Ortner씨 증후군은 다양한 심장 질환에 이차적으로 생긴 반회 후두 신경 마비에 의한 쉰소리를 특징으로 하는 임상 질환이다. 저자들은 흉통없이 발생한 쇤소리를 주소로 내원한 환자에서 좌측 반회 후두 신경의 손상에 따른 성대 마비의 1차원적인 원인으로 대동맥궁 부위에서의 대동맥류를 진단하였기에 문헌 고찰과 더불어 보고하는 바이다. Ortner's syndrome is a clinical entity with hoarseness due to a left recurrent laryngeal nerve palsy caused by cardiac disease. It is a recognized complication of a number of conditions including, atrial septal defect, patent ductus arteriosus, primary pulmonary hypertension, Eisenmenger's syndrome and aortic aneurysm. A 71-year-old woman with a history of hypertension and diabetes complained hoarseness and hemoptysis for 3 days. Chest X-ray showed a soft tissue mass shadow in the left upper lung field and chest CT scan revealed a thoracic aortic aneurysm. We report a case of left recurrent laryngeal nerve palsy due to a huge thoracic aortic aneurysm in an old woman. (Korean J Med 64:105-108, 2003)

<P>Triple-negative breast cancer (TNBC) is an aggressive cancer with a poor prognosis due to its epithelial???to-mesenchymal transition (EMT) phenotype. Cancer patients often experience several detrimental effects of cancer treatment, such as chemoresistance, radioresistance and the maintenance of cancer stem cells due to EMT. Thus, EMT signaling is considered to be a valuable therapeutic target for cancer treatment, and its inhibition is being attempted as a new treatment option for TNBC patients. Previously, we showed that 3-(2-chlorobenzyl)-1,7-dimethyl-1H-imidazo[2,1-f]purine???2,4(3H,8H)-dione (IM-412) inhibits transforming growth factor-관 (TGF-관)-induced differentiation of human lung fibroblasts through both Smad-dependent and??-independent pathways. In the present study, we examined the inhibitory effect of IM-412 on EMT pathways and invasiveness in TNBC cells since the TGF-관 signaling pathway is a typical signaling pathway that functions in EMT. IM-412 not only potently suppressed the migration and invasion of MDA-MB-231??cells, but also lowered the expression of mesenchymal markers and EMT-activating transcription factors in these cells. IM-412 inhibited the activation of several signaling proteins, including Smad2/Smad3, p38MAPK, Akt and JNK, and it also attenuated the phosphorylation of FGFR1 and FGFR3. Collectively, our findings suggest that the synthetic compound IM-412 suppressed the EMT process in MDA-MB-231??cells and thereby effectively inhibited the migration and invasion of these cancer cells. Thus, IM-412 could serve as a novel therapeutic agent for malignant cancers.</P>

본 연구에서는 여러가지 in vivo 동물모델을 이용한 항염증제의 효력 검색법을 확립하여 항암증제의 효력시험 수탁고시에 따른 시험법을 확립하고, 비교적 부작용이 적은 천연물을 대상으로 항염증작용 검색 및 ㅡㄱ 기전을 연구하고자 하였다. 대표적인 급성 염증 모델인 rat에서의 carrageenan 및 PAF 유도 족척부종법, 염증반응 1기의 대표적인 현상인 백혈구 유주도를 측정하는 모질인 meuse에서의 zymosan운토 백혈구 유주 억제도 측정법, 염증만응 3기의 섬유화 현상을 측정하는 모델인 rat에서의 cotton pellet유도 육아종 형성 측정법, 만성 염즈으이 대표적 모델인 ratd에서의 adjubant 유도 관절염법 등 6종으 in vivo호력 시험법을 표준화하고 당귀와 원방풍 물 , 추출액, 은행엽메탕올 추출액, 죽염 수영액의 항염증작용을 검색하엿다. 당귀 물 추출액(1g/kg), 은행엽 메탄올 추출액 (1g/kg),죽염(1.5g/kg)은 carrageenan 및 PAF 유도 부종을 유의하게 억제하였고, 이중 은해엽 메틴올 추출액은 모세혈관 투과도를, 죽염은 육아종 형성을 유의하게 억제하였다. 검색한 천연물 중 zyinosan 유도 백혈굴 육주 및 adjuva띠:유도 관절염에 대해 항염증 작용을 갖는 물질은 얼었다. Carrageenan 유도 족척부종에 대한 항염증 작용 중은행13탄을 추출액과 죽염은 초기 및 후기 부종 전시간대(0.511r~6hr)에 걸쳐 항투종 작용이 있었으나 당귀 물 추출액은 carrageenan 유』A 부종 쵸기(1시간 이내)와 PAF 유도 부종에 대해 항루종 작용이 있어 carrageenan유도 분종의 초기 단계는 매개물질로 PAF가 관련한다는 점을 고려한다면 당귀의 항염증작용은 염증 매개 물질인 PAf의 작용을 억제하여 나'타날 가능성이 있을 것으로 사료된다. 이상의 실험을 통하여 염증반응 각 단계에 대한 항염증작용 겋색법을 화립하여 항염증제의 효력시험 수탁 고시에 따른 시혐법을 표준화하였고, 비교적 부작용이 적은 수종 천연물을 대상으로 항영킁제로의 이용 가능성롸 그 기전을 연구하였다. The antiinflammatory effects of fngeff-ca gigat fedeDoHriefBa sefefoifer, eiHfgo fifaD3 and Bamboo salt (Jukyom ) were investigated by in ri:same administration of methanolic extract of Ginkgo folium also inhibited the vascular permeability in - mice. The aqueous solution of Bamboo salt inhibited the formation of granuloma in SD rat at a oral dose -of Igfg. ABgelica gigantis rad;ix seems to have antiinflammatory effect by inhibition of PAF.
Pulmonary Leukocytoclastic Vasculitis as an Initial Presentation of Myelodysplastic Syndrome

Systemic vasculitis involving the lung is a rare manifestation of myelodysplastic syndrome (MDS), and secondary vasculitis is considered to have poor prognosis. A 44-year-old man presented with fever and dyspnea of 1 month duration. A chest radiograph revealed bilateral multiple wedge shaped consolidations. In addition, the results of a percutaneous needle biopsy for non-resolving pneumonia were compatible with pulmonary vasculitis. Bone marrow biopsy was performed due to the persistence of unexplained anemia and the patient was diagnosed with MDS. We reported a case of secondary vasculitis presenting as non-resolving pneumonia, later diagnosed as paraneoplastic syndrome of undiagnosed MDS. The cytopenia and vasculitis improved after a short course of glucocorticoid treatment, and there was no recurrence despite the progression of underlying MDS.

The tensile deformation behavior of copper, brass, nickel silver, phosphorous bronze and cold rolled steel sheets were studied. Based on true stress-true strain curves obtained by the uniaxial tension test at a constant cross-head speed, several constitutive equations were determined utilizing the regression analysis, and applicability of these equations to describe the stress-strain behavior of the non-ferrous alloys was investigated. In the present study, a Modified Voce equation, σ= B - (B - A) exp(-nε-C) is suggested, where a new term "C" has been incorporated into the exponential term of the Voce equation. The Modified Voce equation fits best with the experimental data of the nonferrous alloys and cold rolled steel sheets tested. From detail analysis of the exponential term, the value of "C/n" is found to be equal to the amonunt of pre-strain.
