developed for determination of limit state functions and iterations of the first order reliability method (FORM) procedure. By the method presented herein the multivariable analysis of a complicated reliability problem is reduced to an S-R problem. The procedure for iterations has been tested by a known problem for the purpose of avoiding convergence problems. The reliability indices for many cases have been obtained and also the effects of the coefficient of variation of load and resistance have been investigated.

        Reliability of Steel Truss Roof Systems Under Variable Snow Load Profiles

        The probabilistic safety analysis of steel truss roof systems under variable snow load profiles is investigated. The roofs of structures such as industrial buildings or sports halls, which require wide areas, are frequently subjected to unexpectedly high loads. Therefore, compared to the residential buildings, that type of buildings often comes across failure or prohibition of usage. Probabilistic techniques are utilized for the analysis of the problem. Thirty-six steel roof structures with different structural dimensions and load variations are modelled and their failure probabilities are calculated. In this paper, a complicated stochastic analysis is reduced to the solution of a load–resistance (S–R) problem by utilizing sensitivity analyses. Firstly, the structures were designed through structural analyses, and then the sensitivity analyses were conducted to understand the response of the structure to the load and to the span-length parameters. In this paper, a reliability study which gradually monitors the effect of geometrical parameters on the failure trend is presented. Three different snow load distribution functions were used. The relation between failure, and load or structural dimension variations was investigated. The results obtained in this study are discussed and compared with the results from the literature for similar structures subjected to snow loads. It is observed that for the snow load distributions with high standard deviations the structural reliability indices may give results below the target safety levels of the design codes. Finally, the assessment of the results shows that the effect of the standard deviation of the snow load on failure probability is much more than the effect of intensity of the nominal snow load.

        Objective: Brucellosis is a characterized zoonotic disease by multisystem involvement. Although musculoskeletal involvement is the most frequent case, the pulmonary system is rarely involved. Nodular involment is one of the rarest amoung pulmonary involvements. This study presents a 21- year-old-male with pulmonary involvement without any musculoskeletal system involvement and he was received a diagnosis in the early days of hospitalization. Case: The patient had complaints of one -month-long cough and sputum production and he informed that he used different antibiotics. After the last center he went detected hypogonadism in the patient, he was referred to our hospital`s endocrinology clinic. Physical examination showed fever:37.2,nb:112 beats/min and fi ndings in accordance with the hypogonadism and also late inspiratory crackles had been heard by oscultation. The captured chest radiograph indicated suspicious nodular appearance and suspected infi ltrated area. The laboratory tests erythrocyte sedimentation rate:41mm/saat,C-Reactive Protein:14.1,WBC : 4.08 x103/ul. The patient stated that he was working as a village shephard 1 month ago who did not suffer from signifi - cant weight loss nor fever. Detailed medical history stated that the last examination indicated brucella rosebengal positive, and the antiglobulin (coombs) test in brucellosis 1/160 was also positive. Altough the sputum culture and tuberculosis acid fast stain were negative, the patient had a positive blood culture regarding brucella melitensis. The thorax CT image showed nodules in the lungs with the ground glass opacity and infi ltrative image has been detected in subpleural area. There was nothing in bones scintigraphy. The patient was treated rifampicin 1x600 mg and doxycycline 2x100 mg for 6 weeks. Along with the treatment the patient`s complaints disappeared, and post-treatment blood cultures was negative. Control thorax CT reveals disappearanceof lesions in the lung area. Conclusion: Anamnesis and disease suspicion are the most important things regarding diagnosis of Brucellosis.

        Potts Model with the Simplest Modulated Phase

        We study the phase diagrams for the Potts model on the Bethe lattice with competing nearest-neighbor interactions J1 and ternary interactions J_p and J_t: A_t vanishing temperature, the phase diagram is fully determined for all values and signs of J_p = J_1 and J_t = J_1; in particular, we show that the phase diagrams contain ferromagnetic, paramagnetic and < 2 > phases only; <i>i.e.</i>, the set of modulated phases consists of the < 2 > phase only for J_1 > 0 and J_p = J_1 < 0: We verify that values of T = J_1 in the very narrow strip -0.11 < T = J_1 < 0 with -J_p = J_1 < 0 favor the modulated phase, which is either a commensurate phase with a very large period or an incommensurate phase. The transition lines are obtained from stability conditions, and characteristic points in the phase diagram are analyzed by using numerical iterations. Also the wavevectors versus temperature are plotted for some critical points in the modulated phases.

