        Effects of Robot-Mediated Gait Training Combined with Virtual Reality System on Muscle Activity: A Case Series Research

        Background: Previous robot-mediated gait training has been proven several limitations such as pointless repeated motion training, decreased presence, etc. In this research, adult stroke patients were participated in robot-mediated gait training accompanied with or without virtual reality program. Objectives: Exploring whether the results indicated virtual reality system has contribution to muscle strength and balance ability. Design: A case series research, cross-over trial. Methods: Eleven participants (male 4, female 7) with adults diagnosed as stroke from medical doctor ware engaged. The participants received 2 treatment sessions of identical duration, robot-assisted gait training with virtual reality and robot-assisted gait training with screen-off randomly crossed over include 1-day for each person of wash-out period. The parameter was muscle activity, the researchers assessed sEMG (surface electromyography). Results: The result showed less muscle activities during training in robotassisted gait training with virtual reality circumstances, and these indicated muscles were gluteus medius muscle, vastus medialis muscle, vastus intermedius and vastus lateralis muscle, semimembranosus muscle, gastrocnemius- lateral head, and soleus muscle (P<.05). Conclusion: In this study, we analyzed the outcome of muscle activity for clinical inference of robot-assisted gait training with virtual reality (VR). Less muscle activity was measured in the treatment accompanied by VR, therefore, a more systematic, in-depth and well-founded level of follow-up research is needed.

        Development of a Computer Pointing Operation Assessment Program and Usability Test with a Commercial Mouse and Assistive Devices: Case-series of Clinical Trial for Patients with Rheumatoid Arthritis

        Rheumatoid arthritis (RA) is inflammation of tissue and muscles around joints. Rheumatoid patients generally make up much of the computer-related work, but they lack evaluation tools to assess usability, which are rare for specialized computing and other input systems developed for this purpose. In this paper, computerized pointing and scrolling tasks were developed for patients with RA using C++ from MFC (Microsoft<SUP>®</SUP> Foundation Class Library). Those were organized for Parallax Scrolling Design and Single Page Design of web pages which have been actively used in Korea. Two RA patients, one male and one female, were examined three times a day, 10 minutes each time for one week to determine the actual usability. The results showed a higher success rate in most tasks except for the pointing tags of female patients. This study developed an assessment program that can evaluate the capabilities regarding the computing input devices, focusing on evaluating mouse operational capability. Therefore, the computer input device proficiency of people with physical limitations, such as RA, could be examined.

        장애인을 위한 3차원 인체 스캐닝 보조프레임 개발 연구

        연구배경 현대 사회의 장애인구는 상대적으로 증가하고 있으며, 그중 지체장애인의 경우 장애인구의 절반을 넘는 비율을 차지하고 있다. 이들 장애인들은 일상생활을 영위하기위해 다양한 보조기들을 필요로 한다. 이러한 보조기는 최근 입체적인 3차원 스캔을 통해 신체 치수를 정밀하게 측정하여 제작된다. 하지만 종래의 3차원 인체 스캔장치로는 뇌성마비, 뇌병변장애 등으로 인해 정적인 자세가 불가능한 장애인이나 심신이 허약하여 정적인 자세유지가 어려운 노약자를 대상으로 팔과 다리에 대한 3차원 수치 데이터를 정확하게 획득하기에는 많은 어려움이 있다. 연구방법 본 연구에서는 성인남녀신체치수조사 보고서의 근거와 COMP, VAS를 가지고 실시된 사용성 평가를 통해 정적 자세유지가 어려운 장애인의 팔과 다리를 안정적으로 고정하여 3차원 스캐닝을 편리하게 수행할 수 있도록 인체 스캔용 프레임 장치를 제작하는데 목적을 두고 있다. 연구결과 국가기술 표준원의 한국인 성인남녀신체치수조사 보고서를 근거로 장치의 가로길이를 120㎝, 세로길이를 60㎝로 결정하였고, 실제 스캐닝 실험을 통하여 높이58㎝를 산출하였다. 색상은 메인프레임의 경우 반사가능성 방지를 위해 무광 검은색으로 처리하였으며, 주요 조절 부재는 사용자가 쉽게 인지할 수 있도록 색상을 배치하였다. 장치는 크게 메인프레임과 가이드레일, 이동프레임, 고정 스트랩 및 스캐너트레이로 구성 된다. 작동방식은 대상자의 측정 환부의 길이에 맞게 이동프레임의 위치를 조정하고 고정 스트랩으로 고정한 후 스캐너트레이에 안착된 3차원스캐너를 작동시켜 정적인 자세에서 3차원 인체 스캔 작업을 수행하도록 한다. 사용성 평가결과 COMP에서 높은 사용성과 만족도를 나타내었지만 조작의 편의성과 사용자의 편의성부분 에서는 점수가 다소 낮게 나타나 개선책이 마련되어야 할 것으로 판단되었다. 그리고 VAS에서는 스트랩 부착부위와 지지면에서 높은 통증을 보고하였기에 수정이 필요할 것으로 판단되었다. 결론 본 연구를 통한 결과물이 장애인을 위한 3차원 인체 측정을 위한 기초자료로 활용될 수 있을 것으로 기대한다. Background Currently, the number of people with disabilities is increasing and people with physical disabilities account for more than half among them. They require assistive devices for activities of daily living. Assistive devices have been producted through accurate 3D body scanning lately. But it is hard to collect the accurate body data with the conventional 3D body scanning system because the disabled people and the elderly people are hard to keep static posture during scanning Methods In this study, it was aid to product frame structure for three-dimensional human body scan for the disabled people and the elderly people whose problem with 3D scanning is hard to keep static posture. The producted frame was Based on the reports from korean Agency for technology and standards Korean adults and then COMP and VAS tests are conducted for a usability test. Result For The description of the device for 3D scanner is conducted that 120cm in width, 60cm in height length based on the reports from korean Agency for technology and standards Korean adults and through actual scanning process, the researchers decided 58cm in height. The color of the frame structure is matte for preventing reflection of light and the other parts’ color was set as visible materials. The whole device is consisted of a main frame, two guide rails, a movable handy frame and two straps. In evaluating clinical utility of the device, COMP show that user satisfaction and usability were highly recorded but operation and user convenience need to be improved at operation and user convenience. and The pain was checked at attaching part of strap and support surface of frame through VAS test. Conclusion This results could provide the assistive frame structure for three-dimensional human body scan could be used not only disabled people but also elderly people and help the previous problems the conventional methods has shown.