        La Projection Des Familles Ayant Enfant Handicapé Dans La Littérature: « Avez-vous du Bleu » - « Un Problème Personnel »

        N’importe où et quelle période qu’ils soient, les parents mettant au monde des bébés en situation, avec leurs caractéristiques physiques ou psychologiques, éprouvent des difficultés par rapport aux familles normales. Le degré de difficulté varie selon la situation de handicap de l’enfant/bébé. C’est pourquoi les mettre au monde en santé occupe une importance au regard d'eux- même, de leur famille et de la société. L'objectif de cette étude est de déceler la situation dans laquelle les individus ayant des enfants à besoins spécifiques, les changements dans leur niveau de vie, la façon dont la société les considère et les effets psychologiques de cette situation sur elles. Deux ouvrages prennent place, l'une par turque et l'autre par littérature japonaise, dans cette étude comparative effectuée. Parallèlement à l'œuvre « Avez-vous du Blue ? » par Uzun Ö, publiée comme une étude autobiographique dans la littérature turque et vue même dans le média ; l’œuvre « Un Problème Personnel » par Kenzaburo Oe rémunérée par le prix Nobel et comme l'une des figures les plus importantes dans la littérature japonaise contemporaine forme le cadre de l'étude. Originellement publié sous le nom de Kojintekina Taiken, « Un Problème Personnel » différencie par l'usage des noms réels des personnes de chez l'autre. Cependant, cela ne change ni l’importance de l’œuvre dans le monde littéraire ni la place de l’auteur dans sa vie privée. Comme ils sont deux de type autobiographique et donnent l'exemple des familles avec un enfant en situation, ces ouvrages différents par deux auteurs de différentes cultures marquent le même dénominateur. Par entremise de cette étude, on tentera de révéler les similitudes, les différences et les modes de vie s'accrochant à la vie parmi des familles ayant un /des enfant(s) handicapé(es). Les troubles que l'on éprouve individuellement parmi les membres de la famille et leur positionnement à un point différent dans la société exigent une approche multifonctionnelle au travail. Par conséquent, les travaux seront discutés en utilisant les données de la psychologie et de la sociologie et évalués dans la méthode éclectique (pluraliste) sous le toit de la littérature scientifique comparative.

        Introduction and Aim:While ischemia and nefrotoxins account for almost 45 % of all acute renal failure cases, prerenal (caused by congestive cardiac failure, liquid insuffi - ciency, sepsis and medication) azotemia accounts for around 20%. Among nefrotoxins, nonstereoid anti-infiammatory drugs (NSAII) may affect kidneys through multiple mechanisms (causing hemodynamic, acute allergic interstitial nephritis, papillary necrosis or glomerular nephritis). Herein, we present a patient who developed analgesic nephropathy associated with total anuria. Case: A patient with a medical history of benign prostatic hyperplasia, coronary artery disease, hypertension and tuberculosis was admitted to our department of emergency with complaints of dyspnea and headache. Laboratory examination revealed the following Results: urea: 138 mg/dL, kreatinin: 5,20 mg/dL, BK: 19300, ALT: 319 U/L AST: 324 U/ L LDH: 1947 U/L. The patient was diagnosed with total anuria. Because the patient had a history of analgesic usage, he was scheduled to undergo hemodialysis 3 times per week with a prediagnosis of analgesic nephropathy. Although initially renal function tests demonstrated a slight deterioration (urea: 177 mg/dL and creatinine: 8 mg/dL) they displayed a gradual decrease following ongoing hemodialysis sessions and consequently returned to normal (urea: 48 mg/dL and creatinine: 1, 25 mg/dL). The patient also received piperacillin/tazobactam 3x 2, 25 gr/day due to a concern for aspiration pneumonia. Diagnostic bronchoscopy results were in compliance with fi brosis at microscopic level. Conclusion/Recommendations: Analgesic nefropathy should be investigated when inquiring for the etiology of acute renal failure. Patients with total anuria should receive comprehensive treatment comprising renal replacement therapy such as hemodialysis. Additionally, interstitial lung disease and overlapping aspiration pneumonia may complicate the condition. Therefore, they also should be considered. Even patients with total anuria may return to normal life following a comprehensive and rigorous approach.