        Deep Learning Frameworks for Cervical Mobilization Based on Website Images

        Background: Deep learning related research works on website medical images have been actively conducted in the field of health care, however, articles related to the musculoskeletal system have been introduced insufficiently, deep learning-based studies on classifying orthopedic manual therapy images would also just be entered. Objectives: To create a deep learning model that categorizes cervical mobilization images and establish a web application to find out its clinical utility. Design: Research and development. Methods: Three types of cervical mobilization images (central posteroanterior (CPA) mobilization, unilateral posteroanterior (UPA) mobilization, and anteroposterior (AP) mobilization) were obtained using functions of 'Download All Images' and a web crawler. Unnecessary images were filtered from 'Auslogics Duplicate File Finder' to obtain the final 144 data (CPA=62, UPA=46, AP=36). Training classified into 3 classes was conducted in Teachable Machine. The next procedures, the trained model source was uploaded to the web application cloud integrated development environment (https://ide.goorm.io/) and the frame was built. The trained model was tested in three environments: Teachable Machine File Upload (TMFU), Teachable Machine Webcam (TMW), and Web Service webcam (WSW). Results: In three environments (TMFU, TMW, WSW), the accuracy of CPA mobilization images was 81-96%. The accuracy of the UPA mobilization image was 43~94%, and the accuracy deviation was greater than that of CPA. The accuracy of the AP mobilization image was 65-75%, and the deviation was not large compared to the other groups. In the three environments, the average accuracy of CPA was 92%, and the accuracy of UPA and AP was similar up to 70%. Conclusion: This study suggests that training of images of orthopedic manual therapy using machine learning open software is possible, and that web applications made using this training model can be used clinically.

        Short-term Benefits of Mobilization for Patients with Non-Specific Neck Pains: Executive Function and Neck Pain Intensity

        Background: Cervical mobilization has been applied mainly for the improvement of arm and neck movements and pain reduction, and little research has been done to improve the executive function. Since this kind of so-called mechanical neck pain is one of most common symptoms, there are controversial issues about this with spine alignment. Posteroanterior (PA) mobilization from the Maitland concept is a process of examination, assessment, and treatment of neuromusculoskeletal disorder by manipulative physical therapy. Objective: To examine the short-term benefits of mobilization for patients with non-specific neck pain. Design: Dual-group Pretest-Posttest Design from the Quasi-Experimental research Methods: Fourteen participants (male 8, female 6; 20's of their age) with non-specific neck pains which are distributed all the unilateral or bilateral body side were recruited. Participants were categorized to Neck Pain with Movement Coordination Impairments (NPMCI) and Neck Pain with Mobility Deficits (NPMD) groups according to the results of physical examination. Professional physical therapist who has over 15-years-of clinical experience applicated manipulative therapy for the neck pain, an occupational therapist only conducted evaluations; K-NDI (Korean version of the Neck Disability Index), VAS (Visual Analog Scale), BDS-K (Korean version of Behavioral Dyscontrol Scale) for decreasing possible adverse effects; there were no person who reported other symptoms followed 4 weeks from the trial. Results: In the NPMCI group, data analysis indicated statistical differences between the PA mobilization interventions in NDI and BDS-K; even though, pain was reduced in VAS, this is not a significantly differ. In the NPMD group, data analysis represented statistical differences between the PA mobilization interventions in NDI, VAS and BDS-K; the scores were represented to be increased or the pain got relief. Conclusions: PA mobilization techniques according to Maitland concept have beneficial effects in patients with neck pain and other clinical positive effects which included neck disability, pain itself and motor function of upper extremity.