      • Evaluation of alveolar bone loss following rapid maxillary expansion using cone-beam computed tomography

        Objective: To evaluate the changes in cortical bone thickness, alveolar bone height, and the incidence of dehiscence and fenestration in the surrounding alveolar bone of posterior teeth after rapid maxillary expansion (RME) treatment using cone-beam computed tomography (CBCT). Methods: The CBCT records of 20 subjects (9 boys, mean age: 13.97 ± 1.17 years; 11 girls, mean age: 13.53 ± 2.12 year) that underwent RME were selected from the archives. CBCT scans had been taken before (T1) and after (T2) the RME. Moreover, 10 of the subjects had 6-month retention (T3) records. We used the CBCT data to evaluate the buccal and palatal aspects of the canines, first and second premolars, and the first molars at 3 vertical levels. The cortical bone thickness and alveolar bone height at T1 and T2 were evaluated with the paired-samples t-test or the Wilcoxon signed-rank test. Repeated measure ANOVA or the Friedman test was used to evaluate the statistical significance at T1, T2, and T3. Statistical significance was set at p < 0.05. Results: The buccal cortical bone thickness decreased gradually from baseline to the end of the retention period. After expansion, the buccal alveolar bone height was reduced significantly; however, this change was not statistically significant after the 6-month retention period. During the course of the treatment, the incidence of dehiscence and fenestration increased and decreased, respectively. Conclusions: RME may have detrimental effects on the supporting alveolar bone, since the thickness and height of the buccal alveolar bone decreased during the retention period.

        Cross-Cultural Adaptation, Validity, and Reliability Study of the Turkish Version of the Back Pain Functional Scale

        Study Design: Prospective study. Purpose: To investigate the cross-cultural adaptation, validity, and reliability of the Turkish version of the Back Pain Functional Scale (BPFS). Overview of Literature: Low back pain is a common disorder in the population that negatively affects the patient’s daily, professional, and social life. Self-report questionnaires are important to precisely evaluate back pain and making better and appropriate treatment decisions. Currently, there are several questionnaires for investigating mobility in patients with low back pain. Methods: We recruited 360 patients with chronic low back pain. For assessing the reliability of the scale, we performed test–retest and internal consistency analyses. Test–retest analysis was performed using intraclass correlation coefficient (ICC). Internal consistency was analyzed by calculating Cronbach’s alpha value. Construct validity of the questionnaire was evaluated by comparing total scores on the BPFS with those on Roland Morris Disability Questionnaire (RMDQ) and Oswestry Disability Index (ODI) using Pearson’s correlation coefficient analysis. Results: Cronbach’s alpha value was 0.910, indicating high internal consistency. The test–retest reliability was excellent (ICC, 0.958; 95% confidence interval, 0.710–0.908). No floor or ceiling effects were observed. Factor analysis indicated that the scale had a single-factor structure. Pearson correlation coefficient was −0.669 when BPFS was compared with RMDQ and −0.701 when compared with ODI. These values indicate a significant correlation of BPFS with RMDQ and ODI. Conclusions: These data indicate the validity and reliability of the Turkish version of the BPFS.

        Microvascular Decompression for Primary Trigeminal Neuralgia : Short-Term Follow-Up Results and Prognostic Factors

        Objective : The aim of this prospective study was to demonstrate the influence of some factors on the prognosis of microvascular decompression in 37 patients with trigeminal neuralgia. Methods : The results of microvascular decompression (MVD) in 37 patients with trigeminal neuralgia were evaluated at 6 months after surgery and were compared with clinical and operative findings. Results : The sex of the patient, the patient's age at surgery, the side of the pain, and the duration of symptoms before surgery did not play any significant roles in prognosis. Also, the visual analogue scale (VAS) of the patient, the duration of each pain attack, and the frequency of pain over 24 hours did not play any significant roles in prognosis. In addition, intraoperative detection of the type of conflicting vessel, the degree of severity of conflict, and the location of the conflict around the circumference of the root did not play any roles in prognosis. The only factors affecting the prognosis in MVD surgery were intraoperative detection of the site of the conflict along the root and neuroradiological compression signs on preoperative magnetic resonance imaging (MRI)/magnetic resonance angiography (MRA). Conclusion : These findings demonstrated that if neurovascular compression is seen on preoperative MRI/MRA and/or compression is found intraoperative at the root entry zone, then the patient will most likely benefit from MVD surgery.