        압력 분포도 측정을 통한 휠체어 시트 디자인 임상적용 사례 연구

        연구배경 우리나라의 장애인구는 점차적으로 증가하는 추세이며 현재 250만 명에 이른다. 이중 보행이 불가한 장애인 과 노약자 및 뇌졸중, 척수마비로 인환 많은 환자들은 휠체어를 이동수단으로 사용하고 있다. 이로 인해 보조기구 중 휠체어는 높은 지급률과 수요를 보이고 있다. 이러한 휠체어 사용자들은 착석시스템이 적절하지 않은 경우 2차 질환을 맞이하기 쉽기 때문에 이를 해소하고자 많은 연구들이 진행되었다. 많은 연구를 통해 사용자의 체중에 대한 착석면의 압력분포가 골고루 분산되어야 한다는 점이 중요하게 나타났고 이에 대한 대안으로 전방웨지 시트에 대한 연구가 이루어져 긍정적인 결과를 도출했다. 연구방법 본 연구에서는 휠체어 사용자를 모집하여 전방 웨지 시트의 웨지높이와 시트의 경도를 변수로 두고 이에 따른 압력 분포를 측정하였다. XSENSOR사의 X-sensor<SUP>®</SUP>를 실험 도구로 사용하였으며 실험은 1차 2차로 나누어져 1차는 전방 웨지의 높이, 2차는 시트 재질의 경도에 대한 압력분포를 측정하였다. 이러한 실험을 통해 본 연구에서는 휠체어 사용자에게 최적의 압력분포를 보이는 전방웨지 시트를 제작 하는데 목적이 있다. 연구결과 실험결과 사용자에게 가장 적절한 시트의 경도는 18㎏/㎠로 도출되었고, 전방 웨지의 높이는 6㎝로 나타났다. 최종적으로, 실험을 통해 도출된 18㎏/㎠의 경도, 6㎝의 전방웨지 시트를 대상자의 신체 접촉 부위의 만곡이나 돌출부에 맞추어 추가적인 시트가공을 하였으며, 휠체어에 적용할 수 있도록 부가작업을 실시하였다. 결론 환자 맞춤형 휠체어 착석 시스템 디자인에 대한 본 연구는 향후 휠체어를 사용하는 장애인 삶의 질을 높이고, 2차 질환을 예방하며 통증을 감소시킴으로써, 이동성의 질을 높이는데 기여할 수 있을 것으로 예상된다. Background The population of disabled people in Korea are getting raised as almost 2.5 million people. Among those who are not able to walk such as the patient of stroke and spinal cord injury generally use a wheelchair for a final use of transportation system. Consequently, the wheelchair is the most important assistive technology among assistive devices. but, If the wheelchair user’s sitting system are not appropriate, secondary disorders are easily occurred, So many researches were done for that. The current researches conducted that seating pressure distribution are important for wedged seat system for optimal pressure distribution. Methods In this study, the main focus is aimed to design personalized and customized wedge seating system through repeated experiments of seating pressure distribution. There were 2 divisions of the trial experiment. First session is for density of the material, the other one is for determining the height of wedge using X-sensor® of XSENSOR company as a outcome measurement tool. Result As a result, suitable density of seat is 18㎏/㎠ and hight of wedge is 6㎝. finally, we fabricated the seat based on our results and additional manufacture system was proceeded for customize. Conclusion This results could be able to help wheelchair user patients’ quality of life by preventing secondary disorders and reducing pain.

        Laboratory Experiments for the Force and Load with Pseudo-Dynamic Test: Ex-vivo Study for the Manual Therapy

        Background: Because of the lack of accurate values for applied forces in manual therapy, manual therapists relies on the magnitude of the individual’s perception during applying the force. However, excessive loading maneuvers carry risks for patients. Objective: To establish the relationship between the maximal force applied to swine skin with the specific region, sex, and baseline parameters of the subject. Design: Ex-vivo Study and laboratory Experimental research Methods: 3.5 kg of Korean pork sirloin that is a piece of swine was handled and it was set 3 dimensions; #A; #B; #C. Forty-seven participants who has no experience in physical therapy randomly carried out the experiment, indicated to push each place of the pressure spots with same posture and process under supervision from the instructor who has over 15 years of manual therapy, and we measured the pressure force in each time. Results: The biggest pressure force was recorded in spot #A, and #B was represented after #C. Pressure on #A showed certain statistic relation with height (r=.317, p<.05) and weight (r=.434, p<.01); pressure on #B showed certain relation which has statistical meaning with only height (r=.401, p<.01); pressure on #C emerged to have statistic relationship with height (r=.308, p< .05)and weight (r=.428, p<.01). The age aspect revealed relation with pressure on #A, #B and #C, but that was not statistically significant. Conclusions: It can be inferred that there is the most loss of pressure in the area where cartilage is like an island in the middle.
